Renin-angiotensin system in osteoarthritis: A new potential therapy.

Osteoarthritis (OA) is one of the most common chronic joint diseases. However, the mechanism remains unclear. The traditional renin-angiotensin system (RAS) is an important system for regulating homeostasis and controlling balance. In recent years, RAS-related components have played an important role in the occurrence of OA. The purpose of this review is to summarize the research results of RAS-related components that are associated with OA. This study systematically searched e-medical databases such as PubMed, Embase, Medline, and Web of Science. The search targets included English publicatio

The art of matching brain tissue from patients and controls for postmortem research.

The quality of postmortem research depends strongly on a thorough clinical investigation and documentation of the patient's disorder and therapies. In addition, a systematic and professional neuropathologic investigation of both cases and controls is absolutely crucial. In the experience of the Netherlands Brain Bank (NBB), about 20% of clinical neurologic diagnoses, despite being made in first-rate clinics, have to be revised or require an extra diagnosis after a complete and thorough review by the NBB. The neuropathology examination may reveal for instance that the "controls" already have pr

A neurological disease on the face of an ancient Roman sculpture.

BACKGROUND: The present case report describes a complication after a percutaneous spine surgery technique that is highly uncommon in clinical practice: a bone cement cardiac embolism. This rare complication emphasizes the importance of this case, which is also interesting considering the midterm follow-up. Documented cardiac embolisms published in the literature (which are scarce) describe the acute phase of these cases but lack follow-up. There are no systematic reviews on this topic, only case-by-case presentations, and surgeons are not aware of its real implications. CASE: We report a case

[Uveitis associated with juvenile idiopathic arthritis : Optimization of immunomodulatory therapy].

BACKGROUND: Uveitis associated with juvenile idiopathic arthritis (JIA-associated uveitis) is a vision-threatening disorder with a high complication rate. Besides early diagnosis within screening programs an adequate therapy is essential for improvement of the long-term prognosis. Corticosteroid therapy is often insufficient. In addition to conventional immunosuppression, immunomodulatory drugs, so-called biologicals, are novel highly effective treatment modalities. OBJECTIVE: A systematic search of the literature was carried out for biologicals currently used in the treatment of JIA-associat

The role of azithromycin in healthcare-associated pneumonia treatment.

WHAT IS KNOWN AND OBJECTIVE: Healthcare-associated pneumonia (HCAP) is an important presentation among hospitalized patients. Standardized definitions of this entity are almost a decade old, and practice patterns have shifted from published guidelines to include empiric coverage with a macrolide, such as azithromycin. Azithromycin is oftentimes included in the empiric treatment regimen for HCAP because of the importance of appropriate empiric antimicrobial coverage, the perceived concern regarding atypical organisms, potential anti-inflammatory effects of the medication, and positive clinical

Evidence-based guidance on venous thromboembolism in patients with solid tumours.

Venous thromboembolism (vte) is a serious, life-threatening complication of cancer. Anticoagulation therapy such as low molecular weight heparin (lmwh) has been shown to treat and prevent vte. Cancer therapy is often complex and ongoing, making the management of vte less straightforward in patients with cancer. There are no published Canadian guidelines available to suggest appropriate strategies for the management of vte in patients with solid tumours. We therefore aimed to develop a clear, evidence-based guideline on this topic. A systematic review of clinical trials and meta-analyses publis

Pyoderma gangrenosum: a systematic review.

Pyoderma gangrenosum (PG) is a rare, chronic neutrophilic dermatosis of unknown etiology. The world wide incidence is estimated to be around 3-10 cases per million population per year. In 50-70% of cases inflammatory bowel diseases, hematological malignancies or rheumatologic disorders are associated to PG. Although the etiology is uncertain, the dysregulation of the immune system appears to be implied. Pathergy is the most important triggering factor of PG. Indeed, 20-30% of patients report the onset of PG following trivial trauma. Four main variants of PG have been described, namely classic,

HIV-Antiretroviral Therapy Induced Liver, Gastrointestinal, and Pancreatic Injury.

The present paper describes possible connections between antiretroviral therapies (ARTs) used to treat human immunodeficiency virus (HIV) infection and adverse drug reactions (ADRs) encountered predominantly in the liver, including hypersensitivity syndrome reactions, as well as throughout the gastrointestinal system, including the pancreas. Highly active antiretroviral therapy (HAART) has a positive influence on the quality of life and longevity in HIV patients, substantially reducing morbidity and mortality in this population. However, HAART produces a spectrum of ADRs. Alcohol consumption c

Erythema multiforme and Stevens-Johnson syndrome/toxic epidermal necrolysis associated with lupus erythematosus.

