Disability Prevalence and the Awareness-Utilization Gap for Disability Services in an Urban Community of Delhi, India: A Cross-Sectional Study.

INTRODUCTION: Disability-related welfare schemes form a critical component of India's social protection architecture, yet community-level evidence on the gap between awareness and utilization in urban settings remains limited. This study aimed to estimate the prevalence of disability and its socio-demographic correlates and to assess awareness and utilization of disability-related services among adults in an urban community of Delhi, generating baseline data on the awareness-utilization gap for this setting. METHOD: A community-based cross-sectional study was conducted in Palam, an urban rese

Decision Framework for Prioritizing Systematic Reviews Versus Primary/translational Research in Low- and Middle-income Countries.

Systematic reviews and meta-analyses (SR/MAs) are widely regarded as the highest level of evidence and play a crucial role in informing clinical practice, public health policy, and guideline development. However, the rapid proliferation of evidence syntheses has raised concerns regarding research redundancy, publication-driven incentives, and the potential diversion of resources away from primary and translational research, particularly in low- and middle-income countries (LMICs). In settings characterized by large and diverse populations, evolving health systems, and substantial unmet health

Responding to a warming planet: a scoping review of climate change literacy, perceived health impacts, and mental health outcomes among African students.

BACKGROUND: Climate change represents an urgent global health and environmental challenge, with youth in Africa, who account for over 60% of the continent's population, being disproportionately affected due to limited adaptive capacities, inadequate education, and fragile mental health systems. Climate change literacy, defined as the ability to obtain, process, and apply climate-related information, is crucial for fostering awareness, adaptive behaviours, and psychological resilience among students. Simultaneously, growing evidence points to the mental health consequences of climate change, in

Translational repression of 5' TOP mRNAs in chronically activated CD8+ T cells in vitro.

T cell exhaustion is a dysfunctional state that arises during chronic infections and cancer, characterized by impaired effector functions and sustained expression of inhibitory receptors. While transcriptional, epigenetic, and metabolic rewiring have been well documented in exhausted T cells, a comprehensive understanding of how translation is regulated in this state remains incomplete. To address this gap, we performed ribosome profiling and RNA sequencing on in vitro chronically activated human CD8+ T cells to globally assess translational control during a model of T cell exhaustion. Our ana

Kidney Transplants From Marginal Donors: From Brenner's Abstract to Double Kidney Transplantation in Humans.

"In 1993, based on findings from their experimental studies of kidney mass manipulation in a rat kidney transplant model, Barry Brenner and his colleagues postulated that augmenting the number of viable nephrons in kidney transplantation to meet the recipient's metabolic and excretory demands may protect against long-term graft loss. This enlightening idea set the stage for the development and worldwide implementation of dual kidney transplant programs using organs from marginal-including older-donors. This concept was then complemented by robust clinical evidence that showed that a scoring sy

Korean Society of Heart Failure Guidelines for the Palliative Care and Hospice for Heart Failure Patients.

Heart failure (HF) is a leading cause of mortality and morbidity in South Korea and imposes substantial physical, emotional, and economic burdens on patients, families, and society. Despite the high symptom burden and complex care needs of patients with HF, palliative care and hospice services remain underutilized owing to cultural, institutional, and knowledge-related barriers. This guideline from the Korean Society of Heart Failure (KSHF) provides evidence-based recommendations to integrate palliative and hospice care across the HF disease trajectory, with the aim of improving quality of lif

Preoperative fasting: when failure to measure perpetuates harm.

Preoperative liquid fasting durations remain unacceptably long worldwide and represent one of the most common yet under-recognised sources of avoidable perioperative harm. Recent international multidisciplinary consensus has emphasised the need for institutional implementation strategies that recommend, where appropriate, more liberal clear liquid protocols to minimise prolonged fasting. Meaningful improvement therefore requires active implementation through routine measurement and audit of actual fasting durations relative to society recommendations, institutional accountability, and quality

Deeper delving Nav1.8 inhibitor Suzetrigine in chronic pain.

Chronic pain is a complex disorder but has no satisfactory therapeutic strategies, though its mechanisms have been extensively studied, including sodium channels such as Nav1.7 and Nav1.8. The Nav1.8 channel inhibitor Suzetrigine was approved for acute pain treatment by the FDA in 2025, while its effects in chronic pain are still not clear and remain to be further studied, considering the more complex mechanism of chronic pain. Recent studies using Suzetrigine on neuronal excitability of human dorsal root ganglion partially support its effects on chronic pain, while some case reports demonstra

An Early Miocene ape from the biogeographic crossroads of African and Eurasian Hominoidea.

The Early Miocene fossil record documenting hominoid evolution has long been restricted primarily to sites in East Africa, whereas contemporaneous North African sites have only yielded remains of cercopithecoid monkeys. Here, we describe a fossil ape from North Africa, a new genus (Masripithecus) from the Early Miocene (~17 million to 18 million years) of northern Egypt, on the basis of mandibular remains. A combined molecular-morphological Bayesian tip-dating analysis positions Masripithecus closer to crown hominoids than coeval fossil apes from East Africa, thereby filling a phylogenetic and

Third-Party Perspective in Medical Research Ethics: A Case Series of Clinical Trials Involving Individuals with Sexual Interest in Children.

It is a central principle in modern research ethics that the rights, safety, and well-being of research participants are the most important considerations. This should also apply to third-party individuals, that is, individuals who are not research participants but may be affected by the research. Despite this, international research ethics guidelines give limited attention to protecting third parties. This gap became evident when we were involved in the design and conduct of a series of clinical trials involving individuals with sexual interest in children, where children may constitute an id

Congenital babesiosis in China: first molecularly confirmed case of vertical transmission of Babesia microti.

