Primary use of silicone oil tamponade in the management of perforating globe injury secondary to inadvertent local anaesthesia injection for ophthalmic surgery.

Perforating and penetrating globe injuries secondary to peribulbar and retrobulbar anaesthesia are often complicated by vitreous haemorrhage and retinal detachment. We describe the effectiveness of primary silicone oil tamponade in the repair of three perforated globes secondary to local anaesthesia for ophthalmic surgery. Three patients with axial myopia had peribulbar and retrobulbar anaesthesia for extracapsular cataract extraction (two patients) and cryotherapy (one patient). All eyes sustained a vitreous haemorrhage obscuring the view to the fundus. Retinal detachments were detected by B-

Local/Regional Anesthesia Versus General Anesthesia in Phalanx Fractures/Dislocations.

Background: Traditionally, surgical repair of phalanx fractures was performed under general anesthesia. However, the emergence of regional and local anesthesia, otherwise known as Wide-awake Local Anesthesia No Tourniquet, provides an alternative approach where general anesthesia is undesirable. The choice of anesthetic approach resides with clinicians, though it is important to factor in the evidence that regional/local provides not only an alternative anesthesia approach but also potentially avoids comorbidities associated with general anesthesia. This study hypothesizes that the use of loca

Effect of Perioperative Intravenous Lidocaine on Rebound Pain After Interscalene Brachial Plexus Block for Arthroscopic Rotator Cuff Repair: A Randomized Placebo-Controlled Trial.

BACKGROUND: Rebound pain after single-shot interscalene brachial plexus block may diminish the early analgesic benefit of regional anesthesia after shoulder arthroscopy. We evaluated whether perioperative intravenous lidocaine reduces rebound pain after arthroscopic rotator cuff repair. METHODS: In this single-center randomized, placebo-controlled Phase II trial, 120 adults receiving interscalene block plus general anesthesia were allocated 1:1 to saline or lidocaine. Lidocaine was given as a 1 mg/kg ideal-body-weight loading dose before induction followed by 2 mg·kg-1·h-1 from skin incision

Modified Yoong Ultrasound-Guided Rotator Interval Injection for Adhesive Capsulitis: A 2-Year Follow-Up.

OBJECTIVE: Adhesive capsulitis (AC) is characterized by progressive fibrosis of the glenohumeral capsule, with the rotator interval (RI) as a key pathological site. Ultrasound-guided hydrodilation of the RI (Yoong technique) is an established intervention, but traversal of a thickened coracohumeral ligament (CHL) can be technically challenging and painful. This study reports preliminary 2-year outcomes of a modified technique targeting the subacromial-subdeltoid (SASD) bursa at the RI, combined with immediate postinjection manipulation. METHODS: A retrospective case series was conducted on 13

A Retrospective Cohort Assessment of Labor Epidural Catheter Replacement With Programmed Intermittent Epidural Bolus and Continuous Epidural Infusion.

BACKGROUND: Labor epidural analgesia (LEA) catheter failure and replacement is a common problem in obstetrical anesthesia. The relationship between LEA catheter replacement and labor analgesia maintenance regimen is not well defined. The primary study objective was to investigate whether maintenance with programmed intermittent epidural boluses (PIEB) is associated with fewer epidural catheter replacements than continuous epidural infusions (CEI). A secondary study objective was to investigate whether initiation of LEA by the combined spinal epidural (CSE) technique was associated with fewer c

Butorphanol decreased the median effective concentration of ropivacaine in ultrasound-guided interscalene brachial plexus block.

BACKGROUND: Combining butorphanol with local anesthesia can enhance the analgesic effect of local anesthesia and prolong the duration of action for nerve block anesthesia. In this study, we aimed to determine the effects of butorphanol on the median effective concentration (EC50) of ropivacaine in ultrasound-guided interscalene brachial plexus block (ISB). METHODS: This is a randomized, double-blind, up-and-down sequential allocation study. Fifty patients undergoing arthroscopic shoulder surgery under general anesthesia were randomly assigned to the ropivacaine group or the butorphanol group

Combined iPACK and adductor canal block versus two-level erector spinae plane block in elderly patients undergoing total knee arthroplasty: a randomized, triple-blinded clinical trial.

