Mental health literacy in Hong Kong.

BACKGROUND AND AIMS: The aim of this study was to investigate Hong Kong nationals' ability to recognize 13 different mental disorders and to examine whether there may be a relationship between their mental health literacy (MHL) and their tendency to describe/explain symptoms of mental disorders in physical terms. METHODS: A total of 299 participants took part in this study and the vignettes depict post-traumatic stress disorder, depression, obsessive-compulsive disorder (OCD), dependent personality disorder, schizotypal personality disorder, generalized anxiety disorder, agoraphobia, bipolar

State effects of major depression on the assessment of personality and personality disorder.

OBJECTIVE: The authors sought to determine whether personality disorders diagnosed during a depressive episode have long-term outcomes more typical of those of other patients with personality disorders or those of patients with noncomorbid major depression. METHOD: The authors used 6-year outcome data collected from the multisite Collaborative Longitudinal Personality Disorders Study (CLPS). Diagnoses and personality measures gathered from the study cohort at the index assessment using interview and self-report methods were associated with symptomatic, functional, and personality measures at

The metacognitive paradox of OCD: confidence is globally reduced but shows increased sensitivity to local evidence.

Confidence is a critical metacognitive signal that guides performance. Biases in confidence, such as excessive doubt, are hallmark features of mental health disorders, especially obsessive-compulsive disorder (OCD). Yet, the underlying neurocognitive mechanisms and how they link to learning and decision-making remain elusive. We asked patients with OCD and matched healthy controls to perform a novel rule-shifting task incorporating trial-by-trial confidence ratings. Using a Bayes-optimal model, we identified two distinct confidence biases: while patients with OCD indicated lower overall confid

Breaking Barriers With Sound: Focused Ultrasound in the Brain.

Focused ultrasound (FUS) is rapidly redefining the landscape of brain therapy, offering a noninvasive, highly precise alternative to traditional neurosurgical techniques. Enabled by advances in phased-array transducer technology, MRI-guided targeting and thermometry, and sophisticated treatment planning software, FUS delivers sub-millimeter accuracy through the skull while sparing surrounding tissue. This article provides a comprehensive yet accessible overview of the core technologies that make FUS possible, including phase correction for skull variability and real-time imaging for safety. We

Ligand coupling mechanism of the human serotonin transporter differentiates substrates from inhibitors.

The presynaptic serotonin transporter (SERT) clears extracellular serotonin following vesicular release to ensure temporal and spatial regulation of serotonergic signalling and neurotransmitter homeostasis. Prescription drugs used to treat neurobehavioral disorders, including depression, anxiety, and obsessive-compulsive disorder, trap SERT by blocking the transport cycle. In contrast, illicit drugs of abuse like amphetamines reverse SERT directionality, causing serotonin efflux. Both processes result in increased extracellular serotonin levels. By combining molecular dynamics simulations with

Year of Zoom in a Year of Doom: Lessons Learned Delivering ERP Remotely During the COVID-19 Pandemic.

In response to the COVID-19 pandemic and consequential shutdown measures, many mental health professionals started providing therapy to patients exclusively via telehealth. Our research center, which specializes in studying and treating obsessive-compulsive disorder (OCD), historically has provided in-person exposure and response prevention (ERP) to adults with OCD, but shifted to telehealth during the pandemic. Unlike in other modes of talk therapy, ERP's emphasis on therapist-supervised exposures presented unique opportunities and challenges to delivering treatment entirely via a virtual pla

Expanding the phenotype of HNRNPU-related neurodevelopmental disorder with emphasis on seizure phenotype and review of literature.

Pathogenic variants in heterogeneous nuclear ribonucleoprotein U (HNRNPU) results in a novel neurodevelopmental disorder recently delineated. Here, we report on 17 previously unpublished patients carrying HNRNPU pathogenic variants. All patients were found to harbor de novo loss-of-function variants except for one individual where the inheritance could not be determined, as a parent was unavailable for testing. All patients had seizures which started in early childhood, global developmental delay, intellectual disability, and dysmorphic features. In addition, hypotonia, behavioral abnormalitie

A case of late-onset OCD developing PLS and FTD.

