Fibromyalgia syndrome: practical strategies for improving diagnosis and patient outcomes.

Recent advances in the understanding of the etiology, epidemiology, diagnosis, and treatment of fibromyalgia must be applied in clinical practice to achieve optimal patient outcomes. Current evidence indicates that fibromyalgia is a hyperalgesic state, resulting from a generalized problem with augmented pain processing that likely results from the way the spinal cord and the brain process pain and other sensory information. The descending pain pathway involving serotonin, norepinephrine, and dopamine, as opposed to the descending opioid pain pathway, appears to be selectively attenuated. Newer

Reduced hippocampal neurogenesis and skill reaching performance in adult Emx1 mutant mice.

Mammalian homeobox gene Emx family is involved in the development of the rostral brain. Loss-of-function studies suggest that, despite the agenesis of corpus callosum, the Emx1 mutants display relatively modest defects compared to the Emx2 mutants. However, the role of the Emx1 in neurogenesis and brain function has never been explored. We used unbiased stereology to determine the number of proliferating progenitors and immature neurons in the adult neurogenic zones. Although previous studies have established that the formation of the dentate gyrus (DG) requires Emx2, we found that the adult E

Attenuating corticosterone levels on the day of memory assessment prevents chronic stress-induced impairments in spatial memory.

This study investigated whether chronic stress-induced spatial memory deficits were caused by changes in the hypothalamic-pituitary-adrenal axis, such as corticosterone (CORT) elevations on the day of memory assessment, rather than the consequence of structural changes in the hippocampus. Male Sprague-Dawley rats were restrained for 6 h/day/21 days, and spatial memory was assessed on the Y-maze on day 22. Ninety minutes before training, rats received a subcutaneous injection of vehicle or metyrapone, a CORT synthesis inhibitor, and then spatial memory was determined 4-h later. The highest dose

Selective loss of hilar neurons and impairment of initial learning in rats after repeated administration of electroconvulsive shock seizures.

Prolonged seizures induced by neurotoxins or intracranial electrical stimulation provoke death of hippocampal neurons, which results in conspicuous learning and memory deficits. We examined whether repeated brief seizures elicited by electroconvulsive shock (ECS) can also deteriorate hippocampal structure and function. Adult Wistar rats were administered six ECS seizures, the first five of which were 24 h apart, whilst the last two were spaced by a 2-h interval. Following a 2-month recovery period, the cognitive status of the animals was assessed using the water maze task. ECS-treated animals

The role of exposure with response prevention in the cognitive-behavioural therapy for bulimia nervosa.

BACKGROUND: One hundred and thirty-five women with bulimia nervosa participated in a randomized clinical trial designed to determine whether the addition of exposure with response prevention to a core of cognitive-behavioural therapy (CBT) leads to greater clinical improvement and lower risk of relapse. We present results from the end of treatment and 6- and 12-month follow-up. METHODS: Participants received eight sessions of CBT and were then randomized to either exposure to pre-binge cues (B-ERP), exposure to pre-purge cues (P-ERP), or a relaxation training control condition (RELAX). Legionella pneumophila-induced visual learning impairment reversed by anti-interleukin-1 beta.

Infecting mice with the opportunistic intracellular pathogen Legionella pneumophila markedly inhibited place learning of infected C57BL/6 mice as determined by the Morris water maze test. Mice infected with L. pneumophila evinced much less ability to learn the position of a hidden platform than did normal noninfected mice, which quickly learned the location of the hidden platform and escaped from the cool water of the pool with increasing efficiency. However, infected mice treated with anti-interleukin-1 (anti-IL-1) neutralizing antibody learned the task with about the same efficiency as the c

Stigma and social symptoms in Parkinson's disease: A scoping review.

BACKGROUND: The incidence of Parkinson's disease (PD) is increasing more rapidly than any other neurodegenerative disorder. Motor and non-motor symptoms, and deficits in cognition and communication, negatively impact people with PD's social functioning and can result in enacted and felt stigma. The aim of this scoping review is to explore the determinants and effects of stigma in PD. METHODS: This scoping review followed the Joanna Briggs Institute methodology for scoping reviews. Quantitative and qualitative studies from January 2000 to April 2026 focusing on stigma in PD were considered for

Reasoning in machine vision by learning fast and slow thinking.

