The prognostic value of HFA-PEFF score in connective tissue disease-associated PAH: evidence from a cohort study.

BACKGROUND: Connective tissue disease-associated pulmonary arterial hypertension (CTD-PAH) patients with left heart dysfunction may have worse prognosis. This study was to investigate the prognostic value of HFA-PEFF score in connective tissue disease-associated PAH. METHODS: This single-center retrospective cohort study enrolled 147 CTD-PAH patients diagnosed via right heart catheterization (RHC), divided into two groups based on their HFA-PEFF scores: < 5 (n = 74) and ≥ 5 (n = 73). The clinical characteristics were compared between the two groups. The primary end point was all-cause mortali

Distinct role of minimal and maximal left atrial volumes in assessing LV filling pressures and prognosis in chronic heart failure.

Current guidelines recommend assessing left atrial (LA) maximal volume to grade left ventricular (LV) diastolic dysfunction. However, recent studies suggest a strong association between LA minimal volume and prognosis. The present study aims to investigate the relative merits of minimal and maximal LA volume in reflecting LV filling pressures and on prognosis in chronic heart failure (HF). We retrospectively analyzed 1135 chronic HF patients (75% men; mean age 66 ± 12 years) in sinus rhythm. LV end-diastolic pressure (EDP) ≥ 20 mmHg (estimated with echocardiography) was considered as increased

Ventricular remodelling after pulmonary valve replacement: comparison between pressure-loaded and volume-loaded right ventricles.

OBJECTIVES: In patients with severe pulmonary valve regurgitation or stenosis, pulmonary valve replacement (PVR) has a favourable effect on right ventricular (RV) volume and pressure unloading. PVR thereby decreases the progression of RV dilatation and/or hypertrophy. This study investigates RV remodelling patterns after PVR in patients with either pressure, volume or combined volume- and pressure-loaded RVs. METHODS: We evaluated 79 consecutive patients who had undergone PVR, between 1999 and 2012 beyond the age of 14 years. Comparisons were made according to the RV loading condition, i.e. i

Comparative study of the effect of pacing mode on the interaction of the right and left ventricles.

This study was designed to evaluate ventricular dyssynchrony from the viewpoint of the interaction of right and left ventricular contractions. Forty-three patients, 24 with sick sinus syndrome, 9 with complete atrioventricular block, and 10 with normal sinus rhythm were involved in this study. Microtip transducer catheters were advanced into both the left and right ventricles and ventricular pressure and the associated dp/dt were recorded simultaneously. Hemodynamic differences in various pacing modes were analyzed using pressure and dp/dt recordings obtained from the left and right ventricles

Impact of Area-Level Socioeconomic and Environmental Measures on Reduced Visual Acuity Among Children and Adolescents.

IMPORTANCE: Calcium-release deficiency syndrome (CRDS), which is caused by loss-of-function variants in cardiac ryanodine receptor 2 (RyR2), is an emerging cause of ventricular fibrillation. However, the lack of complex polymorphic/bidirectional ventricular tachyarrhythmias during exercise stress testing (EST) may distinguish it from catecholaminergic polymorphic ventricular tachycardia (CPVT). Recently, in the first clinical series describing the condition, mouse and human studies showed that the long-burst, long-pause, short-coupled ventricular extra stimulus (LBLPS) electrophysiology protoc

[Dangerous arrhythmias].

The usual frequent tachyarrhythmias well known to the specialists in internal medicine, such as ventricular tachycardia and ventricular fibrillation, are not discussed in this publication; nor are the bradycardias connected with the sick sinus syndrome or with atrioventricular block of higher degrees (with one exception). In the first section a stratification of the risk after myocardial infarction is presented including the therapeutic implications. Severely reduced left ventricular function is of most negative prognostic value. After the poor results of the CAST study, which revealed a three

Interactions between preischemic hypothermia and cardioplegic solutions in the neonatal lamb heart.

Hypothermia is believed to improve the tolerance to both ischemia and cardiopulmonary bypass and is commonly used during heart operations, particularly in the neonate. However, hypothermia also causes calcium to accumulate in the myocyte experimentally, and an increase in intracellular calcium during ischemia may worsen the effect of ischemia and impair the postischemic recovery of function. This effect of hypothermia on intracellular calcium has generally not been considered in experiments that attempt to optimize the composition of cardioplegic solutions. We have evaluated the impact of hypo

Selective chemical ablation of transient receptor potential vanilloid 1 expressing neurons in the left stellate ganglion protects against ischemia-induced ventricular arrhythmias in dogs.

OBJECTIVES: Findings from prior investigations show that left stellate ganglion (LSG) inhibitory approaches protect the heart from ventricular arrhythmias (VAs) caused by acute myocardial infarction (AMI), which still remain many side effects. Targeted transient receptor potential vanilloid 1/tyrosine hydroxylase (TRPV-1/TH) expressing sympathetic neurons ablation is a novel neuro-ablative strategy. The aim of this investigation was to explore if targeted molecular neuro-ablative strategy by resiniferatoxin (RTX) stellate microinjection could protect against ischemia-induced VAs. METHODS: Twe

Selectivity Filter Mutation in NaV1.5 Promotes Ventricular Tachycardia.

