National data system on near miss and maternal death: shifting from maternal risk to public health impact in Nigeria.

BACKGROUND: The lack of reliable and up-to-date statistics on maternal deaths and disabilities remains a major challenge to the implementation of Nigeria's Road Map to Accelerate the Millennium Development Goal related to Maternal Health (MDG-5). There are currently no functioning national data sources on maternal deaths and disabilities that could serve as reference points for programme managers, health advocates and policy makers. While awaiting the success of efforts targeted at overcoming the barriers facing establishment of population-based data systems, referral institutions in Nigeria c

Pharmacokinetics of 8-hour intravenous infusion of NXY-059: a phase I, randomized, double-blind (within dose panels), placebo-controlled study in healthy Chinese volunteers.

BACKGROUND: NXY-059 is a free radical-trapping neuroprotectant that has been reported to reduce infarct size and preserve brain function in experimental models of acute ischemic stroke. NXY-059 administered as an 8- or 72-hour IV infusion has been reported to be well tolerated in healthy young (age, 20-45 years) and older (55-75 years) white volunteers. NXY-059 is no longer in development following a lack of efficacy found in a Phase III trial in patients with acute ischemic stroke. OBJECTIVES: The primary objectives of this study were to determine the pharmacokinetic (PK) properties of an 8-

Neuroprotection in cerebral ischaemia: facts and fancies--the need for new approaches.

UNLABELLED: 'Neuroprotection' is a term used to describe the putative effect of interventions protecting the brain from pathological damage. In occlusive stroke, the concept of neuroprotection involves inhibition of a cascade of pathological molecular events occurring under ischaemia and leading to calcium influx, activation of free radical reactions and cell death. This article will summarize neuroprotection trials to date, some facts and fancies about neuroprotection, ischaemic pathophysiology and possible reasons for the apparent failure of human neuroprotective stroke trials. FACTS: In th

Prevention of disabling back injuries in nurses by the use of mechanical patient lift systems.

Occupational back pain in nurses (OBPN) constitutes a major source of morbidity in the health care environment. According to the National Institute for Occupational Safety and Health (NIOSH), occupational back injury is the second leading occupational injury in the United States. Among health care personnel, nurses have the highest rate of back pain, with an annual prevalence of 40-50% and a lifetime prevalence of 35-80%. The American Nursing Association believes that manual patient handling is unsafe and is directly responsible for musculoskeletal disorders encountered in nurses. It has been

Commissioning and clinical implementation of an MLC tracking system: An evaluation of AAPM TG-264 guidelines.

BACKGROUND: Precise radiotherapy relies on accurately targeting tumours while minimising exposure to healthy tissue, yet patient and organ motion complicate treatment delivery. To address intra-fractional motion, multi-leaf collimator (MLC) tracking systems have recently been adopted, adapting beam shapes in real-time. The American Association of Physicists in Medicine (AAPM) Task Group 264 (TG-264) provides guidelines for safely commissioning such tracking systems, yet these guidelines were initially developed for conventional linear accelerators and require evaluation, especially for newer p

Maximising the public health benefits of smokefree prisons: a synopsis from a mixed-methods study.

BACKGROUND: High levels of smoking among people who experience imprisonment contribute to their high mortality and morbidity rates and to inequalities. Scotland's prisons became smokefree in 2018. However, questions remain about how to prevent high relapse to smoking post release. OBJECTIVES: Summarise evidence on supporting people to reduce tobacco-related harms post release. Understand experiences, opportunities and challenges for reducing tobacco-related harms for people leaving smokefree prisons and for families. Feasibility test a household-targeted intervention to support people release

Ketogenic diet for status epilepticus in adult intensive care unit patients: a standard operating procedure.

INTRODUCTION: Refractory and super-refractory status epilepticus (RSE and SRSE) are life-threatening neurological emergencies with significant morbidity and mortality. When conventional treatment strategies, including antiseizure medications (ASM) and anesthetic agents, fail, alternative metabolic approaches, such as the ketogenic diet (KD), have been considered. KD is a high-fat, low-carbohydrate, and adequate-protein diet that induces ketosis, exerting anticonvulsant effects. Different types of KD are available, including the classical KD and the modified Atkins diet (MAD). Most published ev

An exploration of UK speech and language therapists' treatment and management of functional communication disorders: A mixed-methods online survey.

BACKGROUND: Functional Communication Disorders (FCDs) are one specific presentation of Functional Neurological Disorder (FND). FND is characterised by neurological symptoms, such as sensory and motor symptoms, which are not explained by neurological disease. Speech and language therapists (SLTs) have expertise in managing communication disorders, including FCDs, though is not known is what clinicians do in practice to treat and manage FCDs. AIM: To explore the clinical practices of SLTs who regularly manage FCDs in the UK, including the assessment and intervention approaches taken. METHODS &

The perinatal bereavement project: development and evaluation of supportive guidelines for families experiencing stillbirth and neonatal death in Southeast Brazil-a quasi-experimental before-and-after study.

BACKGROUND: For most parents, getting pregnant means having a child. Generally, the couple outlines plans and has expectations regarding the baby. When these plans are interrupted because of a perinatal loss, it turns out to be a traumatic experience for the family. Validating the grief of these losses has been a challenge to Brazilian society, which is evident considering the childbirth care offered to bereaved families in maternity wards. Positively assessed care that brings physical and emotional memories about the baby has a positive impact on the bereavement process that family undergoes.

[The effect of needle type and immobilization on postspinal headache].

