Standardization of bone mineral density and microstructure from high-resolution CT-scans of the spine in a multicenter setting.

PURPOSE: Quantitative Computed Tomography (QCT) has not fully addressed the need to reduce intra- and inter-scanner variability for Osteoporosis and bone-related studies, which can lead to inaccuracies when pooling data from different CT manufacturers, models, devices, or protocols. In this context, the aim of this work was to develop ex vivo methods for the standardization of bone mineral density and microstructural parameters. MATERIALS AND METHODS: Six human vertebral body specimens embedded in poly-methyl methacrylate (PMMA) were scanned ex vivo inside an anthropomorphic abdomen phantom i

[Expert consensus on the diagnosis and treatment of pulmonary aspergillosis in patients with chronic obstructive pulmonary disease].

The prevalence of pulmonary aspergillosis is increasing in patients with chronic obstructive pulmonary disease (COPD) and can manifest in different forms such as invasive pulmonary aspergillosis (IPA), chronic pulmonary aspergillosis (CPA) and allergic bronchopulmonary aspergillosis (ABPA). With the variations of individual conditions such as immune status, these forms of the disease may transform into each other or even overlap. Moreover, the atypical clinical manifestations and the limited use of invasive sampling techniques have posed a challenge to the diagnosis and treatment of invasive p

Torasemide-induced Vascular Purpura in the Course of Eosinophilic Granulomatosis with Polyangiitis.

Torasemide is a loop diuretic with a molecule that is chemically similar to the sulphonamides described as eosinophilic granulomatosis with polyangiitis (EGPA) triggering drugs. The presented case is probably the first description of torasemide-induced vascular purpura in the course of EGPA. Any diagnosis of vasculitis should be followed by an identification of drugs that may aggravate the disease. A 74-year-old patient was admitted to the Department of Dermatology with purpura-like skin lesions on the upper, and lower extremities, including the buttocks. The lesions had appeared around the an

Necrotizing soft tissue infection following a peripheral bypass.

BACKGROUND: Necrotizing soft tissue infection (NSTI), formerly referred to as necrotizing fasciitis, is a rare but serious postoperative complication. NSTI following arterial bypass is seen only once in the literature (for a coronary artery bypass) and is not mentioned following peripheral bypass. Although surgical site infections have been studied extensively, there are limited published data on postoperative NSTI and no data for NSTI following peripheral arterial bypass. CASE PRESENTATION: Here we present the first, to our knowledge, reported instance of an NSTI following a lower extremity

Medical lessons learnt from the US and Canadian experience of treating combat casualties from Afghanistan and Iraq.

The Winston Churchill Memorial Trust, established in 1965, funds Travelling Fellowships and both authors visited hospitals in Germany, Canada and the USA regarded as centres of excellence with expertise in the early care, reconstruction and rehabilitation of the combat casualties of our NATO Allies, as recipients of these Fellowships. This article presents some of the lessons learnt in the field of musculoskeletal trauma and rehabilitation from the Canadian and US military medical systems. In trauma, there were significant differences in wound debridement policy, use of external fixators for f

Contrast-enhanced magnetic resonance angiography findings prior to hemodialysis vascular access creation: a prospective analysis.

PURPOSE: To determine prospectively the clinical value of contrast-enhanced magnetic resonance angiography (CE-MRA) for assessment of the arterial inflow and venous outflow prior to vascular access (VA) creation. METHODS: Seventy-three patients underwent duplex ultrasonography (DUS) and CE-MRA prior to VA creation for detection of stenoses and occlusions. Two observers read the CE-MRA images for determination of inter-observer agreement. A VA was considered functional if it could be used for successful two-needle hemodialysis therapy within 2 months after creation. RESULTS: CE-MRA detected 6

Sequential development of peripheral t-cell lymphoma post immunochemotherapy of diffuse large B cell lymphoma.

Reports of sequential occurrence of two or more types of lymphoma are rare, especially when they involve different cell lineages. Herein, we report a rare case of sequential development of peripheral t-cell lymphoma following treatment of diffuse large B cell lymphoma. In a 73-year-old Chinese male patient, diffuse large B-cell lymphoma (DLBCL) was diagnosed in September 2011 based on the result of a tongue biopsy. Afterwards, he received rituximab combined with chemotherapy and local radiotherapy. Though he achieved completed remission, he had a new symptom of one enlarged left inguinal lymph

Comparative electrothermal analysis of single and small-electrode grid configurations for HD-inspired peripheral transcutaneous neurostimulation.

BACKGROUND: High-density (HD)-inspired peripheral transcutaneous neurostimulation systems employing clusters of small-area electrode arrays may improve spatial controllability during electrical stimulation of superficial limb tissues, as used in TENS/NMES/FES-like wearable applications. However, the electrothermal consequences of small electrodes can affect usability due to complexities in safety considerations. Therefore it is of critical interest to understand the effects of these systems, and to determine how subdivision topology shapes localized current concentration and thermal accumulati

Artificial intelligence, the magic circle, and CBT: a Wizard-of-Oz framework for adjunctive treatment.