BACKGROUND: Effective postoperative pain management after total knee arthroplasty (TKA) in elderly patients is challenging because of their increased susceptibility to opioid-related adverse events. Motor-sparing regional anesthesia techniques such as the adductor canal block (ACB), the interspace between the popliteal artery and the posterior capsule of the knee (iPACK) block, and the erector spinae plane block (ESPB) may improve analgesia while preserving quadriceps strength and modulating the systemic stress response. METHODS: In this single-center, prospective, randomized, triple-blinded t

Combined erector spinae plane and serratus posterior superior interfascial plane blocks for comprehensive pain management in breast surgery: A potential route to pectoral nerve coverage.

Postoperative pain management in breast cancer surgery remains challenging, particularly in procedures involving submammary tissue expander placement, where patients often report inadequate pain control. The complex innervation of the breast region, involving intercostal nerves (T2-T6), brachial plexus branches, and supraclavicular nerves (C3-C4), necessitates comprehensive anaesthetic coverage. We present four cases utilising a novel combined approach of erector spinae plane (ESP) and serratus posterior superior interfascial plane (SPSIP) blocks for perioperative pain management. The techniqu

Perioperative intravenous lidocaine for gastrointestinal recovery and pain control in elective colorectal surgery: a systematic review and updated meta-analysis of randomized controlled trials.

BACKGROUND: Minimally invasive colorectal surgery, including laparoscopic and robotic techniques, has enhanced perioperative safety and recovery. However, postoperative gastrointestinal dysfunction remains frequent, delaying discharge and impairing outcomes. Intravenous lidocaine, owing to its analgesic and anti-inflammatory properties, has been proposed to improve recovery, but evidence remains inconsistent, especially after the retraction of a major trial. This updated systematic review and meta-analysis assessed the effects of perioperative intravenous lidocaine on gastrointestinal and clin

Perioperative 24-Hour Lidocaine Infusion Does Not Prevent Chronic Postthoracotomy Pain: A Triple-Blinded Randomized Controlled Trial.

BACKGROUND: Managing chronic postsurgical pain is challenging. Evidence supporting the use of perioperative lidocaine to prevent it is inconsistent, potentially due to insufficient infusion duration. OBJECTIVES: To determine the effect of extending lidocaine infusion to 24 hours for treating chronic postsurgical pain in patients undergoing video-assisted thoracoscopic surgery. STUDY DESIGN: Triple-blind, controlled randomized trial. SETTING: A tertiary center in a university hospital. METHODS: Seventy-one patients undergoing elective video-assisted thoracoscopic surgery were randomized 1:1

Quantifying the Phoenix Sign: A Double-Blind Pilot Study on Lidocaine and Papaverine-Induced Vascular and Motor Changes After Common Peroneal Nerve Block in Diabetic Neuropathy.

BACKGROUND: Focal entrapment of the Common Peroneal (Fibular) nerve (CPN) is the most frequent lower-extremity nerve entrapment yet it can be difficult to diagnose clinically. The Phoenix Sign-an increase in extensor hallucis longus (EHL) motor strength following lidocaine injection-may assist diagnosis. Additional observed effects include improved arterial perfusion and Doppler waveforms. METHODS: In this double-blinded, randomized small pilot study, only four patients (N = 4) with diabetic peripheral neuropathy underwent bilateral peripheral nerve blocks with lidocaine or papaverine. Gelatin methacryloyl/hyaluronic acid hydrogel for localized ropivacaine delivery alleviates neuropathic pain and supports functional recovery after spinal cord injury.

Neuropathic pain caused by spinal cord injury severely compromises patients' quality of life. The clinical application of ropivacaine is limited by its short duration of action and the significant side effects associated with repeated administration. In this study, we developed a Gelatin methacryloyl/hyaluronic acid-based hydrogel (Ropi-GelMA/HA) to enable localized and controlled delivery of ropivacaine by photo-crosslinking. In a rat model of spinal cord contusion, Ropi-GelMA/HA was associated with lower Nav1.3 and TNF-α expression and higher NGF and BDNF expression, together with improved m

Effect of Ultrasound-Guided Lateral Infraclavicular Brachial Plexus Block With Lidocaine or Ropivacaine for Closed Reduction of Distal Radius Fractures: A Randomized Controlled Noninferiority Trial.

