[Pathophysiology of spasticity].

Lance's definition of spasticity focuses on the exaggeration of the tonic stretch reflex as one component of the upper motor neuron syndrome. In daily practice, many different symptoms are referred to as spasticity. Experimental studies stress the particular role of the premotor cortex and the medial reticular formation for the genesis of spasticity. Physiological studies clearly demonstrate the two components, phasic and tonic, of the stretch reflex. Whatever the pathology, the clinical picture of spasticity seems to depend less upon the etiology of the lesion and more upon its location in th

Projections from the pericruciate cortex to the nucleus of Darkschewitsch and other structures at the mesodiencephalic junction in the cat.

After injecting a mixture of tritiated amino acids into various regions of the frontal cortices of cats, autoradiograms of the mesodiencephalic junctional region were processed. When the injections were placed in the lateral part of the motor area of the hand or arm regions, silver grains were manifested in the nucleus of Darkschewitsch (ND) in its whole rostrocaudal extent, and they were observed also in the ventrolateral part of the anterior pretectal nucleus (PA) and in the caudal portion of the posterior pretectal nucleus (PP). Following injections made into the medialmost part of the ante

Sex Differences in Progression of Neurodegeneration: The Age, Gene/Environment Susceptibility-Reykjavik Study.

INTRODUCTION: Advancing age is associated with global brain atrophy. Cross-sectional studies have found sex differences in neuroanatomy; however, longitudinal studies assessing sex differences in neurodegeneration are currently scarce. The effects of age and sex on brain atrophy may not be uniform across the whole brain and may partially explain the sex differences observed in dementia. The current study aimed to examine sex differences in longitudinal atrophy patterns in gray and white matter regions in older adults. METHODS: The study sample included 1,480 individuals from the Age, Gene/Env

Precise detection of awareness in disorders of consciousness using deep learning framework.

Diagnosis of disorders of consciousness (DOC) remains a formidable challenge. Deep learning methods have been widely applied in general neurological and psychiatry disorders, while limited in DOC domain. Considering the successful use of resting-state functional MRI (rs-fMRI) for evaluating patients with DOC, this study seeks to explore the conjunction of deep learning techniques and rs-fMRI in precisely detecting awareness in DOC. We initiated our research with a benchmark dataset comprising 140 participants, including 76 unresponsive wakefulness syndrome (UWS), 25 minimally conscious state (

Optimal Brain Targets for Enhancing Vocal Performance With Transcranial Direct Current Stimulation.

Transcranial direct current stimulation (tDCS) can be used to temporarily and reversibly modulate brain functions and is an increasingly utilized tool to investigate the relationships between brain and behavior. This study aimed to identify the brain area where tDCS has the greatest effect on vocal quality, vocal range, neuromotor functioning of the larynx, and self-reported vocal effort in vocally healthy individuals. Sixteen adults, non-singers, with a mean age of 22.5 (±1.9) years and no vocal symptoms or brain dysfunction, participated in this double-blinded randomized crossover clinical t

Cortical microstructural gradients capture memory network reorganization in temporal lobe epilepsy.

Temporal lobe epilepsy (TLE), one of the most common pharmaco-resistant epilepsies, is associated with pathology of paralimbic brain regions, particularly in the mesiotemporal lobe. Cognitive dysfunction in TLE is frequent, and particularly affects episodic memory. Crucially, these difficulties challenge the quality of life of patients, sometimes more than seizures, underscoring the need to assess neural processes of cognitive dysfunction in TLE to improve patient management. Our work harnessed a novel conceptual and analytical approach to assess spatial gradients of microstructural differenti

Abnormal percent amplitude of fluctuation and functional connectivity within and between networks in benign epilepsy with centrotemporal spikes.

BACKGROUND: Benign epilepsy with centrotemporal spikes (BECTS) is one of the most common childhood epilepsy syndromes. The neural basis of BECTS is still poorly understood. This study aimed to further investigate the possible neural mechanisms of BECTS by comparing percent amplitude of fluctuation (PerAF) of resting-state functional magnetic resonance imaging (RS-fMRI) signal of each brain voxel and connectivity within and between related networks in children with BECTS and healthy controls (HCs). METHODS: Firstly, we used PerAF method to investigate brain functional alteration and defined th

Central Autonomic Network Alterations in Anorexia Nervosa Following Peripheral Adrenergic Stimulation.

