Disorder-specific and transdiagnostic vulnerability to posttraumatic stress symptoms: A machine learning approach.

A wide range of biological, cognitive, affective, and behavioral risk factors have been studied in relation to posttraumatic stress disorder. Previous work has often isolated a single risk factor or a small number of risk factors, making it is difficult to know which may be the most important to study or target in interventions. We used a supervised machine learning technique, elastic net, to test the associations between posttraumatic stress symptoms (PTSS) and several self-reported risk factors at the full-scale, subscale, and item levels in a large online sample (N = 1,186) of individuals w

Blast injury and chronic psychiatric disability in military personnel: Exploring the association beyond posttraumatic stress disorder.

INTRODUCTION: Blast injuries are common among military personnel, yet their long-term psychiatric consequences, beyond posttraumatic stress disorder (PTSD) and traumatic brain injury (TBI), remain underexplored. This study investigates the association between blast injuries and non-PTSD psychiatric conditions, including psychotic, neurocognitive, mood, anxiety, adjustment and personality disorders resulting in functional impairment. METHODS: Data were collected from three cross-referenced registries that span the continuum of trauma care from 2006 to 2021. Demographics, injury mechanisms, and

Spring PGD versus waiting list control in the treatment of prolonged grief disorder (PGD): protocol for a feasibility randomised controlled trial (RCT).

Background: Prolonged Grief Disorder (PGD) is characterised by persistent longing or preoccupation with a deceased loved one, accompanied by intense emotional pain that lasts six-months or more and significantly impairs functioning. While Cognitive Behavioural Therapy (CBT) with a grief focus is effective, access is limited due to high costs and therapist shortages. Guided digital therapy, which delivers psychological support via an app or website with professional guidance, may offer a scalable solution. Building on the success of a guided digital intervention for post-traumatic stress disord

Psychological intervention in patients with central sensitization syndromes and comorbid psychosocial trauma: a systematic review and meta-analysis.

The co-occurrence of central sensitization syndromes (CSS) and post-traumatic symptoms (PTSS) or post-traumatic stress disorder (PTSD) exacerbates impairment, and treatment is unsatisfactory in many cases. This systematic review and meta-analysis examined the effectiveness of psychological therapy in individuals with CSS and comorbid PTSS/PTSD. A search of Medline, PsycInfo, Web of Science, Pubmed, and Scopus databases (2000 to 2023) was conducted following PRISMA guidelines. Nineteen articles were selected, including nine for meta-analysis. The main psychological therapies were pain-focused c

Vagus nerve stimulation therapy for treatment-resistant PTSD.

BACKGROUND: Posttraumatic stress disorder (PTSD) is common and debilitating, and many individuals do not respond to existing therapies. We developed a fundamentally novel neuromodulation-based therapy for treatment-resistant PTSD. This approach is premised on coupling prolonged exposure therapy, a first-line evidence-based cognitive behavioral therapy that directs changes within fear networks, with concurrent delivery of short bursts of vagus nerve stimulation (VNS), which enhance synaptic plasticity. METHODS: We performed a first-in-human prospective open-label early feasibility study (EFS)

The Impact of a Brief Psycho-Education and Skills Intervention on Trauma Symptoms in a Prison Setting: Proof of Concept Using Group and Case Analysis.

The links between trauma and poorer physical health and psychological well-being in adulthood are now well established. Research shows levels of trauma are especially high amongst those who are incarcerated although evidence on what works to help this group is limited. To examine the effectiveness and acceptability of participation in a new brief, structured group intervention in fostering stabilization through reducing the symptoms of post-traumatic stress disorder (PTSD) and improving mental health in male inmates who have experienced trauma. Participants completed questionnaire measures bef

Serum lipid and plasma fatty acid profiles in PTSD patients and healthy individuals: Associations with symptoms, cognitive function, and inflammatory markers.

Increasing evidence suggests that posttraumatic stress disorder (PTSD), a serious mental health condition, is associated with physical health problems. Lipid-related molecules are crucial for central nervous system functions associated with PTSD symptoms; however, case-control studies exploring the relationship between PTSD and lipid-related molecules are scarce. We examined 68 civilian PTSD patients and 97 healthy controls, evaluating PTSD symptoms, childhood maltreatment history, suicidality, and cognitive functions. Cholesterol, triglycerides, and inflammation-related marker levels were ana

Intersections of Community Violence Exposure, Posttraumatic Stress Symptoms, and Alcohol and Marijuana Use Among Young Adults.

INTRODUCTION: Community violence exposure (CVE) is an understudied facet of traumatic experiences, and much of the relatively limited literature investigating outcomes associated with CVE, such as Posttraumatic Stress Symptoms (PTSS) and substance use, have centered other trauma types and overlooked differences related to sex, race, and ethnicity. With respect to CVE, direct victimization or CVE broadly has often been the focus. Regarding substance use, alcohol use has been the primary substance studied in the literature, and marijuana has been relatively understudied. METHODS: Considering th

The role of pubertal development in the association between trauma and internalising symptoms in female youth.