BACKGROUND: The occurrence of erythema multiforme (EM)-like lesions in association with lupus erythematosus (LE) is often referred to as "Rowell syndrome" (RS). However, the existence of RS, or at least its nosographic independence from LE, is questioned. The association of Stevens-Johnson syndrome (SJS)/toxic epidermal necrolysis (TEN) with LE is also controversial. OBJECTIVE: We sought to define the features of EM and SJS/TEN in the setting of LE. METHODS: The worldwide literature on the topic was systematically collected and reviewed. RESULTS: A total of 132 citations were found, from wh

Late-onset bipolar illness: the geriatric bipolar type VI.

In parallel to considerable progress in understanding and treatment of bipolarity and despite growing interest in old age psychiatry, late-onset bipolar illness (LOBI) has remained relatively understudied so far, probably in reason of its complexity. To update available data, a systematic review was conducted, focusing on the main issues addressed in literature in regard to this topic. In addition to data on epidemiology, clinical features and treatment, five main issues could be identified: LOBI as secondary disorder, LOBI as expression of a lower vulnerability to the disease, LOBI as subform

A scoping review of self-management interventions for adults with multiple sclerosis.

The purpose of this scoping review is to identify self-management tasks and skills that are being taught in existing interventions for persons with multiple sclerosis and to describe intervention strategies used to facilitate the learning of self-management tasks and skills. Multiple strategies were used to search for studies published or in press between 1980 and 2008 that evaluated self-management interventions. The main exclusion criteria were case studies, inadequate description of the intervention, and traditional clinical patient education interventions. Two independent coders categorize

Incomplete pregnancy and risk of ovarian cancer: results from two Australian case-control studies and systematic review.

Although full-term pregnancies reduce the risk of ovarian cancer, it has not been conclusively established whether incomplete pregnancies also influence risk. We investigated the relationship between a history of incomplete pregnancy and incident epithelial ovarian cancer among over 4,500 women who participated in two large Australian population-based case-control studies in 1990-1993 and 2002-2005. They provided responses to detailed questions about their reproductive histories and other personal factors. Summary odds ratios (OR) and confidence intervals (CI) derived for each study using the

Waiting for child developmental and rehabilitation services: an overview of issues and needs.

Concern about the length of time that children, young people, and families may have to wait to access assessment, diagnostic, interventional, therapeutic, and supportive child developmental and rehabilitation (CDR) services is widespread, but adequate data collection and research on this issue remain limited. We review key concepts and issues relevant to waiting for CDR services from the published literature, a national workshop devoted to this topic, and international experience. We conclude that gaps in data, evidence, and consensus challenge our ability to address the issue of waiting for C

The role of the nose in the pathogenesis of obstructive sleep apnoea and snoring.

Data from observational studies suggest that nasal obstruction contributes to the pathogenesis of snoring and obstructive sleep apnoea (OSA). To define more accurately the relationship between snoring, OSA and nasal obstruction, the current authors have summarised the literature on epidemiological and physiological studies, and performed a systematic review of randomised controlled trials in which the effects of treating nasal obstruction on snoring and OSA were investigated. Searches of bibliographical databases revealed nine trials with randomised controlled design. External nasal dilators w

[In Nigeria: sexual permissiveness and sex education].

Considering the population explosion in Nigeria which is due both to medical advances and traditional beliefs in large families as well as the recent trend of sexual permissiveness which involves the teenagers population, the author of the article considers that sex education is imperative in Nigeria. However, he sees many obstacles standing in the way of general acceptance of sex education. The husband-wife relationship does not encourage free communication on the subject and colonization has removed the traditional forms of sex education through initiation rites and pre-marital counseling by

Central Retinal Artery Occlusion: A Review of Pathophysiological Features and Management.

Central retinal artery occlusion (CRAO) is a form of acute ischemic stroke that results in painless vision loss attributable to retinal infarction. A keen understanding of clinical presentation and underlying pathophysiological features is key to timely intervention and development of new treatment modalities. In CRAO, the time between initial insult to presentation is significant because, analogous to ischemic stroke, the duration of ischemia is inversely related to viable retinal tissue. A major challenge in CRAO is delayed presentation, which reduces the amount of salvageable retina. In add

LLM-Based Classifiers Can Reduce the Proportion of Records to Screen with Minimal Loss of Relevant Studies: A Case Study.

BACKGROUND: The Epistemonikos Sustainable Knowledge Platform (SKP) integrates a suite of machine learning tools designed to make evidence synthesis more efficient, including access to the Epistemonikos Database of Trials (ED-Trials). One feature is the use of classifiers combined with large language models (LLMs) to support abstract screening during study selection. LLM-based classifiers can be applied to populations, interventions, and study design features. Their primary aim is to reduce the proportion of records needed to be screenedby automatically excluding records with a high probability
