Femoral neck fractures. 165 cases treated by multiple percutaneous pinning.

A prospective series of 165 displaced subcapital fractures of the femoral neck treated with reduction and percutaneous multiple pinning is presented. The method offers the advantages of a short operating time, no loss of blood, immediate weight-bearing and no infection problems. Furthermore the operative procedure can be performed under local anaesthesia. In Garden stage 3 fractures union occurred in 97 per cent and late segmental collapse in 8 per cent. In Garden stage 4 fractures union occurred in 72 per cent and late segmental collapse in 14 per cent. Failures were almost exclusively encoun

[Dislocations of the face and skull. Report of 20 cases].

The authors present a serie of 20 cases of craniofacial injuries with or without CSF leakage. They propose a simple classification in 2 groups following the impact: braniofacial injury or orbitofacial injury. For their, craniofacial or orbitofacial injuries are to be operated in one stage by an neurosurgical and maxillofacial team. The best time for operation is approximately around the 10th day following trauma.

Primary Total Ankle Replacement for Acute Comminuted Pilon Fractures in Older Adults: Proposed Indications and a Report of 29 Cases.

BACKGROUND: Management of severe distal tibia pilon fractures (AO/OTA 43-C3) in older adults is exceptionally challenging. Open reduction and internal fixation is associated with a high incidence of posttraumatic arthritis, whereas primary arthrodesis results in significant functional limitations. We investigated primary total ankle replacement (TAR) as a definitive, single-stage, motion-preserving solution. MATERIALS AND METHODS: We retrospectively reviewed 29 older adult patients (age ≥60 years) treated with primary TAR since 2016 for comminuted pilon fractures. Inclusion required severe ar

Incidence of Unapparent Preoperative Deep Vein Thrombosis in Patients with Traumatic Intraarticular Tibial Plateau Fracture.

Objective: In patients with leg injuries, typical symptoms of thrombosis such as painful swelling may be misinterpreted as a consequence of the trauma. This has the potential to result in an unexpected embolism, especially during the perioperative period. This study investigates the incidence of unapparent preoperative deep-vein thrombosis (DVT) in patients with traumatic intraarticular tibial plateau fracture (TPF). A comprehensive analysis was conducted to identify possible risk factors, with particular attention paid to fracture severity and soft tissue injury. Methods: This retrospective s

The neurologically intact patient with TLICS 4 or 5 burst fracture should be given a trial of nonoperative management.

Thoracolumbar burst fracture treatment in neurologically intact patients is controversial with many classification systems to help guide management. Thoracolumbar Injury Classification and Severity score (TLICS) provides a framework, but evidence is limited, and recommendations are primarily based on expert opinion. In this retrospective cohort study, data was reviewed for patients with thoracolumbar burst fractures at a Level-1 Trauma Center in New England from 2013 to 2018. Neurologically intact patients without subluxation/dislocation on supine computed tomography were included. Multimodal

Current strategies for the treatment of solitary and aneurysmal bone cysts: A review of the literature.

This review of the literature aims to compare the etiology, the pathogenesis, the clinical diagnostics and the relevant treatment options of two different types of cystic bone lesions: the solitary bone cyst (SBC) and the aneurysmal bone cyst (ABC). Whereas the clinical symptoms and the radiographic appearance can be similar, the diagnostic pathway and the treatment options are clearly different. The solitary bone cyst (SBC) represents a tumor-like bone lesion, occurring most frequently in the humerus and femur in children and adolescents. Pain caused by intercurrent pathological fractures is

Lateral tibial plateau fractures-functional outcomes and complications after open reduction and internal fixation.

PURPOSE: Tibial plateau fractures are severe intra-articular injuries; their treatment is accompanied by numerous complications and requires extensive surgical experience. In this manuscript, we compared our experiences with data from the literature. METHODS: Retrospectively, we analyzed 41 patients with closed lateral tibial plateau fracture (Schatzker's I-III). All fractures were treated surgically with open reduction and internal plate fixation. Post-operative complications and final outcomes were monitored according to the Tegner Lysholm Knee Scoring System. The average follow-up was 5.7

Reverse shoulder arthroplasty for proximal humeral fractures: outcomes comparing primary reverse arthroplasty for fracture versus reverse arthroplasty after failed osteosynthesis.

