The role of the methacholine inhalation challenge in adult patients presenting with chronic cough.

One hundred and twenty two patients who were presenting with chronic persistent cough for more than 3-4 weeks were studied. Using anatomical and stepwise approach, we could identify the cause of chronic cough in 96% of the patients. The most common causes were due to postnasal drip syndrome (PND) (45%) and bronchial asthma (26%). We also demonstrated the important role of methacholine inhalation challenge in the diagnostic work up. The procedure could identify the patients who had bronchial hyperresponsiveness (BHR) in 52% of the cases. The severity degree of the BHR was classified into three

High-rate continuous biodegradation of concentrated chlorinated aliphatics by a durable enrichment of methanogenic origin under carrier-dependent conditions.

The simultaneous biodegradation of toxic compounds in mixtures is a major current concern. To bioremediate a toxic mixture, we designed a strategy combining an ad-sorbent carrier with an ecological and nutritional system which allowed work close to heavily polluted conditions in nature. Starting from a methanogenic community, we developed a microbial consortium acclimated to a mixture of about 30 chlorinated aliphatics in a fixed-film stationary-bed bioreactor. Prior to the establishment of a durable period of dechlorination, an interval of progressive dechlorination of the toxic mixture was o

Control of subretinal fluid: experimental and clinical studies.

Experimental work shows that subretinal fluid is removed both by active transport across the retinal pigment epithelium (RPE) and by passive hydrostatic and oncotic forces that work most effectively when the RPE barrier has been damaged. The retina will stay attached whether or not the RPE is intact--but retinal function requires the RPE barrier and thus active transport is the primary mechanism of subretinal fluid control. RPE fluid transport is normally limited by the retina (which resists water flow from the vitreous) but can be quite powerful when a reservoir of subretinal fluid is present

Anaesthesia for the patient with cardiac disease.

I have tried to summarize experimental work in human and animal models. This work is complex and difficult to interpret. Clinicians should not take isolated experimental studies and jump to conclusions about clinical use of anaesthetics. Experimenters should not arrive at sweeping conclusions based on a particular experimental study performed under some or other carefully or not so carefully controlled conditions. Editorialists, such as Becker in a recent issue of Anesthesiology, should not conclude that an anaesthetic is "dangerous," based on these kinds of studies. It is especially invalid t

Dissociation of adenosine from metabolic regulation of coronary flow in the lamb.

The objective of these studies was to evaluate the contribution of an adenosine mechanism to metabolic regulation of coronary flow. Cardiac O2 metabolism (MVO2) was altered by changing cardiac output while aortic pressure and heart rate were held constant (paced). Ganglionic (tetraethylammonium chloride) and beta blockade (propranolol) were employed. Relationships of coronary flow to MVO2 were determined in control and alloxan diabetic animals. The latter have previously been shown to have reduced sensitivity to adenosine [Am. J. Physiol. 243 (Heart Circ. Physiol. 12): H252-H258, 1982]. In eac

Prolonged eye-hand decoupling deficits in young adults with a history of concussion from adolescence.

Previous studies reported that adolescents with a sport-related concussion history showed prolonged visuomotor deficits during an eye-hand decoupling task until around 1.5-2 years post-event. The present study expands this work, examining whether such deficits do or do not emerge when testing individuals in young adulthood, i.e. later post-event. Twenty-one non-athlete college students with sport-related concussion history from adolescence (CH; M = 21 yrs.; M = 46 months post-concussion, range 10-90 months) and twenty controls with no history of concussion (NoH; M = 21 yrs.) performed two touc

LACK OF OXYGEN AND/OR GLUCOSE DIFFERENTIALLY POTENTIATES Aβ40 E22Q - AND Aβ42-INDUCED CEREBRAL ENDOTHELIAL CELL DEATH, BARRIER DYSFUNCTION AND ANGIOGENESIS IMPAIRMENT.

BACKGROUND: Disrupted brain hemodynamics and cerebrovascular damage resulting in cerebral hypoperfusion occur early within Alzheimer's Disease (AD) pathogenesis. Cerebral hypoperfusion is also an extremely common consequence of cardiovascular risk factors and diseases (CVRFs/CVDs), which usually manifest in midlife, when AD pathology initiates, and actively contribute to AD onset and progression. Previously our lab has demonstrated that the vasculotropic Dutch mutant, AβQ22, and Aβ42 promote endothelial cells (ECs) apoptosis, barrier permeability, and angiogenic impairments. Prior research has

Work-focused cognitive behavioural therapy for common mental disorders: a systematic review and meta-analysis.

BACKGROUND: Common mental disorders (CMDs) are a major cause of sickness absence and work disability. Although cognitive behavioural therapy (CBT) effectively reduces symptoms, effects on return-to-work (RTW) outcomes are limited. Work-focused CBT (W-CBT) integrates RTW processes into treatment, but the evidence base remains heterogeneous with unresolved methodological challenges. This systematic review and meta-analysis evaluated the effectiveness of W-CBT on psychiatric and work-related outcomes, examined moderators of treatment effects, assessed methodological rigour, and identified common

Medical and genetic differences in the adverse impact of sleep loss on performance: ethical considerations for the medical profession.

The Institute of Medicine recently concluded that-on average-medical residents make more serious medical errors and have more motor vehicle crashes when they are deprived of sleep. In the interest of public safety, society has required limitations on work hours in many other safety sensitive occupations, including transportation and nuclear power generation. Those who argue in favor of traditional extended duration resident work hours often suggest that there are inter- individual differences in response to acute sleep loss or chronic sleep deprivation, implying that physicians may be more res

Comparative case study on the implementation of the multi- space concept in a healthcare facility: study protocol.

