Supratentorial metastatic enteropathy-associated T-cell lymphoma: A case report and literature review.

BACKGROUND: We are describing a rare case of supratentorial metastatic enteropathy-associated T-cell lymphoma (EATL). While these lesions are a rare complication of EATL, the implications are grave and they must be evaluated as a diagnostic possibility when a patient with known celiac disease presents with acute neurological deterioration. In addition, multidisciplinary care teams are recommended by the authors as critical to providing the most comprehensive patient care. CASE DESCRIPTION: A 65-year-old female presented to the emergency room with uncontrolled abdominal pain, nausea, and vomit

Ultrasound-guided central venous catheter placement decreases complications and decreases placement attempts compared with the landmark technique in patients in a pediatric intensive care unit.

OBJECTIVE: To determine whether ultrasound (US) increases successful central venous catheter (CVC) placement, decreases site attempts, and decreases CVC placement complications. DESIGN AND SETTING: A prospective observational cohort study evaluating a transition by the Pediatric Critical Care Medicine service to US-guided CVC placement. Medical and surgical patients in a 21-bed quaternary multidisciplinary pediatric intensive care unit had CVCs placed by attendings, fellows, residents, and a nurse practitioner. PATIENTS: Ninety-three patients were prospectively enrolled into the landmark (LM

Fetal programming: the perspective of single and twin pregnancies.

Multiple pregnancy is associated with increased risk of adverse consequences for both mother and fetus(es), including increased rates of maternal hypertension and pre-eclampsia, spontaneous abortion, Caesarean delivery, low birthweight, birth prematurity, perinatal mortality, admission to neonatal intensive care and extended length of care, respiratory distress, cerebral palsy, developmental delay, contact with disability services and mortality to age 5 years. Premature birth, which affects 97% of triplets and 53.3% of twins in Australia, is not the sole factor involved. The rate of multiple p

Integrating Correctional and Community Health Care: An Innovative Approach for Clinical Learning in a Baccalaureate Nursing Program.

PROBLEM: With an evolving focus on primary, community-based, and patient-centered care rather than acute, hospital-centric, disease-focused care, and recognition of the importance of coordinating care and managing transitions across providers and settings of care, registered nurses need to be prepared from a different and broader knowledge base and skills set. A culture change among nurse educators and administrators and in nursing education is needed to prepare competent registered nurses capable of practicing from a health promotion, disease prevention, community- and population-focused cons

Promoting psychological resilience during the perinatal period with a digital tool (the HealthyMoms4MentalHealth-app): a study protocol for a randomised controlled trial in Sweden.

BACKGROUND: Pregnancy and childbirth are major life transitions that can significantly impact women's mental health, with depression and anxiety being the most commonly reported. International and Swedish studies estimate that up to one in four women experience anxiety or depressive symptoms during pregnancy or postpartum. Poor maternal mental health is associated with adverse outcomes for both mother and child, including preterm birth and low birth weight. Psychological resilience has emerged as a key protective factor, linked to reduced psychological distress and improved emotional regulatio

Operational Determinants of Preventable Hospital Readmissions From US Post-Acute Care Settings: A Structured Narrative Review (2021-2026).

Despite more than a decade of value-based reimbursement under the Hospital Readmissions Reduction Program and the Skilled Nursing Facility Value-Based Purchasing program, 30-day readmission rates from US post-acute care (PAC) settings remain at 20% to 25% among skilled nursing facility residents and contribute more than $15 billion annually to Medicare expenditure. Elevated rates have persisted despite sustained policy attention. This suggests that the dominant clinical quality frame captures only part of the problem. The operational substrate of PAC transitions, therefore, warrants systematic

Review: Posterior urethral valves (PUV) … what do we know?

INTRODUCTION: Posterior urethral valves (PUV) remain the most common cause of severe congenital lower urinary tract obstruction in boys and a leading pediatric cause of chronic kidney disease (CKD) and end-stage kidney disease (ESKD). Despite decades of research, management decisions across antenatal counseling, postnatal stabilization, definitive ablation, bladder management, and long-term surveillance remain heterogeneous. OBJECTIVE: To map the breadth, design, and strength of contemporary evidence informing PUV management across the full clinical pathway, and to identify priority gaps for

Antiretroviral Postexposure Prophylaxis After Sexual, Injection Drug Use, or Other Nonoccupational Exposure to HIV - CDC Recommendations, United States, 2025.

