Icodextrin-induced acute generalized exanthematous pustulosis in a patient with peritoneal dialysis.

Icodextrin has been widely prescribed for peritoneal dialysis (PD) patients with inadequate ultrafiltration, but icodextrin induced acute generalized exanthematous pustulosis (AGEP) has been not well recognized in clinical practice. We described a young-aged female with IgA nephropathy and end stage kidney disease under continuous automated peritoneal dialysis. She developed skin erythema with exfoliation over the groin 7th day after initiation of icodextrin based PD dialysate. Initially, her scaling skin lesion with pinhead-sized pustules affected the bilateral inguinal folds, and then it ext

Biphasic Peritoneal Mesothelioma Is a Rare Tumor and a Diagnostic Challenge: A Case Report.

Malignant peritoneal mesothelioma (MPM) is a rare subtype of mesothelioma. There are three main histological subtypes of mesothelioma: epithelioid, sarcomatoid, and biphasic (mixed). Risk factors include asbestos exposure, previous radiation, and some germline mutations. Treatment includes surgical resection of amenable tumors or cytoreductive surgery and hyperthermic intraperitoneal chemotherapy. We present a 34-year-old male who presented with weight loss, night sweats, and pleuritic chest pain and was found to have ascites with peritoneal nodularity on abdominal imaging. He had a history of

Establishment of permanent peritoneal PleurX catheter as palliative treatment of malignant ascites.

INTRODUCTION: Refractory malignant ascites (RMA) is a manifestation of end-stage cancer disease with a major impact on quality of life due to the symptom burden and need for repeated hospitalisations. We previously described the implantation of the permanent PleurX catheter as a treatment of RMA. The present study aimed to evaluate and describe our experience with the permanent PleurX catheter system in the largest cohort to date. METHODS: A total of 97 consecutive patients had a PleurX catheter implanted from 2015 to 2021. We retrieved retrospective data on patients and procedures using the

Laparoscopic peritoneal dialysis catheter insertion with omentopexy-sparing the omentum in children.

INTRODUCTION: Continuous ambulatory peritoneal dialysis is an important modality of renal replacement therapy in children. Catheter dysfunction (commonly obstruction) is a major cause of morbidity and is a significant concern that hampers renal replacement therapy. As omentum is a significant cause of obstruction, some recommend routine omentectomy during insertion of the peritoneal dialysis catheter. Omentopexy rather than omentectomy has been described in adults to spare the omentum as it may be needed as a spare part in many conditions. Laparoscopic approach is commonly preferred as it prov

Determination of Extra- and Intraperitoneal Fluid During Peritoneal Dialysis Using Bioimpedance.

OBJECTIVE: In peritoneal dialysis (PD), ultrafiltration (UF) failure is commonly attributed to dysfunction of the peritoneal membrane, resulting in decreased ultrafiltration volume (UFV). Our objective was to evaluate whether fluid absorption and UF can be assessed by monitoring intraperitoneal fluid using segmental bioimpedance analysis (sBIA). METHODS: Twenty PD patients were studied during either a peritoneal equilibration test (PET; n = 7) or automated PD (APD; n = 13). Eight electrodes were positioned on the lower abdomen and connected to a bioimpedance device (Hydra 4200). A physical mo

The relationship between changes in peritoneal permeability with CA-125 and HIF-1α.

BACKGROUND: Peritoneal fibrosis (PF) is a major, persistent complication of prolonged peritoneal dialysis that eventually leads to peritoneal ultrafiltration failure and termination of peritoneal dialysis. Prolonged exposure to high glucose concentrations, degradation products, uremic toxins, and episodes of peritonitis can cause some changes in the peritoneal membrane, resulting in intraperitoneal inflammation and PF, leading to failure of ultrafiltration and dialysis. CA-125 can be used as a biomarker of peritoneal mesothelial cell count in the peritoneal dialysate and for monitoring cell co

Altered Glycolysis, Mitochondrial Biogenesis, Autophagy and Apoptosis in Peritoneal Endometriosis in Adolescents.

