Episodic and Long-term Costs of Acute Pancreatitis Requiring Hospitalization Among Adults in US Clinical Practice.

OBJECTIVES: Acute pancreatitis (AP) is associated with significant morbidity and mortality. While most patients fully recover following the acute phase of illness, some develop long-term complications. The objective of this study was to estimate short- and long-term costs among adults hospitalized with AP in US clinical practice, overall and within subgroups defined by AP cause. METHODS: A retrospective cohort design and health care claims database were employed. The study population comprised adults hospitalized for AP (first admission = index admission), and was considered overall as well a

COMBINATION OF ANTIOXIDANTS AND TARGETED TEMPERATURE MANAGEMENT AMELIORATES POSTCARDIAC ARREST SYNDROME IN A RAT MODEL OF ASPHYXIAL CARDIAC ARREST.

Background: Postcardiac arrest syndrome (PCAS) is a complex pathophysiological processes occurring after return of spontaneous circulation (ROSC) following cardiac arrest (CA), which may lead to multiple organ failure and death. The clinical efficacy of conventional therapies such as oxygen therapy and targeted temperature management (TTM) are remaining limited. Given the central role of oxidative stress in the progression of PCAS, we investigated the effect of combining antioxidants with TTM in an asphyxial CA rat model. Methods and Results: After 7 min of untreated asphyxial CA in Sprague-Da

Early Amyloid Diagnosis and Treatment: Experience of Single Hand Surgeon With a Series of 182 Carpal Tunnel Biopsies.

BACKGROUND: Amyloidosis is a systemic disease characterized by buildup of abnormal amyloid deposits that may lead to multiple organ failure and is associated with early onset musculoskeletal manifestations. Orthopedic surgeons are positioned to aid with early diagnosis in the form of a biopsy which allows for workup for other areas of disease, specifically cardiac manifestations, and intervention prior to irreversible cardiac damage. In this review, we describe the biopsy results of a series of 182 patients and workup and treatment of the 46 amyloid positive patients biopsied by a single surge

Enzyme Replacement and Immunosuppression in Heart Transplant Recipients with Fabry Cardiomyopathy: A 7-Year Case Study.

BACKGROUND Fabry disease (FD) is a rare storage disorder caused by the absence or deficiency of alpha-galactosidase A, which leads to accumulation of glycosphingolipids in tissues, chronic inflammation, fibrosis, and multiple-organ dysfunction. There are only a few cases of orthotopic heart transplantation (OHT) in patients with FD in the literature. This report shows the manifestation of FD, its course in OHT recipient and outcome of 4-year enzyme replacement therapy (ERT) combined with post-transplant immunosuppression. CASE REPORT A 54-year-old female patient had cardiomegaly revealed by a

Uterine Dehiscence and Abdominal Wall Necrotizing Fasciitis Following Cesarean Section: A Case Report.

BACKGROUND Necrotizing soft tissue infections, known as NSTIs, are well known for being acute, aggressive, and rapidly progressive and can cause systemic sepsis, toxic shock syndrome, and multi-organ failure. Clinical presentation varies from tense edematous skin to blisters and necrosis. The correlation between necrotizing fasciitis and cesarean section has been reported to be 1.8 per 1000 women, but its pathogenesis is not fully understood. CASE REPORT We present a case of a 28-year-old woman who underwent emergency cesarean section and presented 1 week later with septic shock and profuse fo

THE EFFICACY OF COAGULATION FACTOR CONCENTRATES IN THE MANAGEMENT OF PATIENTS WITH TRAUMA-INDUCED COAGULOPATHY: A SYSTEMATIC REVIEW AND META-ANALYSIS.

Background: Death in the early phase of trauma is primarily attributable to uncontrolled bleeding exacerbated by trauma-induced coagulopathy (TIC). A comprehensive synthesis of the available evidence on interventions for TIC is needed. Methods: We conducted a systematic review and meta-analysis of blood component products and tranexamic acid administrations for severe trauma patients with TIC. We included randomized and nonrandomized controlled trials. We included studies with patients who required transfusion with any coagulopathy associated with trauma and a detailed definition. Enhanced Antimalarial and Antisequestration Activity of Methoxybenzenesulfonate-Modified Biopolymers and Nanoparticles for Tackling Severe Malaria.

Severe malaria is a life-threatening condition that is associated with a high mortality. Severe Plasmodium falciparum infections are mediated primarily by high parasitemia and binding of infected red blood cells (iRBCs) to the blood vessel endothelial layer, a process known as sequestration. Here, we show that including the 5-amino-2-methoxybenzenesulfonate (AMBS) chemical modification in soluble biopolymers (polyglutamic acid and heparin) and poly(acrylic acid)-exposing nanoparticles serves as a universal tool to introduce a potent parasite invasion inhibitory function in these materials. Successful treatment of severe splenic lymphoma‑associated hemophagocytic syndrome by splenectomy and subsequent chemotherapy: A case report.