We describe a 64-year-old woman, suffering from late-onset obsessive-compulsive disorder (OCD) from the age of 57, who developed dysarthria and dysphagia, spastic diplegic, and proximal muscles weakness. Needle electromyography showed no active denervation. Neuropsychological evaluation showed intact cognitive functioning. We diagnosed upper motor neuron disease (MND), with no known genetic correlates. Brain magnetic resonance (MRI) detected bilateral hippocampal atrophy with sclerosis of right hippocampus. 18F-FDG positron emission tomography (PET) showed moderate right temporal cortex thinni

Mouse models of neurodevelopmental disease of the basal ganglia and associated circuits.

This chapter focuses on neurodevelopmental diseases that are tightly linked to abnormal function of the striatum and connected structures. We begin with an overview of three representative diseases in which striatal dysfunction plays a key role--Tourette syndrome and obsessive-compulsive disorder, Rett's syndrome, and primary dystonia. These diseases highlight distinct etiologies that disrupt striatal integrity and function during development, and showcase the varied clinical manifestations of striatal dysfunction. We then review striatal organization and function, including evidence for stria

Abnormal subjective experiences of schizophrenia: evaluation of the Eppendorf Schizophrenia Inventory.

The Eppendorf Schizophrenia Inventory (ESI) is a new clinical questionnaire aimed at self-experienced cognitive disturbances that are considered to be diagnostically specific for schizophrenia. Two ESI validation studies are presented in this report. The aim of study I was to search for objective correlates of the ESI in 100 schizophrenic inpatients. In study II, the diagnostic validity of the ESI was examined by group comparisons (inpatients with first episode or chronic schizophrenia, alcoholism, depression, or obsessive-compulsive disorder, and healthy adults). Study I yielded numerous plau

Computer-aided CBT self-help for anxiety and depressive disorders: experience of a London clinic and future directions.

This article describes a broad-spectrum, computer-aided self-help clinic that raised the throughput of anxious/depressed patients per clinician and lowered per-patient time with a clinician without impairing effectiveness. Many sufferers improved by using one of four computer-aided systems of cognitive behavior therapy (CBT) self-help for phobia/panic, depression, obsessive-compulsive disorder, and general anxiety. The systems are accessible at home, two by phone and two by the Web. Initial brief screening by a clinician can be done by phone, and if patients get stuck they can obtain brief liv

The relationship between disgust sensitivity and avoidant behavior: studies of clinical and nonclinical samples.

Researchers have reported links between specific phobias (of small animals and blood/ injection/injury (BII)) and disgust sensitivity. Avoidance is one route by which disgust sensitivity may play a role in the etiology or maintenance of pathological fear. This paper discusses a series of three studies to examine behavior associated with disgust sensitivity. Study I examined disgust sensitivity and avoidance of disgust-evoking tasks among spider fearful and nonfearful participants. Study 2 reanalyzed previously published data on the relationship between disgust sensitivity and visual avoidance

Development of behavioral and emotional problems in Tourette syndrome.

Tourette syndrome is a movement disorder with multiple neuropsychiatric features. Using the parent form of the Child Behavior Checklist by Achenbach and Edelbrock, we identified behavioral and emotional difficulties in 78 males, 6-16 years of age, with Tourette syndrome. Symptoms most often identified included obsessive-compulsive behavior, aggressiveness, hyperactivity, immaturity, withdrawal, and somatic complaints. Abnormal checklist scores were most prevalent in adolescents with Tourette syndrome. Tic severity was not a statistically significant predictor of behavioral disturbance, althoug

When imagination turns into disorder: the case of maladaptive daydreaming.

