Misdiagnosis of Congenital Pubic Symphysis Diastasis as Post-Traumatic Pubic Diastasis: A Case Report.

In patients with congenital pubic diastasis, who present with polytrauma injury, pubic diastasis could be falsely attributed to the traumatic event. This generally occurs in asymptomatic patients whose anomaly is not diagnosed before the traumatic event. In this report, we present a case of a 26-year-old male with fracture-dislocation of the left hip and congenital agenesis of pubic bones that was initially misdiagnosed as post-traumatic pubic diastasis due to the patient's reduced consciousness. A closer examination led to noticing his micropenis and the scar from the earlier surgical interve

Replacement of a destroyed scaphoimodelled prosthesis.

Preiser disease is a rare condition where ischaemia and necrosis of the scaphoid bone occurs without previous fracture. A female patient in her 30s presented with severe right wrist pain caused by Preiser disease. Initially, she was treated by a pedicled vascularised bone graft, but unsuccessfully. Four years after the initial diagnosis, the affected scaphoid was replaced by a 3D modelled prosthesis. Three years after the operation, the patient had significant pain reduction, good wrist function and grip strength. Final X-rays demonstrated no signs of dislocation of the implant or signs of per

Enhanced Mechanical Properties and Isotropy of Mg-2Al-0.8Sn Alloy through Ca Addition.

Calcium (Ca), with abundant and cheap reserves, is a potential element to facilitate the further application of Mg-Al-Sn based alloys. Here, effects of Ca content on the microstructure and tensile properties of Mg-2.0Al-0.8Sn (wt.%) alloys were systematically studied. The experimental results illustrated that the strength, ductility and isotropy of the alloys improved simultaneously with the increase of Ca content. The better ductility and isotropy could be contributed to the weakened texture via particle stimulation nucleation mechanism. The higher strength benefited from the combination of f

Complex Focal Pain Syndrome: An Unusual Variant of Complex Regional Pain Syndrome.

Complex regional pain syndrome (CRPS) is a chronic neuropathic pain condition that is often overlooked by clinicians and typically occurs within an entire limb. There is considerable clinical variability in presentation among patients with CRPS. We report a case of extremely focal CRPS localized to the left small finger (LSF) following crush injury. A 48-year-old right-handed male presented with LSF stiffness and severe pain of three months' duration following crush injury. He endorsed severe allodynia and minimal flexion at the proximal interphalangeal and distal interphalangeal joints of the

Lisfranc injuries: patient- and physician-based functional outcomes.

The purpose of this study was to assess functional outcome of patients with a Lisfranc fracture dislocation of the foot by applying validated patient- and physician-based scoring systems and to compare these outcome tools. Of 25 injuries sustained by 24 patients treated in our institution between January 1995 and June 2001, 16 were available for review with a mean follow-up period of 36 (10-74) months. Injuries were classified according to Myerson. Outcome instruments used were: (a) Medical Outcomes Study 36-Item Short Form Health Survey (SF-36), (b) Baltimore Painful Foot score (PFS) and (c)

Fatal injuries of the brain stem and/or upper cervical spinal cord in traffic accidents: nine autopsy cases.

Nine forensic autopsy cases were studied. All had injuries of the brain stem and/or upper cervical spinal cord due to traffic accidents. Among the nine subjects, eight were pedestrians and one was a left, front seat occupant of a vehicle. Examination of these 9 cases revealed that three had ponto-medullary avulsion, two had medullary avulsion and the other four had laceration of the upper cervical spinal cord. Atlanto-occipital dislocation was observed in five cases, and a ring fracture around the foramen magnum in the other two cases. Intraventricular haemorrhage, probably due to tears of the

Long-term results of extensor tendon repair.

A retrospective analysis was done in 62 patients with 101 digits having extensor tendon injury. Quality of outcome and parameters that might influence outcome were evaluated. The majority of patients were treated with conventional static splinting. Sixty percent of all fingers sustained an associated injury (fracture, dislocation, joint capsule or flexor tendon damage). Patients without associated injuries achieved 64% good/excellent results, and total active motion of 212 degrees. This difference was statistically significant (p less than 0.05). Distal zones (1 to 4) had a significantly poore

Arterial injuries of the extremities following blunt trauma.

Thirty-three peripheral arterial injuries were observed in 29 patients following blunt trauma. Motor vehicle accidents were the most common initiating event. The mechanism of injury was fracture or dislocation in 21 patients, acute traction on the extremity in five patients, and contusion in three patients. Pulses were absent distal to the site of arterial injury in all patients. Complete arterial disruption was documented in 19 arteries; intimal or intimal and medial tears were observed in 10 arteries. Four arteries not surgically explored. If possible, debridement and primary anastomosis was

[Hinged Elbow External Fixation for Severe Elbow Stiffness].

