Tricks for improving compliance with acne therapy.

Adherence to a medical regimen is the most important aspect of therapeutic efficacy. It goes without saying that a drug that is not used will not be effective. Reasons for noncompliance can be related to the specific disease state, to characteristics of the drugs chosen, to patient personality traits, and to effectiveness of the clinician in dealing with the individual patient. Recognizing, confronting, and improving noncompliant behavior are all part of an effective doctor-patient relationship.

In vitro candidacidal activity of a synthetic killer decapeptide (KP) against Candida albicans cells adhered to resin acrylic discs.

BACKGROUND: Oral Candida spp., and C. albicans in particular, are considered as important aetiological agents in the pathogenesis of denture-induced stomatitis. Several studies have reported that C. albicans is able to easily adhere to different medical devices, such as vascular and urinary catheters or acrylic denture surfaces, and that adhesion is a fundamental step in the initial pathogenic process of colonization and further possible infection. Recently, a synthetic decapeptide (KP) derived from the sequence of a single-chain recombinant anti-idiotypic antibody, acting as a functional inte

Brown Tumor: An Uncommon Manifestation of Severe Hyperparathyroidism.

Brown tumor is now an uncommon form of osteitis fibrosa cystica. It results from an imbalance in bone remodeling, which leads to osteolytic lesions. This process, marked by microfractures and discoloration, is triggered by persistently elevated parathyroid hormone levels in end-stage renal disease (ESRD) patients with tertiary hyperparathyroidism. Our case highlights a large brown tumor in the maxilla causing significant facial deformity. A 34-year-old female patient with ESRD had multiple admissions to our facility for various reasons. She was non-adherent to her dialysis treatment regimen an

Recent advances in Wilson disease.

Wilson disease (WD) is rare genetic disorder that presents with varied phenotype that can at times make the diagnosis challenging. Medical treatments are available, but there are still unmet needs for patients. Since life-long therapy is necessary, adherence to medical therapy and best practices for monitoring and individualizing therapy continue to evolve. Studies are ongoing that address some of these issues. In the current review we focused our attention to recent advances in the diagnosis of WD, current medical treatments, future potential therapies and treatment monitoring. We include dis

Exploring the impact of financial barriers on secondary prevention of heart disease.

BACKGROUND: Patients with coronary artery disease experience various barriers which impact their ability to optimally manage their condition. Financial barriers may result in cost related non-adherence to medical therapies and recommendations, impacting patient health outcomes. Patient experiences regarding financial barriers remain poorly understood. Therefore, we used qualitative methods to explore the experience of financial barriers to care among patients with heart disease. METHODS: We conducted a qualitative descriptive study of participants in Alberta, Canada with heart disease (n = 13

Reducing mortality from anthrax bioterrorism: strategies for stockpiling and dispensing medical and pharmaceutical supplies.

A critical question in planning a response to bioterrorism is how antibiotics and medical supplies should be stockpiled and dispensed. The objective of this work was to evaluate the costs and benefits of alternative strategies for maintaining and dispensing local and regional inventories of antibiotics and medical supplies for responses to anthrax bioterrorism. We modeled the regional and local supply chain for antibiotics and medical supplies as well as local dispensing capacity. We found that mortality was highly dependent on the local dispensing capacity, the number of individuals requiring

The quality of health care delivered to adults in the United States.

BACKGROUND: We have little systematic information about the extent to which standard processes involved in health care--a key element of quality--are delivered in the United States. METHODS: We telephoned a random sample of adults living in 12 metropolitan areas in the United States and asked them about selected health care experiences. We also received written consent to copy their medical records for the most recent two-year period and used this information to evaluate performance on 439 indicators of quality of care for 30 acute and chronic conditions as well as preventive care. We then co

Beyond stigma: reframing antipsychotics as medical treatment.

Antipsychotic non-adherence, driven partly by stigma, worsens outcomes in severe mental illness. Beyond ameliorating psychosis, evidence shows sustained antipsychotic treatment reduces mortality, cardiovascular risk, suicide, inflammation and cognitive decline, while improving quality of life. Reframing antipsychotics as essential medical care may reduce stigma and improve engagement with patients and clinicians.

Dilated pulmonary venous confluence presenting as a mass in mitral stenosis.

Pulmonary varices are rare venous anomalies which typically present as masses. Since they are usually clinically insignificant, varices must be diagnostically distinguished from parenchymal tumors and mediastinal adenopathy so one may adhere to a conservative medical approach. This report reviews the literature and illustrates a convergence of varices that presented as a larger sized mass than previously described by others.

Oral 5-aminosalicylic acid for maintenance of remission in ulcerative colitis.

BACKGROUND: Oral 5-aminosalicylic acid (5-ASA; also known as mesalazine or mesalamine) preparations were intended to avoid the adverse effects of sulfasalazine (SASP) while maintaining its therapeutic benefits. In an earlier version of this review, we found that 5-ASA drugs were more effective than placebo for maintenance of remission of ulcerative colitis (UC), but had a significant therapeutic inferiority relative to SASP. In this version, we have rerun the search to bring the review up to date. OBJECTIVES: To assess the efficacy, dose-responsiveness, and safety of oral 5-ASA compared to pl

The positive impact of journaling on adolescents with cystic fibrosis.

BACKGROUND: Individuals with cystic fibrosis (CF) often have psychological difficulties on top of their medically complex care, such as anxiety, depression, and medical mistrust. These have been shown to be associated with worse adherence, pulmonary function test results, and other health outcomes. In this pilot trial, we implemented a journaling program based on narrative therapy methodology to improve mental and physical health outcomes for individuals with CF. METHODS: Eight adolescents aged 12-17 with a confirmed diagnosis of CF followed in a single center CF clinic were emailed weekly jo

Effects of Patient Sexual Orientation and Gender Identity on Medical Students' Decision Making Regarding Preexposure Prophylaxis for Human Immunodeficiency Virus Prevention: A Vignette-Based Study.

INTRODUCTION: Preexposure prophylaxis (PrEP) is a pillar of our national strategy to end the human immunodeficiency virus (HIV) epidemic. However, one of the largest obstacles to realizing the effectiveness of PrEP is expansion of prescription to all patients at risk for HIV. In this vignette-based study, we sought to investigate medical students' decision making regarding PrEP by presenting fictional patients, all of whom had HIV risk factors based on sexual behavior. METHODS: We systematically varied patients' sexual orientation or gender identity (heterosexual female, gay male, bisexual ma
