Detection of the primary lesion in patients with cervical metastases from unknown primary tumors with narrow band imaging endoscopy: preliminary report.

BACKGROUND: We investigated whether the addition of narrow band imaging (NBI) to standard diagnostic workups could enhance the detection of primary lesions in patients with carcinoma of unknown primary (CUP). METHODS: Thirty patients with CUP underwent NBI endoscopy and fluorodeoxyglucose-positron emission tomography/CT (FDG-PET/CT) after thorough conventional diagnostic workups between 2009 and 2011. Sites suspected of harboring primary tumors, as determined by NBI and/or FDG-PET/CT, were biopsied for histologic confirmation. RESULTS: Occult primary tumors were identified in 33.3% (10/30),

Assessment and Optimization of Cardiovascular Risk and Frailty Before Noncardiac Surgery: A Comparative Analysis of Recent European and American Guidelines.

Perioperative cardiovascular complications and frailty are major drivers of morbidity, mortality, and loss of functional independence after noncardiac surgery, particularly in older adults. Updated European and American guidelines published between 2022 and 2025 address these challenges with differing approaches. This review provides a comparative analysis of contemporary European (European Society of Cardiology 2022, European Society of Anaesthesiology and Intensive Care 2025) and American (American Heart Association 2024) guidelines on preoperative cardiovascular risk and frailty assessment

AI-driven dual-task prediction model for co-stratifying efficacy and toxicity in NSCLC immunotherapy.

While effective against non-small cell lung cancer (NSCLC), PD-1 inhibitors can induce immune-related adverse events (irAEs), occurring in up to 15.2% of patients and potentially fatal. Currently, effective predictive biomarkers capable of simultaneously forecasting both irAEs and immune checkpoint inhibitor (ICI) responders remain elusive. This limitation hinders the safe clinical application of these agents. This study enrolled 333 advanced NSCLC patients treated with PD-1 inhibitor monotherapy or combination therapy. CT imaging features were extracted using radiomics and deep-learning appro

Artificial intelligence for predicting surgical difficulty in laparoscopic cholecystectomy: a systematic review and meta-analysis.

BACKGROUND: Accurately predicting operative difficulty in laparoscopic cholecystectomy (LC) is foundational to personalized surgical planning and patient safety assurance. However, the reliability, generalizability, and true clinical utility of current Artificial Intelligence (AI) models are currently unsubstantiated. This review aimed to evaluate the predictive performance and methodological quality of AI models designed to predict LC surgical difficulty. METHODS: PubMed, Embase, Web of Science, and the Cochrane Library were searched from inception to March 2, 2026. The Prediction Model Risk

Artificial Intelligence and Multi-Omics Approaches in the Precision Management of Pulmonary Hypertension: From Early Diagnosis to Therapeutic Stratification.

Pulmonary hypertension (PH) is a heterogeneous clinical syndrome in which similar haemodynamic abnormalities may arise from distinct vascular, cardiac, pulmonary, thromboembolic, and molecular mechanisms. This complexity limits the ability of conventional classifications and risk scores to fully capture individual disease trajectories and treatment responses. Artificial intelligence (AI) offers a framework for integrating clinical data, electrocardiography, multimodal imaging, invasive haemodynamics, biomarkers, and multi-omics information across the PH care pathway. This review summarises cur

Development and Internal Validation of the CLIMPOUT Score: A Multimodal Prediction Model for Neurological Outcome After Pediatric Intracerebral Hemorrhage.

Spontaneous pediatric intracerebral hemorrhage (sICH) is a rare but devastating neurological emergency associated with high mortality and long-term disability. Existing prognostic scores rely on a reduced number of variables and have shown limited validation, highlighting the need for comprehensive tools. This study describes the protocol for the development and internal validation of the CLIMPOUT (Clinical, Laboratory, and Imaging Predictive Outcome) score, a multimodal model designed to improve early prognostic assessment in pediatric sICH. This single-center retrospective cohort study will

Gut microbiome variability and brain alterations in schizophrenia: A scoping review of structural and functional MRI studies.