BACKGROUND: Anorexia nervosa (AN) is characterized by low body weight, disturbed eating, body image disturbance, anxiety, and interoceptive dysfunction. However, the neural processes underlying these dysfunctions in AN are unclear. This investigation combined an interoceptive pharmacological probe, the peripheral β-adrenergic agonist isoproterenol, with resting-state functional magnetic resonance imaging to examine whether individuals with AN relative to healthy comparison participants show dysregulated neural coupling in central autonomic network brain regions. METHODS: Resting-state functio

Altered sensorimotor cortex noradrenergic function in idiopathic REM sleep behaviour disorder - A PET study.

INTRODUCTION: Noradrenergic denervation is thought to aggravate motor dysfunction in Parkinson's disease (PD). In a previous PET study with the norepinephrine transporter (NART) ligand 11C-MeNER, we detected reduced NART binding in primary sensorimotor cortex (M1S1) of PD patients. Idiopathic rapid-eye-movement sleep behaviour disorder (iRBD) is a phenotype of prodromal PD. Using 11C-MeNER PET, we investigated whether iRBD patients showed similar NART binding reductions in M1S1 cortex as PD patients. Additionally, we investigated whether 11C-MeNER binding and loss of nigrostriatal dopamine sto

The effects of add-on non-invasive brain stimulation in fibromyalgia: a meta-analysis and meta-regression of randomized controlled trials.

OBJECTIVES: The effects of non-invasive brain stimulation (NBS), including repetitive transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (TDCS), in treating FM remain inconclusive. The aim of this study was to investigate present evidence of using NBS as an add-on treatment in treating FM. METHODS: We conducted a database search of the Medline, Embase, PsycINFO and Cochrane Library electronic databases, from inception to July 2015, to analyse randomized controlled trials of NBS in treating FM. A total of 16 studies were included in the current meta-analysis.

Distinct medication-state modulation of motor-cortical low-beta power in tremor-dominant and postural instability/gait difficulty Parkinson's disease: a source-space resting-state EEG study.

BACKGROUND: Source-space electroencephalography (EEG) provides a noninvasive approach for probing oscillatory activity in motor-cortical and cerebellar networks in Parkinson's disease (PD). However, electrophysiological evidence differentiating tremor-dominant (TD) and postural instability/gait difficulty (PIGD) subtypes under a harmonized medication OFF/ON protocol remains limited. We therefore examined whether TD and PIGD differ in medication-state modulation of motor-cortical low-beta power and cerebellum-related connectivity using 64-channel resting-state source-space EEG. METHODS: We pro

Transcranial magnetic stimulation combined with functional near-infrared spectroscopy to elucidate the neurophysiological mechanisms of post-stroke hemiplegia: a systematic review.

Patients with hemiplegia after stroke often suffer from motor dysfunction, and their recovery is frequently limited by the imbalanced interhemispheric competition and abnormal neural network reorganization. The combined application of transcranial magnetic stimulation (TMS) and functional near-infrared spectroscopy (fNIRS) has provided a novel closed-loop neuromodulation strategy for the assessment and intervention of post-stroke motor function. This systematic review aims to systematically synthesize clinical research evidence regarding the combined use of TMS and fNIRS in post-stroke motor f

Modulation of Brainstem-Cortical Pathways by Injection Laryngoplasty in Unilateral Vocal Fold Paralysis: A Longitudinal Diffusion MRI Case Study.

OBJECTIVE: This study investigates the potential role of injection laryngoplasty with hyaluronic acid in supporting spontaneous reinnervation and the recovery of vocal fold mobility, following unilateral vocal fold paralysis (UVFP). Building on previous findings suggesting functional activation of the brainstem during a sustained vowel task before vocal fold mobility recovery (Dedry, Dricot, Van Parys, Boucquey, Delinte, van Lith-Bijl, Szmalec Maryn & Desuter, 2022), this study aims to explore the neural plasticity in response to this peripheral intervention. Specifically, we hypothesize that

Neuroprotective effects of a cannabidiol nanoemulsion in a rotenone-induced rat model of Parkinson's disease: Insights into the gut-brain axis.