BACKGROUND: Modern healthcare organizations face increasing psychosocial and organizational demands driven by digitalization, flexible work models, and rising workload. In large university hospitals such as the Medical Center - University of Freiburg, administrative and clinical staff work in highly complex, interdisciplinary environments where well-being, performance, and patient care are closely linked. The physical office environment plays a crucial role in shaping employees' well-being, motivation, and health. To meet the high cooperative demands in medical centers, multi-space offices are

Night work effects on salivary cytokines TNF, IL-1β and IL-6.

Shift work is unavoidable in modern societies, but at the same time disrupts biological rhythms and contributes to social distress and disturbance of sleep, health and well-being of shift workers. Shift work has been associated with some chronic diseases in which a chronic inflammatory condition may play a role. However, few studies investigating the association of cytokine and other inflammation markers with shift workers have been published in recent years. In this study we evaluated the effects of permanent night work on the production of tumor necrosis factor (TNF), interleukin-1β (IL-1β),

Radiological abnormalities and asbestos exposure among custodians of the New York City Board of Education.

Six hundred sixty custodians employed by the New York City Board of Education underwent examination from 1985 through 1987 for asbestos-related disease and other general medical conditions by the clinical staff of the Division of Environmental and Occupational Medicine of the Mount Sinai School of Medicine of the City University of New York. Two-thirds of the men (no women were examined) were 20 or more years from onset of any custodial work, with 44% having had at least 20 years of employment as custodial workers in New York City Board of Education schools. Twenty-four percent had begun custo

Individual variation in the motivational and neurobiological effects of an opioid cue.

A discrete cue associated with intravenous injections of cocaine acquires greater control over motivated behavior in some rats ('sign-trackers', STs) than others ('goal-trackers', GTs). It is not known, however, if such variation generalizes to cues associated with other drugs. We asked, therefore, whether a discrete cue (a light) associated with the intravenous administration of an opioid drug (the short-acting mu receptor agonist, remifentanil) acquires incentive motivational properties differently in STs and GTs, as indicated by tests of Pavlovian conditioned approach and conditioned reinfo

The effects of heat stress on neuromuscular activity during endurance exercise.

This study analysed the effect of hot (35 degrees C) and cold (15 degrees C) environments on electromyographic (EMG) signal characteristics, skin and rectal temperatures and heart rate during progressive endurance exercise. Eight healthy subjects performed three successive 15-min rides at 30%, 50% and 70% of their peak sustained power output and then cycled at increasing (15 W/min) work rates to exhaustion in both 35 degrees C and 15 degrees C environments. Skin and rectal temperatures, heart rate and EMG data were measured during the trials. The skin temperatures were higher and the subjects

Multimorbidity as a predictor of mortality in companion dogs.

Multimorbidity, the presence of two or more conditions, is associated with a higher risk of death as individuals age. However, modeling multimorbidity in laboratory animals is difficult, if not impossible, because specific conditions are seldom individually diagnosed and treated in these settings. Because of their shared environment, physiology, and genetic diversity, and because they are medically managed as individuals, companion dogs have potential to serve as a translational multimorbidity model. Yet it is unknown how diagnoses accumulate over time, how these diagnoses are associated with

Practical Test-Time Domain Adaptation for Industrial Condition Monitoring by Leveraging Normal-Class Data.

Machine learning has driven significant advancements across diverse domains. However, models often experience performance degradation when applied to data distributions that differ from those encountered during training, a challenge known as domain shift. This issue is particularly relevant in industrial condition monitoring, where data originate from heterogeneous sensors operating under varying conditions, hardware configurations, or environments. Domain adaptation is a well-known method to address this problem; however, the proposed methods are not directly applicable in real-world conditio

Wayfinding in a virtual environment and Down syndrome: The impact of navigational aids.

OBJECTIVE: Individuals with intellectual disabilities (ID), such as those with Down syndrome (DS), sometimes get lost, even when walking familiar routes. Among the spatial cognition models addressing the dynamic construction of spatial representations, the theoretical framework of Siegel and White (1975) remains a reference. The aim of this work is to use virtual reality technology to (a) elucidate the spatial deficits associated with ID in reference to the Siegel and White framework and (b) to evaluate the effectiveness of 3 wayfinding assistive procedures (signified landmarks, arrows, and el

Heat and cold: what does the environment do to marathon injury?

The medical work load seems to increase both with heat and humidity, and with cold and rainy conditions. Heat tolerance during exercise is variable and heat intolerance may contribute to collapse and increase medical encounters. Exposure to cold, wet conditions results in increasing incidence of hypothermia in exhausted marathon runners. Finish-line encounters and course dropouts increase as conditions cool and warm away from the most advantageous conditions in the 4.4-15 degrees C (40-59 degrees F) wet bulb globe temperature (WBGT) range. The risk of requiring medical attention and not finish

Mesolimbic dopaminergic pathways in fear conditioning.

One of the most common paradigms used to study the biological basis of emotion, as well as of learning and memory, is Pavlovian fear conditioning. In the acquisition phase of a fear conditioning experiment, an emotionally neutral conditioned stimulus (CS)--which can either be a discrete stimulus, such as a tone, or a contextual stimulus, such as a specific environment--is paired with an aversive unconditioned stimulus (US), for example a foot shock. As a result, the CS elicits conditioned fear responses when subsequently presented alone during the expression phase of the experiment. While cons
