A patient with Mullerian abnormalities, renal dysplasia, cervical spine fusion, cataracts and intellectual disability: MURCS-plus?

We report a 15-year-old girl with features of the MURCS (Mullerian abnormalities, renal agenesis/ectopy and cervicothoracic somite dysplasia) association and birth defects not typically associated with MURCS. In addition to seizures and intellectual disability, she has cortical brain heterotopia, bilateral subclinical cataracts, submucous cleft palate and patent ductus arteriosus. We propose that this patient represents a more severe form of MURCS, or 'MURCS-plus', which may represent a defect of or insult to mesodermal morphogenesis.

Cervical radiculopathy: pathophysiology, presentation, and clinical evaluation.

Cervical radiculopathy is a common condition that usually results from compression and inflammation of the cervical nerve root or roots in the region of the neural foramen. It is frequently caused by cervical disc herniation and cervical spondylosis. The diagnosis can be established by history and physical examination, but care should be taken, as diagnoses can mimic or coexist with cervical radiculopathy, such as entrapment neuropathies. The pathophysiology, presentation, and clinical evaluation of cervical radiculopathy are discussed.

Zenker diverticulum: a rare complication after anterior cervical fusion.

Esophageal injury is a rare but serious complication occurring after anterior cervical spine surgery. Pharyngoesophageal, or Zenker, diverticulum is an acquired outpouching of the pharyngeal musculature just proximal to a functional esophageal stricture, clinically manifesting as dysphagia, aspiration, and weight loss. We report a case in which a patient developed a pharyngoesophageal diverticulum, accompanied by retropharyngeal abscess, first identified 2 years after a 3-level anterior cervical fusion with allograft and anterior plating. The inferior portion of the cervical plate was dislodge

A biomechanical analysis of C2 corpectomy constructs.

BACKGROUND CONTEXT: Reconstruction of C2 after tumor destruction and resection remains a significant challenge. Most constructs use a strut graft with plate or screw fixation. A novel C2 prosthesis (cage/plate construct) combining a titanium mesh cage with bilateral C1 shelves and a T-plate has been used successfully in 18 patients. Supplemental posterior instrumentation includes C0-C3 or C1-C3 fixation. Biomechanical comparisons of this C2 prosthesis with traditional fixation options have not been reported. PURPOSE: To investigate and compare the stability of the novel cage/plate construct w

Cervical spine and temporomandibular joint arthritis in a child with Kawasaki disease.

Severe neck pain, stiffness, and torticollis in a child with Kawasaki disease commonly represent aseptic meningitis or lymphadenitis. Here we present an unusual case of severe cervical spine and bilateral temporomandibular joint arthritis in a 5-year-old boy with a relapse of Kawasaki disease and coronary artery ectasia. The patient had a favorable response to a second course of intravenous immunoglobulin G and indomethacin therapy, with complete resolution of his symptoms.

Motion compensation associated with single-level cervical fusion: where does the lost motion go?

STUDY DESIGN: Seven adult human cadaveric cervical spines (C2-T1) were biomechanically tested in a programmable testing device. OBJECTIVE: Compare the effects of incremental single-level fusion at different levels of the cervical spine. SUMMARY OF BACKGROUND DATA: Clinical studies have reported degenerative symptomatic disc disease at disc levels adjacent to fusion. No known study has attempted to delineate the effects of single-level fusion at different levels of the cervical spine. METHODS: The spines were tested in flexion, extension, right and left lateral bending, and right and left ax

Traumatic C6-7 subluxation associated with C-7 fracture treated with a pedicle screw system under navigation guidance--case report.

A 51-year-old female presented with traumatic C6-7 subluxation associated with C-7 fracture due to an automobile accident. She underwent pedicle screw fixation at C-6, C-7, and T-1 for stabilization of the spinal column. A neuronavigation system was used to obtain accurate placement of the pedicle screws. The patient recovered well without further neurological compromise. Postoperative cervical radiography showed reasonable restoration of the vertebral column without delayed kyphotic deformity.

Noninvasive treatments for headache.

Primary headache disorders are often accompanied by neck pain or other symptoms referable to neck muscles. Therefore, physical therapy and other physical treatments are commonly prescribed for headache management. A medical literature review was completed in order to gather information regarding the efficacy of selected physical modalities in the treatment of primary and cervicogenic headache disorders. After analyzing the collected data, expert opinions were developed regarding the utility and efficacy of selected physical modalities in the management of primary and cervicogenic headaches. Ba

Anterior cervical osteophyte causing dysphagia as a complication of laminectomy.

Asymptomatic osteophytes of the anterior margins of the cervical vertebral bodies (VB) may occur in 20 - 30% of the population. On rare occasions, dysphagia or dyspnoea may be caused by such cervical osteophytes. We present the case of a 27-year-old woman with progressive dysphagia caused by a voluminous osteophyte secondary to chronic cervical instability after a laminectomy performed 8 years before. Progressive growth of the osteophyte was observed in sequential roentgenograms. As far as we know, this complication of cervical laminectomy has never been reported before. After resection of the

[Spinal cord injury due to osteoarticular causes].

INTRODUCTION: Spinal cord injury is one of the most tragic disabilities, because of its sequelae. Incidence ranges between 9 and 53 cases per million inhabitants. Injuries due to non-traumatic causes account for a significant number of cases but they have not been studied in depth. Our objective was to conduct a descriptive study of osteoarticular-type non-traumatic spinal cord injuries in our centre. PATIENTS AND METHODS: We carried out a retrospective study of the medical records of patients referred to our centre because of non-traumatic, degenerative spinal cord injury between 1998 and 20

Intracranial hypotension caused by cervical cerebrospinal fluid leak: treatment with epidural blood patch.

UNLABELLED: This report describes treatment with cervical epidural blood patch of low cerebrospinal fluid (CSF) pressure headache resulting from spontaneous CSF leak via a tear in a cervical dural cuff. The leak was diagnosed by a dynamic computed tomography (CT)-myelography study followed by gadolinium enhanced magnetic resonance imaging(MRI)-scan. The epidural needle was inserted with the aid of image intensifier and CT-scan to guide the needle to the precise site of the CSF leak. Blood mixed with gadolinium was injected, and subsequent MRI scanning provided the first description of spread o

Radiological considerations of posterior cervical lateral mass fixation using plate and screw.

The aim of this study is to determine whether the posterior cervical fusion methods with the plate-screw system applied to the lateral mass of cervical spine are radiologically safe to patients. The lateral cervical X-rays and CT scans were done on 40 normal adults without cervical problem. Based on Roy-Camille and Magerl's method, the theological trajectory of screw was shown on films and the parameters were measured. The study based on Roy-Camille's method showed less than one percent chance of injury on the facet joint and the mean depth of the screw to be 10.5 +/- 1.4 mm. On the other hand
