Enhanced antimicrobial de-escalation for pneumonia in mechanically ventilated patients: a cross-over study.

BACKGROUND: Antibiotics are commonly administered to hospitalized patients with infiltrates for possible bacterial pneumonia, often leading to unnecessary treatment and increasing the risk for resistance emergence. Therefore, we performed a study to determine if an enhanced antibiotic de-escalation practice could improve antibiotic utilization in mechanically ventilated patients with suspected pneumonia cared for in an academic closed intensive care unit (ICU). METHODS: This was a prospective cross-over trial comparing routine antibiotic management (RAM) and enhanced antimicrobial de-escalati

A brain abscess of probable odontogenic origin in a child with cyanotic heart disease.

The purpose of the present report was to present a rare case of a brain abscess in a child with heterotaxy syndrome, severe cardiac anomalies, and extensive dental caries. The pathogen was Streptococcus intermedius isolated from the cerebrospinal fluid. The source of the pathogen was probably an infection of a primary molar with a dentoalveolar abscess involving the bud of the permanent successor. After a long course of antibiotic regimens followed by a craniotomy with abscess drainage, a shunt, and comprehensive dental treatment, the patient was discharged from the hospital without any neurol

Isolated reversible splenial lesion in tick-borne encephalitis: a case report and literature review.

Here, we demonstrate a first case of tick-borne encephalitis (TBE) associated with an isolated reversible splenial corpus callosum lesion (IRSL) and highlight the wide range of different clinical entities in which such alterations have been observed. A 42-year-old man showed fever, cephalgia and mild disturbance of coordination and gait. Diagnosis was ascertained by slight CSF-pleiocytosis and positive TBE-IgG as well as by positive intrathekal specific antibody index on follow-up. MRI demonstrated a single ovoid hyperintensity in T2 and DWI with reduction in ADC in the splenium of corpus call

Streptococcal meningitis in adults: therapeutic outcomes and prognostic factors.

Thirty-eight patients with streptococcal meningitis, aged 17-75 years, have been identified over a period of 13.5 years. Among these 38 patients, 35 had community-acquired infections, and the other three had nosocomial infections. Twelve of the 38 patients were found to have postneurosurgical forms and 26 to have spontaneous forms. These 38 cases of streptococci included Streptococcus (S.) pneumoniae in 19 cases, viridans group streptococci in 13, non-A, non-B, and non-D streptococci in three, Group D streptococci in one, and Group B streptococci (S. agalactiae) in two. Although one case was f

Laboratory-based surveillance of antimicrobial non-susceptibility in Enterobacter species over 12 years at a tertiary-care hospital in Saudi Arabia.

BACKGROUND: Enterobacter species are increasingly associated with antimicrobial non-susceptibility, but long-term surveillance of temporal trends across antibiotic classes is limited in southern Saudi Arabia. METHODS: We retrospectively studied non-duplicate Enterobacter isolates from adults at a tertiary-care hospital in southern Saudi Arabia (August 2012-August 2024), interpreted by contemporaneous CLSI criteria. Temporal trends were modelled by logistic regression as the annual percent change (APC) in the odds of non-susceptibility, adjusted for care setting, specimen source, age, and sex.

Lemierre Syndrome After Influenza A Virus Infection.

Lemierre syndrome is a rare condition characterized by septic thrombophlebitis of the internal jugular vein with metastatic septic emboli, most commonly affecting the lungs. We report a case of Lemierre syndrome temporally associated with influenza A infection in a previously healthy 21-year-old male. The patient initially presented to the emergency department with severe headache and tested positive for influenza A. Over the following seven days, his symptoms progressed to neck pain with decreased range of motion, hemoptysis, cough, and persistent myalgias. Despite four additional emergency d

Impact of a Pharmacist-Led Antimicrobial Stewardship Program on Empiric Prescribing Appropriateness in the Emergency Department: A Retrospective Cohort Study.

BACKGROUND: Inappropriate antimicrobial use in emergency departments (EDs) contributes significantly to antimicrobial resistance (AMR). Pharmacist-led antimicrobial stewardship programs (ASPs) can potentially optimize antimicrobial prescribing and improve patient outcomes; however, their specific impact in the ED setting warrants further evaluation. METHODS: This retrospective cohort study evaluated adult patients receiving empiric antimicrobial therapy in the ED of a large, tertiary-care, community teaching hospital in South Florida before (pre-ASP) and after (post-ASP) the implementation of

Quantifying the Silent Selection Pressure: Antimicrobial Stewardship and Gut Microbiome Integrity in the NICU and PICU.

Background: Antimicrobial stewardship in Neonatal (NICU) and Pediatric Intensive Care Units (PICUs) is complicated by rapid physiological maturation and the high vulnerability of the developing gut microbiome. Traditional metrics fails to capture the true utilization density of antibiotics in these settings. This study evaluated antimicrobial consumption patterns and alignment with the WHO AWaRe framework in two Indonesian hospitals and its impact towards patients' length of stay. Methods: A retrospective multicenter study was conducted at a public hospital (Haji Hospital) and a private univer

Patterns of inpatient antibiotic use and antimicrobial resistance in the surgical wards of a Ugandan tertiary hospital: A mixed methods study.

BACKGROUND: Inappropriate antibiotic use in surgical wards is a major driver of antimicrobial resistance (AMR), especially in low- and middle-income countries, where empirical prescribing is common. This study assessed inpatient antibiotic use and AMR patterns, integrating these findings with qualitative in-depth interviews (IDIs) which explored determinants of prescribing practices. METHODS: A convergent mixed methods study design was employed on the surgical wards of Mulago National Referral Hospital (MNRH) in Uganda. Quantitative data was collected through Point Prevalence Surveys (PPS) of
