Hypocomplementemic idiopathic membranous glomerulopathy.

Two asymptomatic teenage females with proteinuria, hematuria, and persistent hypocomplementemia are presented. Renal biopsy studies revealed membranous glomerulopathy. During follow-up periods of 4.0 and 4.5 years, the clinical courses and sequential monitoring of immunologic studies failed to demonstrate lupus erythematosus or other systemic disorders. Serum complement levels were consistent with activation of the alternate complement pathway in the first patient and the classic pathway in the second. In both cases further depression in serum complement levels occurred during intervals of gre

Post-streptococcal glomerulonephritis. A quantitative glomerular investigation.

A light microscopical, quantitative glomerular examination was performed on renal biopsies from 14 patients with acute post-streptococcal glomerulonephritis. A significant increase of 58% (p less than 0.0025) in the total number of glomerular cells was found which was mainly due to an increase of endothelial cells (97%). Mesangial cells were, however, also increased (61%) and mesangial area was increased by 34% (p less than 0.05). Biopsies from patients with persisting proteinuria and/or hematuria revealed slight hypercellularity and an increase in mesangial area during the first six months of

Renal involvement in patients with hepatosplenic Schistosomiasis mansoni.

In a prospective study of renal involvement in 100 consecutively hospitalized patients with hepatosplenic schistosomiasis mansoni, 15 exhibited persistent proteinuria of varying degree, which in 6 instances was accompanied by hypertension. Nine patients had the nephrotic syndrome. The most common glomerular lesion in this group was membrano-proliferative glomerulonephritis. Surgical biospy obtained during splenectomy in 15 patients without clinical evidence of renal involvement showed glomerular lesions in 6 instances. Focal proliferative glomerulonephritis was the most common lesion in this g

Fine-needle aspiration biopsy of spleen in diagnosis of generalized amyloidosis.

Fine-needle aspiration biopsy of the spleen was performed on 18 patients shown to have amyloid deposits in other organs and on 17 control patients being investigated for proteinuria. Of the 18 patients with amyloid disease smears of splenic aspirate were positive in all cases, renal biopsy was positive in 16 out of 16 cases, and rectal biopsy was positive in seven out of 11 cases. None of the splenic smears were positive in the 17 control patients and no amyloid was found in the kidney in 15 of these patients on whom renal biopsy was performed. Splenic aspirate biopsy seems to be a simple and

A Case of Autosomal Dominant Alport Syndrome with Normal Type IV Collagen α5 Staining Diagnosed by a Genetic Analysis.

A 30-year-old woman with persistent hematuria, mild proteinuria, and a family history of end-stage kidney disease was initially diagnosed with thin basement membrane nephropathy based on renal biopsy findings of a nearly normal glomerular basement membrane with only focal thinning, no lamellation, and normal type IV collagen α5 staining. However, genetic testing identified a heterozygous COL4A4 missense variant (p.Gly527Cys), thus confirming a diagnosis of autosomal dominant Alport syndrome. After starting angiotensin receptor blocker therapy, the patient's kidney function remained stable. Thi

Primary Health Care approach to managing chronic kidney disease.

Chronic kidney disease (CKD) is a growing global health issue among ageing populations, driven by increasing diabetes and hypertension. Managing advanced stages of CKD with dialysis or transplants is costly. Early stages often have no symptoms, making early detection important for prevention. Primary care providers can help identify and manage CKD early by performing annual screening for high-risk patients, including blood pressure checks, urine tests for albuminuria or proteinuria and estimating glomerular filtration rate from serum creatinine. For those with CKD, renal protective measures ar

Chronic Kidney Disease with Sjögren's Syndrome and Hyperthyroidism: Discrepant Creatinine- and Cystatin C-Based eGFR.

We report the case of a 70-year-old woman with chronic kidney disease, hyperthyroidism, and Sjögren's syndrome who exhibited a marked discrepancy between creatinine-based and cystatin C-based estimated glomerular filtration rates(eGFRs). A kidney biopsy revealed chronic tubulointerstitial nephritis with lymphoplasmacytic infiltration, and electron microscopy showed focal subendothelial widening without electron-dense deposits. Methimazole improved the thyroid function and reduced discrepancies between eGFR estimates. Proteinuria and tubular injury markers improved after corticosteroid therapy.

Relapse of Membranous Nephropathy Following Adrenalectomy for Cushing Syndrome: A Case Report.

