Challenges for interpreting results of antimicrobial susceptibility testing for treating infections of foals with Rhodococcus equi.

Rhodococcus equi is a facultative intracellular pathogen and a major cause of pneumonia in foals worldwide. The standard treatment for affected foals is a combination of a macrolide with rifampicin. The widespread use of this combination has led to the emergence of multidrug-resistant (MDR) strains of R. equi. Consequently, antimicrobial susceptibility testing is essential for guiding treatment of rhodococcal pneumonia. Unfortunately, clinical breakpoints for macrolides and other antimicrobials against R. equi for foals are lacking, thereby complicating clinical interpretation of in vitro resu

Shared Decision-Making in the Surgical Management of Rectal Cancer: Comparing Low Anterior Resection and Abdominoperineal Resection Using a Patient Decision Aid.

Low rectal cancer is surgically managed with one of two primary procedures: low anterior resection (LAR) or abdominoperineal resection (APR). Each procedure has a unique profile of potential complications, oncologic outcomes, and quality-of-life impacts. The acceptability of these outcomes is highly driven by patient values. Consequently, shared decision-making is essential to selecting the optimal procedure for each patient. Evidence has shown that patient decision aids (PtDAs) improve patient knowledge, reduce decisional conflict, and support value-congruent decisions. This review describes

The Peritoneal Membrane and Its Role in Peritoneal Dialysis.

A healthy and functional peritoneal membrane is key to achieving sufficient ultrafiltration and restoring fluid balance, a major component of high-quality prescription in patients treated with peritoneal dialysis (PD). Variability in membrane function at the start of PD or changes over time on treatment influence dialysis prescription and outcomes, and dysfunction of the peritoneal membrane contributes to fluid overload and associated complications. In this review, we summarize the current knowledge about the structure, function, and pathophysiology of the peritoneal membrane with a focus on c

Donor-derived infections in solid organ transplant recipients.

PURPOSE OF REVIEW: The potential for transmission of donor-derived infections (DDIs) is impossible to eliminate, but a thoughtful and systematic approach to donor evaluation can mitigate the risk. Prevention is a key issue and clinicians must maintain a high index of suspicion and remain vigilant in staying up to date on emerging infections. COVID-19 and Monkeypox have represented a new challenge for infectious disease screening and recommendations have been evolving, as knowledge in the field has grown. Additional considerations for pretransplant deceased donor screening include testing for n

Gaps in the knowledge of thyroid hormones and placental biology†.

Thyroid hormones (THs) are required for the growth and development of the fetus, stimulating anabolism, and oxygen consumption from the early stages of pregnancy to the period of fetal differentiation close to delivery. Maternal changes in the hypothalamic-pituitary-thyroid axis are also well known. In contrast, several open questions remain regarding the relationships between the placenta and the maternal and fetal TH systems. The exact mechanism by which the placenta participates in regulating the TH concentration in the fetus and mother and the role of TH in the placenta are still poorly st

Behavior in Avalanche Terrain: An Exploratory Study of Illegal Snowmobiling in Norway.

Snowmobilers make a grim and significant contribution to avalanche fatality statistics in Norway. However, there is limited knowledge on the behavior of this group in avalanche terrain and the factors influencing this behavior. Our study documents what snowmobilers do and not do in avalanche terrain, how their behavior relates to managing complex avalanche conditions and if there is a mismatch between avalanche competence, education and riding preferences. This ethnographic study observed snowmobiler tracks and thus avalanche terrain usage in Northern Norway during 2018 and 2019, supported by

RADIOGRAPHIC ANATOMY OF THE FORELIMB IN THE NORTHERN TAMANDUA (TAMANDUA MEXICANA).

The thoracic limb anatomy of anteaters in the family Myrmecophagidae is specialized for accessing termite and ant nests and for defense purposes. In the case of the northern tamandua (Tamandua mexicana), the forelimbs are also adapted for arboreal and terrestrial locomotion. Unfortunately, this species faces many conservation threats, such as habitat loss and traffic accidents, and injured individuals are frequently taken to wildlife rehabilitation centers. However, lack of knowledge of the radiographic osteoanatomy of this species may prevent appropriate management of injuries and thereby red

Endometrial receptivity and pregnancy outcome.

Human implantation is a highly complex and multifactorial process. Successful implantation requires the presence of a healthy embryo, a receptive endometrium, and a synchronized molecular dialogue between the two, as well as immune tolerance/protection from the host. The endometrial receptivity refers to a hormonally limited period in which the endometrial tissue acquires a transient functional status allowing blastocyst implantation and pregnancy initiation. Global knowledge of endometrial receptivity grew up in recent years. Improvements in genetics, new biomarkers, noninvasive methods, new

Emergence of Coronavirus (COVID-19) Outbreak: Anthropological and Social Science Perspectives.

With the ongoing coronavirus (severe acute respiratory syndrome coronavirus-2, SARS-CoV-2), the entire community of health professionals is working to control disease and investing crores in vaccine development. The present discussion is to bring the focus on various social issues that emerge during outbreak and calls for equal attention as that of other health-care interventions. These issues are summarized in three categories: first, stigmatization due to lack of knowledge about the source of infection; second, speculations and their consequences around lack of knowledge about transmission;

Strigolactones: how far is their commercial use for agricultural purposes?

