Nutritional factors in cataract.

Age-related cataract is a condition characterized by multiple mechanisms and multiple risk factors. The mechanisms that bring about a loss in transparency include oxidation, osmotic stress, and chemical adduct formation. Risk factors for cataract include diabetes, radiation (ultraviolet B, x-ray), certain pharmaceutical substances, certain nutritional states, and possibly acute episodes of dehydration. Interaction occurs between and among mechanistic factors and risk factors. Thus nutrition must be considered as one part of a tapestry of intertwined events and responses. Certain experimental m

Acceptability and feasibility of a comprehensive fall prevention model for independent older adults: A qualitative evaluation.

BACKGROUND: Falls amongst the elderly represent a global public health challenge because of their potential to cause illness, death, and reduce the autonomy of this group. They also impact the emotional, family, social and economic well-being of those involved. Various strategies to prevent falls have been reported in the literature, focusing mainly on addressing individual risk factors, and on the continuous assessment of the risk of falls in older people. OBJECTIVE: This study evaluated user satisfaction and acceptability of a comprehensive model, implemented in the community, to prevent fa

Association between a regional reimbursement policy and treatment escalation in pulmonary arterial hypertension: a real-world cohort study with long-term Markov cost-effectiveness analysis.

BACKGROUND: Pulmonary arterial hypertension (PAH) requires long-term treatment with high-cost targeted therapies. However, evidence regarding the associations of reimbursement policies with real-world treatment escalation and long-term economic outcomes remains limited. OBJECTIVES: To examine the association between implementation of a regional reimbursement policy and real-world treatment patterns and to evaluate the long-term cost-effectiveness of the observed post-policy treatment distribution compared with the pre-policy distribution. METHODS: A retrospective single-center before-and-aft

Economic Evaluation of Thymectomy for the Treatment of Nonthymomatous Myasthenia Gravis.

IMPORTANCE: Myasthenia gravis (MG) is a chronic autoimmune disease that significantly impairs patients' quality of life by causing fluctuating muscle weakness and fatigue. While pharmacological treatment with corticosteroids remains a cornerstone of treatment, long-term use is often associated with substantial adverse effects. Although thymectomy is an established and effective alternative and adjunct to pharmacological treatment, its long-term cost-effectiveness within the UK National Health Service (NHS) is unknown. OBJECTIVE: To evaluate the cost-effectiveness of thymectomy in addition to

Clinical and economic impact of universal screening for cytomegalovirus infection among pregnant women in Italy.

OBJECTIVE: To evaluate the clinical and economic impact of universal screening for cytomegalovirus (CMV) in pregnant women in Italy, with valacyclovir (VCV) therapy in the case of maternal primary CMV infection, compared with no screening. METHODS: We developed a decision-analytic model using a deterministic decision tree and compared the no-screening strategy (Scenario 1) with universal screening until 13 + 6 weeks' gestation (Scenario 2), and universal screening until 23 + 6 weeks' gestation (Scenario 3) as recommended by the Italian National Health Service. The model was applied in a hypot

Excess HIV Infections and Costs Associated With Reductions in HIV Prevention Services in the US.

IMPORTANCE: Pre-exposure prophylaxis (PrEP) is a proven effective intervention to reduce the risk for HIV infection. Critically, changes in policies that lead to increased out-of-pocket PrEP costs or that decrease access to proximate PrEP locations could reduce PrEP coverage, resulting in excess HIV infections and costs. OBJECTIVE: To estimate how decreases in PrEP coverage that would be likely results of federal policy changes may be associated with new HIV infections and their costs. DESIGN, SETTING, AND PARTICIPANTS: In this economic evaluation, US population-based data sources were used

Comparative Analysis of Osteoarthritis Therapeutics: A Justification for Harnessing Retrospective Strategies via an Inverted Pyramid Model Approach.

In this review, we seek to explore two distinct approaches to the clinical management of OA: a prospective approach, addressing primarily one's genetic predisposition to OA and generating early intervention options, and the retrospective approach, aimed at halting or reversing OA progression post-symptom onset. The clinical management of OA remains challenging, largely due to the limited availability of preventative treatments and failure of existing therapies to modify or reverse the underlying pathophysiology. The prospective approach involves the identification of genetic markers associated

Consumer Medicine's Origins and Harms.

Consumer medicine consists of medical interventions pursued for non-health-related goals with the locus of the goals residing solely with the patient. Currently, contraceptives, abortion, cosmetic procedures, and physician-assisted suicide (PAS)/euthanasia fall in this category. Consumer medicine originates from the fusion of expressive individualism with its sole focus on the subjective psychological well-being intersecting with an expansion of health now including well-being combined with an exaltation of autonomy. Expressive individualism is inward-focused and entirely subjective reducing t

Reflecting the Health Opportunity Costs of Funding Decisions Within Value Frameworks: Initial Estimates and the Need for Further Research.

PURPOSE: Evaluating whether a new health technology provides good value for money requires an assessment of its opportunity cost. If the opportunity cost of the new health technology exceeds the benefits, however measured, a net loss is produced. Value frameworks using economic evaluation methods have been developed to guide the assessment of the value of new technologies within health care in response to rising spending. However, few explicitly consider health opportunity costs and fewer still base health opportunity costs on empirical estimates. This may partly be due to the dearth of estima

A value-based analysis of hemodynamic support strategies for high-risk heart failure patients undergoing a percutaneous coronary intervention.

BACKGROUND: The economic burden of heart disease is heavy and growing. As advanced technologies for treating heart disease become available, decision makers need to be able to assess the relative value of such options against existing standards of care. OBJECTIVES: To compare the clinical and economic benefits of a percutaneous ventricular assist device (pVAD) versus an intra-aortic balloon pump (IABP) observed during the 90-day duration of the PROTECT II clinical trial, and to supplement these findings with a simulation of the longer-term value of this technology through the use of a Markov

Supporting teachers and children in schools: the effectiveness and cost-effectiveness of the Incredible Years teacher classroom management programme in primary school children: a cluster randomised controlled trial, with parallel economic and process evaluations.

BACKGROUND: Childhood antisocial behaviour has high immediate and long-term costs for society and the individual, particularly in relation to mental health and behaviours that jeopardise health. Managing challenging behaviour is a commonly reported source of stress and burn out among teachers, ultimately resulting in a substantial number leaving the profession. Interventions to improve parenting do not transfer easily to classroom-based problems and the most vulnerable parents may not be easily able to access them. Honing teachers' skills in proactive behaviour management and the promotion of

Prenatal and infancy home visitation by nurses: recent findings.

This article describes a 20-year program of research on the Nurse Home Visitation Program, a model in which nurses visit mothers beginning during pregnancy and continuing through their children's second birthdays to improve pregnancy outcomes, to promote children's health and development, and to strengthen families' economic self-sufficiency. The results of two randomized trials (one in Elmira, New York, and the second in Memphis, Tennessee) are summarized, and an ongoing trial in Denver, Colorado, is briefly described. Results of the Elmira and Memphis trials suggest the following: The progra

Sex differences in the social motivation of rats: Insights from social operant conditioning, behavioural economics, and video tracking.

BACKGROUND: Social behaviour plays a key role in mental health and wellbeing, and developing greater understanding of mechanisms underlying social interaction-particularly social motivation-holds substantial transdiagnostic impact. Common rodent behavioural assays used to assess social behaviour are limited in their assessment of social motivation, whereas the social operant conditioning model can provide unique and valuable insights into social motivation.
