Endovascular management of aortic dissections involving the common carotid and subclavian arteries.

Aortic dissection is a life-threatening condition, in which rupture of the internal wall of the aorta is observed. The aim of the study was to present the techniques used in patients with type A Stanford aortic dissection treatment by means of carotid-carotid by-pass surgery and implantation of the aortic stent-graft with intentional occlusion of the left common carotid and subclavian arteries. Surgical methods were presented on the basis of three patients treated at the Department of Vascular, General and Oncological Surgery, Memorial Copernicus Hospital in Łódź. Different carotid- carotid by

The use of flow diverter stents in the management of traumatic vertebral artery dissections.

Flow diverters constitute a new generation of flexible self-expanding stent-like devices with a high metal-surface area coverage, specifically designed for the endovascular management of complex cerebral aneurysms. Recently, other potential applications for these devices in the field of occlusive cerebrovascular disease have been described. In vertebral artery dissections causing occlusion associated with a burden of extensive clots, we have found that the reduced porosity of the diverter mesh serves as an effective barrier to in-stent clot protrusion and distal embolization. We describe the n

Number of entry tears is associated with aortic growth in type B dissections.

BACKGROUND: Aortic growth rate in acute type B aortic dissection (ABAD) is a significant predictor for aortic complications and death. To improve the overall outcome, radiologic predictors might stratify patients who benefit from successful medical management vs those who require intervention. This study investigated whether the number of identifiable entry tears in ABAD patients is associated with aortic growth. METHODS: ABAD patients with uncomplicated clinical conditions and therefore treated with medical therapy were evaluated. Those with a computed tomography angiography (CTA) obtained a

Ultrasound of the sural nerve: normal anatomy on cadaveric dissection and case series.

The sural nerve is a small sensory nerve innervating the lateral aspect of the ankle and foot. Clinical symptoms of pathology may present as atypical sensory changes in this region. We present the normal anatomy and ultrasound technique for examination of the sural nerve based on an anatomical dissection, as well as imaging in a normal volunteer. We also present a case series (n=10) of different conditions of the sural nerve that we encountered based on a review of interesting cases from 4 institutions. The pathological conditions included neuropathy related to stripping or venous laser surger

Technical nuances of temporal muscle dissection and reconstruction for the pterional keyhole craniotomy.

The supraorbital keyhole approach offers a limited access for aneurysms located at the middle cerebral artery (MCA) bifurcation with long M(1) segments or proximal M(2) aneurysms. Alternative minimally invasive routes centered on the pterion have been developed to address these aneurysms. Appropriate dissection and reconstruction of the temporal muscle are important for optimal exposure and best cosmetic results with the pterional keyhole craniotomy. The authors describe the technical nuances of temporal muscle dissection and reconstruction adapted to the pterional keyhole craniotomy. After in

Flow-diverting stent for ruptured intracranial dissecting aneurysm of vertebral artery.

BACKGROUND: The treatment of ruptured dissecting aneurysms of the intracranial vertebral artery (VA) with parent vessel preservation is a challenge for neurosurgeons and interventional neuroradiologists. OBJECTIVE: To propose an indication for flow-diverting treatment for reconstruction of a dissecting VA with acute subarachnoid hemorrhage. METHODS: Two male patients transferred after acute subarachnoid hemorrhage and dissecting aneurysm on the V4 segment of the dominant VA. An occlusion test was not performed because of their poor clinical state. A flow-diverting stent represented by the Pi

Analysis of intimal extent and predictors of renal atrophy in patients with aortic dissection.

BACKGROUND: The intimal flap of aortic dissection may extend to the abdominal branches and probably lead to malperfusion syndrome. Renal malperfusion and renal atrophy are significantly related to patient outcomes. PURPOSE: To study the extent of the intimal flap and predisposing factors for renal atrophy in patients with aortic dissection. MATERIAL AND METHODS: From January 2001 to June 2008, 176 (137 men, aged 21-86 years, mean 51.9 years) of 225 subjects with aortic dissection and computed tomography (CT) met the inclusion criteria for this study. Of these 176 patients, 35 (19.9%) develop

Patterns of visceral and renal artery involvement in type B aortic dissections.

OBJECTIVE: The main objective of this study was to describe the visceral and renal vessel involvement and the outcomes in thoracic dissections. METHODS: Computed tomography scans of 39 patients were reviewed (30 type B, 9 type A). Visceral and renal arteries were recorded as perfused by the true lumen (TL) or false lumen (FL). Radiologic follow-up was 3.3 years (range 0-10). RESULTS: A total of 156 vessels were analyzed, and 49 (31%) were supplied by the FL. The most common patterns were FL supply of a renal artery (RA) with the visceral arteries and contralateral RA from the TL. In the foll

Endovascular treatment of ruptured dissecting aneurysms of the posterior inferior cerebellar artery.

BACKGROUND: Dissecting aneurysms of the posterior inferior cerebellar artery (PICA) are rare, with optimal treatment strategies not well established according to the location of the aneurysm on the PICA. We present our single-center experience of endovascular treatment of ruptured dissecting aneurysms of the PICA. METHODS: Nine patients with ruptured dissecting aneurysms of the PICA were treated with endovascular embolization at our facility from August 1997 to December 2011. Dissecting the causes of atopic dermatitis in children: less foods, more mites.

