The role of depressive symptoms in recovery from injuries to the extremities in older persons. A prospective study.

BACKGROUND: Previous research suggested that depressive symptoms play a role in recovery after hip fracture. However none of these studies were prospective and included only patients with hip fractures. OBJECTIVE: To examine the effect of depressive symptoms on the recovery of (instrumental) activities of daily living after fall-related injuries to the extremities in older persons. DESIGN: Prospective cohort study. METHODS: Data were collected from 168 older persons at baseline, prior to their injuries (hip fractures, other fractures or contortions and dislocations), and 8 weeks, 5 months a

[Fractures of the calcaneus].

The authors present the results of another investigation of a group of casualties with fractures of the calcaneum treated by open reposition and stabilization with a splint and screws in 1994 to 1996, followed up after an interval of 57-74 months after the injury, on average after 66 months. Sixteen patients (20 calcanea) could be followed up, 7 patients were lost (8 calcanea). Again two evaluation patterns were compared. According to the authors the results were evaluated 12x as excellent, 6x as good and 2x as poor. On subjective evaluation 13 patients were satisfied, another three satisfied

Transoral miniplate osteosynthesis of condylar neck fractures.

OBJECTIVE: In a retrospective study, we reviewed a series of 55 consecutive patients with 57 fractures of the condylar neck that were treated with transoral miniplate osteosynthesis. STUDY DESIGN: Forty-one patients were included in a clinical follow-up study; the median length of study was 26.5 months (minimum, 7 months; maximum, 79 months). In a radiographic study, the positions of the condyle before open reduction, after open reduction, and more than 6 months postoperatively were evaluated in 3 radiographic planes. A statistical analysis was performed to determine factors that lead to seco

[The Gamma Nail in per- and intertrochanteric femoral fractures--alternative or supplement to the dynamic hip screw? A prospective randomized study of 100 patients with per- and intertrochanteric femoral fractures in the surgical clinic of the City Hospital of Triemli, Zurich, September 1989 - June 1990].

In a prospective randomised trial between September 89 and June 90 one hundred patients with per- and subtrochanteric fractures were consecutively treated by Gamma-nail or DHS. The average age of both groups was about 80 years. The operation time for Gamma-nailing was longer than for DHS-implantation and also the postoperative blood loss was higher in the Gamma-nail-group. We found no difference of intraoperative blood loss, of perioperative letality and in duration of hospital care. 90% of Gamma-nail-patients and 80% of DHS-patients were successfully able to walk four days after operation wit

Proximal humerus fractures: A review of current practice.

The management of proximal humeral fractures (PHF) remains controversial. Its incidence is increasing. Patients should be meticulously assessed clinically for co-morbidities and neuro-vascular injuries. Radiological investigation helps provide information on the fracture configuration and dislocations. Enhanced by 3-dimensional CT scanning, these further help in decision making and operative planning. PHF classifications have been demonstrated to have poor intra-observer and inter-observer reliability. Research has identified some radiographic predictive factors for humeral head ischaemia and

Lag screw osteosynthesis of simple olecranon fractures: A biomechanical comparative study.

Olecranon fractures are most frequently stabilized by tension band wiring (TBW), which unfortunately leads to relevant implant removal rates due to K-wire migration and soft tissue irritation. As lag screw osteosynthesis (LSO) might be a gentle and effective alternative in simple fracture patterns, the goal of the present study was to biomechanically compare LSO with TBW in simple olecranon fractures at a cadaver model. A simple olecranon fracture (Mayo type IIA) was created in eight pairs of human cadaver elbows, which were pairwise fixed by either TBW or two transcortical 4.0 mm lag screws.

Comparison of different pedicle screw fixation schemes in the treatment of neurosurgical spinal fractures: systematic review and meta-analysis.

BACKGROUND: Thoracolumbar fractures have the characteristics of acute onset, rapid change, and severe trauma. The best way to treat thoracolumbar fractures is through fracture reduction surgery. The surgical methods include percutaneous pedicle screw, posterior percutaneous pedicle screw internal fixation, and open pedicle screw internal fixation. METHODS: We searched the PubMed, Embase, and Medline English database from April 1991 to April 2021, and the keywords included "percutaneous tablet screen", "posterior percutaneous tablet screen fixation", "open tablet screen fixation", "fracture of

An irreducible variant of femoral neck fracture: a minimally traumatic reduction technique.

