[Clinical practice guidelines "Congenital hyperinsulinism"].

Congenital hyperinsulinism (CHI) is a rare, genetically heterogeneous disorder characterized by inappropriate hypersecre tion of insulin by pancreatic beta cells and is the most common cause of persistent hypoglycemia in young children. The in cidence of CHI in the general population ranges from 1:30,000 to 1:50,000 live births. Timely diagnosis and treatment of CHI are critical, as persistent hypoglycemia can lead to irreversible damage to the central nervous system. This review summa rizes current understanding of the etiology and pathogenesis of CHI and presents an up-to-date classification

Clinical guidance and expert opinion on the use of prostatic urethral lift implants for benign prostatic hyperplasia.

INTRODUCTION: Minimally invasive surgical therapies (MISTs) for benign prostatic hyperplasia (BPH) are increasingly favored for their efficacy in symptom relief and reduced morbidity. Prostatic urethral lift has gained traction in the last decade due to its non-ablative properties and favorable clinical outcomes. AREAS COVERED: In this article, we provide a narrative review including articles prior to July 2026 focusing on prostatic urethral lift technology. We provide a brief technical overview of the implant and implantation process, outlining key differences in the first- and second-genera

The kidney in the middle: Kidney Disease: Improving Global Outcomes (KDIGO) guidelines address multiple key components of cardiovascular-kidney-metabolic syndrome.

Chronic kidney disease (CKD) is closely intertwined with obesity, diabetes, hypertension, dyslipidemia, and cardiovascular disease. In 2023, the American Heart Association formally recognized these interconnections as a unified entity, the cardiovascular-kidney-metabolic (CKM) syndrome. The CKM syndrome brings renewed attention to the importance of CKD in cardiovascular disease and reinforces the need for effective, evidence-based, interdisciplinary approaches to diagnose and prevent its intersecting components. The Kidney Disease: Improving Global Outcomes (KDIGO) organization has long led ef

[Current treatment, foreseeable newcomer and new treatment approaches for Sjögren's disease].

Although Sjögren's disease (SjD) is the most frequent connective tissue disease, there are still no approved medications for systemic treatment. Many disease-modifying antirheumatic drugs (DMARD) and biologicals have not shown sufficient efficacy in studies. The reasons for this are the complex immunological mechanisms and a high degree of clinical and immunological heterogeneity of the patients; however, significant progress has been made in recent years: for the first time 2 phase 3 studies (ianalumab and telitacicept) reported positive results. The current treatment recommendations are base

A Korean Clinical Practice Framework for Thyroid Eye Disease: Integrating International Recommendations with Survey-Informed Consensus.

Thyroid eye disease (TED), the most common extrathyroidal manifestation of Graves' disease, requires individualized management that integrates medical, radiotherapeutic, and surgical interventions. In this article, we present a society-endorsed, survey-informed Korean clinical practice framework, including consensus elements, for TED management. This framework integrates the 2022 American Thyroid Association/European Thyroid Association consensus, the 2021 European Group on Graves' Orbitopathy (EUGOGO) guideline, and contemporary Korean specialist practice patterns. A 2025 nationwide survey of

2025 ESC/EACTS valvular heart disease guidelines: key updates and surgical implications for cardiac surgeons.

PURPOSE OF REVIEW: The 2025 ESC/EACTS Guidelines for valvular heart disease introduce substantial changes in timing and modality of intervention, making a focused appraisal of their surgical implications highly relevant for contemporary cardiac practice. RECENT FINDINGS: The guidelines formalize imaging-based staging of cardiac damage, lower thresholds for intervention in aortic stenosis and primary mitral regurgitation, and refine age- and risk-stratified allocation of surgery vs. transcatheter therapies across aortic, mitral, and tricuspid disease. They expand indications for transcatheter

Modernizing antibiotic regimens for obstetric infections through antimicrobial stewardship.

Maternal sepsis remains a major and largely preventable contributor to maternal morbidity and mortality, with intra-amniotic infection and postpartum endometritis remaining common obstetric sources of sepsis. Intra-amniotic infection complicates approximately 2%-5% of term deliveries, and postpartum endometritis occurs in up to 7% of births. Both conditions are typically polymicrobial, arising from ascending infection involving gram-positive, gram-negative, and anaerobic organisms. Despite rising antimicrobial resistance, the recommended antibiotic regimens for obstetric infections have remain

[Ten questions about cardiac sarcoidosis].

Cardiac sarcoidosis is a rare but potentially life-threatening condition characterized by the formation of non-caseating granulomas in the myocardium. Clinical manifestations range from asymptomatic forms to atrioventricular blocks, ventricular arrhythmias, heart failure, and sudden cardiac death. Diagnostic work-up requires a multimodality approach combining advanced imaging, clinical criteria, and, when possible, histological confirmation. Immunosuppressive therapy remains the cornerstone of treatment, aimed at suppressing myocardial inflammation and preventing irreversible damage. Risk stra

Maritime Applications of Prolonged Casualty Care Training Scenario: Burn Injury on a Destroyer During Distributed Maritime Operations.

As the U.S. Navy further develops the concept of distributed maritime operations (DMOs), where individual components of the naval force will be more geographically dispersed, smaller vessels may be operating at a significant time and distance from more advanced medical capabilities. Therefore, Role 1 maritime caregivers will need to manage injured and disease non-battle injury patients for prolonged periods during current and future contested DMOs. We developed a hypothetical burn injury patient scenario to present an innovative approach to teaching complex operational medicine concepts includ

The ACR Medical Image Quality Assessment System (MIQAS): A Unified Approach to Image Quality Assessment in Radiology.

Image quality is central to the accurate interpretation of medical imaging, yet it remains inconsistently defined and assessed across clinical practice. To address this, the ACR has developed the Medical Image Quality Assessment System (MIQAS), a standardized, descriptive framework that characterizes image quality based on its alignment with relevant clinical task requirements. This framework will serve as the image quality assessment standard for all relevant ACR programs, including the ACR Accreditation Program, the Reporting and Data Systems programs, Practice Parameters and Technical Stand

Intravenous Fluids in Hospitalized Patients: A Review of Best Practices.

Judicious use of intravenous fluids in hospitalized patients can improve outcomes and prevent adverse events. For many clinical scenarios, balanced crystalloids have some advantages over normal saline, though the differences in outcomes are small. Excessive administration of normal saline can cause metabolic acidosis and kidney injury. For conditions with sodium and/or chloride deficits, normal saline is preferred. Hypertonic saline is used to rapidly correct severe acute hyponatremia, and hypotonic fluids are indicated for replacement of free water deficits. Intravenous albumin is indicated i