INTRODUCTION: Schizophrenia (SCZ) is increasingly considered a multifactorial disorder involving gut-brain interactions. Current evidence supports gut microbiome alterations in SCZ, along with well-established structural and functional brain abnormalities. However, findings linking gut microbiome variability to neuroimaging alterations in SCZ have not yet been comprehensively integrated. METHODS: Following the Joanna Briggs Institute methodology for scoping reviews and the PRISMA-ScR checklist, a literature search was performed in PubMed, Scopus, and Web of Science, selecting structural magne

Rethinking Congestion in Heart Failure from Volume Overload to Venous Pressure and Organ Disfunction with VExUS.

Congestion is a major driver of symptoms, hospitalization, and adverse outcomes in heart failure (HF), yet its clinical assessment remains challenging. Traditional approaches based on physical examination, biomarkers, and isolated imaging surrogates often fail to capture the complexity of systemic venous congestion and its impact on organ function. In HF, congestion should be interpreted as a multifactorial process resulting from the interaction between intravascular volume burden, venous compliance, cardiac filling pressures, neurohormonal activation, blood volume redistribution, and organ-sp

Rheumatic Digital Twin: Proposed Machine Learning-Based Multimodal Framework to Inform Clinical Decision-Making.

Rheumatic diseases are chronic, immune-mediated conditions characterized by significant heterogeneity in presentation and disease course. However, current clinical approaches often rely on snapshot-based assessments that fail to capture the complex longitudinal evolution of these conditions. To address these limitations and support the implementation of precision medicine, we present the design for the Rheumatic Digital Twin, a novel, modular conceptual framework intended to integrate heterogeneous multimodal data, ranging from electronic health records and clinical notes to imaging and omics,

Circulating tumor DNA for minimal residual disease and recurrence surveillance in hepatocellular carcinoma: current evidence and a translational roadmap.

Recurrence remains a major cause of failure after curative-intent therapy for hepatocellular carcinoma (HCC), including resection, ablation, and liver transplantation. Current surveillance relies on imaging and serum biomarkers, but radiologic relapse may lag behind biological recurrence. This gap has focused attention on circulating tumor DNA (ctDNA) and related cell-free DNA (cfDNA) approaches as candidate tools for minimal residual disease (MRD) assessment and molecular surveillance. In HCC, however, recurrence is biologically heterogeneous. Post-treatment relapse may arise from residual ma

Advances in Multimodality Monitoring in Traumatic Brain Injury.

PURPOSE OF REVIEW: Multimodal monitoring (MMM) has the potential to transform the management of secondary brain injury following traumatic brain injury (TBI) to enable more personalized care. Despite its promise, the optimal combination of monitoring modalities and the interpretation of complex data streams remain insufficiently mature for MMM to be considered standard of care. This review evaluates the current state of MMM technologies, their clinical applications, and their emerging role in improving TBI management and guiding the path toward broader clinical adoption. This review focuses on

Leakage-Free Multimodal Depression Screening: Controlled Evaluation of Text, Facial Behavior, and Prosodic Fusion.

Multimodal behavioral sensing may support depression screening, but evaluation on small clinical-interview datasets is particularly vulnerable to data leakage and model-selection bias. We present a leakage-audited trimodal framework evaluated on DAIC-WOZ (n=180, PHQ-8 ≥10), combining SBERT text embeddings, OpenFace facial-behavior descriptors, and COVAREP prosodic features. Participant-level partitioning is performed before augmentation, while decision thresholds and neural-model checkpoints are selected exclusively from internal validation data. A controlled five-seed experiment showed that a

Alleviating cognitive impairments in bipolar disorder with a novel DTI-guided multimodal neurostimulation protocol: a double-blind randomized controlled trial.

BACKGROUND: Traditional neuromodulation strategies show promise in enhancing cognitive abilities in bipolar disorder (BD) but remain suboptimal. This study introduces a novel multimodal neurostimulation (MNS) protocol to improve therapeutic outcomes. METHODS: The novel MNS protocol used individualized diffusion tensor imaging (DTI) data to identify fiber tracts between the dorsolateral prefrontal cortex and dorsal anterior cingulate cortex. The highest structural connectivity point is selected as the individualized stimulation site, which is then targeted using a combination of optimized tran
