Systemic Conditions Affecting Sleep: An Overview for the Clinician.

The impact of sleep on health has been the topic of recent research. Numerous systemic conditions and diseases may have a significant impact on sleep, and vice versa. There are complex interrelationships between sleep and health in special populations as well. This article, therefore, explores and highlights the key elements in this regard. In this article, a few of the systemic factors and conditions with the most robust effect on sleep have been selected. It is obvious that systemic conditions such as cardiovascular, respiratory, endocrine, renal, and other disorders have robust impact on sl

[The lungs: starting point for many diseases].

The lungs are a frequent site for the manifestation of systemic, neoplastic and immunological multiorgan diseases. In the clinical routine, patients frequently present with symptoms from the respiratory spectrum of disorders, such as dyspnea. After a clinical examination, lung function testing and imaging an initial pulmonary manifestation can often be detected; however, the ultimate assignment to a systemic disease is usually only successful in the synopsis of the clinical results, pulmonary involvement, extrapulmonary manifestation and further diagnostics. This review article presents three

APPLICATION OF LAPAROSCOPIC LIFTING SYSTEMS IN PATIENTS WITH HIGH CARDIORESPIRATORY RISK.

Intra-abdominal hypertension during laparoscopic operations increased the risk of complications from cardiovascular and respiratory systems. An application of laparolifting systems allowed doctors to avoid changes of pneumoperitoneum, although it was associated with technical difficulties in operation performance. The authors used a test in order to determine cardiorespiratory reserve in preoperative period. The reserve was characterized by decrease of stroke volume of the heart against the background of intra-abdominal hypertension. There was noted a reliable increase of complication rate in

[Bronchial reactivity in diabetic patients].

The data of the literature concerning bronchial reactivity in diabetic patients are controversial. Therefore, we studied the influence of the presence of a diabetic cardiac autonomic neuropathy (CAN) on the ventilatory parameters measured during a methacholine-induced bronchoconstriction test. Ten insulin-dependent diabetic patients without CAN, ten insulin-dependent diabetic patients with CAN and ten healthy volunteers, all non-smokers and free of respiratory symptoms, have undergone a functional respiratory check-up before the methacholine test. The presence of CAN was classically studied by

Wheat leaf dark respiration acclimates more strongly at night than in the day when responding to nocturnal warming.

Rising night temperatures pose a significant threat to wheat productivity, yet the physiological basis of wheat adaptation to nocturnal warming remains poorly understood. We evaluated leaf photosynthetic and respiratory traits in response to warm nights under temperature-controlled environments in 10 Australian wheat cultivars released between 1901 and 2012. When exposed to warmer nights, rates of leaf net CO2 assimilation measured at 25 °C (Anet25) remained stable across cultivar release date despite declines in photosynthetic capacity (Vcmax and J1500) in newer cultivars. In most cultivars l

Personalization of Caffeine Therapy for Apnea of Prematurity: A Potential Role for Sensor Technologies?

Apnea of prematurity (AOP) remains a critical challenge in neonatal care, with caffeine citrate serving as the cornerstone of pharmacological intervention. However, the current standardized dosing schedule fails to account for significant inter-individual variability in caffeine pharmacokinetics and clinical response. This narrative review explores the transformative potential of integrating wearable sensor technologies and multi-modal data analytics into a closed-loop framework for personalized caffeine therapy. Based on a synthesis of current monitoring literature, we propose a theoretical,

Single-System Laryngeal Anaphylaxis Following Gadobutrol Administration.

Gadolinium-based contrast agents (GBCAs) are widely used in magnetic resonance imaging (MRI) and are generally considered safe, with immediate hypersensitivity reactions being rare. While anaphylaxis typically presents with classic skin reactions, respiratory distress, or cardiovascular collapse, exposure to a known allergen can cause single-system respiratory or laryngeal variants. An 83-year-old male with multiple chronic medical conditions underwent a diagnostic brain MRI with contrast. Immediately after receiving 8.5 mL of gadobutrol, the patient developed hoarseness and a sporadic cough.

Ketamine in ambulatory anesthesia: dose-dependent effects on breathing and cortical function.

PURPOSE OF REVIEW: Ketamine was introduced into medical practice in 1965 and is widely utilized to provide sedation and analgesia. Low-dose ketamine can be used in ambulatory surgery following an assessment of risks and benefits. RECENT FINDINGS: Ketamine is an N-methyl-D-aspartate receptor antagonist that decreases the firing rate of fast-spiking gamma-aminobutyric acidergic interneurons, leading to a disinhibition of pyramidal neurons and enhanced cortical and respiratory arousal. The consequences are an activation of the sympathetic nerve system and an increased drive to upper airway dilat

Acceptance and vagal reactivity: Insights from a laboratory-based study of emotion regulation in late life.

Acceptance (i.e., embracing emotions and thoughts without judging them) is an emotion regulation strategy that can benefit mental and physical health. To shed more light on its proximal physiological mechanisms, the present laboratory-based study examined links between the habitual use of acceptance and vagal reactivity during instructed emotion regulation in late life, when acceptance may become particularly important. In a sample of 129 older adults (aged 64-83), we examined habitual emotional acceptance (using questionnaires) and vagal reactivity (using respiratory sinus arrhythmia) during

Evaluating the socioeconomic value of adult respiratory immunization in Spain: a benefit-cost analysis.

