Exposure assessment and risks associated with wearing silver nanoparticle-coated textiles.

BACKGROUND: Silver (Ag) nanoparticles (NPs) are used increasingly in consumer and healthcare fabrics due to their antimicrobial properties. Abrasive leaching experiments have shown that AgNPs can be released during textile wear and cause a dermal exposure. Derived-no-effect-limit value for AgNPs ranges from 0.01 to 0.0375 mg/kg-body-weight, and thus, low exposures levels can cause relevant risk. METHODS: In this study AgNP release from textiles by artificial sweat immersion and mechanical stress was investigated. A mass balance model was used to calculate dermal Ag exposure and potential inta

Managing a difficult airway due to supraglottic masses: successful videolaryngoscopic intubation after induction of general anesthesia.

BACKGROUND: While awake, flexible bronchoscopic intubation has long been considered the gold standard for managing anticipated difficult airways, the videolaryngoscope has emerged as a viable alternative. In addition, the decision to perform awake intubation or to proceed with airway management after induction of general anesthesia should be grounded in a comprehensive assessment of risks and benefits. CASE PRESENTATION: A 41-year old female patient was scheduled for excision of bilateral, mobile, and pedunculated masses on both aryepiglottic folds, which covered almost the entire upper part

Dynamic assessment of spine movement patterns using an RGB-D camera and deep learning.

In clinical practice, functional limitations in patients with low back pain are subjectively assessed, potentially leading to misdiagnosis and prolonged pain. This paper proposes an objective deep learning (DL) markerless motion capture system that uses a red-green-blue-depth (RGB-D) camera to measure the kinematics of the spine during flexion-extension (FE) through: 1) the development and validation of a DL semantic segmentation algorithm that segments the back into four anatomical classes and 2) the development and validation of a framework that uses these segmentations to measure spine kine

Obstructive Sleep Apnea in Adults: Common Questions and Answers.

Obstructive sleep apnea (OSA) is a common disorder that affects quality of life and is associated with comorbidities such as hypertension, atrial fibrillation, heart failure, coronary heart disease, type 2 diabetes mellitus, and stroke. OSA is characterized by a reduction or cessation of breathing during sleep, resulting in intermittent hypoxemia, autonomic fluctuation, and sleep fragmentation. The U.S. Preventive Services Task Force states that there is insufficient evidence to recommend routine screening for OSA in the absence of symptoms. OSA should be considered in patients with excessive

Excess Folic Acid and Vitamin B12 Deficiency: Clinical Implications?

BACKGROUND: In the 1940s to 1950s, high-dose folic acid supplements (>5 mg/d) were used clinically to reverse the megaloblastic anemia of vitamin B12 deficiency caused by pernicious anemia. However, this treatment strategy masked the underlying B12 deficiency and possibly exacerbated its neuropathological progression. The issue of masking and exacerbating B12 deficiency has recently been rekindled with the institution of folic acid fortification and the wide-spread use of folic acid supplements. OBJECTIVES: The objectives of this review are to describe clinical and epidemiological evidence th

Evaluation of the relationship between the STOP-Bang score with oxygen reserve index and difficult airway: a prospective observational study.

BACKGROUND: Patients diagnosed with Obstructive Sleep Apnea (OSA) syndrome have a tendency towards hypoventilation, hypoxia, and hypercarbia in the perioperative period. This study hypothesized that the Oxygen Reserve Index (ORi) could predict possible hypoxia and determine difficult airways in patients at risk for OSA, as determined by the STOP-Bang questionnaire. METHODS: This prospective study included adult patients undergoing elective surgery under general anesthesia with endotracheal intubation, divided into two groups: low risk (0-2 points) and high risk (3-8 points) based on their STO

Joint Contributions of Auditory, Proprioceptive and Visual Cues on Human Balance.

One's ability to maintain their center of mass within their base of support (i.e., balance) is believed to be the result of multisensory integration. Much of the research in this literature has focused on integration of visual, vestibular, and proprioceptive cues. However, several recent studies have found evidence that auditory cues can impact balance control metrics. In the present study, we sought to better characterize the impact of auditory cues on narrow stance balance task performance with different combinations of visual stimuli (virtual and real world) and support surfaces (firm and c

Reuse of Personal Protective Equipment: Results of a Human Factors Study Using Fluorescence to Identify Self-Contamination During Donning and Doffing.

BACKGROUND: At least 115,000 health and care workers (HCWs) are estimated to have lost their lives to COVID-19, according to the the chief of the World Health Organization (WHO). Personal protective equipment (PPE) is the first line of defense for HCWs against infectious diseases. At the height of the pandemic, PPE supplies became scarce, necessitating reuse, which increased the occupational COVID-19 risks to HCWs. Currently, there are few robust studies addressing PPE reuse and practice variability, leaving HCWs vulnerable to accidental contamination and harm. OBJECTIVE: The objective of thi

New technologies in airway management: A review.

The evolution of medical knowledge and technological growth have contributed to the development of different techniques and devices for airway management. These appear to play a role in optimizing the number of attempts and overall success, ultimately reducing the negative consequences of airway manipulation. In this literature review, we highlight the recent evidence regarding new technologies applied to airway management. Before intubation, every patient should have an individualized structured airway management plan. Technology can help with both airway evaluation and tracheal intubation. P

A denoising method for multidimensional magnetic resonance spectroscopy and imaging based on compressed sensing.

Both in spectroscopy and imaging, t1-noise arising from instabilities such as temperature alterations, field-related frequency drifts, electronic and sample-spinning instabilities, or motions in in vivo experiments, affects many 2D Magnetic Resonance experiments. This work introduces a post-processing method that aims to attenuate t1-noise, by suitably averaging multiple signals/representations that have been reconstructed from the sampled data. The ensuing Compressed Sensing Multiplicative (CoSeM) denoising starts from a fully sampled 2D MR data set, discards random indirect-domain points, an

Explainable multiple abnormality classification of chest CT volumes.

Understanding model predictions is critical in healthcare, to facilitate rapid verification of model correctness and to guard against use of models that exploit confounding variables. We introduce the challenging new task of explainable multiple abnormality classification in volumetric medical images, in which a model must indicate the regions used to predict each abnormality. To solve this task, we propose a multiple instance learning convolutional neural network, AxialNet, that allows identification of top slices for each abnormality. Next we incorporate HiResCAM, an attention mechanism, to

MallesNet: A multi-object assistance based network for brachial plexus segmentation in ultrasound images.

Ultrasound-guided injection is widely used to help anesthesiologists perform anesthesia in peripheral nerve blockade (PNB). However, it is a daunting task to accurately identify nerve structure in ultrasound images even for the experienced anesthesiologists. In this paper, a Multi-object assistance based Brachial Plexus Segmentation Network, named MallesNet, is proposed to improve the nerve segmentation performance in ultrasound image with the assistance of simultaneously segmenting its surrounding anatomical structures (e.g., muscle, vein, and artery). The MallesNet is designed by following t
