A novel homozygous frameshift mutation in CFAP65 is associated with multiple morphological abnormalities of sperm flagella in a consanguineous Pakistani family.

Multiple morphological abnormalities of the sperm flagella (MMAF) represent a severe form of asthenozoospermia that leads to male infertility, and it is commonly associated with genetic defects affecting flagellar components. Although the gene CFAP65 has been implicated in MMAF, its full mutational spectrum and clinical relevance within highly consanguineous populations remain poorly characterized. To elucidate the genetic basis of this condition, whole-exome sequencing followed by Sanger sequencing was performed in two infertile individuals from a consanguineous Pakistani family. Sperm morpho

Waist-to-height ratio and renal dysfunction as independent correlates of low handgrip strength in patients with type 2 diabetes.

INTRODUCTION: Handgrip strength (HGS) is a key indicator of functional decline. We investigated whether central adiposity (waist-to-height ratio, WHtR) and renal dysfunction independently or additively correlate with low HGS across body mass index (BMI) categories in Asian patients with type 2 diabetes. MATERIALS AND METHODS: This retrospective cross-sectional study included 1,468 patients. Low HGS was defined by the Asian Working Group for Sarcopenia 2019 criteria. Multivariable logistic regression evaluated independent and combined correlates. Subgroup analyses examined the interactions bet

A new era in atherothrombosis.

Atherothrombosis is a complex vascular disorder in which a thrombus forms on a vascular lesion, typically following the disruption and/or erosion of an atherosclerotic plaque. Atherothrombosis is the main pathological mechanism underlying atherosclerotic cardiovascular diseases (ASCVDs) [i.e., myocardial infarction (MI), ischemic stroke, and peripheral artery disease (PAD)]. In recent years, substantial advances have reshaped our understanding of ASCVD, spanning its pathophysiology, risk stratification, and therapeutic management. Emerging insights into the mechanisms of atherosclerosis progre

Skin Application of Inflammasome Inhibitor MCC950 Prevents Psoriasis-like Cutaneous Manifestations and Associated Systemic Inflammation.

Psoriasis is a chronic, systemic inflammatory disease associated with cardiovascular and metabolic comorbidities. It is driven by a sustained immune response in the skin, orchestrated by keratinocytes and both innate and adaptive immune cells. The initial phase of the pathology remains largely unclear. It likely involves the activation of dendritic cells by stress signals from keratinocytes with the NLRP3 inflammasome potentially acting as a key innate sensor. Here we investigate the effects of the topical administration of the selective NLRP3 inhibitor MCC950, in the imiquimod-induced psorias

Delayed Recognition of Hemophagocytic Lymphohistiocytosis in a Child With Refractory Fever: A Case From Pakistan.

Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening, exceedingly rare hyperinflammatory syndrome that typically presents with nonspecific symptoms such as fever and cytopenia. An early diagnosis of HLH is a significant clinical challenge, especially in children with presentations mimicking common infectious diseases. A 9-year-old girl came with a 2-week history of intractable high-grade fever, cough, and generalized weakness, unresponsive to broad-spectrum antibiotics. Physical examination showed hepatosplenomegaly and pallor. Laboratory tests showed pancytopenia, significantly elev

Anti-Platelet Factor 4 Antibody Clonal Heterogeneity and MGUS Status in HIT.

BACKGROUND: Monoclonal gammopathy of thrombotic significance (MGTS) is a recently described chronic prothrombotic condition characterized by monoclonal anti-PF4 antibodies that are detected above the polyclonal antibody background in patient sera (i.e. present as monoclonal gammopathy of undetermined significance, MGUS). Due to conflicting data in the published literature on antibody clonality in heparin-induced thrombocytopenia (HIT), we evaluated clonality and abundance of anti-PF4 antibodies in HIT, including investigating whether an MGUS, if present in HIT, represents the causative anti-PF

Epstein-Barr virus antibodies and risk of non-Hodgkin lymphoma: a pooled analysis of four prospective cohorts.