Cognitive behavioral therapy (CBT) is a cornerstone of evidence-based psychological treatment, but its implementation continues to be limited by poor patient adherence and shortages of clinical resources. At the same time, most applications of artificial intelligence (AI) and games in mental health have focused on direct intervention, although the evidence base remains underdeveloped and AI has important limitations in diagnosis and treatment decisions. In response, this article proposes a Wizard-of-Oz (WoZ) framework for adjunctive treatment that positions AI and board games as tools that sup

Reciprocal regulation of hMENA and TGF-β signaling in cancer-associated fibroblasts promotes EMT, immunosuppression, poor prognosis, and ICT resistance in NSCLC.

BACKGROUND: Cancer-associated fibroblasts (CAFs) significantly impact cancer progression and CAF subtypes are key determinants of response to immune checkpoint therapy (ICT). The transforming growth factor-β (TGF-β) signaling is a main pathway in protumorigenic activity of CAFs and resistance to ICT. The actin cytoskeleton regulator hMENA plays crucial roles in epithelial-mesenchymal transition (EMT) and regulates pathways critical to antitumor immune response, such as interferon-I and Axl-GAS6. METHODS: We constructed a single-cell atlas of CAFs using integrated public dataset. Experimental

Single-cell transcriptomic profiling reveals CD27+ cytotoxic T Cell heterogeneity and exhaustion dynamics in colorectal cancer tumor microenvironment.

BACKGROUND: CD27+ cytotoxic T cells play critical roles in anti-tumor immunity, yet their heterogeneity and functional states within the colorectal cancer (CRC) tumor microenvironment remain poorly characterized. Understanding the molecular mechanisms underlying T cell exhaustion is essential for developing effective immunotherapeutic strategies. METHODS: We performed comprehensive single-cell RNA sequencing analysis of CD27+ cytotoxic T cells from CRC tumor tissues and matched normal adjacent tissues using the GSE144735 dataset. Multiple dimensional reduction techniques, trajectory inference

Gravlax: an annotation-independent molecular evidence archive for single-cell RNA-seq.

A cell-by-gene count matrix is the artifact of a single-cell RNA-seq experiment that is most often stored, shared, and reanalyzed. It is the output of a computation whose inputs are the sequenced molecules and a gene annotation, and while the molecules never change, the annotation is revised continually. Once the matrix has been produced, the evidence behind it can no longer be reinterpreted. Recovering that evidence means returning to raw reads or alignments that are large, costly to process, and frequently unavailable. We ask whether a concise representation of the molecules themselves can b

Cognitive behavioural therapy for insomnia and epigenetic ageing: secondary analysis from a randomised controlled trial.

BACKGROUND: Insomnia in later life has been associated with accelerated biological ageing; however, little is known about the effects of insomnia treatment on it. In this study, we evaluated the effects of cognitive behavioural therapy for insomnia (CBT-I), compared with sleep education therapy (SET), an active comparator condition, on epigenetic indicators of biological ageing in older adults with insomnia. METHODS: This secondary analysis was conducted within a larger single-site, investigator-initiated randomised controlled trial (ClinicalTrials.govNCT01641263) of adults aged 60 years or o

Correlation of Ankle-Brachial Index With NT-proBNP and Left Ventricular Ejection Fraction in Patients With Type 2 Diabetes Mellitus and Heart Failure: A Cross-Sectional Study.

BACKGROUND AND OBJECTIVES: Coexistence of type 2 diabetes mellitus (T2DM) and heart failure increases the risk of morbidity and mortality. Ventricular dysfunction and lower limb ischemic changes are assessed with left ventricular ejection fraction (LVEF), NT-proBNP, and ankle-brachial index (ABI). We conducted this study to determine the correlation of ABI with NT-proBNP and LVEF. METHODS: We conducted this cross-sectional study from March 2024 to February 2026 at the Kalinga Institute of Medical Sciences (KIMS), Bhubaneswar, Odisha, India. Early Anterior Cruciate Ligament Reconstruction Mitigates the Development of Posttraumatic Osteoarthritis in a Murine Anterior Cruciate Ligament Rupture Model.

BACKGROUND: Up to 60% of patients develop posttraumatic osteoarthritis (PTOA) 10 to 25 years after anterior cruciate ligament reconstruction (ACLR). While some studies suggest that early ACLR may mitigate PTOA, other studies, by contrast, have found no difference in the timing of ACLR on the development of PTOA. Therefore, the optimal timing of ACLR for the mitigation of PTOA has yet to be fully elucidated in the literature. PURPOSE: To evaluate the effect of timing of ACLR on the development of PTOA using a murine noninvasive (closed) anterior cruciate ligament (ACL) rupture model, as well a