BACKGROUND: Surgical treatment of proximal humeral fractures in the elderly pose challenges in decision making. Reverse total shoulder arthroplasty (RTSA) has been established as a reliable option for salvage of failed hemiarthroplasty, although few studies have analyzed RTSA after failed open reduction with internal fixation (ORIF). This study evaluated the outcomes of patients with failed osteosynthesis who undergo salvage RTSA compared with patients undergoing primary RTSA for proximal humeral fractures. METHODS: We retrospectively reviewed 18 patients who underwent primary RTSA for acute

Direct visualization for syndesmotic stabilization of ankle fractures.

BACKGROUND: Ankle fractures with syndesmotic injury treated via standard trans-syndesmotic fixation have a high percentage of syndesmotic malreduction.(10) We established a protocol involving both direct syndesmosis visualization and meticulous tibial incisura reconstruction via the posterior malleolus fracture fragment, when present, via the attached, intact PITFL, then compared this with historic controls to assess improvement after this type of syndesmosis reconstruction. MATERIALS AND METHODS: One hundred forty-nine consecutive direct visualization patients were treated prospectively with

Midterm results with a bipolar radial head prosthesis: radiographic evidence of loosening at the bone-cement interface.

BACKGROUND: Metal prostheses are useful for restoring elbow and forearm stability when the radial head cannot be fixed after a fracture. Because the anatomy of the radial head is difficult to reproduce with a prosthesis, two different options have been proposed: a bipolar prosthesis with a fixed stem and a mobile head, and a monoblock prosthesis with a smooth stem that is intentionally fixed loosely in the neck of the radius. One concern with a fixed-stem implant with a mobile head has been the risk of osteolysis. The purpose of this study was to evaluate radiographic changes reflecting or sug

[Surgical treatment of thoracolumbar spinal fractures with internal fixator and transpedicular spongiosa-plasty].

We report on 30 unstable fractures of the thoracolumbar spine which were operatively treated between 1987 and 1992 with the AO Internal Fixator and transpedicular bone grafting. There were 26 flexion-compression fractures, 2 flexion-distraction injuries and 2 fracture-dislocations. Follow-up ranged from 2 to 5 years. All patients were examined and their histories reviewed. New radiographs were obtained and a standardized questionnaire on pain and on functional and economical status was answered. The radiographical analysis included measurement of the vertebral, segmental and local kyphosis and

Scaphoid fractures and wrist pain--time for new thinking.

As with the practice of all medicine, an early accurate diagnosis is essential. The same applies after injury. The diagnosis should be made at the earliest opportunity--this should be done as soon as an appropriate expert is able to examine the patient, assess adequate radiographs and, where necessary, order special investigations such as a bone scan. By this means, distal radial fractures, injuries of the radioulna joint, dislocated lunates, Bennett's fractures, dorsal capsular wrist sprains, avulsion fractures etc. can be managed accordingly. Those patients in whom clinical examination, plai

Radiographic Healing and Observed Complications Following Light-Cured Polymer Immobilization: A Retrospective Cohort Study of 108 Patients.

Purpose: Traditional plaster and fiberglass casts remain widely used for fracture immobilization but are associated with recognized challenges, including skin irritation, hygiene limitations, and distress during cast removal, particularly in pediatric populations. Light-cured polymer immobilization (LCPI) systems have been introduced as an alternative method of fracture support. The primary objective of this study was to describe radiographic healing and alignment outcomes among patients treated with an LCPI system. Secondary objectives were to document skin- and device-related events and to i

Complication profile and surgical trends in hemiarthoplasty for femoral neck fractures: analysis from a national referral hospital in Ethiopia.

BACKGROUND: Hemiarthroplasty is one of the standard treatment options for displaced femoral neck fractures in the elderly. While surgical approaches, implant choices, and complication profiles are well-documented in high-income countries, there is a critical lack of data from low-resource settings like Ethiopia, where system constraints and resource availability may significantly influence practice and outcomes. This study aimed to analyze the surgical trends, implant choices, and complication profiles of hemiarthroplasty at a major Ethiopian trauma center. METHODS: A retrospective cross-secti

Does the order matter? Comparing the order of stem placement and fracture reduction in revision total hip arthroplasty for Vancouver B2 and B3 periprosthetic femur fractures.

BACKGROUND: Vancouver B2 and B3 periprosthetic femur fractures (PPFF) have posed significant treatment challenges due to stem instability and lack of adequate femoral bone stock. This study investigated subsidence, survivorship, and outcomes of Vancouver B2 and B3 fractures, based on the order in which revision stem placement and fracture reduction occurred during revision total hip arthroplasty (rTHA). METHODS: This retrospective, cohort study included 46 rTHAs between June 2011 and April 2023. Included patients underwent rTHA for Vancouver B2 or B3 PPFF with minimum one-year radiographic an
