Kinetic characterization of adolescent scoliosis patients with Lenke 1B.

Purpose: The purpose of this study was to investigate dynamic responses of Lenke1B+ spines of adolescent scoliosis patients to different frequencies. Methods: Modal analysis, harmonic response analysis and transient dynamics of a full spine model inverted by the finite element method using Abaqus. Results: The first-order axial resonance frequency of 4.51 Hz produced a maximum axial displacement of 30.15 mm. Comparison of the five frequencies indicated that the 10 Hz frequency response curve was smoothest, while the amplitude-frequency curve at 4 Hz showed the greatest fluctuations accompanied

Tree shrew as a new animal model for musculoskeletal disorders and aging.

Intervertebral disc degeneration (IDD), osteoarthritis (OA), and osteoporosis (OP) are common musculoskeletal disorders (MSDs) with similar age-related risk factors, representing the leading causes of disability. However, successful therapeutic development and translation have been hampered by the lack of clinically-relevant animal models. In this study, we investigated the potential suitability of the tree shrew, a small mammal with a close genetic relationship to primates, as a new animal model for MSDs. Age-related spontaneous IDD in parallel with a gradual disappearance of notochordal cell

The Combination Strategy of the Drainage and Resection under the Microendoscope for Giant Discal Cyst: A Case Report.

INTRODUCTION: Discal cysts have been reported as intraspinal extradural cysts connected with corresponding intervertebral discs. The resection of the discal cyst has illustrated the good result. However, we report a much larger discal cyst, which was seemed to be difficult to resect all of them under the microendoscope safety. CASE REPORT: A 21-year-old man had experienced pain in the right lower limb for at least a month and was diagnosed with radiculopathy of the right S1 nerve root. Magnetic resonance image showed that mild disc degeneration and cyst at L5/S1, which was measured 12.0 mm on

Analysis of Risk Factors for Postoperative Progressive Segment Degeneration at the Decompression and Non-decompression Segments after Minimally Invasive Lumbar Decompression Surgery: A 5-year Follow-up Study.

INTRODUCTION: The risk factors for the development of progressive segment degeneration (PSD) after decompression surgery are still unknown. In this study, the risk factors for PSD in patients who undergo decompression surgery for lumbar spinal stenosis with and without coexisting spondylolisthesis and scoliosis were examined, focusing on decompression and non-decompression segments. METHODS: We reviewed the data of patients with >5 years of postoperative follow up. Radiographic PSD was defined as either the development of an anterolisthesis or retrolisthesis of >3 mm or a decrease in disc hei

Intervertebral vacuum phenomenon - prevalence and severity CT-scan analysis in patients older than 50 years: a retrospective cohort study.

BACKGROUND: Chronic low back pain secondary to degenerative changes in the spine is a common cause of disability, and disc degeneration is one of the most frequent imaging findings. Intervertebral vacuum phenomenon (IVP) is usually observed in advanced degeneration. Recently, this phenomenon has gained interest due to a relatively new surgical technique called percutaneous discoplasty, aimed at treating low back pain secondary to degenerative disc disease in elderly patients. PURPOSE: To analyze the prevalence and related factors of the vacuum phenomenon in adult patients. MATERIAL AND METHO

Development and validation of a geometrically personalized finite element model of the lower ligamentous cervical spine for clinical applications.

Epidemiological and clinical studies show that the magnitude and scope of cervical disease are on the rise, along with the world's rising aging population. From a biomechanical perspective, the cervical spine presents a wide inter-individual variability, where its motion patterns and load sharing strongly depend on the anatomy. This study aimed to first develop and validate a geometrically patient-specific model of the lower cervical spine for clinical applications, and secondly to use the model to investigate the spinal biomechanics associated with typical cervical disorders. Based on measure

The Incidence of Intradiscal, Intrathecal, and Intravascular Flow During the Performance of Retrodiscal (Infraneural) Approach for Lumbar Transforaminal Epidural Steroid Injections.