Strigolactones are a class of natural and synthetic compounds that in the past decade have been exciting the scientific community not only for their intriguing biological properties but also for their potential applications in agriculture. These applications range from their use as hormones to modify and/or manage plant architecture, to their use as stimulants to induce seed germination of parasitic weeds and thus control their infestation by a reduced seed bank, to their use as 'biostimulants' of plant root colonisation by arbuscular mycorrhizal fungi, improving plant nutritional capabilities

Women's perceptions of the personal and family impact of genetic cancer risk assessment: focus group findings.

Women with a personal or family history of breast or ovarian cancer are increasingly presenting for genetic cancer risk assessment (GCRA). To explore the personal and family impact of GCRA, four focus groups were conducted of women seen for risk assessment. Participants were 22 primarily non-Latina White women with a personal or family history of breast or ovarian cancer. Analysis of the data identified new themes related to balancing time to assimilate risk information with the need to make timely healthcare decisions, physicians' lack of sufficient genetic knowledge, and concern for daughter

Ethnomedicinal plant knowledge and practice of the Oromo ethnic group in southwestern Ethiopia.

An ethnomedicinal study was conducted to document the indigenous medicinal plant knowledge and use by traditional healers in southwestern Ethiopia from December 2005 to November 2006. Data were collected from 45 randomly selected traditional healers using semi-structured interviews and observations. Sixty-seven ethnomedicinal plant species used by traditional healers to manage 51 different human ailments were identified and documented. Healers' indigenous knowledge was positively correlated with their reported age but not with their educational level. High degree of consensus was observed amon

Advances in lupus and Sjögren's syndrome: a tribute to Josep Font.

Dr. Josep Font (Barcelona, 1953-2006) devoted his professional career to the care of patients with systemic autoimmune diseases (SAD). In 1995, he created the Department of Autoimmune Diseases at the Hospital Clinic in Barcelona, a pioneering unit in Europe specifically dedicated to the clinical management of these patients. The research output of Dr. Font has been prodigious, with a total of over 500 scientific articles published in 25 years, and was especially focused on the clinical and immunological characterization of systemic lupus erythematosus (SLE) and primary Sjögren's syndrome (SS).

The use of everyday technology by people with dementia living alone: mapping out the difficulties.

While the technological development available to society is taking quantum leaps, we have little knowledge of how people with mild dementia manage to cope with familiar technology at home, such as television and electronic household machines, or new technology, such as remote controls, cell phones and computers. As this technology represents a potential problem area, the aim of this qualitative, exploratory study was to identify and characterize difficulties with and hindrances to using everyday technology, as they appeared in data, for persons with early stage dementia. Eight participants wit

More than just a bump: the hamartoma syndromes.

The hamartoma syndromes detailed in this review are just a few examples of the many genodermatoses now known to be associated with uncontrolled tumor proliferation secondary to mutations in tumor suppressor genes. Knowledge gained through the study of these syndromes has not only improved our understanding of patients afflicted with such conditions, but has also led to significant insight into the important role tumor suppressor genes play in preventing tumor formation and in carcinogenesis. As major strides continue to be made in the identification of causative mutations in the hamartoma synd

Urinary incontinence in men: current status and future directions.

BACKGROUND: Despite similar rates of voiding dysfunction in older men and women, most funded research has focused on women. Strategic treatment plans for managing urinary incontinence and other lower urinary tract symptoms in men are limited by sparse or absent direct clinical evidence with most interventions supported by data extrapolated from studies in women. OBJECTIVES: To explore what is known about the epidemiology and etiology of incontinence in men, highlight some of the gaps in the current knowledge, address limitations in existing research, and consider future directions in men's co

TB vaccines at the turn of the century: insights into immunity to M. tuberculosis and modern approaches for prevention of an ancient disease.

Tuberculosis (TB) remains a leading cause of infectious disease mortality worldwide, accounting for nearly 2 million deaths annually. Despite the availability of effective anti-TB therapy, the world's case burden of TB continues to climb, in part owing to the concurrent acquired immune deficiency syndrome pandemic. The widespread use of the current TB vaccine, the bacille Calmette-Guérin strain of M. bovis (BCG), has failed to curtail the TB epidemic and therefore strategies for the eradication of TB have centered on aggressive case-finding and managed treatment, such as directly observed trea

Mortality as an outcome in hospital-acquired pneumonia.

The most common route of entry of pathogens into the lung in patients managed in the intensive-care unit is aspiration of contaminated oropharyngeal secretions or gastric contents. In intubated patients, the risk of this type of infection is particularly high. Knowledge of specific risk factors for specific microorganisms, along with the origin of acquisition (primary endogenous, secondary endogenous, or exogenous), will permit a more rational and effective method of prevention. Attributable mortality is highly dependent on the institution of a correct initial antibiotic choice, as well as the