Hypercortisolemia in Cushing syndrome (CS) may exert a protective effect against the membranous nephropathy (MN) activity. We report the case of a 59-year-old woman who was diagnosed with stage I MN by renal biopsy 10 years earlier, and achieved complete remission after one year of glucocorticoid therapy. After adrenalectomy for CS (preoperative cortisol level, 18.3 μg/dL), her proteinuria increased. A repeat renal biopsy revealed progression to stage III MN (cortisol level, 0.3 μg/dL). Glucocorticoid and calcineurin inhibitor administration resulted in complete remission. The temporal associa

Antinuclear antibody (ANA) screening based on EULAR/ACR 2019 clinical domain criteria among PLHIV and profile of diagnosed HIV-SLE cases.

Systemic lupus erythematosus (SLE) in people living with HIV (PLHIV) is rare, with limited prevalence data. We performed symptom-directed antinuclear antibody (ANA) screening in 129 PLHIV, followed by extractable nuclear antigen testing in ANA-positive individuals. SLE was diagnosed using the 2019 ACR-EULAR criteria. ANA positivity was seen in 54 patients, while anti-dsDNA antibodies were detected in four, all diagnosed with SLE. Three patients were male, with polyarthritis as the most common presentation, followed by proteinuria. Sjögren syndrome, dermatomyositis, and primary systemic scleros

Spontaneous remission in primary membranous nephropathy: mechanisms, predictive factors, and implications for personalized management.

Primary membranous nephropathy (PMN) is a major cause of adult nephrotic syndrome and demonstrates considerable clinical heterogeneity. This review summarizes current evidence on the immunological mechanisms and clinical predictors underlying spontaneous remission (SR) in PMN. We discuss key factors including the dynamics of anti-PLA2R antibodies, proteinuria trends, renal function indicators, histopathological features, and emerging biomarkers. A staged immune modulation process is proposed, involving suppression of autoreactive responses and promotion of tissue repair. Integrating these insi

Case Report: Bevacizumab-induced renal-limited TMA and FSGS-like lesions in a kidney transplant recipient.

Bevacizumab, an anti-VEGF agent commonly employed in cancer therapy, can induce renal vascular endothelial and podocyte damage by inhibiting VEGF, leading to renal disease. We present a case of nephrotic syndrome and renal insufficiency arising from small intestinal mucinous adenocarcinoma 17 years post-renal transplantation, treated with bevacizumab and sindilizumab. Renal biopsy confirmed renal-limited thrombotic microangiopathy (TMA) with FSGS-like lesions in the transplanted kidney, attributed to bevacizumab. Case Report: Membranous nephropathy associated with mantle cell lymphoma: a rare case.

Mantle cell lymphoma (MCL) is a rare, aggressive non-Hodgkin lymphoma (NHL) that can affect the kidneys. In this research, we described a very rare case of secondary membranous nephropathy (MN) associated with MCL. A 67-year-old man manifested with impaired renal function and nephrotic syndrome (NS). Kidney biopsy revealed atypical MN and MCL cell infiltration. After eight rounds of modified R-CHOP therapy, which includes rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisolone, the patient achieved complete remission, accompanied by improved renal function and a reduction in pr

Anti-PLA2R Antibody-Positive Membranous Nephropathy Superimposed by Pleural Tuberculosis a Year Later: Mere Association or Actual Cause?

Tuberculosis-associated glomerulonephritis is difficult to diagnose, as it usually presents with hematuria, proteinuria, edema, hypertension or renal insufficiency, which are similar to the symptoms of primary glomerulonephritis. Membranous nephropathy is uncommonly seen in tuberculosis patients. We report a case of a 21-year-old male who presented with adult-onset nephrotic syndrome and was diagnosed by renal biopsy as anti-phospholipase A2 receptor antibody-positive membranous nephropathy. After one year of conservative treatment with ramipril alone without achieving a remission, the patient

Successful Management of Critical Acute Respiratory Distress Syndrome following COVID-19 through Extracorporeal Membrane Oxygenation in a Patient with Concurrent Nephrotic Syndrome Relapse.

