Liver transplantation in the United States.

Based on the data reported to the OPTN/UNOS Liver Transplant Registry between 1987-2005, we found: 1. The number of deceased-donor liver transplantations increased slowly each year, with most of the increase being in adult recipients. The number of LD transplants, on the other hand, decreased sharply after 2002, following 3 years of rapid increase from 1998-2001 in both pediatric and adult recipients. 2. The number of DD liver recipients with non-cholestatic liver diseases increased very quickly during the past 18 years. Malignant disease as a cause of end-stage liver disease increased after i

Insulin Resistance in Long COVID-19 Syndrome.

Background: The COVID-19 pandemic has caused severe health issues worldwide and contributed to huge financial losses. Key comorbidities linked to an increased risk of severe COVID-19 and higher mortality rates include cardio-metabolic disorders such as type 1 and type 2 diabetes mellitus (T1DM and T2DM), atherosclerotic cardiovascular disease, chronic kidney disease, hypertension, heart failure, and obesity. The persistence of symptoms even after the acute phase is over is termed long COVID-19 syndrome. This study aimed to evaluate the relationship between long COVID-19 syndrome and the develo

Effectiveness of 8-week Treatment with Glecaprevir/Pibrentasvir in Treatment-naïve or -experienced HCV Patients: Results from an Observational Retrospective Study in Real-life Settings (ODYSSEY).

BACKGROUND AND AIMS: Pan-genotypic ribavirin-free oral direct-acting antivirals, including the glecaprevir/pibrentasvir combination, are recommended for the treatment of most patients with chronic hepatitis C virus (HCV) infection. In Romania, the HCV-infected patient population receiving glecaprevir/pibrentasvir is not well characterized and data on treatment effectiveness is lacking. The ODYSSEY study aimed to provide insights into the characteristics and treatment outcomes of HCV-infected Romanian patients receiving 8-week therapy with glecaprevir/pibrentasvir. METHODS: This observational,

Association of Human Immunodeficiency Virus Infection and Risk of Peripheral Artery Disease.

BACKGROUND: The effect of human immunodeficiency virus (HIV) on the development of peripheral artery disease (PAD) remains unclear. We investigated whether HIV infection is associated with an increased risk of PAD after adjustment for traditional atherosclerotic risk factors in a large cohort of HIV-infected (HIV+) and demographically similar HIV-uninfected veterans. METHODS: We studied participants in the Veterans Aging Cohort Study from April 1, 2003 through December 31, 2014. We excluded participants with known prior PAD or prevalent cardiovascular disease (myocardial infarction, stroke, c

Tofacitinib as Induction and Maintenance Therapy for Ulcerative Colitis.

BACKGROUND: Tofacitinib, an oral, small-molecule Janus kinase inhibitor, was shown to have potential efficacy as induction therapy for ulcerative colitis in a phase 2 trial. We further evaluated the efficacy of tofacitinib as induction and maintenance therapy. METHODS: We conducted three phase 3, randomized, double-blind, placebo-controlled trials of tofacitinib therapy in adults with ulcerative colitis. In the OCTAVE Induction 1 and 2 trials, 598 and 541 patients, respectively, who had moderately to severely active ulcerative colitis despite previous conventional therapy or therapy with a tu

Long COVID associated with SARS-CoV-2 reinfection among children and adolescents in the omicron era (RECOVER-EHR): a retrospective cohort study.

BACKGROUND: Post-acute sequelae of SARS-CoV-2 infection (PASC) remain a major public health challenge. Although previous studies have focused on characterising PASC in children and adolescents after an initial infection, the risks of PASC after reinfection with the omicron variant remain unclear. We aimed to assess the risk of PASC diagnosis (U09.9) and symptoms and conditions potentially related to PASC in children and adolescents after a SARS-CoV-2 reinfection during the omicron period. METHODS: This retrospective cohort study used data from 40 children's hospitals and health institutions i

Exhaustion, immuno-inflammation, and pathogen burden after cardiac surgery: an exploratory study.