BACKGROUND: Closed reduction of distal radius fractures is painful, and current analgesic strategies may be inadequate. Ultrasound-guided lateral infraclavicular brachial plexus block may offer complete analgesia and muscle relaxation, potentially improving patient comfort and reduction quality. However, benefits and challenges regarding anesthetic agents for this procedure remain unclear. METHODS: In this randomized, controlled, blinded, noninferiority trial, 63 adults with distal radius fractures requiring closed reduction received a lateral infraclavicular block with either 30 mL of ropiva

Ultrasound-guided infraspinatus-teres minor plane block for acute post traumatic shoulder pain in the emergency department: A case series.

Acute traumatic shoulder injuries are often associated with severe pain that hinders examination, radiographic positioning, and timely orthopaedic management. Conventional systemic analgesia may be inadequate, while interscalene and superior trunk blocks can be limited by hemidiaphragmatic paresis. The ultrasound-guided infraspinatus teres minor (ITM) inter-fascial plane block is a recently described diaphragm-sparing technique that targets the suprascapular and axillary nerves through a single posterior injection and has shown promising anatomic and postoperative analgesic coverage in earlier

Relief increases select shoulder muscle activity, following subacromial injection in subacromial pain syndrome.

Subacromial pain syndrome (SAPS) is commonly linked to disrupted neuromuscular function, yet the effects of pain reduction on shoulder muscle activity remains unclear. This study examined how immediate and long-term (six weeks) decreases in shoulder pain influence muscle activation during humeral elevation in individuals with SAPS. Twenty-one participants with clinically diagnosed SAPS completed three sessions of humeral elevation at: baseline (T1), immediately after a subacromial injection (T2), and after six weeks of physical therapy (T3). Electromyography (EMG) was recorded from eight shoul

Functional recovery after trigger point injection in pediatric anterior cutaneous nerve entrapment syndrome: clinical implications for multidisciplinary management.

UNLABELLED: Anterior Cutaneous Nerve Entrapment Syndrome (ACNES) is an under-recognized cause of chronic abdominal pain in children. Although trigger point injections (TPI) provide pain relief, their relationship with functional recovery remains unclear, particularly in patients with coexisting comorbidities. This study aimed to evaluate the association between pain relief and functional recovery in pediatric ACNES, stratified according to comorbid conditions. This study was a retrospective cohort study. We retrospectively reviewed 27 pediatric patients with ACNES who underwent TPI between 202

King's Mill anterior-posterior and transverse versus paracervical block in operative outpatient hysteroscopy: a randomised trial.

BACKGROUND: Pain control during operative outpatient hysteroscopy remains a clinical challenge, with the paracervical block (PCB) showing inconsistent efficacy. The King's Mill Anterior-Posterior and Transverse (KAPT) block targets the Lee-Frankenhauser plexus within the uterosacral ligament complex, where the highest density of uterine and cervical sensory fibres lies. We compared the KAPT block with the PCB in operative outpatient hysteroscopy. METHODS: Single-centre, participant- and outcome assessor-blinded randomised controlled trial in a UK district general hospital. Forty-eight women u

Low-dose perineural dexamethasone in supraclavicular brachial plexus blocks: Optimising postoperative analgesia.

BACKGROUND: Orthopaedic procedures often cause intense postoperative pain, posing challenges for effective management. Brachial plexus blocks offer relief but optimising analgesia with minimal local anaesthetic is still challenging. Perineural dexamethasone, with anti-inflammatory effects, shows promise in lower doses but lacks sufficient research. OBJECTIVE: The study aims to assess low-dose perineural dexamethasone in ultrasonography-guided brachial plexus blocks for extending analgesia and reducing opioid use in upper limb surgeries. METHODS: Double-blinded trial on 90 American Society of
