Bone marrow-derived mesenchymal stem cells for ischemic stroke: mechanisms, delivery strategies, clinical evidence, and translational challenges.

Ischemic stroke remains a major global health burden because of its high incidence, disability, and mortality. Current reperfusion therapies are constrained by narrow therapeutic windows and have limited capacity to reverse established neuronal injury or promote long-term functional recovery, highlighting the need for strategies that support neural repair and neurovascular remodeling. Bone marrow-derived mesenchymal stem cells (BMSCs) have been widely investigated as candidate therapeutic products for ischemic stroke because of their paracrine, immunomodulatory, trophic, angiogenic, and microe

Construction and anti-osteoporotic activity evaluation of dual-targeted exosomes derived from bone marrow mesenchymal stem cells.

Osteoporosis is a major public health concern worldwide, and mesenchymal stem cell (MSC)-derived exosomes hold promise as therapeutic agents and drug carriers for its treatment. However, the clinical translation of natural exosomes is limited by poor targeting and short retention. Here, we developed a bone/magnetic dual-targeted exosome delivery system (S-Z@ALN-SPIONs-EXO) by functionalizing BMSC-derived exosomes with alendronate (ALN) and superparamagnetic iron oxide nanoparticles (SPIONs) and co-loading them with danshensu and ZnO quantum dots. The system exhibited excellent biocompatibility

Mechanobiological modulation: physiological pressure preconditioning boosts the paracrine-driven lymphangiogenic capacity of bone marrow mesenchymal stem cells.

INTRODUCTION: Lymphatic system remodeling and regeneration are essential for tissue repair and homeostasis, and these processes are precisely regulated by both biochemical and biomechanical signals in the tissue microenvironment. The mechanobiological mechanisms governing this process remain largely elusive, particularly in oral and maxillofacial tissues which are constantly exposed to complex mechanical forces. Bone marrow mesenchymal stem cells (BMSCs) hold great promise for regenerative medicine due to their robust paracrine activity, but how mechanical preconditioning modulates their pro-l

Reconstruction of the hematopoietic system using bone marrow, thymus, and spleen organoids: Construction strategies and research advances.

Hematopoietic development and systemic homeostasis depend critically on specific three-dimensional (3D) microenvironments within the bone marrow, thymus, and spleen. Traditional two-dimensional (2D) cultures and animal models frequently fail to recapitulate human physiology due to spatial limitations and interspecies genetic divergence. While 3D organoids offer a compelling alternative, achieving anatomical fidelity and sustaining long-term functionality remain significant bioengineering hurdles. Navigating these challenges, this review synthesizes emerging strategies for constructing high-fid

Wild-type bone marrow cells repopulate tissue resident macrophages and reverse the impacts of homozygous CSF1R mutation.

Adaptation to existence outside the womb is a key event in the life of a mammal. The absence of macrophages in rats with a homozygous mutation in the colony-stimulating factor 1 receptor (Csf1r) gene (Csf1rko) severely compromises pre-weaning somatic growth and maturation of organ function. Transfer of wild-type bone marrow cells (BMT) at weaning rescues tissue macrophage populations permitting normal development and long-term survival. To dissect the phenotype and function of macrophages in postnatal development, we generated transcriptomic profiles of all major organs of wild-type and Csf1rk

Blood Serum from Patients with Acute Leukemia Inhibits the Growth of Bone Marrow Multipotent Mesenchymal Stromal Cells.

Background/Objectives: Acute leukemia (AL) alters both hematopoiesis and the bone marrow stromal microenvironment. Attempts to develop a culture of multipotent mesenchymal stromal cells (MSCs) from AL patients' bone marrow are not always successful, as opposed to healthy donors' bone marrow. Methods: To unveil the reason, healthy donors' MSCs were cultured in the presence of sera from healthy donors (control group) or AL patients at the onset of the disease, in short- and long-term remission, and before and after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Results: The cell

Bone marrow microenvironment-responsive polymeric-drug/siRNA regulates leukemia stem cells assisting for prevention of AML relapse.

Acute myeloid leukemia (AML) remains a formidable hematological malignancy with a poor prognosis, largely due to the limited success of existing treatments in effectively targeting leukemia stem cells (LSCs). These cells are adept at migrating into hypoxic niches within the bone marrow, thereby evading chemotherapeutic agents and retaining their stem-like properties. This evasion facilitates their survival and subsequent regeneration following treatment, significantly contributing to disease relapse and highlighting the need for novel therapeutic interventions. Herein, we present the developme

Galectin-3 Release in the Bone Marrow Microenvironment Promotes Drug Resistance and Relapse in Acute Myeloid Leukemia.

Reciprocal signaling between acute myeloid leukemia (AML) cells and the surrounding bone-marrow microenvironment (BMME) promotes AML progression through several mechanisms. One of the most important mechanisms is the induction of Galectin-3 (Gal-3) expression by AML cells and bone marrow mesenchymal stromal cells (BM-MSCs). Emerging evidence indicates that Gal-3 upregulation in the BMME promotes AML cell adhesion and survival, leading to the development of chemotherapy resistance, relapse, and poor prognosis. Identifying the biological function and critical signaling pathways of Gal-3 may cont

Unique Bone Marrow Findings of FDG-PET/CT in Acute Leukemia in Children: Comparison to Inflammatory Diseases.