BACKGROUND: Exhaustion, a consequence of prolonged stress characterized by unusual fatigue, is associated with increased risk of cardiac morbidity and mortality. In patients recovering from coronary artery bypass (CABG), little is known about the relationship of 1) immune-mediated inflammation and resultant endothelial activation, and 2) cumulative exposure to infectious pathogens (pathogen burden (PB)) implicated in coronary atherosclerosis to exhaustion. AIM: The aim of this exploratory study was to investigate the association of PB, inflammatory markers (interleukin (IL)-6, IL-10) and a ma

Longitudinal retrospective cohort study to examine liver, cardiovascular and metabolic factors as predictors of all-cause mortality in a rural population in South-West Uganda.

INTRODUCTION: Markers of liver and metabolic disease have not been well described for many populations in Africa, but could be important to inform individual and public health interventions that reduce morbidity and mortality. METHODS: We studied longitudinal data from a population in rural Uganda, aiming (1) to describe trends in liver function tests and (2) to investigate how liver, metabolic and cardiovascular parameters are associated with all-cause mortality. Demographic and laboratory data were collected through the General Population Cohort in South-Western Uganda. We summarised cohort

Acute and chronic kidney injury following COVID-19 infection and vaccination: a narrative review.

This narrative review examines acute and chronic kidney injury following COVID-19 infection and vaccination, discussing the mechanism of SARS-CoV-2 entry into host cells through the ACE2 receptor - highly expressed in renal tissues - facilitating the viral invasion. Viral RNA has been detected in the urine of patients infected with SARS-CoV-2, suggesting direct renal involvement. The incidence of Acute Kidney Injuriy (AKI) among hospitalized COVID-19 patients was particularly higher in the early stages of the pandemic and largely varied by 29%-46%, depending on population studied and COVID-19

Japanese evidence on Janus kinase inhibitors for rheumatoid arthritis: a narrative review of risk-optimized use.

BACKGROUND: Janus kinase inhibitors (JAKi) have expanded treatment options for rheumatoid arthritis (RA) by providing rapid and effective oral therapy. However, their optimal use has become increasingly complex after the emergence of safety concerns involving serious infections, herpes zoster (HZ), major adverse cardiovascular events (MACE), venous thromboembolism (VTE), and malignancy. This issue is particularly relevant in Japan, where the RA population is older and has a higher prevalence of comorbidities. OBJECTIVE: To review the risk-optimized use of JAKi for RA based on Japanese evidenc

Lower Extremity Amputation in the Unhoused Population.

BACKGROUND: Unhoused patients are a group at high risk for lower extremity amputation because of higher rates of cardiovascular disease prevalence, higher rates of foot infection treatment failure, and poorer diabetes control. The aim of this study is to identify any disparities in preoperative optimization or postoperative outcomes following lower extremity amputation between housed and unhoused patients, with the intention of modifying patient management and enhancing long-term outcomes in this high-risk population. METHODS: This is a single institution, retrospective analysis of 1,304 pati

Surgical Challenges in Infective Endocarditis: State of the Art.

Infective endocarditis (IE) is still a life-threatening disease with frequent lethal outcomes despite the profound changes in its clinical, microbiological, imaging, and therapeutic profiles. Nowadays, the scenario for IE has changed since rheumatic fever has declined, but on the other hand, multiple aspects, such as elderly populations, cardiovascular device implantation procedures, and better use of multiple imaging modalities and multidisciplinary care, have increased, leading to escalations in diagnosis. Since the ESC and AHA Guidelines have been released, specific aspects of diagnostic an

Retrospective Analysis of the SARS-CoV-2 Infection Profile in COVID-19 Positive Patients in Vitoria da Conquista, Northeast Brazil.

Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) is responsible for causing Coronavirus Disease-2019 (COVID-19), a heterogeneous clinical condition that manifests varying symptom severity according to the demographic profile of the studied population. While many studies have focused on the spread of COVID-19 in large urban centers in Brazil, few have evaluated medium or small cities in the Northeast region. The aims of this study were: (i) to identify risk factors for mortality from SARS-CoV-2 infection, (ii) to evaluate the gene expression patterns of key immune response pathways
