Alcohol and tobacco use disorder comorbidity in young adults and the influence of romantic partner environments.

BACKGROUND: Although there is considerable evidence that the development of tobacco dependence (TD) and that of alcohol use disorder (AUD) are intertwined, less is known about the comorbid development of these disorders. The present study examines tobacco dependence and alcohol use disorder comorbidity in young adulthood within the context of romantic partner relationships. METHODS: Data were drawn from the Seattle Social Development Project, a contemporary, ethnically diverse, and gender balanced longitudinal panel including 808 participants. A typological person-centered approach was used t

Lack of evidence to support policy development for management of contacts of multidrug-resistant tuberculosis patients: two systematic reviews.

BACKGROUND: Existing international guidelines provide different recommendations for the management of contacts of multidrug-resistant tuberculosis (MDR-TB) patients. OBJECTIVE: To conduct two systematic reviews with the aim of identifying chemoprophylactic approaches that are effective in contacts of MDR-TB patients to assist in policy making. DESIGN: We systematically searched the Medline, Embase, Central, LILACS, TRIP and BIOSIS Preview databases for studies on the effectiveness of anti-tuberculosis drugs in preventing active TB in persons at risk of developing MDR-TB. This was done as an

Yoga spinal flexion positions and vertebral compression fracture in osteopenia or osteoporosis of spine: case series.

OBJECTIVE: The objective of this report is to raise awareness of the effect of strenuous yoga flexion exercises on osteopenic or osteoporotic spines. We previously described subjects with known osteoporosis in whom vertebral compression fractures (VCFs) developed after spinal flexion exercise (SFE) and recommended that SFEs not be prescribed in patients with spinal osteoporosis. METHODS: This report describes 3 healthy persons with low bone mass and yoga-induced pain or fracture. RESULTS: All 3 patients had osteopenia, were in good health and pain-free, and had started yoga exercises to impr

[Predementia Alzheimer's disease. Benefits of early diagnosis].

Given population aging and the rise in the number of persons with Alzheimer's disease, measures that aim not only to delay but also to prevent the development of this disease are increasingly required. Advances in the diagnosis of Alzheimer's disease support the need for a review of current clinical standards for mild cognitive impairment and provide new goals in the early treatment of this disease. The current diagnostic process should be refocussed toward the pathological substrate of this disease rather than symptoms in order to initiate therapeutic measures as soon as possible without wait

Barriers to accessing surgical care in Pakistan: healthcare barrier model and quantitative systematic review.

Inadequate access to surgical services results in increased morbidity and mortality from a spectrum of conditions in Pakistan. We employed a modification of Andersen's model of health services utilization and developed a 'Healthcare Barrier Model,' to characterize the barriers to accessing health care in developing countries, using surgical care in Pakistan as a case study. We performed a literature search from MEDLINE, EMBASE, CINAHL, SCOPUS, Global Health Database, and Cochrane Central Register of Controlled Trials, and selected 64 of 3113 references for analysis. Patient-related variables i

[Psychological distress in applicants for genetic screening for colorectal cancer].

UNLABELLED: Introduction. The development of a DNA based diagnostic test has allowed for the genetic screening of many hereditary diseases. In addition to the identification of the deleterious gene, this screening process has led to the recognition of developing illnesses at high risk. In recent years, a number of genes predisposing to an inherited cancer syndrome have been identified. AIM OF THE STUDY: Our purpose in this study was to determine whether subjects at risk who test for inherited colorectal cancer, are likely to develop a higher level of psychological distress than the norm, taki

How important are client characteristics to understanding treatment process in the therapeutic community?

Prior research has demonstrated that therapeutic communities (TCs) are effective at improving posttreatment outcomes for substance abusers. However, little is known about the in-treatment experience for clients with different backgrounds, experiences, and needs. The aim of this study is to examine the in-treatment experience for different clients by exploring the relationships between treatment process and client characteristics. A comprehensive measure of treatment process, operationalized as Community Environment and Personal Change and Development and change was administered to 447 adults a

Machine learning ensemble meets clinical practice: developing a real-world risk prediction model for metabolic syndrome using super learner and scorecard approaches.

INTRODUCTION: Metabolic syndrome (MetS) refers to a cluster of metabolic abnormalities that significantly increase the risk of developing cardiovascular diseases (CVDs). Traditional binary definitions of MetS fall short in capturing its severity spectrum, limiting personalized risk stratification. OBJECTIVES: We aimed to develop a super learner model and multilevel risk scorecard to improve MetS risk prediction and support early cardiovascular risk identification. METHODS: This study included a total of 460,256 health examination records from Zhejiang, China (2018-2023), with 344,925 used fo

Current status and challenges of breast cancer prevention~DNA methylation would lead to groundbreaking progress in breast cancer prevention~.

