Hypothyroidism is one of the common causes of the secondary hypercholesterolemia. The prevalence of hypothyroidism in the general population is estimated to be as high as about 1.5%. Frequency of the hypothyroidism in patients with hyperlipidemia is high, and can be observed in 4.2-10% in different populations. Most commonly, there is no need to treat the hypothyroid patients with the hypolipidemic drugs. Substitution treatment with the thyroid hormones usually results in either normalization or significant decreasing of the lipid levels. Hypothyroidism with symptoms of involvement of skeletal

BACKGROUND: Inactivation of muscles of facial expression by chemodenervation with botulinum toxin remains an off-label indication. Nevertheless, it continues to be a safe and effective technique to improve dynamic rhytides and is the treatment of choice for the hypertrophic lateral fibers of the orbicularis oculi muscle that can cause the superimposed crow's feet.
OBJECTIVE: Although infrequent and self-limiting, the complication of unexpected muscle weakness from toxin diffusion or erroneous placement is documented.
METHODS: However, injection into the pretarsal portion of the orbicularis o

Myotonia occurs with abnormalities of the muscle membrane, and there have been reports of various channel abnormalities including Cl, Na, and Ca channels. Historically in 1966, Lipicky and Bryant found the abnormality of the muscle Cl channel in Thomsen's disease and hereditary myotonia of goat. Thereafter, Na channel abnormality was found in myotonia associated with familial hyperkalemic periodic paralysis and paramyotonia congenita by using patch clamp technique. Myotonic dystrophy is the most common myotonic disorder and genetic studies disclosed the presence of abnormal triplet repeat in t

Three patients with McArdle's disease exercised for 2 h at 30% VO2max. During exercise two phases occurred. During the first 15 min they experienced progressive fatigue and weakness of exercised muscles, with a rapid and complete recovery (adaptation phase). Following this, all 3 patients were able to continue exercise without difficulty ('second wind' phase). During the adaptation phase, patients have to cope with their inability to use muscle glycogen as a fuel. Processes occurring during this phase are as follows. An increase in cardiac output. This might be expected to increase muscle bloo

Camptocormia is a postural affliction characterised by abnormal flexion of the thoracolumbar spine, which appears upon standing, increases with walking, and disappears in the supine position. Only five cases of camptocormia secondary to idiopathic inflammatory myopathies have been described in the literature. In this case report, we describe the case of a male patient 67 years of age who suffered from polymyositis for 18 years, which was associated with manifestations compatible with camptocormia. The clinical manifestations were stable and did not progress. Polymyositis is an idiopathic infla

Separation of the levator ani muscles from pubic bone is a common major levator trauma that may occur in vaginal delivery and is associated with pelvic floor dysfunctions. We describe a novel ultrasound-guided technique to repair these muscles. A 33-year-old woman presented with a history of difficult vaginal delivery and complaint of numbness and weakness of the vagina. In evaluation, bilateral levator defects were diagnosed by physical examination, three-dimensional endovaginal ultrasound, and magnetic resonance imaging. With ultrasound guidance the detached ends of muscles were tagged and s

A prospective study was carried out on 24 volunteers who had suffered from paralytic poliomyelitis up to 51 years earlier. A quantitative EMG was carried out in each subject. Grossly raised mean amplitudes of the interference patterns were found in many strong muscles as well as in weak muscles. Such muscles frequently showed "contraction fasciculation", a manifestation of loss of normal small motor units. In many subjects the clinical and EMG evidence of chronic partial denervation were more widespread than the subjects had realised. It is suggested that if, with advancing age, minor damage o

INTRODUCTION: Both genetic and infectious diseases can result in skeletal muscle degeneration, inflammation, pain, and/or weakness. Duchenne muscular dystrophy (DMD) is the most common congenital muscle disease. DMD causes progressive muscle wasting due to mutations in Dystrophin. Influenza A and B viruses are frequently associated with muscle complications, especially in children. Infections activate an immune response and immunosuppressant drugs reduce DMD symptoms. These data suggest that the immune system may contribute to muscle pathology. However, roles of the immune response in DMD and

RATIONALE: Rhabdomyolysis (RM) may cause acute kidney injury (AKI), but the classic triad of myalgia, muscle weakness, and dark-colored urine is often absent. Fenofibrate-associated RM is rare and may be overlooked in older patients with chronic kidney disease (CKD), reduced renal reserve, or polypharmacy.
PATIENT CONCERNS: A 70-year-old man with CKD was admitted with nausea, poor appetite, and a marked increase in serum creatinine for 1 week. Two months earlier, atorvastatin had been discontinued, and micronized fenofibrate had been started for hypertriglyceridemia. He had no obvious myalgia

Autoimmune Myasthenia Gravis (MG) is a disease characterized by fatigable muscle weakness and autoantibodies. It can be divided by the presence of serum autoantibodies into two major categories where Immunoglobulin G (IgG) against either the acetylcholine receptor (AChR), or muscle specific kinase (MuSK) causes fatigable muscle weakness. The clinical relevance of Low-density lipoprotein-receptor related protein-4 IgG (LRP4) is debated. These antibodies disrupt neuromuscular transmission via different mechanisms: AChR antibodies, mostly of IgG1 and IgG3 subclass, can activate complement leading

