The association between lack of circumcision and HIV, HSV-2, and other sexually transmitted infections among men who have sex with men.

BACKGROUND: Observational studies evaluating the association of circumcision and HIV infection among men who have sex with men (MSM) have yielded mixed results. We examined the relationship between circumcision and HIV, herpes simplex virus type-2 (HSV-2), syphilis, urethral gonorrhea, and urethral chlamydia among MSM stratified by anal sexual role. METHODS: Between October 2001 and May 2006, 4749 MSM who reported anal intercourse in the previous 12 months attended the Public Health-Seattle and King County STD clinic for 8337 evaluations. Clinicians determined circumcision status by examinati

Investigation of patients treated by an HIV-infected cardiothoracic surgeon--Israel, 2007.

Transmission of human immunodeficiency virus (HIV) from an infected health-care worker to patients is rare, with the greatest potential for occurrence during exposure-prone, invasive surgical procedures in which the blood of the health-care worker might come into contact with patients' blood or mucous membranes. When a surgeon is discovered to have HIV infection, a decision must be made about notification of patients, but only limited data are available to guide decision-making. Such notifications generally are decided upon on a case-by-case basis, taking into account such factors as the natur

Human papillomavirus genotypes associated with cervical cytologic abnormalities and HIV infection in Ugandan women.

Human papillomavirus (HPV) infection is associated with almost all cases of cervical cancer, and cervical cancer is a common malignancy in women living in developing countries. A cross-sectional study was conducted to determine the prevalence of HPV infection, human immunodeficiency virus (HIV) infection, and cervical cytologic abnormalities in women presenting to a sexually transmitted infections clinic in Kampala, Uganda. In June and July, 2002, 135 women underwent complete physical exams including Papanicolaou (Pap) smears. HIV status was evaluated by serology. Cervical and vaginal swabs we

Selective induction of cell-mediated immunity and protection of rhesus macaques from chronic SHIV(KU2) infection by prophylactic vaccination with a conserved HIV-1 envelope peptide-cocktail.

Infection of Indian-origin rhesus macaques by the simian human immunodeficiency virus (SHIV) is considered to be a suitable preclinical model for directly testing efficacy of vaccine candidates based on the HIV-1 envelope. We used this model for prophylactic vaccination with a peptide-cocktail comprised of highly conserved HIV-1 envelope sequences immunogenic/antigenic in macaques and humans. Separate groups of macaques were immunized with the peptide-cocktail by intravenous and subcutaneous routes using autologous dendritic cells (DC) and Freund's adjuvant, respectively. The vaccine elicited

Human immunodeficiency virus type 1 clade B superinfection: evidence for differential immune containment of distinct clade B strains.

Sequential infection with different strains of human immunodeficiency virus type 1 (HIV-1) is a rarely identified phenomenon with important implications for immunopathogenesis and vaccine development. Here, we identify an individual whose good initial control of viremia was lost in association with reduced containment of a superinfecting strain. Subject 2030 presented with acute symptoms of HIV-1 infection with high viremia and an incomplete seroconversion as shown by Western blotting. A low set point of viremia (approximately 1,000 HIV-1 copies/ml) was initially established without drug thera

Serum concentrations of antimycobacterial drugs in patients with pulmonary tuberculosis in Botswana.

BACKGROUND: We conducted a pharmacokinetic study of antimycobacterial drugs involving a cohort of patients with pulmonary tuberculosis (TB) in Gaborone, Botswana, to assess the prevalence of and risk factors for low drug concentrations in serum. METHODS: Adults participated if they had a history of cough > or =2 weeks, had abnormal chest radiograph findings, consented to testing for human immunodeficiency virus (HIV), had sputum cultures positive for Mycobacterium tuberculosis, and were receiving antituberculous therapy for >7 days. Observed maximum serum concentrations were compared with pub

Prevalence and detection of HIV risk behavior in primary care: implications for clinical preventive services.

Despite their potential to reduce the incidence of HIV infection through primary prevention, family physicians report low levels of routine identification of patients at risk and counseling. This may reflect perceptions that few of their patients are at risk, that patients at risk will self-disclose during consultations, or that a physician-initiated approach is unacceptable to patients presenting for non-HIV-related problems. Our aim was to determine the prevalence of risk factors for HIV infection and HIV testing among patients in general practice, the acceptability to patients of opportunis

Zidovudine update: 1990.

Zidovudine (ZDV) is the only approved antiviral for the treatment of human immunodeficiency virus infection (HIV) in the U.S. Although newer antivirals have reached Phase II testing, ZDV is now the accepted therapy against which all other agents will be compared. Zidovudine 1500 mg/d was previously prescribed only to adult HIV-infected patients who had developed AIDS or AIDS-related complex (ARC). However, results obtained from recently completed studies indicate that a lower daily dose (500 mg) appears to be equivalent. In addition, ZDV therapy appears to be beneficial to asymptomatic HIV-inf

Immunopathogenesis of pediatric acquired immunodeficiency syndrome.

Pediatric acquired immunodeficiency syndrome (PAIDS) results from infection with human immunodeficiency virus (HIV). The majority of infants with AIDS are infected in utero. In most instances, mothers are infected from intravenous drug abuse or bisexual partners. Infected individuals, both mothers and infants, may be asymptomatic for prolonged periods of time. Initially viral replication may be limited and the virus integrated into immune cells. Activation of the virus may result from several factors including infection with other viral agents, such as cytomegalovirus and Epstein-Barr virus. V

CMV and EBV Co-Infection in HIV-Infected Children: Infection Rates and Analysis of Differential Expression of Cytokines in HIV Mono- and HIV-CMV-EBV Co-Infected Groups.

