Sigmoid Volvulus in a Young Adult With Cerebral Palsy: A Case Managed by the Paul-Mikulicz Procedure.

A 25-year-old male with cerebral palsy presented with sigmoid volvulus complicated by ischemia and fecal impaction-an underrecognized but serious risk in this population. Emergency Paul-Mikulicz colostomy achieved safe resection. Clinicians should suspect volvulus early in CP patients, and proactive bowel management is essential.

Non-invasive prenatal screening in practice: an audit of clinical management in high-probability cases.

Non-invasive prenatal screening (NIPS) is widely recommended for fetal chromosomal aneuploidies. This study describes patterns of NIPS requests, including follow-up and post-test genetic counselling, and discusses how these compare to clinical guidelines in Australia and New Zealand. We performed a retrospective analysis of increased-probability NIPS cases reported by a private pathology provider from April 2022 to May 2024. Descriptive statistics were used to summarise referral patterns, the type of anomaly detected, uptake of complimentary genetic counselling, and diagnostic follow-up. Of th

The Importance of Minority Stress and Intersectionality for Cancer Symptom Management: The Case of Cancer-Related Pain.

Although research demonstrates that cancer survivors who are sexual and gender minorities (SGM), racial and ethnic minorities, or of lower socioeconomic status experience a higher prevalence of cancer-related pain, limited studies have examined how these intersecting marginalized identities and additional social experiences interconnect and affect pain outcomes. In their recent study, Boehmer and colleagues conducted an intersectional analysis of cancer-related pain among SGM adults and other marginalized backgrounds. Using data from the Behavioral Risk Factor Surveillance System, they found t

Tibialis Anterior Muscle Herniation Managed with Ovine Extracellular Matrix Patch: Case Report.

INTRODUCTION: Muscle herniation occurs when muscle protrudes through a defect in its deep fascia. The tibialis anterior is most frequently involved and symptomatic cases present with an activity-dependent bulge and pain. Dynamic ultrasonography (USG) confirms the diagnosis while magnetic resonance imaging can delineate the defect and exclude other differentials. CASE REPORT: A 35-year-old male had exertional anterior shin pain with a reducible swelling over mid-tibialis anterior that accentuated on dorsiflexion. Dynamic USG confirmed a focal fascial defect with herniation. After failed conser

Surgical Management of Sporadic Pancreatic Insulinomas in a Resource-Limited Setting: A Report of Two Cases.

Insulinoma is the most common functional pancreatic neuroendocrine tumor, typically benign, solitary, and sporadic, with an estimated incidence of 1-4 cases per million per year worldwide. It causes recurrent hypoglycemia through inappropriate insulin secretion, classically presenting as Whipple's triad and confirmed by a prolonged supervised fasting test. Surgical enucleation remains the preferred treatment for small, well-localized benign insulinomas, allowing maximal preservation of pancreatic parenchyma. We report two cases of sporadic pancreatic insulinoma managed by open enucleation at a

Strangulated left paraduodenal hernia with jejunal necrosis managed in a field hospital: a case report.

Left paraduodenal hernia is the most common congenital internal hernia and a rare cause of acute small-bowel obstruction. Delay in diagnosis may lead to strangulation and bowel necrosis requiring urgent surgery. We report the case of a 30-year-old man presenting to a field hospital with sudden severe abdominal pain, vomiting, tachycardia, and generalized peritonism. Abdominal computed tomography demonstrated a cluster of jejunal loops in the left upper quadrant beneath the superior mesenteric vein, consistent with left paraduodenal hernia and closed-loop obstruction. Emergency midline laparoto

Vaping-associated acute respiratory distress syndrome managed with VV-ECMO: a case report.

Electronic cigarettes (e-cigarettes) have been increasingly promoted as safer alternatives to conventional smoking and as smoking cessation tools. However, emerging evidence has linked e-cigarette use to serious pulmonary complications, including e-cigarette or vaping product associated lung injury (EVALI). Severe cases may progress to acute respiratory distress syndrome (ARDS), requiring advanced respiratory support. We report the case of a previously healthy 25-year-old female who developed severe vaping-associated ARDS refractory to conventional mechanical ventilation and was successfully m

Impact of 18F-FDG-PET/CT in Managing a Case of Fungal Prosthetic Valve Endocarditis.

