The Anatomage Table and the placement of titanium mesh for the management of orbital floor fractures.

OBJECTIVE: The anatomy of the head and neck region is complex as are its implications for maxillofacial pathologic conditions and their surgical treatments. We hypothesize that the assessment of the surgical management of orbital floor fractures by using titanium mesh could represent an appropriate first experimental field for the use of the Anatomage Table in maxillofacial surgery. STUDY DESIGN: Patients with unilateral orbital floor fractures were searched for in the hospital database of Novara University Hospital, Novara, Italy. The Digital Imaging and Communications in Medicine data of pr

Neurosensory recovery after trauma to the orbital floor: a prospective trial with dexamethasone.

Our aims were to document the occurrence of neurosensory disturbances of the infraorbital nerve six months after operation for an orbital blow-out fracture, and to find out whether dexamethasone facilitates neurosensory regeneration. Patients were randomly assigned to one of two groups: the study group was given a total of dexamethasone 30mg, whereas the control group were given neither glucocorticoid nor placebo. Each patient's infraorbital neurosensory state was recorded preoperatively, immediately postoperatively, and six months later. A total of 18 patients were included, eight of whom had

Fatal orbital necrotizing fasciitis secondary to stenotrophomonas maltophilia associated stomatitis.

An 81-year-old woman with diabetes mellitus presented with signs of orbital cellulitis and a gangrenous ulcer on the right palate. Biopsy analysis confirmed a diagnosis of necrotizing fasciitis. Surgical debridement of necrotic tissues and enucleation of the right eye were performed. Stenotrophomonas maltophilia was isolated from the specimens. The patient remained stable under antibiotic therapy and underwent reconstructive surgery. To the best of our knowledge, this represents a rare case of orbital necrotizing fasciitis developing secondary to S. maltophilia infection. Orbital necrotizing f

Three-Dimensional Printed Model and Transantral Endoscopy to Orbital Fracture Repair.

Orbital fractures are high prevalent and result in several complications such as diplopia, muscular entrapment, visual impairment, and enophthalmos. The goal of orbital reconstruction is to restore orbital anatomy, volume, and globe symmetry. This case report aims to describe the use of transantral endoscopy and 3D printed model for treatment of an orbital floor fracture. A 54-year-old woman presented orbital floor fracture with diplopia and extraocular muscle entrapment. The surgical treatment was performed using a standard titanium mesh bended over 3D printed model, and transantral endoscopy

The influence of concomitant medial wall fracture on the results of orbital floor reconstruction.

INTRODUCTION: Up to 35% of orbital floor fractures extend to the medial wall. This results in restriction of both abduction and adduction, leading to horizontal diplopia. The greater the defect, the more pronounced the enophthalmos. AIM OF THE STUDY: The aim of the study was to determine the influence of concomitant medial wall defects on enophthalmos and diplopia, and the influence of intraoperative revision on the results of surgical reconstruction in patients with orbital floor fracture. MATERIAL AND METHODS: 78 cases of orbital floor fracture, with or without concomitant medial wall defe

Complications of orbital endoimplantation in the Eye Clinic of the Lithuanian University of Health Sciences.

The aim of the study was to analyse the rate of complications of orbital endoimplantation in patients operated from 2002 to 2014 at the Eye Clinic of the Lithuanian University of Health Sciences and to compare it with the results in the literature. Enucleation must be performed very carefully in order to prevent any additional trauma, infection, deformation, and to create an optimal conjunctival socket. However, complications occur despite efforts and qualified surgeons. The most common complications described in the literature are thinning and cysts of the conjunctiva, a foreign body reaction

Rhino-Orbital-Cerebral Mucormycosis: A Fatal Complication of Uncontrolled Diabetes Mellitus.

Mucormycosis is a progressively invasive disease, with a fatal outcome, on late presentation. A 38-year-old female presented with diabetic ketoacidosis with right eye ptosis and a frozen globe without any signs of inflammation, externally. She underwent transnasal endoscopic debridement of paranasal sinuses and exenteration of the right eye. The histopathology specimen revealed the growth of mucormycosis. She was treated with intravenous (IV) amphotericin B, IV insulin, and extensive debridement surgery, but had an unfavorable outcome due to rapid mucor invasion to the brain.

[Orbital cellulitis complicated by subperiosteal abscess due to Streptococcus pyogenes infection].

BACKGROUND: Orbital cellulitis is an infectious disease that is very common in pediatric patients, in which severe complications may develop. Etiological agents related to this disease are Haemophilus influenzae B, Staphylococcus aureus, Streptococcus pneumoniae and Moraxella catarrhalis, which correspond to 95% of cases. Moreover, Streptococcus beta hemolytic and anaerobic microorganisms may also be present corresponding to < 5% of the cases. We present an uncommon case of cellulitis complicated by sub-periosteal abscess caused by Streptococcus pyogenes (Group A beta hemolytic streptococcus).

Orbital Cellulitis and Secondary Angle Closure: A Rare Presentation of Choroidal Melanoma.

We describe a case of choroidal melanoma initially presenting with orbital cellulitis, fulminant conjunctival swelling, and secondary angle closure. Despite treatment with intravenous antibiotics, the patient's condition did not improve. With further investigations including ultrasound scan and magnetic resonance imaging, a high-density lesion was found within the globe. Characteristic imaging findings suggested a malignant origin and the lesion was found to be a melanoma on histopathological analysis.

Increased Incidence of Rhino-Orbital-Cerebral Mucormycosis After Colorado Flooding.

In September 2013, central Colorado experienced a record amount of rainfall resulting in widespread flooding. Within 1 month of the flooding, 4 patients presented to the authors' institution with rhino-orbital-cerebral mucormycosis. This represents the largest number of cases ever recorded over a 1-month period. The authors hypothesize that the combination of immunocompromised status and environmental exposure resulted in the increased incidence.