(1) Background: CMV and EBV co-infections can affect the HIV disease progression by modulating the immune system. The disease dynamics can differ in HIV-positive adults and children. In Pakistan, HIV is rapidly expanding, especially in children; however, the prevalence of CMV and EBV co-infection and the effect on immune modulation in HIV-positive children are not known. This study aimed to bridge this gap by estimating the rate of active CMV and EBV co-infection in HIV-positive children, followed by the analysis of differential expression of cytokines in HIV mono- and HIV/CMV/EBV co-infected

Increasing Syphilis Prevalence Among MSM Across India Despite Improvements in the HIV Care Continuum.

INTRODUCTION: Syphilis rates have increased in many regions, especially in men who have sex with men (MSM). Yet data are sparse from low- and middle-income countries. We characterized changes in syphilis prevalence across serial respondent-driven sampling surveys in 10 Indian cities between 2012 and 2024. METHODS: We used respondent-driven sampling of MSM in 2012-2013 (10 cities), 2016-2017 (same 10 cities), and 2023-2024 (3 of 10 cities). Participants were ≥18 years, male, and reported sex with a man in the prior 12 months. Participants completed a survey, had a rapid HIV antibody test, with

Clinical characteristics and diagnostic challenges of otosyphilis in patients with HIV coinfection: a retrospective analysis.

BACKGROUND: The global incidence of syphilis has been rising, and atypical clinical presentations are increasingly recognized. Human immunodeficiency virus (HIV)-induced immune changes can alter syphilis progression and complicate diagnosis. Otosyphilis is a rare but potentially reversible cause of sensorineural hearing loss (SNHL). This study investigates otologic involvement in patients co-infected with syphilis and HIV. METHODS: Medical records of patients diagnosed with both syphilis and HIV infection at Etlik City Hospital between January 2023 and June 2025 were retrospectively reviewed.

Substance Use and HIV Risk in a Syndemic Context: Vulnerabilities Among African Refugee Sex Workers in Italy.

Substance use is a recognized driver of HIV vulnerability among sex workers, yet little is known about how behavioral patterns intersect with structural exclusion for African refugee male sex workers (RMSWs) in Europe. Guided by syndemic theory, we conducted an explanatory mixed-methods study in partnership with a community-based organization in Northern Italy. The qualitative strand included 20 in-depth interviews and two focus group discussions exploring experiences of substance use, stigma, and healthcare engagement. The quantitative strand comprised a structured survey of 150 RMSWs assessi

Late diagnosis of human immunodeficiency virus (HIV) and its associated factors in Asian countries: A systematic review.

BACKGROUND: Despite widespread public health interventions and research on Human Immunodeficiency Virus (HIV), late diagnosis and presentation into care constitute serious challenges to effective prevention and control of the disease. Asia, the continent with the highest human population, accommodates a significant population of people living with HIV (PLWH), highlighting the need for effective prevention and control strategies. OBJECTIVES: This review aims to systematically analyze the prevalence and the trend of late HIV diagnosis, and the associated risk factors in the Asian region. METHO

HIV/AIDS Mortality Trends in Peru: A Natural Experiment of COVID-19's Disruption and Health Disparities (2017-2024).

Peru's already concentrated HIV epidemic confronted an unprecedented shock with the arrival of SARS-CoV-2. With a natural experiment design, leveraging 96 months (2017-2024) of aggregate national death-certificate data (n = 9,917), we applied join-point regression, attributable-fraction calculations of deaths excess, and Newey-West-corrected interrupted time series models to disentangle indirect service disruptions from direct viral coinfection on HIV/AIDS mortality. Using population-level data, we estimated both relative and absolute measures of disparity, providing novel insights into popula

Cervical Cancer in Women With HIV: A Call to Action for Equitable Prevention in Low- and Middle-Income Countries.

Cervical cancer is preventable; however, it remains the leading cause of death in low- and middle-income countries (LMICs). Women with HIV (WWHs) have a sixfold higher risk of developing and dying from cervical cancer than women without HIV. Cervical cancer can be prevented by vaccination against high-risk human papillomaviruses (hrHPVs) and by screening for and treating precancer cervical lesions. While these preventive measures are routinely available to WWHs in developed countries, they are lacking in most LMICs, where the burden of HIV and cervical cancer is the highest. To prevent cervica

Machine Learning-based Prediction of Active Tuberculosis in People With HIV Using Clinical Data.

BACKGROUND: Coinfections of Mycobacterium tuberculosis (MTB) and human immunodeficiency virus (HIV) impose a substantial global health burden. Patients with MTB infection face a heightened risk of progression to incident active TB, which preventive therapy can mitigate. Current testing methods often fail to identify individuals who subsequently develop incident active TB. METHODS: We developed random forest models to predict incident active TB using patients' medical data at HIV-1 diagnosis. Training our model involved using clinical data routinely collected at enrollment from the Swiss HIV C

Comparative study of the prevalence of HIV using the third- and fourth-generation HIV ELISA in blood donors in South India.

INTRODUCTION: Different generations of ELISA have evolved over the years. With the introduction of the fourth-generation ELISA, there is a claim of higher sensitivity over the third-generation HIV ELISA kits. However, few literatures suggest false positive issues. The present study aimed to assess the seroprevalence of HIV and to see the level of agreement between the third- and fourth-generation ELISA for HIV detection. METHODOLOGY: Prospective cross-sectional study conducted in a tertiary care hospital from November 2021 to May 2023. All positive samples by either the third- or fourth-gener