Nonoccupational postexposure prophylaxis (nPEP) for HIV is recommended when a nonoccupational (e.g., sexual, needle, or other) exposure to nonintact skin or mucous membranes that presents a substantial risk for HIV transmission has occurred, and the source has HIV without sustained viral suppression or their viral suppression information is not known. A rapid HIV test (also referred to as point-of-care) or laboratory-based antigen/antibody combination HIV test is recommended before nPEP initiation. Health care professionals should ensure the first dose of nPEP is provided as soon as possible,

Prospective, multi-site evaluation of the Cepheid Xpert Xpress CoV-2 plus test on nasal and nasopharyngeal swabs.

UNLABELLED: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) continues its largely aseasonal spread with millions of cases per year. Highly sensitive, point-of-care testing is critical for rapid detection of coronavirus disease 2019 (COVID-19) cases and initiation of antiviral therapy to avert adverse health outcomes and reduce onward transmission of the virus. While hundreds of COVID-19 diagnostics received emergency use authorization from the FDA during the pandemic, significantly fewer have navigated the course to FDA clearance or approval. Here, we determined the clinical perfo

Effect of hospital discharge plan for children with type 1 diabetes on discharge readiness, discharge education quality, and blood glucose control.

BACKGROUND: Type 1 diabetes is one of the most common chronic diseases in childhood. The number of type 1 diabetes patients in China still ranks fourth in the world. Therefore, children with type 1 diabetes in China are a group that needs attention. The management of type 1 diabetes mellitus (T1DM) involves many aspects of daily life. It is extremely challenging for children and their families. T1DM children have complex medical care needs. Despite the continuous development of therapeutic medicine and treatment technologies, blood glucose control in children with T1DM is still not ideal. They

The Cost-Effectiveness of Platelet-Rich Plasma Injections for Knee Osteoarthritis: A Markov Decision Analysis.

BACKGROUND: Use of platelet-rich-plasma (PRP) injections for treating knee osteoarthritis has increased over the past decade. We used cost-effectiveness analysis to evaluate the value of PRP in delaying the need for total knee arthroplasty (TKA). METHODS: We developed a Markov model to analyze the baseline case: a 55-year-old patient with Kellgren-Lawrence grade-II or III knee osteoarthritis undergoing a series of 3 PRP injections with a 1-year delay to TKA versus a TKA from the outset. Both health-care payer and societal perspectives were included. Transition probabilities were derived from

Use of Combat Casualty Care Data to Assess the US Military Trauma System During the Afghanistan and Iraq Conflicts, 2001-2017.

IMPORTANCE: Although the Afghanistan and Iraq conflicts have the lowest US case-fatality rates in history, no comprehensive assessment of combat casualty care statistics, major interventions, or risk factors has been reported to date after 16 years of conflict. OBJECTIVES: To analyze trends in overall combat casualty statistics, to assess aggregate measures of injury and interventions, and to simulate how mortality rates would have changed had the interventions not occurred. Building an electronic health record integrated quality of life outcomes registry for spine surgery.

OBJECTIVE: Demonstrating the value of spine care requires adequate outcomes assessment. Long-term outcomes are best measured as overall improvement in quality of life (QOL) after surgical intervention. Present registries often require parallel data entry, introducing inefficiencies and limiting compliance. The authors detail the methodology of constructing an integrated electronic health record (EHR) system to collect QOL metrics and demonstrate the effect of data collection on routine clinical workflow. A streamlined approach to collecting QOL data can capture patient data without requiring d

Resilience as a Mediator in a Web-Based Intervention (MINDxYOU) to Reduce Stress Among Health Care Professionals: Stepped-Wedge Cluster Randomized Trial.

BACKGROUND: The mechanisms through which mindfulness and third wave-based digital programs exert their effects on stress reduction remain poorly understood. Identifying these mediators is essential to optimize their implementation, particularly in health care settings. This approach is particularly relevant for health care professionals, who are constantly exposed to high levels of emotional demands, work overload, and risk of burnout, especially in the aftermath of the COVID-19 pandemic. Despite the growing need for scalable and accessible mental health support in this population, such digita

Glutathione peroxidase 4 (GPX4) as an oncogenic factor in human tumors: a pan-cancer analysis.

BACKGROUND: Tracheostomy is one of the most common procedures done in intensive care unit (ICU) patients. Decannulation is the weaning off from tracheostomy to maintain spontaneous respiration and/or airway protection. However, this step needs a near perfect coordination of brain, swallowing, coughing, phonation, and respiratory muscles. However, despite its perceived importance, there is no universally accepted protocol for this vital transition. In this systematic review of decannulation we focus attention to this important aspect of tracheostomy care. AIM: To compare the two methods of dec