Objectives and Background: To present a novel technique of treatment for a patient with basilar invagination. Basilar invagination (BI) is a congenital condition that can compress the cervicomedullary junction, leading to neurological deficits. Severe cases require surgical intervention, but there is debate over the choice of approach. The anterior approach allows direct decompression but carries high complication rates, while the posterior approach provides indirect decompression and offers good stability with fewer complications. Materials and Methods: A 15-year-old boy with severe myelopath

BACKGROUND: Risk for lower extremity musculoskeletal injury increases after sport-related concussion (SRC) and may result from unresolved motor control deficits. Muscle weakness is a deficit that could contribute to musculoskeletal injury risk.
HYPOTHESIS: Athletes with SRC will demonstrate quadriceps and hamstring muscle weakness at the time of return to sport and 30 days later compared with controls.
STUDY DESIGN: Prospective matched cohort.
LEVEL OF EVIDENCE: Level 3.
METHODS: A total of 31 athletes with SRC (CONCUSSION) were matched by sex, age, and activity level to controls (CONTROL)

BACKGROUND: Pompe disease is a lysosomal storage disorder caused by the deficiency of enzyme acid alpha-glucosidase (GAA) which results in accumulation of glycogen, particularly in the skeletal, cardiac, and smooth muscles. The late-onset form with symptoms presenting in childhood through adulthood, is characterized by proximal muscle weakness, respiratory insufficiency, and unlike the infantile-onset form often with no cardiac involvement.
METHODS: We report our experience with 18 adult patients (14 males/4 females) with Pompe disease, several of whom had unique findings and novel pathogenic

BACKGROUND: While dermatomyositis is often associated with malignancy, several autoimmune diseases like myositis can be caused by immune checkpoint inhibitors. Differentially diagnosing malignancy-associated dermatomyositis or myositis caused by immune checkpoint inhibitors is sometimes difficult, particularly when a patient with malignancy shows the symptoms of myositis after checkpoint inhibitor administration. We experienced such a case in which we had difficulties in diagnosing paraneoplastic dermatomyositis or drug-associated myositis. In this case, all of our team initially assumed that

BACKGROUND: Deficiency of electron transfer flavoprotein dehydrogenase (ETFDH) is associated with multiple acyl-CoA dehydrogenase deficiency (MADD). This disorder is an autosomal recessive lipid storage myopathy (LSM) that exhibits a wide range of clinical features, including myopathy, weakness and multisystem dysfunctions. Many patients with late onset of MADD improve when treated with riboflavin and are also referred to as RR-MADD (riboflavin-responsive multiple Acyl-CoA dehydrogenase disorder).
METHODS: In this study, we report the clinical and genetic characterization of a novel RR-MADD p

Nemaline myopathy type 6 (NEM6) is a rare neuromuscular disorder caused by pathogenic variants in the Kelch repeat and BTB domain-containing 13 (KBTBD13) gene. Patients experience muscle weakness, excessive fatigue, and impaired muscle relaxation that affect their daily activities related to abnormal protein aggregation in muscle cells and a predominance of slow-twitch myofibers, and no specific therapies are available. Here, we studied a clinically relevant Kbtbd13:p.Arg408Cys knockin mouse model to elucidate the molecular changes driving disease pathogenesis through measurements of muscle co

Cumulative human exposure to the environmental toxin, bisphenol A (BPA), has raised important health concerns in recent decades. However, the direct genomic regulation of BPA in skeletal muscles and its clinical significance are poorly understood. Therefore, we conducted a genome-wide transcriptome analysis after daily oral administration of BPA at the lowest observed adverse-effect level (LOAEL, 50 mg/kg) in male mice for six weeks to explore the gene-expression regulations in skeletal muscle induced by BPA. The primary Gene Ontology terms linked to BPA-dependent, differentially expressed gen

OBJECTIVE: ACTN2, encoding alpha-actinin-2, is essential for cardiac and skeletal muscle sarcomeric function. ACTN2 variants are a known cause of cardiomyopathy without skeletal muscle involvement. Recently, specific dominant monoallelic variants were reported as a rare cause of core myopathy of variable clinical onset, although the pathomechanism remains to be elucidated. The possibility of a recessively inherited ACTN2-myopathy has also been proposed in a single series.
METHODS: We provide clinical, imaging, and histological characterization of a series of patients with a novel biallelic AC

Total knee arthroplasty (TKA) improves patient-reported function by alleviating joint pain, however the surgical trauma exacerbates already impaired muscle function, which leads to further muscle weakness and disability after surgery. This early postoperative strength loss indicates a massive neural inhibition and is primarily driven by a deficit in quadriceps muscle activation, a process known as arthrogenic muscle inhibition (AMI). To enhance acute recovery of quadriceps muscle function and long-term rehabilitation of individuals after TKA, AMI must be significantly reduced in the early post
