"This is how we should work" - healthcare professionals' past uncertainty and present confidence in management of fibromyalgia.

OBJECTIVE: Fibromyalgia is a chronic illness that is poorly understood by healthcare professionals (HCPs), and HCPs may express uncertainty due to limited available knowledge. Our objective was to explore what HCPs tell about their present confidence in delivering self-management programs to patients with fibromyalgia. METHODS: A narrative research approach was applied, and interviews were conducted with nine HCPs from various professional backgrounds. The HCPs were experienced in the management of fibromyalgia, and one year earlier, they had revised their self-management program to ascertain

Mechanical work derived using markerless motion capture provides a valid indication of acute neuromuscular fatigue in tennis.

Whilst workload monitoring is key to managing performance and injury risk, effective strategies in tennis are not well-established. Mechanical work (external and internal components), measurable through markerless motion capture approaches, provides a novel way to quantify workload. To determine the suitability of this approach, this study investigated the association between mechanical work and acute neuromuscular fatigue. Fifteen tennis players completed a tennis-specific on-court fatiguing protocol interspersed with sprint tests. Peak sprint velocity (measure of acute neuromuscular fatigue)

Can ovarian cancer screening work? A secondary analysis of the UK collaborative trial of ovarian cancer screening.

BACKGROUND: The UK Collaborative Trial of Ovarian Cancer Screening (UKCTOCS; 2001-2020) showed no reduction in disease mortality in its primary intervention arm using multimodal screening (MMS) with longitudinal Cancer Antigen 125 (CA125) and transvaginal ultrasound. Whether this null result reflects the stop-screen design, screening performance, or ovarian cancer natural history remains unclear. METHODS: Using individual-level screening and diagnosis data from the MMS arm, we estimated ovarian cancer natural history, focusing on high-grade serous ovarian cancer (HGSC), the most common and le

Distinct Echocardiographic Phenotypes in Primary vs. Secondary Iron Overload Cardiomyopathy: A Pilot Study on Myocardial Work Indices.

Background: Iron overload cardiomyopathy (IOC) is a major determinant of outcomes in hemochromatosis, and conventional echocardiography may miss early myocardial toxicity. Comparative data on primary (PH) versus secondary hemochromatosis (SH) using myocardial work (MW) indices are limited. Methods: We performed a retrospective cross-sectional study of 34 adults (16 PH and 18 SH patients) at a tertiary center. They all underwent echocardiography with speckle-tracking to obtain LV global longitudinal strain (GLS) and non-invasive MW indices from pressure-strain loops: global work index (GWI), gl

Buccal Mucosa a Narrative Review: How Does It Work, How Is It Used, What Is Coming Next.

Buccal mucosa grafts (BMG) have become the standard for complex urethral reconstruction due to unique regenerative properties, including rapid re-epithelialization, minimal inflammation, and tightly controlled fibroblast action that results in "scarless healing". These characteristics reflect fetal-like wound healing phenotypes with distinct fibroblast subpopulations and favorable inflammatory profiles. BMG have applications across the genitourinary tract from renal pelvis to external genitalia. Emerging technologies-tissue-engineered oral mucosal substitutes, extracellular vesicles, and organ

Muscle work during manual wheelchair propulsion in people with a spinal cord injury: a model-based approach.

Manual wheelchair users are at high risk of developing shoulder pathologies due to the repetitive demands of propulsion. Understanding individual muscle contributions is essential to improve rehabilitation strategies and prevent overuse injuries. This study applied a modeling approach to evaluate the individual muscle contributions, particularly the rotator cuff muscles, to mechanical work during the push and recovery phases of wheelchair propulsion in individuals with a spinal cord injury. We used a thoracoscapular shoulder model in OpenSim, which incorporated accurate scapular kinematics and

The Intersection of Human Rights, HIV and Sex Work: Implications for Ending AIDS in Thailand by 2030.

INTRODUCTION: Sex work significantly shapes the Thai economy and the HIV epidemic, yet its criminalization restricts access to health services and exacerbates human rights violations. This study examines experiences of stigma, discrimination, gender-based violence (GBV) and incarceration among sex workers (SWs), exploring how these factors intersect with HIV services. This research is a component of a larger study investigating factors associated with pre-exposure prophylaxis (PrEP) uptake among SWs in Thailand. METHODS: We conducted a post hoc analysis of a cross-sectional survey (September-

Authentic leadership and organizational citizenship behavior: the mediating role of work engagement in a Confucian cultural context.

INTRODUCTION: In contemporary higher education, particularly within Confucian cultural contexts characterized by high power distance and hierarchical structures, leadership plays a critical role in fostering positive employee behaviors. Despite growing interest in authentic leadership, limited research has examined its influence on organizational citizenship behavior in non-Western settings, especially considering underlying mechanisms and boundary conditions. This study addresses this gap by investigating the relationship between authentic leadership and organizational citizenship behavior, w

Adverse Effects Due to the Use of Upper Limbs Exoskeletons in the Work Environment: A Scoping Review.

Both for design issues and for the study, analysis, and understanding of the interaction of workers with exoskeletons, the study of adverse effects provides criteria to improve the design of more efficient exoskeletons with better ergonomics and long-term usability. In this work, a scoping review was carried out on adverse effects due to the prolonged use of upper-limb exoskeletons, which have been evidenced in the scientific literature. The causes of the effects are described in terms of their impacts on the physiological, psychological, and technological aspects that affect the user. A scopi

Functional electrical stimulation of the soleus redistributes lower limb joint work distally in young and older adults.

Older adults walk with reduced ankle and greater hip mechanical output compared to young adults. This "distal-to-proximal redistribution" likely contributes to the greater metabolic energy expenditure during walking in older versus young adults. Because of the inverse relationship between ankle and hip use, functional electrical stimulation (FES) of the ankle extensors may increase ankle mechanical work and indirectly decrease hip mechanical work. Although FES increases stimulated muscle metabolism, bilateral soleus stimulation may restore more youthful walking kinetics without a detectable ch

Socioeconomic impact of chronic delta hepatitis in Spain: Indirect costs of work absenteeism, presenteeism, and premature mortality.

INTRODUCTION: Patients with chronic delta hepatitis (CDH) exhibit higher levels of morbimortality than those with hepatitis B only, generating higher indirect costs for society. The aim of this study was to estimate the loss of productivity and costs resulting from work absenteeism and presenteeism as well as premature mortality among patients with CDH in Spain. METHODS: Patients with CDH in their working age (between 20-65 years) were estimated by an epidemiological flow model that incorporated the prevalence of infection with the hepatitis B and D viruses. To calculate the costs (year-2023)

Effect of early return to work after laparoscopic total extraperitoneal hernia repair: A retrospective comparative cohort study.

OBJECTIVES: Laparoscopic total extraperitoneal (TEP) hernia repair is one of the widely used surgical methods for symptomatic inguinal hernia. Although laparoscopic procedures provide advantages on postoperative complications and shorter convalescence, there is currently no global consensus on the timing for returning to work following laparoscopic hernia repair. This study compared the outcomes of early and late return to work after laparoscopic TEP inguinal hernia repair. MATERIALS AND METHODS: Between March 2008 and December 2019, we reviewed 506 cases of laparoendoscopic TEP hernia repair
