[Exophthalmometry and thyroid disease. The value of the Hertel measurement evaluated by a group of patients with thyroid diseases and a control group].

Exophthalmometry values from routine referrals to a hospital eye clinic are analysed, including adult patients with and without thyroid disease. In thyroid disease without eye motility complaints (n = 90) the median exophthalmometry value was 18 mm (range 12-28 mm). Thyroid patients with eye muscle involvement (diplopia, n = 47) had significantly higher values (median 22 mm, range 15-31 mm) but did not differ significantly from a subgroup of 51 patients without thyroid disease, but with "big eyes" as a (presumably) physiological finding (median value 20 mm, range 16-25 mm). The above three cat

Early visual deprivation results in persistent strabismus and nystagmus in monkeys.

To understand to what extent visual-pattern deprivation during infancy results in strabismus and nystagmus, the authors examined the long-term consequences of this type of deprivation in monkeys during the first 50 days of life. Three cynomolgus and three rhesus monkeys had the eyelids sutured closed within 24 hr of birth. At 25 days of age, the eyelids were opened, and the eyelids of the fellow eye were sutured closed for an additional 25 days (reverse-eyelid suture). When the eyelids were opened at 50 days of age, each monkey was found to have 20-30 delta of exotropia and nystagmus, which pe

Intermittent downbeat nystagmus secondary to vermian arachnoid cyst with associated obstructive hydrocephalus.

A 27-year-old man presented with a history of dizziness and intermittent vertical oscillopsia after oblique extension of the head and neck, following a neck injury 2 years previously. He had no other symptoms of cerebellar or brainstem dysfunction. On examination, an intermittent downbeat nystagmus lasting about fifty seconds was elicited by head extension and rotation. Radiologic examination showed slight lateral instability of the odontoid on lateral rotation, and computed tomography brain scan with Iopamidol 300 (Niopam) contrast revealed a 5-cm vermian arachnoid cyst in the posterior fossa

Inferior rectus muscle entrapped by Teflon implant after orbital floor fracture repair.

A patient with a tripod fracture had entrapment of the inferior rectus muscle after reconstruction of an orbital floor defect with an alloplastic implant. Prior to insertion of the implant, the operating surgeon reported no motility disturbance. An orbital computed tomography scan suggested that the implant impinged on the inferior rectus muscle. Because of persistent diplopia, the orbit was reexplored. An intraoperative forced duction test prior to removal of the implant was positive in the field of action of the inferior rectus muscle. An intraoperative forced duction test after removal of t

Convergence insufficiency in brain-injured patients.

Disorders of eye movements are relatively common in brain-injured patients. Some of these disturbances are caused by direct trauma to the orbital content, cranial nerves and other brain areas. Convergence, which is a part of the near-vision complex and depends on the integrative function of the cortical and subcortical areas, is commonly affected by traumatic brain injury. Intact vergence needs the integrative function of brain structures associated with acquisition of information on one side and intact mesencephalic function, including function of a vergence integrator, and their interconnect

Adaptive disorders of accommodation and vergence in binocular dysfunction.

Disturbances of binocular vision are described clinically by the Duane-White classification in terms of the magnitude of the accommodative-convergence ratio (AC/A). Convergence excess and convergence insufficiency are assumed to result from high and low AC/A ratios respectively. It is assumed that the abnormal AC/A ratio is an independent variable that underlies abnormal phorias. However, recent studies have demonstrated that the AC/A ratio is inversely related to the adaptability of tonic accommodation (lens adaptation) and directly related to adaptability of tonic vergence (prism adaptation)

Interhemispheric coordination is compromised in subjects with developmental dyslexia.

Developmental dyslexics (DDs) and good readers (GRs) were tested on measures of interhemispheric coordination. All subjects (ages 16 to 47) demonstrated normal oculomotor control and visual acuity, prior to testing. Subjects were instructed to track three different point-light source patterns (single stimulus in one hemifield, dual stimuli in one hemifield and a pair of simultaneous, symmetric, bihemifield stimuli [SSBS]), presented in random sequence and arrayed horizontally at +/- 5, +/- 10, and +/- 15 degrees of eccentricity. Tested with unihemifield stimuli, all subjects showed normal sacc

Ocular surface drying and tear film osmolarity in thyroid eye disease.

Five factors potentially associated with corneal exposure--palpebral fissure width, exophthalmos, blink rate, lagophthalmos, and lid lag--were evaluated in 17 patients with Graves' disease to determine which were associated with ocular surface damage. Multiple regression analysis revealed that increased palpebral fissure width and increased blink rate were both significant predictors of ocular surface damage (as measured by rose Bengal staining). Tear osmolarity and tear film break-up time were measured to determine the type of drying mechanism involved in thyroid eye disease. All eyes examine

Variability and habituation of nystagmic responses to hot caloric stimulation of normal subjects. Evidence that this test may be inapplicable to monitoring drug-induced vestibular toxicity.

