Feeding a high-carbohydrate (CHO) diet and administration of alkalinizing agents have both been shown to improve performance in high-intensity exercise. The effect of these treatments in combination was investigated in the present study. Six healthy male subjects exercised to exhaustion on an electrically braked cycle ergometer at a power output equivalent to 100% of their maximum oxygen uptake (VO2,max) on four separate occasions. Each subject consumed either a diet with the same composition as his normal diet (termed the experimental normal (N) diet; 54 +/- 7% CHO, 13 +/- 2% protein, 33 +/-

INTRODUCTION: Military personnel operate in chaotic and time-dependent environments; however, they are not assessed for readiness or return to duty under those constraints. Physical performance declines when integrated with cognitive challenges. Assessing this integrated performance (i.e., neuromuscular-cognitive) is vital to understanding how an individual may respond to uncertainty. Therefore, the purpose of this study was to determine the test-retest reliability of 3 neuromuscular-cognitive assessments, both with and without a rucksack.
MATERIALS AND METHODS: This study employed a test-ret

Troponin release following exertional vasovagal syncope has not previously been reported. A young man was investigated after being admitted twice with exertional syncope, each time followed by a 10-fold spike in troponin I over 24 h. Treadmill exercise tests reproduced his symptoms and demonstrated a vasovagal mechanism. During recovery, despite lying supine, he remained hypotensive for 5 min, with profound bradycardia and ST segment depression. We suspected that intense cardiovagal neural activity may have caused the troponin leak.

OBJECTIVE: Chronic exertional compartment syndrome (CECS) is an underdiagnosed condition that affects young athletes. CECS is caused by increased compartmental pressure in the fascial spaces during exercise. CECS is diagnosed by direct pressure readings (in mmHg), which is a painful and invasive test. Noninvasive shear-wave elastography (SWE) and subharmonic-aided pressure estimation (SHAPE) using contrast-enhanced ultrasound were compared to compartment pressure testing for the diagnosis of CECS.
METHODS: Ten healthy volunteers and five CECS patients enrolled in this prospective, IRB-approve

BACKGROUND: Immune checkpoint inhibitor (ICI) anti-tumor therapy is commonly associated with reports of significant fatigue. Whether ICI-induced fatigue is linked to subclinical cardiac toxicity is not well understood.
METHODS: We performed a prospective observational study to monitor cardiac function following initiation of ICI in cancer patients. Rest and staged exercise stress transthoracic echocardiograms (TTE) were captured at baseline, 3 and 6 months after ICI initiation.
RESULTS: Thirteen patients completed baseline testing, of whom 10 completed 3 month testing and 8 completed 6 month

BACKGROUND: For patients with congenital heart disease-related pulmonary arterial hypertension (CHD-PAH), cardiopulmonary exercise testing (CPET) can reflect cardiopulmonary reserve function. However, CPET may not be readily accessible for patients with high-risk conditions or limited mobility due to disability. Echocardiography, on the other hand, serves as a widely available diagnostic tool for all CHD-PAH patients. This study was aimed to identify the parameters of echocardiography that could serve as indicators of cardiopulmonary function and exercise capacity.
METHODS: A cohort of 70 pat

People with mental illness are more likely to experience physical health problems and die prematurely than are comparable populations. This study evaluated whether exercise, when offered as part of routine treatment, affects the psychological and physical health of patients in a high-secure forensic unit in Norway. Thirteen patients completed a structured exercise program lasting 8 to 12 weeks. After completion of the program, resting heart rate and systolic blood pressure after treadmill testing were significantly improved. In addition, patients' subjective feelings of well-being and safety i

PURPOSE: Important goals of cardiac rehabilitation maintenance programs (CMP) are to increase leisure time physical activity (LTPA) and improve health-related quality of life (HRQoL). Elderly patients with cardiac disease and low exercise capacity are simultaneously the most severely affected and have the most to gain from exercise-based rehabilitation. Individualized combined exercise (ICE) may be an effective modality to achieve these goals. We compared six months of ICE to CMP in their effects on LTPA and HRQoL.
METHODS: Sixty patients (70 ± 9 years, 39% female) with cardiac disease and lo

BACKGROUND: Vertebral fractures are associated with increased morbidity (e.g. pain, reduced quality of life) and mortality. Therapeutic exercise is a non-pharmacological conservative treatment that is often recommended for patients with vertebral fractures to reduce pain and restore functional movement. This is an update of a Cochrane Review first published in 2013.
OBJECTIVES: To assess the effects (benefits and harms) of exercise intervention of four weeks or greater (alone or as part of a physical therapy intervention) versus non-exercise/non-active physical therapy intervention, no interv

BACKGROUND: Arterial stiffening and atherosclerosis tend to coexist. Strain imaging, using a 2-dimensional speckle tracking (2D-ST) method, has been used for arterial stiffness assessment and early identification of atherosclerosis. We investigated whether the ascending aortic strain assessed by 2D-ST echocardiography at rest can predict the presence of coronary artery disease (CAD).
METHODS AND RESULTS: Two hundred seventy-one consecutive patients with suspected stable angina pectoris sequentially underwent exercise treadmill testing, 2-dimensional echocardiography, M-mode echocardiography,

BACKGROUND: Wrestling, characterized by high-intensity intermittent efforts, demands exceptional anaerobic power and recovery capacity. Nitrate-rich beetroot juice (BRJ), supplemented with vitamin C, has emerged as a potential ergogenic aid through increased nitric oxide bioavailability. However, limited data exists regarding its acute effects on anaerobic performance in combat sport athletes. This study investigated the acute effects of BRJ supplemented with vitamin C on upper- and lower-body anaerobic test performance and selected biochemical markers in collegiate wrestlers. METHODS: In a ra