BACKGROUND: Exposure to trauma in childhood is associated with an increased risk for internalising symptoms. Alterations in pubertal development has been proposed as a potential mechanism underpinning this association. However, longitudinal studies, which are needed to examine pubertal development over time, are scarce. The goal of this pre-registered study was to examine how trauma exposure shapes the timing and tempo of pubertal development, and in turn contributes to risk for internalising symptoms in female youth. METHODS: Using the largest longitudinal sample to date, we characterised pr

Mental health phenotypes of well-controlled HIV in Uganda.

INTRODUCTION: The phenotypic expression of mental health (MH) conditions among people with HIV (PWH) in Uganda and worldwide are heterogeneous. Accordingly, there has been a shift toward identifying MH phenotypes using data-driven methods capable of identifying novel insights into mechanisms of divergent MH phenotypes among PWH. We leverage the analytic strengths of machine learning combined with inferential methods to identify novel MH phenotypes among PWH and the underlying explanatory features. METHODS: A total of 277 PWH (46% female, median age = 44; 93% virally suppressed [<50copies/mL])

Can Masturbation Regulate PTSD Symptoms? Exploring the Mediating Role of PTSD in Childhood Sexual Abuse and Masturbation Motives.

Childhood sexual abuse (CSA) is a significant public health issue with profound and long-lasting effects on survivors' emotional, psychological, and sexual well-being. While extensive research has examined the interpersonal sexual challenges associated with CSA, less is known about its association with solitary sexual behaviors such as masturbation, particularly the underlying motives or reasons. This study examined the mediating role of post-traumatic stress disorder (PTSD) symptoms in the relationship between CSA and three specific masturbation motives: mood improvement, relaxation/stress re

Unveiling Suicidal Risk in Young Child Sexual Abuse Victims: Prevalence and Predictive Markers.

Child sexual abuse has repeatedly been identified as a risk factor for suicidal ideation and behavior, yet most research has focused on adolescents and young adults. Very little is known about suicidality in children exposed to sexual abuse in middle childhood, which is a developmental period marked by unique cognitive and socioemotional specificities. Gaining a better understanding of risk factors in this subgroup is crucial to inform age-appropriate prevention and intervention efforts. This study aimed to: 1) determine the prevalence of suicidal ideation and behavior in a sample of child vic

[Delirium: long-term sequelae and aftercare with special consideration to posttraumatic stress disorder].

Delirium is a frequent syndrome among hospitalized patients, with a prevalence of 10-30% on general wards, up to 50% on palliative care units and 30-80% on intensive care units (ICU). Defined in the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-V) as an acute, reversible disturbance of attention, cognition and consciousness, delirium is associated with infections, surgery, trauma and other disease conditions. Risk factors include advanced age, pre-existing neurocognitive disorders and substance abuse. Delirium is associated with increased mortality, longer hospitali

Improving perinatal mental health outcomes: The role of support in assisting breastfeeding experiences following birth trauma.

BACKGROUND: Experiencing any traumatic event can have long lasting impacts on mental health. In the context of childbirth, a traumatic experience could directly or indirectly impact a mother's ability to successfully breastfeed and affect her long-term mental health. Being able to breastfeed successfully is known to improve mental health outcomes for mothers. AIM: To explore how self-reported birth-related trauma symptoms affect breastfeeding and the role of support in shaping these experiences, to inform trauma-informed breastfeeding support services. METHODS: A convenience sample of 93 mot

The Contribution of Anesthesia Management to Childbirth-Related Posttraumatic Stress Disorder: A Case of Trauma After Denied Neuraxial Analgesia, Unmet Expectations and Remifentanil Labor Analgesia.

Childbirth-Related Posttraumatic Stress Disorder (CB-PTSD) can arise even after low-risk deliveries. This case report describes a 35-year-old woman who, despite a medically uncomplicated vaginal birth, developed severe CB-PTSD after being denied neuraxial analgesia and receiving remifentanil-PCA (remi-PCA) to manage pain. Her distress stemmed from inadequate pain relief, dissociation, and loss of control, exacerbated by unmet expectations for epidural analgesia. Five years later, she continues to process the trauma. This case underscores the importance of informed consent, patient-centered ana

Unraveling the interconnectedness of trauma-related common mental disorders in Somali refugees: a network analysis.

BACKGROUND: Refugee populations, particularly Somali refugees, face disproportionately high rates of trauma-related common mental disorders (CMDs) due to prolonged exposure to war, forced displacement, and post-migration adversities. Traditional diagnostic models often conceptualize CMDs, including posttraumatic stress, depression, anxiety, and somatic symptoms, as distinct disorders, overlooking the symptom-level interconnections that sustain psychological distress. This study employs network analysis to examine the structure of PTSD, depression, anxiety, and somatic symptoms, identifying cen

A verbal prescreen for intrusive symptoms improves posttraumatic stress disorder screening efficiency.

BACKGROUND: The American College of Surgeons Committee on Trauma requires that trauma centers have a structured approach to identifying patients at risk for mental health conditions. However, the college does not recommend any specific screening protocol. The Posttraumatic Stress Disorder Checklist for DSM-5 (PCL-5) is a validated, 20-question screening instrument used at many trauma centers. This study evaluates the diagnostic performance of a verbal, two-question prescreen based on the PCL-5. METHODS: All traumatically injured patients were screened for posttraumatic stress disorder (PTSD)