Background/Objectives: Fluorine-18 fluorodeoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) is a valuable imaging modality for detecting malignancies and diagnosing fever of unknown origin (FUO). However, data regarding FDG accumulation in bone marrow among pediatric acute leukemia (AL) cases are limited. In this study, we aimed to compare FDG-PET/CT findings between children with AL and those with inflammatory diseases (IDs), including FUO, and develop a scoring system for differential diagnoses. Methods: We retrospectively analyzed FDG-PET/CT findings in six children

Peripheral Blood as a Diagnostic Alternative to Bone Marrow in Immunophenotyping Pediatric B-Cell Acute Lymphoblastic Leukemia.

Flow cytometric immunophenotyping is essential for the diagnosis and immunologic classification of B-cell acute lymphoblastic leukemia in children and is traditionally performed using bone marrow aspirates, an invasive procedure that may be delayed or unavailable in certain clinical contexts. However, many patients present with circulating blasts at diagnosis, raising the possibility of using peripheral blood as an alternative source for initial immunophenotypic classification. Although previous studies have shown that peripheral blood can support initial diagnostic classification, they have n

BioMarrow: An accessible and reproducible 3D patient-derived bone marrow model for advancing research and clinical applications.

Preclinical models for multiple myeloma (MM) often fail to recapitulate the complexity of the bone marrow (BM) microenvironment, limiting their utility for drug testing and translational research. There is an urgent need for physiologically relevant, patient-adaptable platforms to support personalized therapeutic evaluation. We developed BioMarrow, a 3D ex vivo BM culture system using unmanipulated patient BM aspirates embedded in Matrigel. Culture conditions were optimized to sustain diverse hematopoietic, stromal and immune populations for up to 7 days. Spatial distribution, cytokine secreti

Creative methods to explore children's experiences of gastrostomy and nasogastric tube feeding during allogeneic bone marrow transplant.

PURPOSE: Children undergoing bone marrow transplant experience severe gastrointestinal toxicity. Most become unable to eat, drink and take medications orally putting them at risk of malnutrition and clinical complications. To meet these demands most children require a feeding tube. This study explored children's experiences of having a nasogastric or gastrostomy tube. METHODS: During this qualitative component in a mixed methods study, children completed scrapbooks as a creative method which acted as stimuli during two interviews: on admission for transplant and 1-2 months post-discharge. Sepsis impairs immunocompetent plasmacytoid dendritic cell reconstitution from hematopoietic stem/progenitor cell through altered bone marrow environment.

Plasmacytoid dendritic cells (pDCs) are crucial components of the immune response during viral infections, yet their function and development in the late phase of sepsis remain poorly understood. In this study, we investigated the impact of prolonged sepsis on pDCs and their progenitors in cecal ligation and puncture (CLP)-induced septic mice. We observed a significant reduction in both pDCs and their progenitors, alongside the presence of mature and regulatory pDCs. These mature and regulatory pDCs exhibited impaired type I interferon (IFN) secretion and antigen presentation capacity. In a Fl

HDAC3-YY1-RAB5A axis remodels AML-supportive niche by modulating mitochondrial homeostasis in bone marrow stromal cells.

Recent studies have shown that the interaction between acute myeloid leukemia (AML) and bone marrow stromal cells (BMSCs) plays a vital role in the progression of leukemia and the development of drug resistance, while the underlying mechanisms remain inconclusive. In this study, we found that AML patient-derived BMSCs exhibit a hyperinflammatory phenotype. Histone deacetylase 3 (HDAC3) in BMSCs enhances mitochondrial reactive oxygen species (ROS) production by RAB5A-mediated blockade of mitophagy. Furthermore, we confirmed that HDAC3 regulates RAB5A expression through transcription factor YY1.

Differentiation, ageing and leukaemia alter the metabolic profile of human bone marrow haematopoietic stem and progenitor cells.

Metabolic cues are crucial for regulating haematopoietic stem and progenitor cells (HSPCs). However, the metabolic profile of human HSPCs remains poorly understood due to the limited number of cells and the scarcity of bone marrow samples. Here we present the integrated metabolome, lipidome and transcriptome of human adult HSPCs (lineage-, CD34+, CD38-) upon differentiation, ageing and acute myeloid leukaemia. The combination of low-input targeted metabolomics with our newly optimized low-input untargeted lipidomics workflow allows us to detect up to 193 metabolites and lipids from a starting

Premature ageing of lung alveoli and bone marrow cells from Terc deficient mice with different telomere lengths.

Telomeres are terminal protective chromosome structures. Genetic variants in genes coding for proteins required for telomere maintenance cause rare, life-threatening Telomere Biology Disorders (TBDs) such as dyskeratosis congenita, aplastic anemia or pulmonary fibrosis. The more frequently used mice strains have telomeres much longer than the human ones which question their use as in vivo models for TBDs. One mice model with shorter telomeres based on the CAST/EiJ mouse strain carrying a mutation in the Terc gene, coding for the telomerase RNA component, has been studied in comparison with C57