Maladaptive daydreaming (MD) is defined as a psychological condition characterized by immersive, narrative-based fantasies that dominate waking consciousness and interfere with daily functioning. While daydreaming is an adaptive cognitive activity, above all in adolescents, MD represents an extreme and compulsive variant associated with significant emotional distress, social withdrawal, academic and occupational impairment. This narrative review offers a comprehensive synthesis of the current literature on MD, with particular emphasis on its clinical manifestations, neurobiological underpinnin

Mapping dysfunctional circuits in the frontal cortex using deep brain stimulation.

Frontal circuits play a critical role in motor, cognitive and affective processing, and their dysfunction may result in a variety of brain disorders. However, exactly which frontal domains mediate which (dys)functions remains largely elusive. We studied 534 deep brain stimulation electrodes implanted to treat four different brain disorders. By analyzing which connections were modulated for optimal therapeutic response across these disorders, we segregated the frontal cortex into circuits that had become dysfunctional in each of them. Dysfunctional circuits were topographically arranged from oc

Closed-loop systems for deep brain stimulation to treat neuropsychiatric disorders.

INTRODUCTION: A closed-loop or feedback-control system is a process which considers the system's output in order to automatically adjust the input. Compared to a traditional open-loop system, a closed-loop system allows for a higher degree of accuracy with minimal human intervention. Novel methods of closed loop 'adaptive' deep brain stimulation DBS (aDBS) are being developed. AREAS COVERED: This review focuses on the current state of aDBS for various neuropsychiatric conditions: common movement disorders such as Parkinson's disease, dystonia, essential tremor, and Tourette syndrome, as well

[Do cognitive-behavioral group therapies in the treatment of anxiety disorders have an advantage over individual treatments?].

Group cognitive-behavior therapies in the treatment of anxiety disorders for adult psychiatric patients were historically developed on the basis of validated individual treatments. They have been widely employed and studied for social phobia, panic disorders, agoraphobia and obsessive-compulsive disorders, with generally positive results similar to those obtained with the corresponding individual methods. The cognitive-behavioural group treatments for generalized anxiety and posttraumatic stress disorders have not yet received sufficient validation. The results of evaluative research show that

[Distribution of alexithymia as a personality-trait in psychosomatically ill in-patients--measured with TAS 20 and LEAS].

Preliminary findings of an ongoing study of the distribution of alexithymia in different diagnostic-groups of psychosomatically ill in-patients (n = 240, will be increased to n = 400) are reported. Alexithymiea is measured simultaneousely by the Levels of Emotional Awareness Scale (LEAS, a performance-test) and the 20-item Toronto Alexithymia Scale (TAS 20, a self-report-scale). Measured by the LEAS and compared with other diagnostic groups (affective, anxiety and compulsive-obsessive disorders; adjustment disorders; eating disorders), patients with somatoform disorders showed a decreased abil

Impaired emotional response to stress in mice lacking galectin-1 or galectin-3.

UNLABELLED: Galectin-1 (Gal-1) and galectin-3 (Gal-3) are multifunctional glycan-binding proteins, expressed in the brain and in its limbic structures that are involved in behavioral control. Gal-1 induces the expression of the brain-derived neurotrophic factor (BDNF) and promotes adult neural stem cells proliferation, biological events impaired in stress-related psychiatric disorders, such as depression and anxiety. Despite that, there is no evidence regarding galectin involvement in emotional control during stressful situations. Thus, we analyzed the behavioral phenotype of Gal-1 or Gal-3 kn

Differences and similarities in the sensory and cognitive signatures of voice-hearing, intrusions and thoughts.

BACKGROUND: Auditory hallucinations are frequently conceptualized as a disorder of input, whereby random discharges in language-related cortical areas lead to sensory irritations that mimic real voices. Alternatively, auditory hallucinations may represent a disorder of interpretation, whereby none of its four prevalent characteristics (the "four A's of hallucinations": acoustic, alien (i.e., appears as non-self), autonomous (i.e., beyond subjective control), authentic (i.e., appears like a real voice)) can reliably discriminate real versus imagined voices. METHOD: The study explored the resem