PURPOSE OF THE STUDY The elbow is a highly constrained synovial hinge joint that frequently becomes stiff after injury. Grade 4 elbow stiffness (ankylosis) is a strongly limiting factor in the patient s daily activities and is difficult to treat. Hinged external fixation of the elbow provides the advantages of static fixation with the benefits of continued motion of the joint. The goal of our study was to assess the improvement in the range of motion of the elbow after surgical arthrolysis using a posterior extensile approach to the joint with or without using external fixation. Incidence and predictors for the need for fasciotomy after extremity trauma: a 10-year review in a mature level I trauma centre.

BACKGROUND: Compartment syndrome is a devastating complication after trauma to the extremities. Prompt fasciotomy is essential for avoiding disability and limb loss. The purpose of this study was to determine the incidence and predictors for the need for fasciotomy after extremity trauma. METHODS: All trauma patients sustaining extremity injuries admitted to the LAC+USC Medical Centre during a 10-year period ending in December 2007 were identified. Demographics, clinical data, blood requirements and outcomes were abstracted. Patients who required an extremity fasciotomy were compared with tho

Limb salvage and outcomes among patients with traumatic popliteal vascular injury: an analysis of the National Trauma Data Bank.

PURPOSE: Popliteal arterial trauma carries the greatest risk of limb loss of any peripheral vascular injury. The purpose of this study was to analyze outcomes after popliteal arterial injuries and identify factors contributing to disability. METHODS: A retrospective analysis was conducted of prospectively collected trauma data from the National Trauma Data Bank (NTDB). We studied all patients with popliteal arterial injury in terms of demographics, injury patterns, interventions, limb salvage, resource utilization, and outcomes. RESULTS: We identified 1395 popliteal arterial injuries among t

Is Repair of a Small Coronoid Fracture Required in the Surgical Treatment of Terrible Triad Injury of the Elbow?

Background: Multiple treatment protocols have been described in literature for the treatment of terrible triad injury (TTI) of the elbow. We believe that repair of the medial collateral ligament (MCL) should be performed in preference to repair of a small coronoid fracture if the elbow is unstable after fixation/replacement of the radial head and repair of the lateral collateral ligament (LCL). The aim of this study is to report the outcomes of surgical treatment of patients with TTI associated with a small coronoid fracture in whom the coronoid fracture was not addressed. Methods: This study

Osteochondral Injuries of the Knee in Pediatric Patients.

Osteochondral injuries in pediatric patients may occur as a result of a traumatic injury or secondary to an osteochondritis dissecans (OCD) lesion. Lateral patella dislocation is a common traumatic cause of osteochondral injury that typically occurs at the medial facet of the patella or at the lateral aspect of the distal femur. Multiple theories have been proposed for the cause of an OCD lesion in the knee, including trauma or repetitive microtrauma, local vascular insufficiency, and family history. The most "classic" location for OCD lesions of the knee is the lateral aspect of the medial fe

Determining the strongest orientation for "Lisfranc's screw" in transverse plane tarsometatarsal injuries: a cadaveric study.

UNLABELLED: The treatment of tarsometatarsal joint fracture-dislocations generally consists of realignment followed by stabilization with rigid internal fixation. The purpose of this study was to determine the strongest orientation for the "Lisfranc's screw" for repair of disruption of the articulation between the first and second metatarsals and the medial and intermediate cuneiforms. To this end, Lisfranc's ligament was sectioned in 6 pairs of fresh-frozen, human cadaver feet, after which a 3.5-mm partially threaded, cannulated screw was placed across the Lisfranc joint in 1 of 2 opposing di

Percutaneous treatment of carpal, metacarpal, and phalangeal injuries.

Percutaneous fixation of hand fractures is a common technique that takes advantage of the subcutaneous nature of hand bones, their small size, and their limited loading potential for stress placed on hardware. Percutaneous wire fixation supplements cast fixation when plaster cannot hold particular reductions, and allow surgical fixation with limited postoperative swelling. In the first part of the current study, the types of wires that are used for hand fixation, fluoroscopy, helpful instruments, and the basic techniques used for this type of surgery are discussed. In the second part of the st

[Therapy of rupture of the great tubercle].

In most cases isolated avulsions of the great tubercle can be treated conservatively. 31 Patients suffering from an isolated avulsion of the great tubercle had to go through an after-examination. 30 of these patients were fixed on the conservative method for three to four days mostly, usually with a Desault-dressing, and, subsequently, they had to undergo a functional after-treatment, even if the dislocation of the fragment was partly more than 1 cm. Only once, in the case of subacromial position of the fragment, an operative fixation was performed. On an average, an after-examination followed