Experiments were performed on 25 otoneurologically "normal' subjects to evaluate the hot caloric test as a screening test for aminoglycoside vestibular toxicity. Using portable equipment under non-ideal conditions, it was found that there was a large inter-subject variability in nystagmic response and that, instead of a random test-retest variability, a systematic variation in response occurred on repeated caloric stimulation with water at 44 degrees C. A response decline (habituation) evident in both the maximum slow phase velocity and the maximum frequency occurred at second test, although t

[The perverted nystagmus (author's transl)].

The perverted nystagmus is a modification of the horizontal beating nystagmus (in classical caloric stimulation) by a vertical or rotatory mainly component. Experimentally, this disorder can be shown after an unilateral destroy of the superior or median vestibular nucleus. We show 32 cases of subjects with a perverted nystagmus. 75% of these subjects have a pure central disease and 20% a vestibular neuronitis. The perverted nystagmus had an upbeat component with the cold stimulation and downbeat component with the hot stimulation. It was accompanied by a spontaneous nystagmus in 66% (and 30% w

Variations in gravitoinertial force level affect the gain of the vestibulo-ocular reflex: implications for the etiology of space motion sickness.

Recordings of horizontal nystagmus were obtained on 16 male subjects exposed to repeated patterns of horizontal angular acceleration, constant velocity rotation, and sudden-stop deceleration in the laboratory and in the free-fall and high-force periods of parabolic flight. Nystagmus intensity was a clear function of gravitoinertial force level: slow phase velocity and beat frequency increased during exposure to high force levels and decreased in free-fall compared to values obtained at I G. These findings indicate that the gain of the vestibulo-ocular reflex decreases in free-fall. This fact l

Cerebellar ataxia, opsoclonus, and occult neural crest tumor. Abdominal computerized tomography in diagnosis.

A 9-month-old female with opsoclonus and ataxia was examined. Computerized axial tomography (CT) of her abdomen identified a retroperitoneal mass of neural crest origin that was not recognized by more conventional roentgenographic methods. The syndrome of cerebellar ataxia, myoclonus, and opsoclonus, and its relationship to neural crest tumors is reviewed along with the usefulness of abdominal CT.

Comparison of effects of pentobarbital and ethanol on the neuronal activity in the posterior parietal association cortex.

The problem of this study was whether the effects produced by alcohol in the posterior parietal association cortex are specific to this drug or shared by other centrally acting depressant drugs such as barbiturates. The effect of graded doses of pentobarbital on multineuronal impulse activity was recorded with transdural microelectrode technique in 30 expts. in Brodmann's area 7 of five stump-tailed monkeys (Macaca speciosa). The results were compared with those from 32 expts. performed with alcohol and published separately. The dosage of the two drugs was determined on the basis of the monkey

Restrictions in the diagnoses and management of strabismus problems.

Restrictions have been found to be a common cause of incomitant strabismus and limitation of ocular rotations. Different types of restrictions have been described and the technique of forced duction testing reviewed. Causes of restriction include endocrine eye disease, orbital floor and medical wall fracture, Brown's syndrome, tight lateral rectus syndrome, congenital tight muscles, scarring, muscle contracture, and orbital celluitis or tumors. The importance of recognizing restrictions in the management of these problems has been emphasized.

A quantitative comparison of traditional reading of the EEG and interpretation of computer-extracted features in patients with supratentorial brain lesions.

The EEGs of adult patients with suspected supratentorial brain lesions were recorded on paper and on magnetic tape, using a small computer. The spectra of 16 channels were computed on a 40 sec sample. For each channel, a ratio of the type (delta + theta)/(alpha + beta) was computed and displayed on the computer terminal, a measure of the asymmetry in slow wave activity between homologous areas of the head was also displayed. This display is called a canonogram. It is believed to be a meaningful representation of the important characteristics of the EEG in the presence of supratentorial lesions

Two visual systems in clinical neurology: readaptive role of the primitive system in visual agnosic patients.

In six patients with visual agnosia syndromes there was a striking dissociation between a preserved possibility of recognition of letters or objects in movement versus the recognition by simple visual static presentation. After a review of the literature it is concluded that at least two visual systems may be individualized in humans, i.e. a more recent one (the geniculo-calcarine) and an older one (the tectal-pulvinar-occipito-temporal system). Both from a physiological and clinical point of view, the most primitive system has two parts, i.e. a tectal and a telencephalic part. In visual agnos