BACKGROUND: Loss-of-function mutations in the SCN5A gene, which encodes for the predominant cardiac NaV isoform, NaV1.5, result in either deficiency in the channel expression or function. Impaired NaV1.5 expression and function underlie reduced peak Na+ current (INa) and result in ventricular conduction velocity slowing, predisposing the heart to conduction block and ventricular arrhythmias clinically associated with Brugada syndrome (BrS). Recently, a missense mutation in NaV1.5 selectivity filter (DEKA motif), K1419E (DEEA), has been identified in patients with BrS. Despite early characteriz

Neonatal Primary Repair of Pulmonary Atresia With Ventricular Septal Defect Without Major Aortopulmonary Collateral Arteries: Midterm Outcomes.

OBJECTIVES: Pulmonary atresia with ventricular septal defect (PA-VSD) without major aortopulmonary collateral arteries (MAPCAs) is a rare congenital heart defect. Surgical management strategies include staged palliation or primary neonatal repair. We evaluated the midterm outcomes of neonatal primary repair in patients without MAPCAs. METHODS: A retrospective single-centre analysis was performed of 31 neonates who underwent primary repair for PA-VSD without MAPCAs between 2005 and 2024. Primary outcomes included mortality, reintervention rates, and right ventricular (RV) function. RESULTS: T

Rationale and Design of the Multicenter Trial on Japan Working Group on the Effects of Angiotensin Receptor Blockers Selection (Azilsartan vs. Candesartan) on Diastolic Function in the Patients Suffering from Heart Failure with Preserved Ejection Fraction: J-TASTE Trial.

BACKGROUND: Previous studies suggest that the pathophysiology of heart failure with preserved ejection fraction (HFpEF) is characterized not only by high ventricular stiffness, but also by vascular stiffness. Azilsartan has higher vascular affinity compared with other angiotensin II receptor blockers (ARBs), which were proven to have no beneficial effects on clinical outcomes in patients with HFpEF in earlier clinical trials. We aimed to test the hypothesis that azilsartan may improve left ventricular diastolic function in HFpEF patients with hypertension in this trial. METHODS: The Effects o

Global and regional myocardial function quantification by two-dimensional strain: application in hypertrophic cardiomyopathy.

OBJECTIVES: Recently, a novel method to measure strain from standard two-dimensional images has been developed. Our goal was to characterize global and regional systolic function abnormalities using this technique in patients with hypertrophic cardiomyopathy (HCM). BACKGROUND: Strain has been proposed as a sensitive tool to detect early systolic function abnormalities in HCM. However, the clinical application of conventional Doppler-derived strain has been limited by poor reproducibility and angle dependency. METHODS: Echocardiographic examinations were performed in 26 patients with nonobstr

Managing Harlequin Syndrome in VA-ECMO - do not forget the right ventricle.

Harlequin Syndrome (also known as North-South Syndrome) is a complication of veno-arterial extracorporeal membrane oxygenation (V-A ECMO) that can occur when left ventricular function starts to recover. While most commonly due to continued impaired gas exchange in the lungs, we present a case caused by right ventricular dysfunction, successfully managed by conversion of the ECMO circuit to a veno-veno-arterial (VV-A) configuration.

Successful Use of Intra-aortic Balloon Counterpulsation for Systemic Ventricular Failure Following Total Pericardiectomy for Calcific Chronic Constrictive Pericarditis.

We report two male patients aged 18 and 19 years, respectively, undergoing total pericardiectomy for chronic calcific constrictive pericarditis who developed systemic ventricular failure unresponsive to medical management following surgery. The failing circulation was successfully reestablished using intra-aortic balloon counterpulsation. Aortic counterpulsation facilitates recovery of ventricular function and appears to be a reasonable alternative in select instances of refractory cardiac failure following pericardiectomy.

Imaging the elusive anomalous origin of left coronary artery from pulmonary artery.

A 7-year-old girl with severe mitral regurgitation and pulmonary artery hypertension with good ventricular function was planned for mitral valve repair, but was subsequently diagnosed to have anomalous left coronary artery from pulmonary artery. We describe the pitfalls in the diagnosis of this rare anomaly, particularly in the presence of severe pulmonary artery hypertension and absence of circumstantial evidence of coronary steal and ventricular dysfunction.

Left anterior descending artery occlusion secondary to blunt chest trauma diagnosed by comprehensive echocardiography and coronary angiography.

Blunt chest trauma can cause serious complications, but coronary artery occlusion is rare. In a 19-year-old man, 40 days after a motorcycle accident, comprehensive echocardiography detected left anterior descending artery occlusion and left ventricular dysfunction, which was confirmed by coronary angiography and ventriculography. Echocardiography was also able to confirm restored left anterior descending artery flow and improved left ventricular function after coronary artery bypass grafting.

Ruptured papillary muscle after mitral valve replacement with preservation of chordae tendineae.

Many cardiac surgeons believe strongly that every effort should be made to preserve the continuity of the mitral anulus, chordae tendineae, and papillary muscles during mitral valve replacement in order to maximize ventricular function and maintain normal ventricular geometry. We treated a patient with spontaneous papillary muscle rupture after mitral valve replacement in whom efforts had been made to preserve continuity of the mitral mechanism.
