Endoscopic papillary large balloon dilation for the removal of bile duct stones.

Endoscopic papillary large balloon dilation (EPLBD) with endoscopic sphincterotomy (EST) has been widely used as the alternative to EST along with endoscopic mechanical lithotripsy (EML) for the removal of large or difficult bile duct stones. Furthermore, EPLBD without EST was recently introduced as its simplified alternative technique. Thus, we systematically searched PubMed, Medline, the Cochrane Library and EMBASE, and analyzed all gathered data of EPLBD with and without EST, respectively, by using a single standardized definition, reviewing relevant literatures, published between 2003 and

Black pleural effusion.

BACKGROUND: Black pleural effusions are extremely rare and have been reported in patients with infection, malignancy, and hemorrhage. However, no review articles appear to have focused on this rare clinical presentation. PURPOSE: To classify and characterize diseases causing "black pleural effusion" based on the pathophysiological mechanisms involved. METHODS: We searched the medical literature to find reports of "black pleural effusion" using the PubMed database. RESULTS: We identified 8 cases and classified the underlying diseases into the following 4 entities based on pathophysiological

Biliary phytobezoar resulting in intestinal obstruction.

Phytobezoar is the most common type of bezoar. It is composed of indigestible vegetable matter and is usually found in the stomach. Biliary phytobezoar is extremely rare and difficult to diagnose preoperatively. The pathogenesis is not clear, and there have been only a few reports of biliary bezoars associated with sphincteric impairment at the ampulla of Vater. Here, we present a report of biliary bezoar that resulted in jejunal obstruction. We were unable to identify the bezoar in the extrahepatic bile duct until it obstructed the small bowel lumen. To our knowledge, this is the first report

Delayed pancreatic fistula after pancreaticoduodenectomy. A case report.

CONTEXT: Pancreatic fistulas still occur despite refinements in both surgical technique and perioperative management after a pancreaticoduodenectomy. However, the occurrence of delayed pancreatic fistulas is very rare. CASE REPORT: A 62-year-old woman diagnosed with a lower biliary carcinoma had undergone a pylorus-preserving pancreaticoduodenectomy. The postoperative course was uneventful and the patient was discharged from the hospital after 16 days. One year after surgery, she was admitted to our hospital with a fluid collection around the pancreaticojejunal anastomosis. A catheter was ins

Indications and results for transduodenal sphincteroplasty in the era of endoscopic sphincterotomy.

BACKGROUND: Transduodenal sphincterotomy (TS) has fallen into disuse since endoscopists developed techniques to treat sphincter problems nonsurgically. However, some patients experience recurrent sphincter strictures after endoscopic sphincterotomy (ES), with the ampulla endoscopically inaccessible, and pancreas divisum (PD); these patients are referred to a surgeon because they are unsuitable for ES. METHODS: The medical records of patients who underwent TS at the First Department of Surgery of the Medical School, University of Rome "La Sapienza," between January 1997 and December 2005 were

Long-term outcome after partial external biliary diversion for progressive familial intrahepatic cholestasis.

BACKGROUND: Though patients with progressive familial intrahepatic cholestasis (PFIC) typically require liver transplantation, initial surgical treatment includes partial biliary diversion (PBD) to relieve jaundice-associated pruritus. This study was undertaken to describe long-term PFIC outcome data, which are currently sparsely reported. METHODS: Retrospective review of 7 patients diagnosed with PFIC who underwent PBD between 2004 and 2008 was directed toward long-term postoperative outcome including resolution of jaundice/pruritus, stoma complications, interval to transplantation, and deat

Laparoscopic splenectomy and/or cholecystectomy for children with sickle cell disease.

BACKGROUND: In 1991, Delaitre reported the first laparoscopic splenectomy (LS). Since then LS has become the procedure of choice to treat hematological diseases requiring splenectomy. The Eastern province of Saudi Arabia is known to have a high incidence of hemoglobinopathies including sickle cell disease (SCD), which is known to be associated with complications necessitating splenectomy and/or cholecystectomy. This report describes our experience with LS and/or laparoscopic cholecystectomy (LC) for children with SCD. PATIENTS AND METHODS: The medical records of all children with SCD who had

Laparoscopic resection of type I choledochal cyst in an adult and Roux-en-Y hepaticojejunostomy: a case report and literature review.

Choledochal cysts are rare cystic dilatations of the extrahepatic biliary tree, the intrahepatic bile ducts, or both and carry a substantial risk of malignant transformation. Type I choledochal cysts, which involve the entire common hepatic and common bile ducts, represent 80% to 90% of these lesions. We report laparoscopic excision of symptomatic type I choledochal cyst in a 37-year-old woman, and review the literature. Laparoscopic excision of the extrahepatic biliary tree from the hepatic confluence to the anomalous pancreatobiliary junction with en bloc cholecystectomy and reconstruction w

Emergent biliary drainage for choledochal cyst.

Two patients with choledochal cyst who needed an emergent biliary drainage are presented. Case 1: Spontaneous rupture of the cyst was demonstrated by hepatobiliary scintigram in a 21-month-old girl and an emergent T-tube drainage relieved her symptom. Acute obstruction by protein plugs was considered to have caused rapid expansion of the cyst, which resulted in rupture eventually. Case 2: Marked hypoproteinemia was demonstrated in a 29-month-old boy with choledochal cyst. Bile drainage through Foley catheter promptly made the serum protein level elevated above normal range. Portal hypertension

Biliary lithiasis in early pregnancy and abnormal development of facial and distal limb bones (Binder syndrome): a possible role for vitamin K deficiency.

