Recidivism, health and social functioning following release to the community of NSW prisoners with problematic drug use: study protocol of the population-based retrospective cohort study on the evaluation of the Connections Program.

INTRODUCTION: The rising rate of incarceration in Australia, driven by high reoffending, is a major public health problem. Problematic drug use is associated with increasing rates of reoffending and return to custody of individuals. Throughcare provides support to individuals during imprisonment through to post-release, improving both the transition to community and health outcomes post-incarceration. The aim of this study is to evaluate the Connections Programme (CP) that utilises a throughcare approach for release planning of people in prison with a history of problematic drug use. The study

Comparing the potential impacts of seasonal immunisation against year-round immunisation to prevent RSV infant hospitalisations in Japan: A modelling study.

The seasonal circulation of respiratory syncytial virus (RSV) in countries such as Japan, together with the transient nature of passive immunity conferred to infants via maternal vaccination or monoclonal antibody administration, may warrant a differential strategy for those born between the RSV seasons. Maximal effectiveness may be achieved by deferring immunisation of this cohort from birth until entry into their first RSV season using catch-up administration of monoclonal antibody through a seasonal and catch-up programme, compared with year-round administration. To estimate the benefit of

Associations between antenatal corticosteroids and neonatal morbidities in a prospective cohort: role of course, timing, and gestational age.

BACKGROUND: Antenatal corticosteroids (ACS) have been widely used to enhance fetal lung maturation in pregnant women at risk of preterm delivery, but gaps remain in understanding how number of courses, gestational age at the first dose, and last dose-to-delivery interval affect neonatal morbidities across different gestational age groups. This study aimed to investigate the associations between ACS, particularly the number of courses, gestational age at the first dose, and last dose-to-delivery interval, and neonatal morbidities. METHODS: This prospective study included 78,642 singleton infan

Assessment of macrolide resistance in the respiratory tract of preterm-born infants during treatment with azithromycin in the AZTEC clinical trial.

BACKGROUND: Our multicentre, double-blind, randomised, placebo-controlled Azithromycin therapy for prevention of chronic lung disease of prematurity trial assessed if early 10-day azithromycin treatment improved survival without development of chronic lung disease of prematurity when compared with placebo in 796 preterm-born infants. Since antibiotic resistance remains a significant global health priority, we had preplanned macrolide-resistance assessment in recruited infants. We, therefore, identified baseline macrolide-resistant genes in respiratory samples and determined if this was altered

Safety and immunogenicity of 13-Valent pneumococcal conjugate vaccine in healthy infants in Shanghai during the COVID-19 pandemic: a prospective cohort study.

BACKGROUND: Streptococcus pneumoniae (S. pneumoniae) is a major cause of morbidity and mortality in infants, leading to diseases like pneumonia and otitis media. Although the World Health Organization (WHO) recommends inclusion of pneumococcal conjugate vaccines (PCVs) in national immunization programs, 13-Valent PCV (PCV13) remains optional in China, with suboptimal coverage, especially in urban settings like Shanghai. This study was performed to evaluate the safety, immunogenicity and impact on nasopharyngeal carriage of S. pneumoniae of PCV13 in healthy infants in Shanghai. MATERIAL AND ME

Combined effects of traffic-related air pollution, climate factors, and greenness on respiratory disease risk in infants.

Prenatal exposure to climate factors, air pollution, and green space has been linked to respiratory diseases in infants. However, the role of the combined effects of exposure to these factors on respiratory ailments remains unclear. Here we investigate the association of combined exposure to traffic-related air pollution (TRAP), climate factors, and green space during the prenatal period with respiratory diseases in infants. We enrolled 454 participants from the ongoing prospective birth cohort known as the Mothers and Children's Environmental Health study. Data on infant respiratory diseases

Efficacy of Donated Milk in Early Nutrition of Preterm Infants: A Meta-Analysis.

