Anatomical comparison across heads, fore- and hindlimbs in mammals using network models.

Animal body parts evolve with variable degrees of integration that nonetheless yield functional adult phenotypes: but, how? The analysis of modularity with Anatomical Network Analysis (AnNA) is used to quantitatively determine phenotypic modules based on the physical connection among anatomical elements, an approach that is valuable to understand developmental and evolutionary constraints. We created anatomical network models of the head, forelimb, and hindlimb of two taxa considered to represent a 'generalized' eutherian (placental: mouse) and metatherian (marsupial: opossum) anatomical confi

National and State-Level Trends in Nontraumatic Lower-Extremity Amputation Among U.S. Medicare Beneficiaries With Diabetes, 2000-2017.

OBJECTIVE: Diabetes is a leading cause of nontraumatic lower-extremity amputation (NLEA) in the U.S. After a period of decline, some national U.S. data have shown that diabetes-related NLEAs have recently increased, particularly among young and middle-aged adults. However, the trend for older adults is less clear. RESEARCH DESIGN AND METHODS: To examine NLEA trends among older adults with diabetes (≥67 years), we used 100% Medicare claims for beneficiaries enrolled in Parts A and B, also known as fee for service (FFS). NLEA was defined as the highest-level amputation per patient per calendar

Congenital limb deficiency in Japan: a cross-sectional nationwide survey on its epidemiology.

BACKGROUND: Congenital limb deficiency is a rare and intractable disease, which impairs both function and appearance of the limbs. To establish adequate medical care, it is necessary to reveal the actual conditions and problems associated with this disease. However, there have been no extensive epidemiological surveys in Japan addressing this disease. This is the first nationwide epidemiological survey of congenital limb deficiency in this country. METHODS: With the cooperation of epidemiology experts, we performed a two-stage nationwide survey to estimate the number of patients with congenit

Canagliflozin and Cardiovascular and Renal Events in Type 2 Diabetes.

Background Canagliflozin is a sodium-glucose cotransporter 2 inhibitor that reduces glycemia as well as blood pressure, body weight, and albuminuria in people with diabetes. We report the effects of treatment with canagliflozin on cardiovascular, renal, and safety outcomes. Methods The CANVAS Program integrated data from two trials involving a total of 10,142 participants with type 2 diabetes and high cardiovascular risk. Participants in each trial were randomly assigned to receive canagliflozin or placebo and were followed for a mean of 188.2 weeks. The primary outcome was a composite of deat

Second transcatheter closure for residual shunt following percutaneous closure of patent foramen ovale.

AIMS: Residual shunt following percutaneous patent foramen ovale (PFO) closure has been described in up to 49% of patients and is associated with recurrent cerebrovascular events. Our aim was to evaluate the safety, feasibility, and midterm outcomes of transcatheter residual shunt closure. METHODS AND RESULTS: From 1994 to July 2016, 2,679 patients underwent transcatheter PFO closure for treatment of presumed paradoxical embolism at our institution. Among them, 100 patients (3.7%) were referred for residual shunt closure. They constituted the study population for which a retrospective analysi

Peripheral nerve crushing to relieve chronic pain in diabetic and ischaemic foot ulcers.

OBJECTIVE: We retrospectively assessed the effectiveness of peripheral nerve crushing (Smithwick operation) in relieving intractable chronic pain associated with foot ulcers caused by diabetes mellitus (DM) or atherosclerosis. METHOD: From April 2009 to April 2012, patients underwent peripheral nerve crushing in the leg affected by foot ulceration. The cause of ulceration was either DM alone, atherosclerosis alone, or both DM and atherosclerosis. Because sensation in the foot is associated with five nerves: the tibial, deep peroneal, superficial peroneal, sural, and saphenous, one or more of

Concept of the central clip: when to use one or two MitraClips®.

AIMS: Percutaneous edge-to-edge mitral valve repair with the MitraClip(®) was shown to be a safe and feasible alternative compared to conventional surgical mitral valve repair. We analyse the concept of the central clip and the predictors for the need of more than one MitraClip(®) in our high-risk surgical population with severe mitral regurgitation (MR). METHODS AND RESULTS: Patients with severe MR (3 or 4+) and high operative risk (as defined by logistic EuroSCORE) refused for conventional mitral valve repair were considered for MitraClip(®). The procedure was performed under general anaest

Effects of low phytate barley (Hordeum vulgare L.) on zinc utilization in young broiler chicks.

Two 21-d experiments were conducted to evaluate the effects of low phytate barley (LPB) on Zn utilization by young broiler chicks and to determine the contribution of endogenous phytase, present in LPB. In the first experiment, ninety-six 1-d-old male chicks were assigned to a 2 x 3 factorial arrangement of treatments (4 pens of 4 chicks/treatment). Factors were barley type [wild-type barley (WTB) and LPB mutant M 955] and supplemental Zn (0, 10, or 20 mg of Zn/kg). In the second experiment, two hundred forty 1-d-old straight-run broiler chicks were assigned to a 2 x 2 x 3 factorial arrangemen

Isolated Flexor Hallucis Longus Weakness Caused by a Popliteal Ganglion Cyst Compressing the Tibial Nerve: A Case Report.

CASE: A 43-year-old woman presented with left foot numbness and progressive difficulty flexing the great toe. Examination revealed isolated flexor hallucis longus (FHL) weakness. Plain radiographs of the left knee showed no degenerative changes. MRI showed a multiloculated ganglion cyst in the left popliteal fossa with increased FHL signal intensity, suggestive of denervation edema. Cyst aspiration resulted in immediate FHL recovery, but the cyst recurred 3 months later, and complete recovery was achieved after surgical excision. At the 3-year follow-up, the patient remained symptom-free with

From ileum to endocardium: a case report of Yersinia pseudotuberculosis prosthetic valve infective endocarditis.

