Contemporary practices and limb outcomes in peripheral venoarterial extracorporeal membrane oxygenation at a high-volume single institution.

OBJECTIVE: To evaluate the evolution of peripheral venoarterial extracorporeal membrane oxygenation access and the impact of adjuncts such as ultrasound guided access, distal perfusion catheters, and decannulation use on vascular complications, involvement, and outcome. METHODS: A retrospective chart review of patients cannulated for peripheral venoarterial extracorporeal membrane oxygenation was performed at a large tertiary center using patients identified from a prospectively maintained extracorporeal life support registry. The primary end point was limb ischemia requiring fasciotomy or am

Adjusting input arterial function to improve the accuracy of hypoperfusion assessment in computed tomography perfusion (CTP) for stroke: a preliminary study.

BACKGROUND: The choice of arterial input function (AIF) during post-processing of computed tomography perfusion (CTP) imaging can strongly influence perfusion maps. At present, there is no consensus on the optimal site for AIF selection. Since CTP often overestimates hypoperfused tissue, some patients with acute ischemic stroke (AIS) may face misdiagnosis and unnecessary treatment. This study aimed to improve the accuracy of hypoperfusion assessments through artificial intelligence to automatically modify the selection of AIFs. METHODS: We retrospectively analyzed 35 patients with AIS caused

Outcomes of prosthetic upper arm arteriovenous graft vs brachiobasilic fistula for hemodialysis access.

OBJECTIVE: Primary arteriovenous access such as radiocephalic and brachiocephalic fistulas are initial choices for creating vascular access in dialysis patients. When neither of these choices is an option, upper arm arteriovenous graft or brachiobasilic transposition is recommended. Although primary fistula is better than prosthetic graft for suitable patients, there is little data to guide the best treatment strategy in the absence of suitable vein for primary access creation. This study identifies factors that influence patency rates and compares outcomes of patients treated with brachiobasi

Comparison of Angiographic Success and Clinical Outcomes Based on Different Percutaneous Coronary Intervention Techniques.

Introduction Primary percutaneous coronary intervention (PCI) is the standard treatment for patients with ST-segment elevation myocardial infarction (STEMI). Various PCI techniques exist, including balloon angioplasty, bare-metal stents (BMS), drug-eluting stents (DES), thrombus aspiration, direct stenting, rotational atherectomy (Rotablation), and cutting balloon angioplasty. Specific approaches for patients with STEMI and multivessel coronary artery disease may involve: 1) culprit vessel-only (CVO) primary PCI, 2) primary PCI followed by multivessel intervention of additional noninfarct lesi

Staged autogenous to prosthetic hemodialysis access creation strategy to maximize forearm options.

OBJECTIVE: When an adequate cephalic vein is not available for fistula construction, surgeons often turn to basilic vein or prosthetic constructions. Single-stage forearm prosthetic hemodialysis accesses are associated with poor durability, and upper arm non-autogenous access options are often limited by axillary outflow failure, which inevitably drives transition to the contralateral arm or lower extremity. We hypothesized that initial creation of a modest flow proximal forearm arterial-venous anastomosis to dilate ("develop") inflow and outflow vessels, followed by a planned second-stage pro

Ischemic Stroke and Vaccine-Induced Immune Thrombotic Thrombocytopenia following COVID-19 Vaccine: A Case Report with Systematic Review of the Literature.

INTRODUCTION: Vaccine-induced immune thrombotic thrombocytopenia (VITT) is a prothrombotic syndrome observed after adenoviral vector-based vaccines for severe acute respiratory syndrome coronavirus 2. It is characterized by thrombocytopenia, systemic activation of coagulation, extensive venous thrombosis, and anti-platelet factor 4 antibodies. Arterial thrombosis is less common and mainly affects the aorta, peripheral arteries, heart, and brain. Several cases of ischemic stroke have been reported in VITT, often associated with large vessel occlusion (LVO). Here, we describe a case of ischemic

Impact of a regional smoking cessation intervention for vascular surgery patients.

OBJECTIVE: Tobacco use is common among vascular surgery patients and negatively impacts outcomes and longevity. In the second quarter of 2018, a statewide vascular quality collaborative launched an initiative across its 35 participating hospitals to promote smoking cessation at the time of surgery. This intervention was based on the Vascular Physician Offer and Report (VAPOR) trial and consisted of 3 components: brief physician-delivered advice, referral to telephone-based counseling, and nicotine replacement therapy. The goal of this study is to evaluate the results of this intervention. MET

Ticagrelor and Aspirin or Aspirin Alone in Acute Ischemic Stroke or TIA.

BACKGROUND: Trials have evaluated the use of clopidogrel and aspirin to prevent stroke after an ischemic stroke or transient ischemic attack (TIA). In a previous trial, ticagrelor was not better than aspirin in preventing vascular events or death after stroke or TIA. The effect of the combination of ticagrelor and aspirin on prevention of stroke has not been well studied. METHODS: We conducted a randomized, placebo-controlled, double-blind trial involving patients who had had a mild-to-moderate acute noncardioembolic ischemic stroke, with a National Institutes of Health Stroke Scale (NIHSS) s

Arterial reconstruction with human bioengineered acellular blood vessels in patients with peripheral arterial disease.

OBJECTIVE: Vascular conduit is essential for arterial reconstruction for a number of conditions, including trauma and atherosclerotic occlusive disease. We have developed a tissue-engineered human acellular vessel (HAV) that can be manufactured, stored on site at hospitals, and be immediately available for arterial vascular reconstruction. Although the HAV is acellular when implanted, extensive preclinical and clinical testing has demonstrated that the HAV subsequently repopulates with the recipient's own vascular cells. We report a first-in-man clinical experience using the HAV for arterial r

Evolving strategies for the management of venous thoracic outlet syndrome.

OBJECTIVE: Traditional management of venous thoracic outlet syndrome (VTOS) has involved catheter-directed thrombolysis (CDT) followed by transaxillary or paraclavicular (PC) first rib resection. More recently, we have adopted an infraclavicular (IC) approach for first rib resection and five other strategies to treat these patients. We report our evolving experience with the treatment of acute VTOS. METHODS: We reviewed our prospectively maintained database to identify patients treated for VTOS. Our strategy includes CDT with pharmacomechanical thrombectomy, IC first rib resection during the

Presentation and Management of Inferior Vena Cava Thrombosis.

BACKGROUND: Inferior vena cava thrombosis (IVCT), although rare, has a potential for significant morbidity and mortality. IVCT is often a result of IVC filter thrombosis, but it can also occur de novo. Although anticoagulation remains the standard of care, endovascular techniques to restore IVC patency have become key adjunctive therapies in recent years. This study examines a single-center experience with diagnosis and management of IVCT. METHODS: A retrospective Institutional Review Board-approved review of a single-center institutional database was screened to identify IVCT thrombosis usin