Background: Preterm birth is associated with an increased risk of many complications, which is a main public health problem worldwide with social and economic consequences. Human milk from breast feeding has been proved to be the optimal nutrition strategy for preterm infants when available. However, the lack of human milk from mothers makes formula widely used in clinical practice. In recent years, donated breast milk has gained popularity as an alternative choice which can provide human milk oligosaccharides and other bioactive substances. Objective: We aimed to conduct a systematic review a

Assessing the appropriateness of the management of gastro-oesophageal reflux in Australian children: a population-based sample survey.

Gastro-oesophageal reflux (GOR) is a common physiological state in infants and young children, with gastro-oesophageal reflux disease (GORD) its pathological manifestation. Management of GOR/GORD requires elimination of possible underlying causes, parental reassurance, modification of feeding and symptom mitigation, monitoring, and referral to paediatricians if warning signs are present. Published clinical practice guidelines (CPGs) seek to support clinicians and improve management. This study aimed to measure the proportion of Australian GOR/GORD paediatric care that was in line with CPG reco

Clinical presentation of congenital syphilis in a rotavirus vaccine cohort study in Lusaka: a case series.

BACKGROUND: Despite an otherwise robust national antenatal clinic program, maternal and congenital syphilis remains an important public health issue in Zambia. This case series reports the clinical presentation of seven infants diagnosed with congenital syphilis in Lusaka, Zambia. CASE PRESENTATIONS: The cases in this series were incidental findings from a cohort of infants enrolled in a rotavirus vaccine immunogenicity study recruiting infants at 6 weeks of age. As part of clinical care for enrolled participants, we screened mothers of children who presented with adverse events of (i) repeat

Should we use Palivizumab immunoprophylaxis for infants against respiratory syncytial virus? - a cost-utility analysis.

BACKGROUND: Passive immunization against RSV (Respiratory Syncytial Virus) is given in most western countries (including Israel) to infants of high risk groups such as premature babies, and infants with Congenital Heart Disease or Congenital Lung Disease. However, immunoprophylaxis costs are extremely high ($2800-$4200 per infant). Using cost-utility analysis criteria, we evaluate whether it is justified to expand, continue or restrict nationwide immunoprophylaxis using palivizumab of high risk infants against RSV. METHODS: Epidemiological, demographic, health service utilisation and economic

A Randomized Trial of Phototherapy with Filtered Sunlight in African Neonates.

BACKGROUND: Sequelae of severe neonatal hyperbilirubinemia constitute a substantial disease burden in areas where effective conventional phototherapy is unavailable. We previously found that the use of filtered sunlight for the purpose of phototherapy is a safe and efficacious method for reducing total bilirubin. However, its relative safety and efficacy as compared with conventional phototherapy are unknown. METHODS: We conducted a randomized, controlled noninferiority trial in which filtered sunlight was compared with conventional phototherapy for the treatment of hyperbilirubinemia in term

[Obstructive sleep apnea syndrome in young infants].

UNLABELLED: Obstructive sleep apnea syndrome (OSAS) has been reported among children aged 3-6 years. Adenotonsillar hypertrophy is the most common cause of OSAS. The upper airway obstruction results in repeated obstructive apneas and hypopneas, associated with oxygen desaturations and sleep fragmentation. OSAS in children is associated with higher respiratory morbidity as well as significant clinical consequences, mainly neurocognitive and behavioral problems, impaired growth and cardiac dysfunction. The objectives of the present study were to determine the clinical and sleep characteristics o

Effectiveness of massage, sucrose solution, herbal tea or hydrolysed formula in the treatment of infantile colic.

AIM AND OBJECTIVE: The aim of the study was to evaluate the effectiveness of massage, sucrose solution, herbal tea or hydrolysed formula, each used individually in the treatment of infantile colic. BACKGROUND: The term colic describes a group of symptoms that occur frequently in infants, consisting of paroxysmal abdominal pain and severe crying. Infant colic is of importance for both parents and the community health services that provide families with care, and is therefore an important clinical problem that is amenable to nursing interventions. DESIGN: This prospective and randomised-contro