Approximately 0.1% of all prosthetic cardiac valves are affected by fungal endocarditis (bacterial endocarditis being the most common cause), which has a high case fatality rate. Post-treatment clinical improvement and a negative blood culture do not definitively rule out the presence of residual active disease. Among imaging techniques, trans-oesophageal echocardiography has higher sensitivity than transthoracic echocardiography, but has its own limitations. Functional imaging with 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) has shown to have high

Asymmetry to Symmetry: A Case Report of Mandibular Asymmetry Managed With Distraction Osteogenesis Followed by Genioplasty.

Mandibular asymmetry is a facial deformity that affects both function and appearance. It may result from developmental disturbances, trauma, or temporomandibular joint pathology. Distraction osteogenesis (DO) offers a biological method for gradual bone elongation along with simultaneous adaptation of surrounding soft tissues. Genioplasty further refines chin position and facial balance. This report describes the management of a young adult patient presenting with facial asymmetry due to unilateral mandibular hypoplasia. Clinical and radiographic examination revealed reduced ramal height on the

Congenital Hyperinsulinism in Neonates: Diagnostic Challenges and Management in Two Cases With KCNJ11 and ABCC8 Mutation.

We report two cases of persistent neonatal hypoglycemia associated with mutations in the KCNJ11 and ABCC8 genes, encoding the Kir6.2 and SUR1 subunits of the adenosine triphosphate-sensitive potassium channel. In both cases, symptoms were non-specific and initially attributed to other conditions, including suspected infection. Standard treatment with enteral and parenteral glucose infusions failed to restore euglycemia. Diazoxide was administered without a clinical response. Both patients were then treated with octreotide, resulting in stabilization of glycemic levels. Genetic testing confirme

Management of a Pilon Fracture Complicated by a Bent Plate in Situ with Nonoperative Correction: A Case Report.

CASE: Hardware complications in the perioperative period of a pilon fracture open reduction internal fixation or fusion procedure present a unique challenge. Revision of hardware can lead to additional complications. In this case report, we share a technique of in situ plate bending to manage damaged hardware in a patient who underwent a primary tibiotalar arthrodesis for a pilon fracture. CONCLUSION: Preoperative planning, protection of the soft tissue envelope, and use of the "F" tool are keys to the success of this treatment method. Proper technique allows for a noninvasive method for corr

Laparoscopic Management of an Intrauterine Device Perforating the Uterus and Adherent to the Rectum and Ovary: A Case Report.

Management of intrauterine device (IUD) perforation can be complex when adjacent pelvic organs are involved. Laparoscopic removal is commonly performed; however, cases involving bowel adherence may require intraoperative assistance from additional surgical services. We report the case of a 31-year-old gravida 2, para 2 (G2P2) woman with a history of cesarean delivery who presented with pelvic pain five weeks postpartum and was found to have an IUD perforating the posterior uterine wall, with adherence to the rectum and left ovary. The Complex Surgical Management of Vertebral Fracture in a Patient With Undiagnosed Ankylosing Spondylitis: A Case Report.

Ankylosing spondylitis (AS) is a chronic inflammatory disorder that primarily affects the spine, resulting in progressive stiffness. Structural spinal changes increase susceptibility to fractures, even from minor trauma. These fractures are often unstable and can lead to neurological complications, posing significant challenges in surgical management. We present the case of a 60-year-old man who developed quadriplegia following a fall. Imaging revealed a displaced T3 vertebral fracture with spinal cord compression and findings consistent with AS. The patient underwent surgical stabilization wi

Rectal melanoma: managing a rare cause of rectal bleeding: case report and review of the literature.

BACKGROUND: Anorectal melanoma is rare and has a poor prognosis. Symptoms are often non-specific, such as bloody diarrhea, anal pain or bowel habit changes. Diagnosis requires tissue sample with immunohistochemistry because of the difficulty of clinical differentiation from other tumor entities since many lesions are amelanotic. Many patients are diagnosed at an advanced stage, precluding surgical treatment. Owing to the rareness of the disease and the lack of data there are no clear treatment guidelines available. A 90-year-old female sought emergency care due to rectal bleeding and changes i

Urethral Duplication in Boys: A Novel Technique to Manage - Report of Seven Cases and Review of Literature.

AIMS AND OBJECTIVES: Urethral duplication is one of the rarest congenital anomalies that affects boys more than girls. It may be complete or incomplete with various types of presentation. Diagnosis is often confirmed with a micturating cystourethrogram. Usually, an open surgical procedure is performed as the standard mode of management. We have performed cystoscopic cauterization of the nondominant urethra as an alternative method of management. MATERIALS AND METHODS: We have performed this procedure in seven children with Effman type IIA anomalies over the period of the last 4 years (2020-20