Non-Hodgkin lymphomas (NHLs) are among the most common cancers in the US, yet their etiology is mostly unknown. Epstein-Barr virus (EBV) is a known driver of rare NHLs, but its role in common subtypes is unclear. We evaluated associations between antibodies to 16 EBV antigens, including 12 emerging markers (anti-: BARF1, BBRF1, BFRF1, BGLF2, BHRF1, BORF1, BPLF1, BRLF1, BXLF1, LF2, LMP1, VCAp40) and NHL risk in prediagnostic blood samples from 2134 case-control sets from 4 prospective cohort studies. Conditional logistic regression estimated odds ratios (ORs) and 95% confidence intervals (CIs).

Macrophage Extracellular Vesicles: Therapeutic Strategies for Corneal Fibrosis in Rare Diseases.

Corneal scarring (fibrosis) is a blinding condition affecting millions of sufferers worldwide. It is not only in common ocular injuries but also in genetically inherited rare diseases such as epidermolysis bullosa (EB), keratitis-ichthyosis-deafness (KID) syndrome and aniridia. In rare diseases like EB or KID syndrome, corneal fibrosis arises from chronic inflammation, structural instability and neuro-immune dysfunction driven by genetic mutations. Current therapies are not effective in addressing the needs of affected individuals due to limited efficacy nor the considerable side effects of tr

A lethal combination: fulminant liver failure due to Babesia microti and Borrelia burgdorferi co-infection.

Babesiosis and Lyme disease are tick-borne infections endemic to the northeastern and upper Midwestern United States. Although both diseases can involve multiple organ systems, acute liver failure (ALF) is exceedingly uncommon. Co-infection with Babesia microti and Borrelia burgdorferi may exacerbate disease severity, create diagnostic uncertainty, and delay appropriate therapy. We describe a 77-year-old man with type II diabetes mellitus, stage IV chronic kidney disease, and heart failure with preserved ejection fraction who presented with dyspnea, cough, and peripheral edema. A rare complication of vascular surgery - subclinical esophageal perforation secondary to supra-aortic debranching and TEVAR: a case report.

INTRODUCTION AND IMPORTANCE: Combined supra-aortic debranching and thoracic endovascular aortic repair (TEVAR) are well-established therapeutic options for complex aortic arch pathologies. Although rare, late complications such as esophageal injury can be life-threatening and require prompt multidisciplinary management. Several cases of delayed esophageal perforation complicated by mediastinitis have been reported in the literature. However, perforation presenting solely as a graft infection causing microembolizations remains exceedingly rare. CASE PRESENTATION: A 56-year-old male patient wit

Oxidized mannan-MOG35-55 conjugate as a platform for antigen-specific immune modulation in multiple sclerosis.

Multiple sclerosis (MS) is a chronic autoimmune disease of the central nervous system (CNS) characterized by autoreactive T-cell responses against myelin antigens. Current disease-modifying therapies (DMTs) broadly suppress immune activity but do not restore antigen-specific immune tolerance and may cause significant adverse effects. Antigen-specific immunotherapies offer a rational alternative aimed at selectively re-educating the immune system without compromising protective immunity. Structural determinants of myelin antigens, including arginine-dependent conformational stability and post-t

Gut-derived butyrate as a key regulator in the pathogenesis of multiple sclerosis.

Multiple sclerosis (MS) is an autoimmune condition that affects the central nervous system (CNS) and leads to neuroinflammation. The chronic inflammatory cascades of the immune system damage the functions of the nerves and myelin tissue. The heterogeneity of MS symptoms depends on the anatomical location of the lesion within the CNS. The identification of the gut microbiota as a crucial modulator of neuroinflammation via the microbiota-gut-brain axis has been the subject of new studies. Butyrate, one of the well-known short-chain fatty acids (SCFAs) produced by gut bacteria through the ferment

The AML immune paradox: decoding escape pathways and pioneering checkpoint, vaccine, and combination strategies.

