STUDY DESIGN: Multicenter cohort.
OBJECTIVES: A report from the International Spine Study Group (ISSG) noted that surgeons failed to achieve alignment goals in nearly two-thirds of 266 complex adult deformity surgery (CADS) cases. We assess whether personalized interbody spacers are associated with improved rates of achieving goal alignment following adult spinal deformity (ASD) surgery.
METHODS: ASD patients were included if their surgery utilized 3D-printed personalized interbody spacer(s) and they met ISSG CADS inclusion criteria. Planned alignment was personalized by the surgeon during i

BACKGROUND: Intervertebral disc degeneration (IDD) is a main contributor to low back pain. Oxidative stress, which is highly associated with the progression of IDD, increases senescence of nucleus pulposus-derived mesenchymal stem cells (NPMSCs) and weakens the differentiation ability of NPMSCs in degenerated intervertebral discs (IVDs). Quercetin (Que) has been demonstrated to reduce oxidative stress in diverse degenerative diseases.
AIM: To investigate the role of Que in oxidative stress-induced NPMSC damage and to elucidate the underlying mechanism.
METHODS: In vitro, NPMSCs were isolated

BACKGROUND: Sarcopenia, defined as loss of skeletal muscle mass, is a novel term associated with adverse outcomes in children. Magnetic Resonance Imaging (MRI) is a safe and precise technique for measuring tissue compartments and is commonly used in most routine paediatric imaging protocols. Currently, there is a lack of MRI-derived normative data which can help in determining the level of sarcopenia. This study aimed to introduce reference values of total psoas muscle area (tPMA), total paraspinal muscle area (tPSMA), and total macroscopic fat infiltrations of the PSMA (tMFI).

The increased incidence of injury demonstrated in epidemiological data for the elderly population, and females compared to males, has not been fully understood in the context of the biomechanical response to impact. A contributing factor to these differences in injury risk could be the variation in geometry between young and aged persons and between males and females. In this study, a new methodology, coupling a CAD and a repositioning software, was developed to reposture an existing Finite element neck while retaining a high level of mesh quality. A 5th percentile female aged neck model (F057

BACKGROUND CONTEXT: Vertebral endplate defects are often implicated in degenerative disc disorders, yet their connection to patient-reported symptoms remains unclear. COX-2 and PGE-2 are known for their roles in inflammation and pain, with EP-4 receptor involvement in pain signaling. Examining their expression in vertebral endplate tissues may provide insights into pathomechanism of low back pain.
PURPOSE: To investigate the association between endplate defects and patient-reported symptoms and to further clarify the role of the COX-2/PGE-2/EP-4 axis in the pathogenesis of chronic low back pa

STUDY DESIGN: In vitro experiment using degenerated human ligamentum flavum (LF) and herniated intervertebral disk (IVD).
OBJECTIVES: To investigate the role and effect of degenerated and herniated IVDs on LF hypertrophy and ossification.
SUMMARY OF BACKGROUND DATA: Spinal stenosis is caused, in part, by hypertrophy and ossification of the LF, which are induced by aging and degenerative process. It is well known that degenerated IVDs spontaneously produce inflammatory cytokines. Therefore, we hypothesized that degenerated IVD may affect adjacent LF through secreted inflammatory cytokines.

BACKGROUND: The transvertebral approach is useful for decompression in patients with cervical radiculopathy; because the intervertebral disk is preserved, moveability is retained. We performed wide deletion of the vertebral body to increase the patient population eligible for treatment with this approach and include patients with compression of the cervical spinal cord.
METHODS: In patients undergoing anterior decompression, we performed vertebrotomy (13 x 8 mm) at the midline of the cervical vertebral body at the upper level using a surgical saw. The resulting hole facilitates decompression

It has been suggested that lumbar sympathectomy can reduce pain behavior, including mechanical allodynia and thermal hyperalgesia, caused by ligation of the spinal nerve. One well-characterized model, which involves application of nucleus pulposus to the spinal nerve and displacement of the adjacent nerve, shows behavioral changes in rats. However, there have been no previous reports regarding sympathectomy performed in this model. Disk incision and adjacent spinal nerve displacement were performed with (n=6) or without (n=6) sympathectomy. Sham surgery was also performed with (n=6) or without

Thoracic disk herniation is a not uncommon pathology faced by the spinal surgeon. The management of massive intradural thoracic disk herniation with ventral cord compression is problematic both in terms of obtaining adequate decompression and ensuring no subsequent leakage of cerebrospinal fluid. A 54-year-old woman presented with a 10 year history of back pain and left leg pain. Over the past 6 months she experienced a progressive spastic paraparesis in both legs with recent urinary incontinence. A left anterolateral thoracotomy for excision of T8/9 thoracic disk protrusion was affected. A tr

The intervertebral disc (IVD)-endplate-vertebra junction forms a structurally heterogeneous soft-hard tissue interface involved in spinal load transmission, yet its microscale structural-mechanical heterogeneity remains incompletely understood. Here, we employ atomic force microscopy (AFM) to quantify spatial variations in apparent elastic modulus across the annulus fibrosus (AF) and endplate in a healthy, young porcine spine model. High-resolution morphological mapping reveals a radial transition from loosely organized fibrillar networks in medial AF to densely packed lamellar bundles in late

BACKGROUND: Thoracic disc herniations are rare but can cause severe myelopathy, particularly when centrally located and calcified. Their ventral position and limited operative corridor make surgical management challenging.
METHODS: A 46-year-old male presented with progressive gait instability, a T5 sensory level, and upper motor neuron signs. Imaging revealed a large, centrally located, partially calcified T5-6 disc herniation with rostral migration causing significant cord compression.
RESULTS: Given the morphology and ventral displacement of the cord, an endoscopic trans-costovertebral ap