Hemophagocytic lymphohistiocytosis (HLH) represents a fatal immunopathology derived from excessive inflammatory reactions. In particular, lymphoma-associated hemophagocytic syndrome (LAHS) is associated with a dismal prognosis. The current study presented a challenging case of splenic LAHS. A 71-year-old man presented with fatigue and anorexia. Laboratory test results revealed anemia, thrombocytopenia, lactate dehydrogenase elevation and markedly elevated levels of ferritin (6,210 ng/ml) and soluble interleukin 2 receptor (sIL-2R; 11,328 U/ml). Abdominal computed tomography revealed marked spl

Yttrium-90 anti-CD25 BEAM conditioning for autologous hematopoietic cell transplantation in Peripheral T-cell lymphoma.

Peripheral T-cell lymphomas (PTCLs) have a poor prognosis with current treatments. High-dose chemotherapy followed by autologous hematopoietic cell transplant (AHCT) is used as a consolidation strategy after achieving clinical remission with first-line therapy, as well as in chemotherapy-sensitive relapse if allogeneic transplant is not an option. CD25 is a targetable protein often highly expressed in PTCLs. In this phase 1 clinical trial, we tested the addition of β-emitting 90yttrium (90Y)-labeled chimeric anti-CD25 basiliximab (aTac) to BEAM (carmustine, etoposide, cytarabine, and melphalan

The Role of microRNAs in Arsenic-Induced Human Diseases: A Review.

MicroRNAs (miRNAs) are noncoding RNAs with 20-22 nucleotides, which are encoded by endogenous genes and are capable of targeting the majority of human mRNAs. Arsenic is regarded as a human carcinogen, which can lead to many adverse health effects including diabetes, skin lesions, kidney disease, neurological impairment, male reproductive injury, and cardiovascular disease (CVD) such as cardiac arrhythmias, ischemic heart failure, and endothelial dysfunction. miRNAs can act as tumor suppressors and oncogenes via directly targeting oncogenes or tumor suppressors. Recently, miRNA dysregulation wa

Immune dysregulation in Kabuki syndrome: a case report of Evans syndrome and hypogammaglobulinemia.

Kabuki syndrome (KS) is a rare multisystemic disease due to mutations in the KMT2D or KDM6A genes, which act as epigenetic modulators of different processes, including immune response. The syndrome is characterized by anomalies in multiple organ systems, and it is associated with autoimmune and inflammatory disorders, and an underlying immunological phenotype characterized by immunodeficiency and immune dysregulation. Up to 17% of KS patients present with immune thrombocytopenia characterized by a severe, chronic or relapsing course, and often associated to other hematological autoimmune disea

Differences in clinical characteristics and chest CT findings between severe and critical H1N1 pneumonia.

INTRODUCTION: Critical H1N1 pneumonia patients usually have one of the symptoms such as respiratory failure, septic shock, multiple organ dysfunction, or other need for intensive care management, which are associated with high risk of mortality. It is essential to differentiate the severity of H1N1 pneumonia and take corresponding target treatments. OBJECTIVES: We aim to investigate the differences in clinical characteristics and chest computed tomography (CT) findings between severe and critical patients with H1N1 pneumonia. METHODS: A total of 27 patients diagnosed with H1N1 pneumonia from

Bilateral Psoas Haematoma Following Extracorporeal Membranous Oxygenation in a Patient with Severe COVID-19 (SARS CoV-2). A Rare Case Report and Literature Review.

Abstract IntroductionAs the COVID-19 pandemic evolves periodically, becomes evident that although affects primarily the respiratory system, its ability to trigger a significant inflammatory response, combined with neuropathy and coagulopathy, often evolves into complex systemic responses as opposed to a purely respiratory disease. This can lead into severe respiratory distress and failure, septic shock, arrhythmia and a multiple organ dysfunction; Circumstances where first-line non-invasive medical therapies become suboptimal and advanced lifesaving intervention including, but not limited to i

A Minimally Invasive and Highly Effective Extracorporeal CO2 Removal Device Combined With a Continuous Renal Replacement Therapy.

OBJECTIVES: Extracorporeal carbon dioxide removal is used to treat patients suffering from acute respiratory failure. However, the procedure is hampered by the high blood flow required to achieve a significant CO2 clearance. We aimed to develop an ultralow blood flow device to effectively remove CO2 combined with continuous renal replacement therapy (CRRT). DESIGN: Preclinical, proof-of-concept study. SETTING: An extracorporeal circuit where 200 mL/min of blood flowed through a hemofilter connected to a closed-loop dialysate circuit. An ion-exchange resin acidified the dialysate upstream, a