Acute myeloid leukemia (AML) remains a high-mortality cancer due to its aggressive nature and immunosuppressive tumor microenvironment (TME), which enables evasion of immune surveillance. Despite chemotherapy and targeted therapies, 5-year survival is ~ 30%, necessitating novel immunotherapies. AML suppresses cytotoxic T/NK cells by co-opting regulatory pathways, creating an "immune paradox." Emerging strategies aim to disrupt this evasion. Immune checkpoint inhibitors (ICIs), such as anti-PD-1 nivolumab and anti-CD47 magrolimab, combined with hypomethylating agents (HMAs), enhance T-cell acti

Farnesol emulsion for elimination of Pseudomonas aeruginosa biofilm in a 3D airway model of cystic fibrosis.

Cystic fibrosis (CF), a life-shortening genetic disease, is hallmarked by mucus obstruction, respiratory deficiency, and chronic bacterial infections. Pseudomonas aeruginosa is the most common virulent respiratory pathogen that is detrimental to the overall survival of CF patients. Here we evaluate the efficacy of farnesol emulsion, a broad-spectrum agent recently used to combat P. aeruginosa biofilm infections, for reducing P. aeruginosa infections in CF using a three-dimensional (3D) airway "organ tissue equivalent" (OTE) model. OTEs are fabricated using cells derived from human primary cell

Enhancing Chimeric Antigen Receptor-Extracellular Vesicles (CAR-EV) Technology: The Future of Cancer Therapy.

CAR cell therapies have significantly advanced personalized treatment for several hematological malignancies. Currently, seven CAR- cell products are approved by the Food and Drug Administration (FDA) and six by the European Medicines Agency (EMA) for treating lymphoma, multiple myeloma, and chronic lymphocytic leukemia. Several challenges and limitations remain, with cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS) being the most significant. Cell-free therapies, such as CAR-EVs, offer substantive advantages over their cellular counterparts. T

Effects of Daily Lifestyle Habits on Non-Neurogenic Orthostatic Hypotension in Older Adults in South Korea: A Cross-Sectional Study.

Background: Orthostatic hypotension (OH) is a chronic, debilitating condition common in older adults. This study examined the effects of daily lifestyle habits on non-neurogenic OH in older adults in South Korea. We further compared the effects of daily lifestyle habits on OH to those of the autonomic nervous system (ANS) function. Methods: In a cross-sectional study, 217 community-dwelling older adults aged ≥65 years were recruited using the convenience sampling method. Data were collected using two questionnaires to assess OH and daily lifestyle habits: OH was measured by Orthostatic Grading

Immune Checkpoint Inhibitor-Induced Ocular Myasthenia Gravis: Case Series and Literature Review.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have transformed cancer management by enhancing the immune system's ability to target cancer cells. However, they are known to be associated with immune-related adverse events, including ICI-induced myasthenia gravis (ICI-MG). There is limited literature on the isolated ocular form, ICI-induced ocular myasthenia gravis (ICI-OMG). The purpose of this study was to characterize clinical presentation and response to treatment of patients with ICI-OMG. METHODS: This case series and literature review focuses on isolated ICI-OMG. The Mayo MGRS Prediction Tool calculates the risk of finding monoclonal gammopathy of renal significance in a kidney biopsy in patients with monoclonal gammopathy.

INTRODUCTION: Monoclonal gammopathy of renal significance (MGRS) is increasingly recognized as an important cause of kidney failure. A kidney biopsy remains an important diagnostic measure. However, a kidney biopsy is not without risks. Here, we devised the Mayo MGRS Prediction Tool to assess the probability of finding MGRS in patients with chronic kidney disease with monoclonal gammopathy (MG). METHODS: We included patients from 2013 to October 2023 who underwent a kidney biopsy at the Mayo Clinic excluding those whose hematological condition required specific treatment due to tumor burden s