We present 25 cases of irreducible variant femoral neck fractures that require surgical management after routine manipulative manoeuvre attempts have failed. In our study, an irreducible variant of femoral neck fractures is defined as a reduction that cannot be achieved after multiple attempts at closed reduction. This was evident radiographically, as seen in displaced–impacted femoral neck fractures when the proximal femur compacts and rotates along with the distal part, and anatomical reduction cannot be achieved with manipulative manoeuvres. Another rare situation also included is when the

Analysis of the Fracture Resistance of Buildings on Composite Foundations with Horizontal Reinforcement Crossing Normal Faults.

To investigate the performance of horizontally reinforced composite foundations in resisting surface rupture of normal faults, this study designed and conducted a series of physical model tests. A systematic comparative analysis was performed on the fracture resistance of sites with three-layer sand, five-layer sand, and three-layer clay geogrid horizontally reinforced composite foundations under 70° normal fault dislocation. The results indicate that significant changes in earth pressure serve as a precursor indicator of fault rupture, and their evolution process reveals the internal energy a

Stemless vs. stemmed reverse total shoulder arthroplasty as primary treatment in the elderly: A meta-analysis.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is widely used for cuff-deficient and degenerative shoulder conditions in older adults. While stemmed implants offer long-term success, stemless designs have gained attention due to bone preservation and ease of revision, but evidence remains limited. METHODS: A systematic search was performed in PubMed, Scopus, Cochrane Library, and Google Scholar till June 2025. Five studies met the inclusion criteria. Key outcomes included satisfaction, Visual Analog Scale for pain, functional scores, shoulder range of motion (ROM), fracture-related co

Motor vehicle - pedestrian occupational accident: A case report study.

A rare case of serious non-fatal pedestrian-motor vehicle accident in the constructions sector is described, leading to severe orthopedic trauma and disability. A 49-year-old male worker was admitted to the orthopedic department due to reported occupational injury. The accident occurred at the workplace during the construction of a cycling road, where the victim as pedestrian was referred to be crashed by a motor vehicle at his left lower extremity. He consequently fell on the ground and the vehicle drifted his whole limb with its caterpillar. The patient bore occupational trauma with extensiv

Unplanned 30-day readmissions in orthopaedic trauma.

INTRODUCTION: 30-day readmission is increasingly used as a hospital quality metric. The objective of this study was to describe the patient factors associated with unplanned 30-day hospital readmission of orthopaedic trauma patients. METHODS: A statewide observational study was undertaken using data from all acute hospitals in California. All hospital inpatients with a primary diagnosis of fracture or dislocation (ICD-9-CM codes 800-829) were included, except for those with isolated injuries to the skull, face, or ribs. The primary outcome measure was unplanned 30-day readmission to any hospi

A new method of transoral endoscopic-assisted treatment for mandibular subcondylar fractures using a template-guided patient-specific osteosynthesis implant: the Ulm protocol.

Intraoral subcondylar fracture repair still remains one of the most controversial and discussed topics in the field of maxillofacial trauma. Despite transoral endoscopic-assisted reduction and internal fixation has attracted wide attention, the surgical approach still is technically challenging and specialised instruments are mandatory. Trauma surgeons need to go through a shallow learning curve in order to achieve suitable and predictable surgical results comparable to the standard extraoral approaches. In our clinic the transoral endoscopic-assisted approach without the use of transbuccal tr

Low bone density and post-operative outcomes following revision total shoulder arthroplasty.

BACKGROUND: Low bone density (LBD) may increase complication risk following revision total shoulder arthroplasty (rTSA), but the magnitude and time course of this risk in the revision setting have not been well characterized, with few studies reporting short-term or midterm outcomes. This study compared 90-day and 2-year outcomes after rTSA in patients with and without a documented diagnosis of LBD. METHODS: We performed a retrospective cohort study including patients from January 1, 2005, through February 1, 2024. LBD was defined by a documented diagnosis of osteoporosis or osteopenia (Inter

Transverse coronoid fracture: when does it have to be fixed?

BACKGROUND: After elbow fracture-dislocation, surgeons confront numerous treatment options in pursuing a stable joint for early motion. The relative contributions of the radial head and coronoid, in combination, to elbow stability have not been defined fully. QUESTIONS/PURPOSES: The purpose of this study was to evaluate the effect of an approximately 50% transverse coronoid fracture and fixation in the setting of an intact or resected radial head on coronal (varus/valgus) and axial (internal and external rotational) laxity in (1) gravity varus stress; and (2) gravity valgus stress models.