BACKGROUND: Lumbar transforaminal epidural steroid injections (TFESIs) are often used in the treatment of radicular pain. In light of safety concerns, many practitioners have proposed adopting the retrodiscal (infraneural) approach with the needle tip positioned into Kambin's triangle. With this technique, the needle may inadvertently be directed too far ventrally and enter the intervertebral disc. In addition, the risk of subarachnoid or subdural extra-arachnoid injection may be higher with this technique as well. OBJECTIVE: To determine the incidence of inadvertent intradiscal, intrathecal,

Clinical and Radiological Outcomes of Unilateral Biportal Endoscopic Decompression by 30° Arthroscopy in Lumbar Spinal Stenosis: Minimum 2-Year Follow-up.

BACKGROUND: Open microscopic laminectomy has been the standard surgical method for degenerative spinal stenosis without instability till now. However, it is associated with complications such as paraspinal muscle injury, excessive bleeding, and wound infection. Several surgical techniques, including microendoscopic decompression, have been introduced to solve these problems. METHODS: Authors analyzed retrospectively 55 patients presenting with neurological symptoms due to degenerative lumbar spinal stenosis refractory to conservative treatment. Patients with foraminal stenosis requiring foram

An unexpected cause of intractable dysphagia after 20 years following cervical spine instrumentation using bone cement alone: a case report.

INTRODUCTION AND IMPORTANCE: Postoperative dysphagia following anterior cervical discectomy fusion (ACDF) for cervical disc herniation is still poorly understood. Dysphagia after anterior spinal cervical approach is mild and transient. Here, the authors present a rare case suffering with severe progressive dysphagia for over 1 year after 20 years of ACDF due to expulsed bone cement abutting the esophagus which was successfully removed after reoperation. CASE PRESENTATION: A 59-year-old homemaker female presented to us with a foreign body sensation in the throat 'globus pharyngeus' and progres

Tailoring selection of transforaminal interbody spacers based on biomechanical characteristics and surgical goals: evaluation of an expandable spacer.

OBJECTIVE: Transforaminal lumbar interbody fusion (TLIF) is commonly used for lumbar fusion, such as for foraminal decompression, stabilization, and improving segmental lordosis. Although many options exist, surgical success is contingent on matching design strengths with surgical goals. The goal in the present study was to investigate the effects of an expandable interbody spacer and 2 traditional static spacer designs in terms of stability, compressive stiffness, foraminal height, and segmental lordosis. METHODS: Standard nondestructive flexibility tests (7.5 N⋅m) were performed on 8 cadave

Acute rib fracture caused by preoperative positioning for direct lateral interbody fusion: A case report.

RATIONALE: Degenerative spondylolisthesis is defined as forward slippage of a vertebra with respect to the underlying vertebra and is associated with the induction of lumbar canal stenosis. The use of anterior column support for degenerative lumbar conditions has been well documented. Direct lateral interbody fusion (DLIF) gains access via a lateral approach through the retroperitoneal fat and psoas muscle. It avoids many of the access-related complications yet comes with its own risks and limitations. The location of the iliac wing precludes exposure of the L5-S1 disc space and may make L4-5

Hybrid surgery of percutaneous transforaminal endoscopic surgery (PTES) combined with OLIF and anterolateral screws rod fixation for treatment of multi-level lumbar degenerative diseases with intervertebral instability.

BACKGROUND: Oblique lumbar interbody fusion (OLIF) has been used to treat lumbar intervertebral instability, which has some advantages including less trauma, less blood loss, faster recovery and bigger cage. However, it usually needs posterior screws fixation for biomechanical stability, and possible direct decompression for relieving neurologic symptoms. In this study, OLIF and anterolateral screws rod fixation through mini-incision were combined with percutaneous transforaminal endoscopic surgery (PTES) for the treatment of multi-level lumbar degenerative diseases (LDDs) with intervertebral

Association between vertebral endplate defects and patient-reported symptoms: an immunohistochemical study investigating the COX-2/PGE-2/EP-4 axis.

BACKGROUND CONTEXT: Vertebral endplate defects are often implicated in degenerative disc disorders, yet their connection to patient-reported symptoms remains unclear. COX-2 and PGE-2 are known for their roles in inflammation and pain, with EP-4 receptor involvement in pain signaling. Examining their expression in vertebral endplate tissues may provide insights into pathomechanism of low back pain. PURPOSE: To investigate the association between endplate defects and patient-reported symptoms and to further clarify the role of the COX-2/PGE-2/EP-4 axis in the pathogenesis of chronic low back pa