BACKGROUND: Binder syndrome is a maxillonasal dysostosis characterized by midface and nasal hypoplasia, sometimes associated with short terminal phalanges of fingers and toes and transient radiological features of chondrodysplasia punctata. Warfarin- or phenytoin-induced vitamin K deficiency during early pregnancy is a well-established etiology for this syndrome, which occurs nevertheless sporadically in most cases. CASE(S): We describe here the first case, to our knowledge, of Binder syndrome in a child whose mother presented with biliary lithiasis in early pregnancy. The mother proved to ha

Extended right hepatectomy with total caudate lobe resection and biliary tree resection for a large colorectal liver metastasis involving both the right and left hepatic lobes and the umbilical fissure: a case report.

Very large right-sided liver tumors may grow up to the base of the umbilical fissure and involve the left hepatic duct and can occasionally reach the bile duct confluence. This kind of involvement has often been considered a contraindication to resection. We report a patient who presented with a large hepatic metastasis from colorectal cancer that reached the umbilical fissure and involved the left hepatic duct just above the bile duct confluence. An extended right hepatectomy including complete resection of caudate lobe was performed. We resected the left and common hepatic ducts, as well as

Imaging diagnosis: biliopleural fistula in a dog.

A 3-year-old intact male Brittany Spaniel was admitted for thoracic gunshot wounds. Despite continued supportive care, the dog's condition deteriorated. Thoracic/abdominal ultrasound revealed a biliopleural fistula. Following fistula repair, the dog made a complete recovery. This case represented a diagnostic challenge because of the uncommon clinical presentation of a severe bile pleuritis in the absence of significant bile peritonitis. Biliopleural fistula is a rare condition but should be considered as a differential diagnosis in trauma patients with pleural effusion, especially those with

Palliative transhepatic biliary drainage and enteral nutrition.

Simultaneous intestinal and biliary obstruction is a rare but agonizing complication of metastatic abdominal cancer. Although endoscopic procedures exist that relieve jaundice or restore enteral nutrition, they can be impossible to perform for technical or anatomical reasons. We propose a palliative approach for these patients that includes transcutaneous common bile duct drainage, progressive dilation of the transhepatic channel over 1 wk, and, finally, insertion of a permanent silicon catheter that drains bile into the duodenum and is combined with an enteral feeding line. We report three pa

[The interrelationship of the intensity of free-radical oxidation to the level of serum bilirubin in lesions of the hepatobiliary system].

The level of lipid peroxidation (LPO) products increases in the blood of patients with chronic diseases of the liver and acute cholecystitis with mechanical jaundice. The values normalize after surgery for acute cholecystitis, whereas therapy for chronic diseases of the liver virtually does not correct them. These data demonstrate the relationship between intensity of free-radical oxidation and severity of hepatobiliary process and between plasma content of LPO products and level of serum bilirubin.

Dyspepsia in infants and children.

Pathological processes and diseases of the upper gastrointestinal tract have become increasingly recognized over recent years as childhood entities responsible for a variety of upper gastrointestinal symptoms previously labelled as functional or non-organic. The term 'dyspepsia' is an adult one whose definition requires clarification before use in the paediatric context, but it encompasses age-dependent symptoms such as feed-associated irritability in the infant, peri-umbilical pain in the younger child, and heart-burn, nausea, and indigestion in the older child as in adults. The possible orga

MR cholangiopancreatography at 0.5 T with a 3D inversion recovery turbo-spin-echo sequence.

The objective of this study was to assess the diagnostic value of magnetic resonance cholangiopancreatography (MRCP) at 0.5 T. The MRCP technique was performed in 28 patients with symptomatology referrable to the biliary system. A three-dimensional (3D) inversion recovery turbo-spin-echo (TSE) sequence was used to create 3D reconstructions of the bile ducts. Dilation of the biliary tree or pancreatic duct in 23 patients due to tumor, calculi, or strictures was depicted with excellent contrast resolution. The approximate level of obstruction and all calculi were accurately predicted by MRCP. In

Magnetic resonance cholangiopancreatography: a novel approach to the evaluation of suspected pancreaticobiliary neoplasms.

BACKGROUND: Magnetic resonance cholangiopancreatography (MRCP) is a new noninvasive diagnostic method for pancreaticobiliary (PB) imaging without endoscopy, sedation, or iodinated contrast. The purpose of this study was to evaluate the ability of MRCP to depict pancreatic and biliary ductal anatomy compared to that of endoscopic retrograde cholangiopancreatography (ERCP) and to evaluate the ability of MRCP to accurately diagnose PB neoplasms. METHODS: Twenty patients had MRCP, and 17 also had ERCP. All studies were read prospectively by experienced reviewers blinded to other imaging data. Pat

[Hepaticocholangiochole cystostomy plus construction of subcutaneous tunnel of the gallbladder in the treatment of hepatolithiasis with bile duct stricture].

A novel operation was designed by using Hartmann's pouch of the gallbladder to anastomose stone-strictured intrahepatic bile duct while leaving the common bile duct and Oddis sphincter intact and fixing the fundus of the bladder subcutaneously. 28 patients with intrahepatic bile duct stones and strictures underwent the operation successfully, and recovered well. There were no postoperative complications such as reflux cholangitis or disturbances of GI tract.