Parkinson's disease (PD) is a neurodegenerative disease characterized by motor and non-motor symptoms, often associated with the accumulation of α-synuclein, neuroinflammation and oxidative stress in the central and peripheral systems. Considering that patients with Parkinson's disease often have gastrointestinal symptoms, this study aimed to investigate whether a rotenone-induced rat model of PD can induce intestinal changes and the neuroprotective effects of a cannabidiol (CBDne) nanoemulsion formulation, focusing on the striatum and intestine. Wistar rats were divided into six groups: contr

Characterizing upper extremity fine motor function in the presence of white matter hyperintensities: A 7 T MRI cross-sectional study in older adults.

BACKGROUND: White matter hyperintensities (WMH) are a prevalent radiographic finding in the aging brain studies. Research on WMH association with motor impairment is mostly focused on the lower-extremity function and further investigation on the upper-extremity is needed. How different degrees of WMH burden impact the network of activation recruited during upper limb motor performance could provide further insight on the complex mechanisms of WMH pathophysiology and its interaction with aging and neurological disease processes. METHODS: 40 healthy elderly subjects without a neurological/psych

Mechanism of Alzheimer type II astrocyte development in hepatic encephalopathy.

Type C hepatic encephalopathy (Type C HE) is a major and complex neurological condition that occurs following chronic liver failure. The molecular basis of Type C HE remains elusive. Type C HE is characterized by mental confusion, cognitive and motor disturbances. The presence of Alzheimer type II astrocytes (AT2A) is the key histopathological finding observed in Type C HE. However, nothing is currently known regarding AT2A development and its involvement in cognitive, and motor deficits in Type C HE. We, therefore, examined in rats the mechanisms by which liver failure contributes to the prog

Normative data of cortical excitability measurements obtained by transcranial magnetic stimulation in healthy subjects.

INTRODUCTION: The assessment of cortical excitability (CE) measurements has been increasingly used in neuropsychiatric research. However, there is scant information on the normative values of these measurements, as well as the possible effect of hemisphere laterality, gender and age on these variables. OBJECTIVES: To obtain normative data for CE measurements by transcranial magnetic stimulation, to assess inter-/intra-investigator variability and the influence of sex, age and oral contraception use. METHODS: A sample of 216 healthy volunteers matched according to age and gender was evaluated

Effects of oropharyngeal exercises on orofacial function and corticomotor excitability in healthy individuals.

Oropharyngeal exercises/myofunctional therapy (OE/MT) offer complementary benefits for obstructive sleep apnea. However, the complex nature of the intervention impedes the understanding of the underlying mechanisms related to sensorimotor function and corticomotor control of involved muscles. This study explored the sensorimotor rehabilitation effects of OE/MT tasks in twenty-two healthy adults (24.1 ± 3.1 years) using a cross-over design to better understand the normal physiology of the tongue before and after these different tasks. Sessions involved breathing training (BT), tongue training (

Beta transcranial alternating current stimulation over the dorsal premotor area impairs response initiation but not inhibition in humans.

BACKGROUND: Response initiation and inhibition are core components of movement control, supported by a distributed frontal network including the dorsal premotor cortex (PMd) and inferior frontal gyrus (IFG). Converging evidence suggests that neural oscillations, particularly in the β- and γ bands, contribute to both response initiation and inhibitory control. Transcranial alternating current stimulation (tACS) enables non-invasive, frequency-specific modulation of cortical oscillations, offering a tool to causally probe these mechanisms. This study investigated whether β- and γ-tACS applied ov

Prior use-dependent plasticity triggers different individual corticomotor responses during persistent musculoskeletal pain.

Movement repetition is crucial for pain interventions. It facilitates the rehabilitation of motor patterns, the acquisition of motor skills, and the genesis of adaptive use-dependent plasticity. However, the influence of prior motor experience and preexisting use-dependent plasticity on pain severity and progression remains poorly investigated. This study investigated the effects of preexisting use-dependent plasticity during the development of prolonged experimental musculoskeletal pain. Using transcranial magnetic stimulation, corticospinal excitability was assessed by measuring the Rest mot