A 44-year-old man with coronavirus disease 2019 (COVID-19) and nephrotic syndrome relapse was admitted to our intensive-care unit for respiratory failure. Despite receiving mechanical ventilation and immunomodulators, the patient experienced refractory hypoxemia, necessitating venovenous extracorporeal membrane oxygenation (VV-ECMO) therapy. Due to a worsening renal function, continuous hemodiafiltration was initiated. After 11 days, his respiratory status gradually improved, and VV-ECMO was withdrawn. The kidney function and proteinuria improved, and hemodialysis was subsequently discontinued

Amyloidosis in a Patient With Congenital Neutropenia Because of G6PC3 Deficiency.

Glucose-6-phosphatase catalytic subunit 3 (G6PC3) deficiency is a recently identified form of congenital neutropenia associated with developmental anomalies. The severity of neutropenia and the clinical spectrum are highly variable. Aside from infectious complications and extrahematologic features, inflammatory bowel disease and autoinflammatory complications are less frequently observed manifestations. However, amyloidosis has never been reported in G6PC3 deficiency. Here, we present a 12-year-old patient with incidentally discovered neutropenia because of the p.E65A (c.194A>C) variant of the

Lipoic acid supplementation associated with neural epidermal growth factor-like 1 (NELL1)-associated membranous nephropathy.

Lipoic acid (alpha lipoic acid, thioctic acid) is a popular over-the-counter antioxidant and insulin-mimetic supplement under investigation in a variety of conditions including multiple sclerosis, diabetes, and schizophrenia. Unfortunately, high-grade proteinuria was an unexpected adverse event specific to the treatment arm of our clinical trial investigating lipoic acid supplementation in patients with multiple sclerosis. This observation led to detection of similar patients in our nephrology practice. Here, we describe four biopsy-proven cases of neural epidermal growth factor-like 1 (NELL1)

A 55-Year-Old Male Presenting With a Lower Extremity Rash: A Case of Immunoglobulin A (IgA) Nephropathy.

Immunoglobulin A (IgA) nephropathy, mesangial deposition of IgA in renal parenchyma, typically presents with hematuria and proteinuria. Leukocytoclastic vasculitis (LCV), a small-vessel vasculitis, can present secondary to IgA. We will discuss a case of secondary IgA nephropathy with concomitant LCV in a patient with reactivated hepatitis C. A 55-year-old male with decompensated alcoholic cirrhosis presented for a bilateral lower-extremity rash. The patient was diagnosed with IgA nephropathy, by kidney biopsy, and skin biopsy showing LCV. Further investigation revealed hepatitis C viral load w

Maternal serum levels of asymmetric dimethylarginine in normal and preeclamptic pregnancies.

The study aims to investigate maternal serum levels of asymmetric dimethylarginine (ADMA) in preeclampsia. Serum samples were collected from 57 women with preeclamptic pregnancies and 30 women with normal pregnancies during the third trimester. ADMA levels were measured with the ELISA method. ADMA levels in preeclamptic pregnancies were significantly higher when compared with normal pregnancies (2.35 ± 3.20 nmol/l versus 0.35 ± 0.10 nmol/l; p < .05). ADMA levels show a significant positive correlation with systolic and diastolic pressure, urea, and creatinine but a negative correlation with pr

Membranous Nephropathy with Proteinase 3-ANCA-associated Vasculitis Successfully Treated with Rituximab.

Membranous nephropathy (MN) with anti-neutrophil cytoplasmic antibody (ANCA)-associated glomerulonephritis (ANCA-GN) is seen infrequently. Previous reports of patients with ANCA-GN with MN showed that the most frequent ANCA subtype was myeloperoxidase-ANCA. We herein present a 73-year-old woman with scleritis, hematuria, proteinuria, and positive serum proteinase 3 (PR3)-ANCA. She underwent a renal biopsy and was diagnosed with MN and ANCA-GN. Immunofluorescence staining for PR3 colocalized with IgG along the glomerular basement membrane were observed. Oral prednisolone and intravenous rituxim

Evolving understanding of cardiovascular protection by SGLT2 inhibitors: focus on renal protection, myocardial effects, uric acid, and magnesium balance.

Robust clinical data indicate that inhibitors of the sodium/glucose cotransporter 2 (SGLT2) dramatically improve clinical outcomes in diabetes, especially heart failure and progression of kidney disease. Factors that may contribute to these findings include: 1) improved glycemic control, 2) diuresis and reduced extracellular fluid volume, 3) reduced serum uric acid levels, 3) direct myocardial effects, 4) reduction in proteinuria and preservation of kidney function, and 5) correction of diabetic magnesium deficiency. Understanding the mechanisms by which SGLT2 inhibitors improve cardiovascular