The number of breast cancer patients is increasing worldwide. Furthermore, breast cancer often develops in young people, even those only in their 30s, who play a central role in their families and society. Results from many cohort studies suggest that dietary factors, alcohol consumption, lack of physical activity, obesity, nulliparity, breastfeeding, oral contraceptive use, fertility treatment and hormone replacement therapy are risk factors for breast cancer. However, the effects of lifestyle habits on the human body are complexly intertwined with various factors, and the effects vary from p

Editor's Choice - The Intersocietal IWGDF, ESVS, SVS Guidelines on Peripheral Artery Disease in People With Diabetes Mellitus and a Foot Ulcer.

Diabetes related foot complications have become a major cause of morbidity and are implicated in most major and minor amputations globally. Approximately 50% of people with diabetes and a foot ulcer have peripheral artery disease (PAD) and the presence of PAD significantly increases the risk of adverse limb and cardiovascular events. The International Working Group on the Diabetic Foot (IWGDF) has published evidence based guidelines on the management and prevention of diabetes related foot complications since 1999. This guideline is an update of the 2019 IWGDF guideline on the diagnosis, progn

Quantitative proteomics for drug toxicity.

The emerging field of toxicoproteomics has been greatly advanced by quantitative proteomic technologies and their increasing applications in toxicology. The discipline is focused on the proteomic study of toxicity caused by toxic substances, including but not limited to drugs, toxins, environmental stressors, chemicals and any other materials that induce significant pathological responses. Drug safety is a major point of concern during the development phase and clinical application. Identification of toxicity biomarkers, potential drug targets and characterization of toxicity mechanisms repres

Risk for hand eczema in employees with past or present atopic dermatitis.

Persons with atopic dermatitis run a considerable risk of developing hand eczema when exposed to occupational agents that are a burden to the skin. This also pertains to those with a history of skin atopy in childhood. This review presents estimates of the risk of developing hand eczema and examines the evidence for an effect modification by skin atopy on exposure. Skin atopy at least doubles the effects of irritant exposure and, thus, doubles the risk in occupations where hand eczema is a common problem. On the basis of this evidence, guidelines for occupational counseling can be given. Furth

Treat-to-target: LLDAS, DORIS remission, and flare prevention strategies.

The goal of care in any medical condition is to optimise a person's current health status, with a view to positively impacting on their long-term outcomes. Since 2014 the concept of treat-to-target (T2T) in systemic lupus erythematosus (SLE) has been in development, enabled by a paradigm shift to focus on attainable treatment targets allowing for the development, validation, and adoption of a T2T approach. In this review, the history of this paradigm change will be reviewed, along with evidence from clinical trials that show increasing likelihood of achievement of these goals with more effecti

Splice-switching antisense oligonucleotides for pediatric neurological disorders.

Pediatric neurological disorders are frequently devastating and present unmet needs for effective medicine. The successful treatment of spinal muscular atrophy with splice-switching antisense oligonucleotides (SSO) indicates a feasible path to targeting neurological disorders by redirecting pre-mRNA splicing. One direct outcome is the development of SSOs to treat haploinsufficient disorders by targeting naturally occurring non-productive splice isoforms. The development of personalized SSO treatment further inspired the therapeutic exploration of rare diseases. This review will discuss the rec

Group schema therapy for patients with severe anxiety disorders.

This article presents an adaptation of group schema therapy (GST) developed for patients with severe anxiety. An anonymized case presentation exemplifies the adaptation. GST was originally developed to treat clients with maladaptive personality traits but has been applied to a variety of disorders in recent years. For patients with severe anxiety, who have not responded to other treatments, GST may be a promising approach. The paper presents the GST model, the evidence supporting it, and its adaptation for the treatment of anxiety. The case presentation illustrates the therapeutic process and

Building a research agenda for mental health assessments in resolving legal dilemmas on adolescent sexual consent.

The dilemma of adolescent consent confounds dispensation of justice in child sexual abuse and juvenile justice contexts, with legal personnel struggling to discern between consent and coercion in cases of adolescents who are sexually abused and those who are accused of alleged sexual offences. Given that capacities for consent also determine personal safety and mental health, there is an imperative to build a child mental health research agenda for developing methodologies that incorporate psychological knowledge on abuse dynamics, adolescent mental health and development, vulnerability and ri