Reasoning is a hallmark of human intelligence, enabling adaptive decision-making in complex unfamiliar scenarios. In contrast, machine intelligence remains bound to training data, unable to dynamically refine solutions at inference. While recent advances have explored machine reasoning - trading inference-time compute for improved performance - they focus on verbal domains such as mathematical problem-solving where explicit rules govern step-by-step solution generation. Many tasks lack sufficient labelled data and require alternative performance improvement mechanisms, such as inference-time c

How Rapid Cycle Deliberate Practice Compares to Traditional Simulation: A Qualitative Investigation of Pediatric Resident Perceptions.

OBJECTIVES: Rapid cycle deliberate practice (RCDP) is a type of within-simulation debriefing that alternates between specific practice and targeted feedback. While RCDP has become increasingly used in conjunction with traditional debriefing, little is known about specific contexts in which RCDP might be most useful. Therefore, our objective was to explore learners' perceptions of RCDP versus traditional simulation, with focus on differences in learning and psychological safety. METHODS: Pediatric residents at a large pediatric residency program attended simulation trainings with both RCDP and

Farm-based therapeutic horticulture for people living with dementia: the Vicenza Farm Project.

Dementia is a major global health challenge, with wide-ranging psychosocial and relational impacts that call for supportive, everyday interventions alongside clinical care. While advances in Alzheimer's disease are promising, dementia includes multiple conditions, including young-onset forms, and existing pharmacological and non-pharmacological interventions still offer valuable but incomplete support, often leaving everyday psychosocial needs unmet for most people living with dementia. This Perspective describes the Vicenza Farm Project, a farm-based therapeutic horticulture program developed

Self-supervised small vessel segmentation with shape-aware geometric models and attention.

Small vessels play a critical role in the development of various brain diseases. Significant advances have been made in high-resolution vascular imaging, but it remains a challenge to precisely segment small vessels to quantitatively assess their integrity. Existing vessel segmentation approaches typically assume, implicitly or explicitly, a tubular representation suitable for large vessels but suboptimal for small vessels because they often exhibit shape irregularity, weak contrast, and discontinuity even in high-resolution imaging data. To address these challenges, we propose a generalized a

A Single-Armed Pilot Study to Evaluate a Psychosocial Intervention for Young People Who Have Experienced a Burn Injury: The Wellbeing Study.

Children with serious burns often suffer psychosocial consequences. This study evaluated the pilot of a 6-session psychoeducational intervention newly designed to support the wellbeing of children who experienced burns. Each 1-h session was delivered online to individuals at times deemed convenient to participants, by trained facilitators (mental health researchers and other allied health professionals). The pilot was single-arm, with psychosocial screening administered at baseline, immediately postintervention, and at 6- and 12-month follow-ups. Eligible participants were 6-17 years, 6-week t

Resting-State Brain Age Predicts Cognitive and Sensorimotor Deficits in Schizophrenia Spectrum Disorders: A Validation & Longitudinal Study.

Brain-age models use neuroimaging features to predict chronological age and thereby estimate normative lifespan patterns; the resulting brain-age gap (BAG) quantifies deviation from age-expected brain characteristics. Structural brain-age acceleration is well established in schizophrenia spectrum disorders (SSD), but the utility of resting-state functional connectivity (rs-FC)-based brain age remains unclear. Here, we trained rs-FC brain-age models on aggregated lifespan data from healthy controls (N≈2,200) and evaluated them in four independent SSD case-control cohorts. Across cohorts and atl

Organizing adult attachment in alternative ways: A qualitative assessment of schemas antithetical to the secure base script.

How adults make meaning of their childhood experiences with caregivers plays a central role in how they anticipate support, manage distress, and interpret relationships. To understand how adults conceptualize these attachment relationships, the Adult Attachment Interview (AAI) probes for childhood memories of general relationships, day-to-day interactions, and instances of loss, threat, and separation. Across these narratives, the AAI captures expectations of whether caregivers are perceived as an available source of support in times of distress, and whether they serve as a secure base from wh

From Inference to Ritual: Why Frequentist Statistics Became Hard to Understand and Teach in Medicine.

INTRODUCTION: Frequentist statistical tools still structure a large part of biomedical research, regulatory evaluation, and medical teaching, even though their correct interpretation remains uncertain among students, clinicians, and readers of the literature. This misunderstanding is usually attributed to pedagogical shortcomings or individual cognitive errors. This article examines the structural reasons for this persistent misunderstanding. It argues that it arises from a combination of historical, methodological, semantic, and institutional factors rather than from a direct lack of technica
