Localized Contrast-Induced Encephalopathy of the Medulla Oblongata Following Endovascular Treatment of Posterior Circulation Lesions: A Report of Two Cases.

Contrast-induced encephalopathy (CIE) is a rare but serious complication of endovascular treatments. Contrast materials can disrupt the blood-brain barrier and subsequently cause encephalopathy. We herein report two cases. Case 1 is a 63-year-old woman presented with an unruptured right vertebral artery aneurysm. The patient underwent the stent-assisted coil embolization of the lesion and experienced dizziness, nystagmus, and numbness in the right upper limb after the procedure. T2-weighted images and fluid-attenuated inversion recovery sequence (FLAIR) images revealed a high-intensity lesion

Heart Failure With Preserved Ejection Fraction.

Heart failure is a complex clinical syndrome in which impaired ventricular filling and ejection of blood into circulation causes decreased stroke volume and cardiac output. Heart failure with preserved ejection fraction (50% or more) is the most common type of heart failure, and up to 1 in 10 adults will be affected in their lifetime. Clinical symptoms such as peripheral edema, dyspnea, and orthopnea, with clinical findings including jugular venous distention, third heart sound, and laterally displaced apical impulse should prompt consideration of heart failure. Laboratory values (eg, elevated

Bilateral Optic Disc Edema in the Course of Idiopathic Intracranial Hypertension: A Case Report.

Idiopathic intracranial hypertension (IIH) is a rare disease characterized by elevated cerebrospinal fluid pressure in the absence of structural changes in the brain. One of the main symptoms of IIH is deterioration of visual acuity with evidence of swelling of the optic nerve disc. Other complaints reported by patients may also include headaches, dizziness, and tinnitus. A 46-year-old female patient presented to the ophthalmology outpatient clinic because of bilateral visual acuity deterioration. The patient had previously been treated for hypertension and dizziness, and she also reported inc

Non-cerebral malaria: does such a thing exist?

Malaria, caused by Plasmodium spp., remains a major public health problem. Cerebral malaria is its deadliest form, with a 15-25% mortality rate, despite artemisinin-based treatments. In addition, the World Health Organization (WHO) strictly defines cerebral malaria as the presence of coma, 1 h after a seizure or the correction of hypoglycemia, in patients with P. falciparum parasitemia. Consequently, 25% of survivors experience neurocognitive and behavioral sequelae, particularly in children. However, more recently, neurocognitive and behavioral impairments were also reported in severe non-cer

Atypical telencephalic reflux pattern in a cavernous sinus dural AV fistula related to an anatomical variation of the basal vein of Rosenthal.

Cavernous sinus dural arteriovenous fistula can cause cerebral edema and hemorrhage due to cortical venous reflux and congestion. Understanding complex venous reflux and drainage routes is crucial for treatment planning. Here, we present a case of a cavernous sinus dural arteriovenous fistula with cortical venous reflux via two separate terminations of the telencephalic veins caused by an aplastic basal vein of Rosenthal. The patient presented with diplopia and eye redness and was diagnosed with a Cognard type IIa + b cavernous sinus dural arteriovenous fistula. The shunt was supplied by the d

Encephalitis and cytokine storm secondary to respiratory viruses in children: Two case reports.

INTRODUCTION: Encephalitis is a syndrome characterized by brain damage secondary to an inflammatory process that is manifested by cognitive impairment and altered cerebral spinal fluid analysis; it may evolve with seizures and coma. Despite viral infections representing the main cause of encephalitis in children, respiratory syncytial virus (RSV) and parainfluenza virus are mostly associated with respiratory presentations. Uncommonly, the inflammatory phenomena from encephalitis secondary to viral agents may present with an exacerbated host response, the so-called cytokine storm. The link betw

TRPM4 Blocking Antibody Protects Cerebral Vasculature in Delayed Stroke Reperfusion.

Reperfusion therapy for acute ischemic stroke aims to restore the blood flow of occluded blood vessels. However, successful recanalization is often associated with disruption of the blood-brain barrier, leading to reperfusion injury. Delayed recanalization increases the risk of severe reperfusion injury, including severe cerebral edema and hemorrhagic transformation. The TRPM4-blocking antibody M4P has been shown to alleviate reperfusion injury and improve functional outcomes in animal models of early stroke reperfusion. In this study, we examined the role of M4P in a clinically relevant rat m

Deferoxamine ameliorates neurological dysfunction by inhibiting ferroptosis and neuroinflammation after traumatic brain injury.

Traumatic brain injury (TBI) is an important reason of neurological damage and has high morbidity and mortality rates. The secondary damage caused by TBI leads to a poor clinical prognosis. According to the literature, TBI leads to ferrous iron aggregation at the site of trauma and may be a key factor in secondary injury. Deferoxamine (DFO), which is an iron chelator, has been shown to inhibit neuron degeneration; however, the role of DFO in TBI is unclear. The purpose of this study was to explore whether DFO can ameliorate TBI by inhibiting ferroptosis and neuroinflammation. Here, our finding

Home Bioelectrical Impedance Analysis Management System in Patients With Heart Failure: Rationale and Study Design.

Body fluid monitoring and management are essential to control dyspnea and prevent re-hospitalization in patients with chronic heart failure (HF). There are several methods to estimate and monitor patient's volume status, such as symptoms, signs, body weight, and implantable devices. However, these methods might be difficult to use for reasons that are slow to reflect body water change, inaccurate in specific patients' condition, or invasive. Bioelectrical impedance analysis (BIA) is a novel method for body water monitoring in patients with HF, and the value in prognosis has been proven in prev

Persistent Reverse End Diastolic Flow in Fetal Middle Cerebral Artery: A Rare Finding with Poor Outcomes.

Doppler findings of persistent reverse end-diastolic flow (PREDF) in a fetal middle cerebral artery (MCA) are a very rare sonographic finding and are a marker of poor fetal condition. This finding often leads to intrauterine fetal death or early neonatal death. Reverse end-diastolic flow in the middle cerebral artery is an advanced hemodynamic event. Fetal cerebral circulation normally has a high impedance; in the event of fetal hypoxemia, impedance decreases, resulting in the central redistribution of blood flow to vital organs, which maintains the oxygen delivery to the brain. Reverse flow i

Modulating Expression of Endogenous Interleukin 1 Beta in the Acute Phase of the Pilocarpine Model of Epilepsy May Change Animal Survival.

The pilocarpine-induced (PILO) model has helped elucidate the electrophysiological and molecular aspects related to mesial temporal lobe epilepsy. It has been suggested that the extensive cell death and edema observed in the brains of these animals could be induced by increased inflammatory responses, such as the rapid release of the inflammatory cytokine interleukin 1 beta (Il1b). In this study, we investigate the role of endogenous Il1b in the acute phase of the PILO model. Our aim is twofold. First, we want to determine whether it is feasible to silence Il1b in the central nervous system us