BACKGROUND: We report a case of Yersinia pseudotuberculosis prosthetic valve infective endocarditis (IE) in a 70-year-old woman. To date, Y. pseudotuberculosis IE has not been described in the literature. CASE SUMMARY: The patient presented with sepsis and a history of acute gastroenteritis, on a background of a long-standing mechanical mitral valve replacement (MVR) for rheumatic valve disease. Initial blood cultures grew Y. pseudotuberculosis, and a trans-oesophageal echocardiogram (TOE) demonstrated a mobile lesion suggestive of vegetations on the atrial surface of the MVR. Ilizarov-assisted periosteal distraction for refractory arteriovenous malformation ulcers: first clinical report in Asia.

Refractory ulcers caused by high-flow arteriovenous malformations (AVMs) pose significant therapeutic challenges due to persistent tissue ischemia and shear stress-induced graft failure. Traditional embolization or flap reconstruction strategies often yield suboptimal outcomes, particularly in weight-bearing regions. We present a 28-year-old female with a non-healing dorsal foot AVM ulcer despite multiple embolizations and radical toe amputations. Genetic testing revealed a KRAS mutation, confirming a somatic etiology of AVM. After a debridement and local flap repair, we employed Ilizarov-assi

Hidden Agony: Foot Pain Linked to Pet Hair Splinter.

Cutaneous pili migrans is a rare and interesting dermatological condition characterized by embedding a hair or its fragment in the epidermis or superficial dermis, sometimes leading to pain and a serpiginous rash. We present a 28-year-old male who came to the clinic concerned about the sudden onset of pain in his foot over 10 hours. Upon meticulous physical examination, we found a white hair embedded in the skin of the fifth toe of his left foot, which the patient immediately recognized as hair from his dog. Upon removal of the hair, the pain immediately resolved. We believe our case represent

Anomalous circumflex artery encircling the aortic annulus: implications for mitral valve repair.

Injury to coronary arteries during mitral surgery is a rare but life-threatening procedural complication, an anomalous origin and course of the left circumflex artery (LCx) increase this risk. Recognizing the anomaly by the characteristic angiographic pattern and identifying its relationship with the surrounding anatomical structure using imaging techniques, mainly transesophageal echocardiography (TOE) or coronary computed tomography angiography (CCTA), is of crucial importance in setting up the best surgical strategy. We report a case of anomalous origin of a circumflex artery (LCx) from the

Phosphaturic mesenchymal tumor: two cases highlighting differences in clinical and radiologic presentation.

Phosphaturic mesenchymal tumors are rare, usually benign neoplasms that occur in the soft tissue or bone and are the cause of nearly all cases of tumor-induced osteomalacia. Tumor-induced osteomalacia due to phosphaturic mesenchymal tumor is a challenging diagnosis to make-patients present with variable clinical and radiologic findings and the culprit neoplasm is often small and can occur anywhere head to toe. We present two cases of phosphaturic mesenchymal tumor in the scapular body and plantar foot. In both cases, the patient endured years of debilitating symptoms before a tissue diagnosis

Borjeson-Forssman-Lehmann Syndrome: Clinical Features and Diagnostic Challenges.

Borjeson-Forssman-Lehmann syndrome (BFLS) is an X-linked recessive disorder resulting from mutations in the PHF6 gene. The syndrome is characterized by short stature, obesity, hypogonadism, hypotonia, intellectual disability, distinctive facial features, fleshy ears, and finger and toe abnormalities. However, the diagnostic challenge in identifying BFLS remains a topic of interest. In this case report, we present the clinical characteristics of a proband with BFLS, highlighting the additional features of hypotonia, intellectual disability, and distinctive facial features. Botulinum Toxin Injection for the Treatment of Compression of Lateral Plantar Nerve by Flexor Digitorum Accessorius Longus.

The flexor digitorum accessorius longus is an anomalous muscle with a reported prevalence of 1.6%-12.2% in cadaveric studies. Flexor digitorum accessorius longus courses through the tarsal tunnel and has been reported as an etiology of tarsal tunnel syndrome in previous case reports. The flexor digitorum accessorius longus is intimately related to the neurovascular bundle and may impinge on the lateral plantar nerves. However, very few cases of lateral plantar nerve compression by the flexor digitorum accessorius longus have been reported. Herein, we report a case of lateral plantar nerve comp

Retrospective View of Four Cases of Cutaneous Manifestations in COVID-19 Patients in an Acute Care Setting.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which is the etiologic agent of coronavirus disease 2019 (COVID-19), causes an excessive inflammatory response and hemostatic abnormalities in the lungs, kidney, and skin. Four patients with COVID-19 admitted to an acute care community hospital developed nonblanchable purpuric macules, patches, and retiform purpura-like lesions at the sacrum, buttocks, lower extremities, and upper back. These lesions can be misdiagnosed as deep tissue pressure injuries. One patient also developed a vesicular-like rash at the upper back and another o

Fatigue Property and Small Crack Propagation Mechanism of MIG Welding Joint of 6005A-T6 Aluminum Alloy.

In this study, metal inert gas welding (MIG) was applied to 4 mm thick 6005A-T6 aluminum alloy welding. Compared with other parts, the hardness of the weld zone (WZ) was the lowest, about 67 HV. There was the Softening in WZ, which might make WZ the weakest zone. Then, fatigue tests were carried out on MIG welded joints. All the fatigue specimens fractured at the weld toe of the lap joint, and the fracture was characterized by a cleavage fracture. Crack closure induced by oxide was observed during the steady propagation of the fatigue crack. Impurities hindered crack propagation, changed the d