Energy metabolism plays a pivotal role in the pathogenesis of endometriosis. For the initial stages of the disease in adolescents, this aspect remains unexplored. The objective of this paper was to analyze the association of cellular and endosomal profiles of markers of glycolysis, mitochondrial biogenesis, apoptosis, autophagy and estrogen signaling in peritoneal endometriosis (PE) in adolescents. We included 60 girls aged 13-17 years in a case-control study: 45 with laparoscopically confirmed PE (main group) and 15 with paramesonephric cysts (comparison group). Samples of plasma and peritone

Peritoneal Metastasis of Osteosarcoma in 99m Tc-MDP SPECT/CT Imaging.

An 11-year-old boy with history of conventional high-grade osteosarcoma in the left distal femur was referred to our department for 99m Tc-MDP whole-body bone scan. In addition to multiple bone lesions and pleura, abnormal high radioactivity was found in the abdomen. SPECT/CT revealed diffuse peritoneal thickening with calcification and increased radioactive uptake, which suggest peritoneal metastases. The most common metastases of osteosarcoma can be found in bone and lungs, whereas peritoneal metastases are extremely rare.

Association of Protein Energy Wasting and Oxidative Stress Markers in Peritoneal Dialysis.

INTRODUCTION: Protein-energy wasting (PEW) is highly prevalent among patients undergoing peritoneal dialysis (PD), and it has been proposed that oxidative stress (OS) may contribute to its pathogenesis. This study was an attempt to determine the association between the presence of PEW and OS levels in PD patients. METHODS: This analytical cross-sectional study involved 62 clinically stable PD patients aged ≥ 18 years, between September 2017 and July 2018. PEW was assessed using PEW definition criteria, 7-point Subjective Global Assessment (SGA), and Malnutrition-Inflammation Score (MIS). Redo

Oxidation State in Peritoneal Dialysis in Patients with Type 2 Diabetes Mellitus.

End-stage renal disease (ESRD) progression is closely related to oxidative stress (OS). The study objective was to determine the oxidant and antioxidant status in peritoneal dialysis (PD) patients with type 2 diabetes mellitus (DM). An analytical cross-sectional study from the PD program was carried out with 62 patients, 22 with and 40 without DM. Lipoperoxides (LPO) levels in patients with DM, 3.74 ± 1.09 mM/L, and without DM, 3.87 ± 0.84 mM/L were found to increase compared to healthy controls (HC) 3.05 ± 0.58 mM/L (p = 0.006). The levels of the oxidative DNA damage marker (8-OH-dG) were fou

Recurrent malignant peritoneal mesothelioma treated by a second resection: A case report.

KEY CLINICAL MESSAGE: Malignant peritoneal mesothelioma, a rare and poor prognosis disease, is seldom treated surgically, especially for recurrence. However, early diagnosis and aggressive treatment of primary and recurrent tumors can achieve long-term patient survival. ABSTRACT: Malignant peritoneal mesothelioma (MPM) is a rare and aggressive tumor, and rarely indicated for surgery, especially for recurrence. In the present case, we report a rare case who could survive long-term after two surgeries in 4 years for MPM.

Gastric Extent of Tumor Predicts Peritoneal Metastasis in Siewert II Adenocarcinoma.

BACKGROUND: Whereas current guidelines recommend staging laparoscopy for most patients with potentially resectable gastric cancer, such a recommendation for patients with adenocarcinoma of the gastroesophageal junction (AEG) is lacking. This study sought to identify baseline clinicopathologic characteristics associated with peritoneal metastasis (PM) among patients with Siewert II AEG. METHODS: Trimodality therapy-eligible patients with Siewert II AEG (2000-2015, single institution) were retrospectively identified. A composite PM outcome was defined as follows: (1) PM at staging laparoscopy;