Atopic dermatitis (AD) is a common, chronic or chronically relapsing, multifactorial skin disease that mainly occurs in children but affects also adults. AD usually begins early in life and often concerns people with a personal or family history of asthma and allergic rhinitis. AD is characterized by eczematous changes in the epidermis and originates from a late, T-cell mediated reaction associated to the formation and production of memory T-cell of TH2 type, occurrence of homing receptor at skin level and cutaneous lymphocyte-associated (CLA) antigens. Extrinsic or allergic AD, but not intrin

Exploring the transcriptome of ciliated cells using in silico dissection of human tissues.

Cilia are cell organelles that play important roles in cell motility, sensory and developmental functions and are involved in a range of human diseases, known as ciliopathies. Here, we search for novel human genes related to cilia using a strategy that exploits the previously reported tendency of cell type-specific genes to be coexpressed in the transcriptome of complex tissues. Gene coexpression networks were constructed using the noise-resistant WGCNA algorithm in 12 publicly available microarray datasets from human tissues rich in motile cilia: airways, fallopian tubes and brain. A cilia-re

[In-vitro study on dissection of inflamed synovial tissue by hydro-jet cutting].

Surgical synovectomy is a useful therapeutic option for rheumatoid arthritis patients with ongoing active synovitis despite optimal medical therapy. The present experimental study evaluated the novel, minimally invasive surgical technique of hydro-jet cutting in vitro using synovial biopsies. Depending on the selected water pressure (30-100 bar) it is possible to achieve precise and selective dissection of the synovial membrane. It was found that application of a water jet at 60 bar for 15 s is ideal for dissecting the stratum synoviale from the stratum fibrosum without any alteration of the j

Genetic dissection of the Canq1 locus governing variation in extent of the collateral circulation.

BACKGROUND: Native (pre-existing) collaterals are arteriole-to-arteriole anastomoses that interconnect adjacent arterial trees and serve as endogenous bypass vessels that limit tissue injury in ischemic stroke, myocardial infarction, coronary and peripheral artery disease. Their extent (number and diameter) varies widely among mouse strains and healthy humans. We previously identified a major quantitative trait locus on chromosome 7 (Canq1, LOD = 29) responsible for 37% of the heritable variation in collateral extent between C57BL/6 and BALB/c mice. We sought to identify candidate genes in Can

A case of an aortic dissection in a young adult: a refresher of the literature of this "great masquerader".

Aortic dissection is often misdiagnosed, especially among young patients, and it is associated with a high mortality rate. We present here a case of fatal acute aortic dissection in a young man who was misdiagnosed with pericarditis. We reviewed the literature of acute aortic dissection in young people and we focused particularly on clinical presentations, outcomes and investigations of aortic dissection. We report a case of a 33-year-old man with a history of uncontrolled hypertension with acute pleuretic chest pain who was transferred to our hospital for suspected pulmonary embolism and died

State of the art in neuroprotection during acute type A aortic dissection repair.

UNLABELLED: Temporary (TND) or permanent neurologic dysfunctions (PND) represent the main neurological complications following acute aortic dissection repair. The aim of our experimental and clinical research was the improvement and update of the most common neuroprotective strategies which are in present use. HYPOTHERMIC CIRCULATORY ARREST (HCA): Cerebral metabolic suppression at the clinically most used temperatures (18-22°C) is less complete than had been assumed previously. If used as a 'stand-alone' neuroprotective strategy, cooling to 15-20°C with a jugular SO(2) ≥ 95% is needed to prov

Causes of misinterpretation of cross-sectional imaging studies for dissection of the craniocervical arteries.

OBJECTIVE: This review presents some of the more common causes of false-positive and false-negative interpretations of cross-sectional imaging studies showing, or designed to show, dissection of the carotid or vertebral arteries. CONCLUSION: Dissection of the craniocervical arteries is a diagnosis that can be very difficult on cross-sectional imaging studies such as CT angiography, MRI, and MR angiography.

Dissecting out the cause: a case of concurrent acute myocardial infarction and stroke.

Acute type I aortic dissection is an uncommon but potentially fatal condition requiring prompt recognition of symptoms and generally surgical intervention. A history of chronic hypertension is the major predisposing risk factor for aortic dissection. Commonly patients experience acute chest pain, but myocardial infarction or stroke due to the dissection involving the coronary or carotid arteries are rare and serious life-threatening complications. The authors describe the case of a 60-year-old male presenting with concomitant features of acute myocardial infarction and cerebrovascular accident

Stroke occurrence and patterns are not influenced by the degree of stenosis in cervical artery dissection.

BACKGROUND AND PURPOSE: In stroke attributable to spontaneous dissection of the cervical artery, it is unclear whether the occurrence and pattern of stroke depend on the degree of stenosis. METHODS: In 147 consecutive dissection of the cervical artery patients with (n=88) and without stroke (n=59), we compared the number, volume, and patterns of cerebral diffusion-weighted imaging stroke lesions among patients with <70% stenosis (Group 1), ≥70% stenosis (Group 2), and occlusion (Group 3).