BACKGROUND: Vaccine-preventable illnesses, such as pneumococcal disease, respiratory syncytial virus, influenza, and COVID-19, place a significant burden on Spain's population health, health system, and economy. However, uptake and funding constraints limit the full value potential of Spain's national immunization programs. METHODS: A benefit-cost analysis was performed to assess the societal return on investment from adult vaccination by estimating benefit-cost ratios and net benefits (NBs). All outcomes were monetized using established methods. Program costs included all direct vaccination-

[Respiratory manifestations associated with cyclic neutropenia and innate immune deficiency (IgG3 deficiency): a case report].

BACKGROUND: Inborn errors of immunity are a heterogeneous group of diseases caused by a diminished or absent response of one or more components of the immune system. IgG subclass deficiency is more common in childhood, especially in boys than in girls. IgG2 deficiency is the most common, followed by IgG3 deficiency. CASE REPORT: A male pediatric patient experienced respiratory distress of viral and bacterial origin and asthma in his early years, with frequent emergency room admissions and/or hospitalizations. At age 5, he was diagnosed with cyclic neutropenia, so the Pediatric Hematology Depa

Video-Assisted Thoracoscopic Surgery (VATS)-Aided Simultaneous Fixation of Scapular Body and Rib Fractures: A Case Report.

Scapular fractures are rare injuries, whereas rib fractures are more common. Scapular fractures are frequently associated with rib fractures, which may lead to significant respiratory compromise. While surgical fixation has demonstrated benefits for both scapular and rib fractures, simultaneous fixation using a single surgical approach is not widely practiced. A 54-year-old male sustained a right scapular body fracture (AO 14B1) and multiple right rib fractures following a crush injury. Due to severe respiratory pain and prolonged mechanical ventilation, surgical fixation was performed on post

Respiratory challenges and oedema development in zebra finch embryos under reduced pore area.

Global warming is one of the primary drivers of habitat loss and population decline in numerous species, including birds, amphibians and marine life. Avian embryos exhibit ectothermic phenotypes during most of their incubation period and are also vulnerable to rising temperatures when parents cannot cool the nests. This vulnerability stems from their unique respiratory mechanisms, which utilize eggshell pores to exchange respiratory gases. The number of pores is fixed at oviposition, and embryos may experience hypoxia during later developmental stages, especially when exposed to elevated ambie

Gene editing of pigs to control influenza A virus infections.

Proteolytic activation of the hemagglutinin (HA) glycoprotein by host cellular proteases is pivotal for influenza A virus (IAV) infectivity. Highly pathogenic avian influenza viruses possess the multibasic cleavage site of the HA which is cleaved by ubiquitous proteases, such as furin; in contrast, the monobasic HA motif is recognized and activated by trypsin-like proteases, such as the transmembrane serine protease 2 (TMPRSS2). Here, we aimed to determine the effects of TMPRSS2 on the replication of pandemic H1N1 and H3N2 subtype IAVs in the natural host, the pig. The use of the CRISPR/Cas 9

[Issues of treatment and prevention of recurrent inflammatory diseases of the upper respiratory tract: An immunologist's view. A review].

Often ill people are, first of all, patients with recurrent infectious and inflammatory diseases of the upper respiratory tract (URT). They put a significant financial burden on the healthcare system. The leading etiological factors of recurrent inflammatory diseases of URT, in addition to pathogenic bacterial microflora, are viral agents (viruses of the viral respiratory infections group, herpes viruses). The chronic infection and recurrence of local inflammation is due to the persistence of resident bacterial and viral flora (including herpes viruses), and as a result, the insufficiency of t

Physiological effects and clinical evidence of high-flow nasal cannula during acute exacerbation in COPD patients: A narrative review.

Chronic obstructive pulmonary disease (COPD) is one of the leading causes of death worldwide. During severe exacerbations, COPD patients may develop acute respiratory failure (ARF), often necessitating hospital admission due to impaired gas exchange. In COPD patients, the diaphragm is subjected to an increased workload resulting from airflow limitations and geometric changes in the thorax due to pulmonary hyperinflation. Noninvasive ventilation (NIV) plays a crucial role in managing type II ARF by improving alveolar ventilation, reducing the work of breathing, minimizing the need for endotrach

Advancing Lung Immunology Research: An Official American Thoracic Society Workshop Report.

The mammalian airways and lungs are exposed to a myriad of inhaled particulate matter, allergens, and pathogens. The immune system plays an essential role in protecting the host from respiratory pathogens, but a dysregulated immune response during respiratory infection can impair pathogen clearance and lead to immunopathology. Furthermore, inappropriate immunity to inhaled antigens can lead to pulmonary diseases. A complex network of epithelial, neural, stromal, and immune cells has evolved to sense and respond to inhaled antigens, including the decision to promote tolerance versus a rapid, ro
