Asymmetry in optic disc morphometry as measured by confocal scanning laser ophthalmoscopy in subjects with hyperopic anisometropia.

PURPOSE: To determine whether there is an asymmetry in optic nerve head morphometry in subjects with hyperopic anisometropia. METHODS: Seventeen men and 20 women with a mean age of 31.43 +/- 2.73 (standard error of mean) years underwent a complete eye examination and optic nerve head topography with the Heidelberg Retina Tomograph II (Heidelberg Engineering, GmBH, Heidelberg, Germany). Differences in optic nerve head parameters between anisometropic eyes and fellow eyes were evaluated using the paired-sample t test or Wilcoxon signed rank test and the correlations between refraction, visual a

Pars plana capsulectomy and vitrectomy for posterior capsular opacification in pseudophakic children.

PURPOSE: To evaluate the safety and efficacy of pars plana capsulectomy and vitrectomy for posterior capsular opacification (PCO) in pseudophakic children. METHODS: Pars plana capsulectomy and vitrectomy was performed for PCO in 63 pseudophakic eyes of 57 children with an infusion through sutureless clear cornea. The patients' ages ranged from 3 to 12 years (mean: 5.8 +/- 1.9 years). Surgical technique, intraoperative and postoperative complications, visual acuity, intraocular pressure (IOP), and corneal endothelial cell density were recorded. RESULTS: The surgical procedure was performed un

Ocular presentation of pediatric Miller-Fisher syndrome.

The authors describe a case of Miller-Fisher syndrome in a child who presented to the ophthalmology department with bilateral abducens nerve palsies. Miller-Fisher syndrome is an important differential diagnosis in any case of bilateral sixth nerve palsies but should only be definitively diagnosed once tumors, infections, and other neurological diseases have been conclusively ruled out.

Chronic anterior uveitis following bacille Calmette-Guérin vaccination: molecular mimicry in action?

A 13-year-old girl developed bilateral chronic anterior uveitis following bacille Calmette-Guérin (BCG) vaccination. HLA testing was negative for B27 but positive for DRB1 *0404, a variant of DR4 often associated with rheumatoid arthritis. The authors propose her HLA repertoire allowed for a BCG-induced abnormal autoimmune response by the mechanism of molecular mimicry.

A rare case of retinocytoma occurring in a 12-year-old child.

A 12-year-old girl with a translucent, partially calcified retinal mass in the posterior pole surrounded by a rim of pigment epithelial changes and chorioretinal atrophy was diagnosed as having retinocytoma. The patient received a single treatment of transpupillary thermotherapy at the slit lamp to prevent malignant transformation. The dimensions of the lesion and the visual acuity remained unchanged at 3.5 years of follow-up with no further treatment. Retinocytoma is a benign retinal tumor that carries the same genetic implications as retinoblastoma. Regular follow-up is necessary because the

Optic nerve hypoplasia potentiates retinopathy of prematurity.

The risk of progression to threshold retinopathy of prematurity correlates with the area of avascular retinas and inversely with the degree of vascular maturity. Asymmetric retinal ablation was performed in two patients and asymmetric retinopathy of prematurity progression was observed in two infants with optic nerve hypoplasia.

Success rate and cause of failure for late probing for congenital nasolacrimal duct obstruction.

PURPOSE: To evaluate the outcome of late probing for congenital nasolacrimal duct obstruction in children 2 to 6 years old and to identify the cause of failure in these children. METHODS: Records of 65 nasolacrimal duct obstructions in 62 children (39 boys and 23 girls) aged 2 to 6 years who underwent primary probing for congenital nasolacrimal duct obstructions were evaluated. The main observations were the type of obstruction encountered during probing (complex/simple), age at the time of probing, and success rate depending on the type of obstruction. Success was the main outcome measure an

Safety and efficacy of silicone rod frontalis suspension surgery for childhood ptosis repair.

PURPOSE: To evaluate the safety and efficacy of silicone rod frontalis suspension surgery for childhood ptosis. METHODS: The authors retrospectively studied 89 consecutive children (110 eyelids) who had silicone rod frontalis suspension surgery for ptosis at Duke University Eye Center from 1983 to 2004. Marginal reflex distance1 (MRD1) elevation of 2 mm or more (vs preoperative MRD1) was considered satisfactory. MRD1 was measured as the vertical distance from the corneal light reflex in primary gaze to the upper eyelid margin. The postoperative eyelid symmetry (< or = 1 mm = satisfactory) was

Parry-Romberg syndrome presenting with recurrent exotropia and torticollis.

Parry-Romberg syndrome, or progressive facial hemiatrophy, is a unilateral, slowly progressive atrophy that affects skin, subcutaneous tissues, muscles, and bones. The authors describe an 8-year-old girl with Parry-Romberg syndrome, amblyopia, recurrent exotropia, and torticollis, and stress that the association between this syndrome and amblyopia, recurrent exotropia, and torticollis should be recognized.

Cataract surgery in pediatric uveitis.

Uveitis in children is associated with several sight-threatening ocular complications, including the formation of cataracts. The surgical management of uveitic cataracts in children is both challenging and controversial and, unlike in adult uveitic cataracts, surgery has historically been associated with poor visual outcomes. Juvenile idiopathic arthritis-associated uveitis in particular poses unique therapeutic challenges and the issue of correction of aphakia in these patients remains a contentious one. The growing use of immunotherapies and, where needed, targeted biologic agents in childho

Pupillography of relative afferent pupillary defects in amblyopia associated with peripapillary myelinated nerve fibers and myopia.

Pupillary responses to light were recorded in three patients with unilateral extensive myelinated nerve fibers and amblyopia by means of binocular infrared video pupillography. All of the patients had an afferent pupillary defect in the eye with myelinated nerve fibers. This finding supports the notion that this type of amblyopia is a severe form of anisometropic amblyopia that is often resistant to treatment.

Corneal superinfection in acute viral conjunctivitis in young children.

Two children suffered from corneal superinfection following acute viral keratoconjunctivitis. Group A beta-hemolytic streptococci were isolated in one child and cultures were negative in the other child. The corneal ulcers resolved with prompt antibiotic therapy, but both patients developed amblyopia. Visual outcome following corneal superinfection in acute viral conjunctivitis may be poor when it occurs in young children.