Congenital babesiosis is rarely reported globally. We report a 74-day-old male infant presented with fever, pallor, and severe, life-threatening haemolytic anaemia (haemoglobin: 45 g/L). The infant had 16% parasitemia with ring forms evident on peripheral blood smear. Babesia microti infection was confirmed in both the mother and infant by PCR and metagenomic next- generation sequencing. Genetic analysis revealed an identical strain in bot. Treatment with intravenous azithromycin and oral atovaquone/proguanil resulted in rapid clearance of parasitemia and resolution of anaemia. This first mole

Management of cystic echinococcosis in British Columbia: outcomes and suggestions to improve guideline adherence.

Cystic echinococcosis (CE) is rare in Canada but continues to appear sporadically, sometimes in patients who have not travelled to endemic regions. Since 2010, World Health Organization (WHO) guidelines have emphasized stage-specific management, yet the extent to which this approach is applied in Canadian practice is uncertain. We reviewed 41 cases of CE managed in British Columbia between 2010 and 2022; only about two-thirds received treatment aligned with WHO guidance. The primary gap was in radiology reporting, which frequently documented cyst size but omitted the staging details essential

The Role of Prefrontal and Posterior Parietal Cortex in Generating Multiple Step Saccades.

While multiple step saccades (MSS) are occasionally reported in the healthy population, they are more evident in patients with Parkinson's disease (PD). Therefore, MSS has been suggested as a biological marker for the diagnosis of PD. However, the lack of clarity on the neural mechanism underlying the generation of MSS largely impedes their application in the clinic. We have proposed recently that MSS are triggered by the discrepancy between desired and executed saccades. Accordingly, brain regions involved in saccadic planning and execution might play a role in the generation of MSS. To test

Should In Vitro Fertilization Policies Use the Body Mass Index?

Current guidance from the American Society for Reproductive Medicine (ASRM) and the American College of Obstetricians and Gynecologists (ACOG) indicates that body mass index (BMI) ought not be used alone for in vitro fertilization (IVF) exclusionary policies, but the reasons for using BMI alongside other criteria are less clear. This article aims to fill a gap in the literature on this point and follow the findings to a logical conclusion regarding the role of BMI in IVF policies of the future. The article discusses why BMI might be useful in IVF policies, explores evidence that other metrics

New perspectives on polycystic ovary syndrome: hypothalamic-sympathetic-adipose tissue interaction.

Polycystic Ovary Syndrome (PCOS) is a prevalent endocrine and metabolic disorder among women of reproductive age. Although the traditional mechanistic framework has focused on dysregulation of the hypothalamic-pituitary-ovarian axis (HPOA), emerging evidence suggests that PCOS-associated metabolic disturbances predominantly originate from dysregulation of the hypothalamic-sympathetic nerve-adipose tissue axis (HSF axis). This article systematically reviews the mechanisms by which hypothalamic nuclei (including the arcuate nucleus and paraventricular nucleus) regulate adipose tissue metabolism

Exercise prescriptions for ischemic cardiomyopathy: a scoping review.

This review explores the critical role of exercise as a non-pharmacological intervention in managing ischemic cardiomyopathy (ICM), a leading cause of heart failure. It highlights the profound cardiovascular benefits of exercise, such as improved cardiopulmonary parameters, decreased morbidity and mortality, and enhanced functional capacity. It also critically evaluates existing literature on the efficacy of various exercise types and intensities, including aerobic, resistance, and high-intensity interval training. There is a significant gap in current clinical guidelines, which lack specific

Rabenosyn-5 suppresses non-small cell lung cancer metastasis via inhibiting CDC42 activity.

Metastasis, the primary cause of death in lung cancer patients, is facilitated by cytoskeleton remodeling, which plays a crucial role in cancer cell migration and invasion. However, the precise regulatory mechanisms of intracellular trafficking proteins involved in cytoskeleton remodeling remain unclear. In this study, we have identified Rabenosyn-5 (Rbsn) as an inhibitor of filopodia formation and lung cancer metastasis. Mechanistically, Rbsn interacts with CDC42 and functions as a GTPase activating protein (GAP), thereby inhibiting CDC42 activity and subsequent filopodia formation. Furthermo

Road map to small bowel endoscopy quality indicators.

PURPOSE OF REVIEW: Quality indicators for upper and lower gastrointestinal endoscopy are well established and linked to patient outcomes. However, there is a perceived gap in the development and implementation of quality indicators for small bowel endoscopy. In this review, we aimed to discuss the development of quality indicators in small bowel endoscopy and their implementation in clinical practice. RECENT FINDINGS: The proposed quality indicators for small bowel endoscopy focus on process measures, which mainly evaluate the procedural aspects, rather than the outcomes or the overall patien

Enhancing Pre-Treatment Motivation Improves Forensic Mental Health Outcomes.

Despite evidence that psychological treatments benefit from pre-treatment intervention, there remains no published research on the value of including a pre-treatment intervention in forensic mental health settings. The present study aimed to address this gap by examining the effects of adding a brief motivational preparatory program (MPP) to standard forensic psychiatric care. The MPP was based on hope theory and motivational interviewing within a cognitive-behavioral therapy approach. MPP participants and a waitlist control group completed a battery of self-report measures of hope and motivat

The Efficacy of Cannabis in Oncology Patient Care and Its Anti-Tumor Effects.

As the legalization of medical cannabis expands across several countries, interest in its potential advantages among cancer patients and caregivers is burgeoning. However, patients seeking to integrate cannabis into their treatment often encounter frustration when their oncologists lack adequate information to offer guidance. This knowledge gap is exacerbated by the scarcity of published literature on the benefits of medical cannabis, leaving oncologists reliant on evidence-based data disheartened. This comprehensive narrative article, tailored for both clinicians and patients, endeavors to br