UNLABELLED: Post-dural puncture headache (PDPH) is a significant complication of spinal anaesthesia. Diameter and tip of the needle as well as the patient's age have been proven to be important determinants. The question of whether post-operative recumbency can reduce the risk of PDPH has not been answered uniformly. And besides, some studies referring to this subject reveal methodical failures, for example, as to clear definition and exact documentation of post-operative immobilization. Furthermore, fine-gauge needles (26G or more) have not been investigated yet. The first aim of our study wa

How do we counsel patients on short- and long-term complications after hypospadias repair? - A survey study.

INTRODUCTION: Hypospadias correction remains one of the most performed pediatric urologic procedures, affecting up to 1/150 males born in the United States. Current studies suggest that surgical counseling has a significant impact on shared decision making, decisional regret, and long-term follow-up. However, no set paradigm currently exists for long-term follow-up or counseling. We sought to obtain consensus from established pediatric urologists the optimal content and potential short, intermediate, and long-term complications to be considered when counseling patients and parents of patients

Speech and Language Therapy Weekend Service in Inpatient Rehabilitation: A Qualitative Study Exploring Perspectives of People With Stroke.

BACKGROUND: An allied health weekend service in subacute inpatient stroke rehabilitation can improve patient and organisational outcomes. However, there is insufficient evidence to justify the role of a speech and language therapy (SLT) weekend service in this setting. Exploring the perspectives of individuals who have received SLT services in inpatient stroke rehabilitation will assist with understanding the current role of SLT and the potential for SLT weekend services in inpatient stroke rehabilitation. AIMS: This study aimed to: (1) determine the aspects of an SLT inpatient rehabilitation

Evaluation of triage methods used to select patients with suspected pandemic influenza for hospital admission: cohort study.

BACKGROUND: Triage methods are necessary in emergency departments to provide clinicians with a reliable method for determining each patient's risk of adverse outcome. Prior to the 2009 H1N1 influenza pandemic the CURB-65 (a risk prediction score for pneumonia, based on confusion, urea level, respiratory rate, blood pressure and age over 65 years) pneumonia score and the Pandemic Modified Early Warning Score (PMEWS) were used to assess adults. In response to the emergence of the pandemic, national guidance produced a new swine flu hospital pathway for use adults and children. However, none of t

Report from the National Transplantation Pregnancy Registry: outcomes of pregnancy after transplantation.

Experience in the field of pregnancy posttransplantation has been gained through continued case reports, center reports, and registry data. The NTPR maintains an ongoing active database to study the safety of pregnancy and includes the outcomes of female transplant recipients as well as male recipients who father pregnancies. Analyses are ongoing and include long-term followup of recipients' graft status and of their offspring. For the most part, guidelines proposed in 1976 for counseling recipients remain applicable (30). While these counseling guidelines were formulated for kidney recipients

Lead concentrations in tibial and calcaneal bone in relation to the history of occupational lead exposure.

INTRODUCTION: Providing local consultant-delivered hospital services in remote and island communities in the United Kingdom is increasingly problematic due to difficulties with recruitment and retention of staff, statutory restrictions to hours worked by health professionals and the expectation each clinician must manage an externally defined volume of cases to maintain clinical standards. This article describes a before-and-after evaluation of a novel method of providing consultant support for acute internal medicine to an island grouping off the Scottish coast. Under the scheme, local GPs pr

Issues in subject recruitment and retention with pregnant and parenting substance-abusing women.

To advance knowledge about the treatment of addiction among pregnant women and other women of childbearing age, investigators must adhere to the requirements of a strict experimental research design while concurrently providing clinical services. This means that researchers must address a variety of difficult questions, including the following: Was the sample large enough? Were the criteria for subject inclusion and exclusion well defined? Did the process of recruitment result in a sample that could be generalized to a larger population, or was the sample biased in some way? Was assignment to

Pain and symptom control in paediatric palliative care.

Important differences become evident in a comparison of cancer pain between children and adults. Management of pain in children is commonly multidisciplinary, is less dependent on invasive measures and relies more on systemic therapy. Children are not little adults: their immaturity, developing cognition and dependence all influence their experience and interpretation of pain. Much progress has been made in altering practices such as under-prescribing and underdosing that have adversely affected adequate control of pain in children. The challenge for paediatric health care providers in the mid

Symptom Management Following Radioactive Iodine Treatment in Adults With Thyroid Cancer: Usability Evaluation and Proof-of-Concept Study.

BACKGROUND: Thyroid cancer is often labeled a "good cancer" due to high survival rates. However, many patients diagnosed with differentiated thyroid cancer (DTC) treated with radioactive iodine (RAI) experience short- and long-term symptoms as a result of treatment. These include salivary dysfunction, dry mouth, dry eyes, and nausea. Despite these challenges, patients report limited knowledge and unmet informational needs regarding RAI symptoms and symptom management. Evidence-based, patient-centered education to support symptom management during DTC survivorship is needed. Our team developed

Views of People With Psychosis About Algorithm-Based Relapse Prediction and Data Sharing: Qualitative Study.

BACKGROUND: Preventing relapses of psychosis is difficult and important. Digital remote monitoring (DRM) systems are being developed and tested to support this. Increasingly, these systems use algorithm-based relapse prediction. Hence, understanding stakeholder views about algorithmic prediction is crucial. Existing qualitative work has explored health professionals' views, but very few studies have examined the perspectives of people with psychosis on this topic. OBJECTIVE: This paper aimed to provide an in-depth examination of the views of people with psychosis regarding algorithmic relapse
