Pain in systemic lupus erythematosus: emerging insights and paradigms.

Systemic lupus erythematosus (SLE) is a chronic autoimmune disease characterized by protean clinical manifestations that are associated with immune system dysregulation. Of these manifestations, pain and pain-related symptoms such as fatigue, mood disturbance and cognitive impairment are the most common features reported by patients and represent important determinants of quality of life. Nevertheless, the relationship of these symptoms to underlying immune mechanisms is unclear. To advance scientific study and patient-centric care, this Review will consider the origin of pain in SLE and the c

Studies on the neurobiology of vitamin E (alpha-tocopherol) and some other antioxidant systems in the rat.

To understand why nervous tissue should be particularly affected by severe deficiency of vitamin E, the distribution of vitamin E (alpha-tocopherol) and some other antioxidant systems was investigated. The concentration of vitamin E and the activities of glutathione peroxidase and superoxide dismutase were determined in different regions of the nervous system in male Wistar rats. The cerebral cortex had the highest, and the cerebellum the lowest concentration of alpha-tocopherol (P less than 0.02). Activity of glutathione peroxidase tended to show an inverse relationship to the alpha-tocophero

Efficacy of non-pharmacological interventions for chemotherapy-induced peripheral neuropathy: a systematic review and network meta-analysis for randomized controlled trials.

BACKGROUND: Chemotherapy-induced peripheral neuropathy (CIPN) is a prevalent adverse effect linked to neurotoxic chemotherapeutic agents. Current pharmacological treatments exhibit limited efficacy and notable adverse effects. The clinical effectiveness of non-pharmacological therapies, like acupuncture, physical exercise (PE), cryotherapy (CR), and compression therapy, requires systematic comparison. This study employs a network meta-analysis (NMA) to appraise the efficacy and preventive effects of various non-pharmacological interventions on CIPN. METHODS: The study adhered to the PRISMA gu

Mitochondrial transfer as a therapeutic target for peripheral neuropathy.

Satellite glial cells transfer mitochondria to sensory neurons via myosin 10-dependent tunneling nanotubes. Ji et al. show that this transfer is impaired in diabetic neuropathy, causing energy failure. Restoring it via cell or mitochondrial transplantation alleviates pain and promotes nerve regeneration, revealing a new therapeutic strategy for peripheral neuropathy.

Monoallelic POLR3A Variants Cause Early-Onset Peripheral Neuropathy.

OBJECTIVE: Biallelic variants in genes encoding the RNA polymerase III complex (Pol III) cause a spectrum of neurological disorders primarily affecting the central nervous system. Monoallelic variants have been reported in the POLR3B subunit only, associated with neurodevelopmental disorder, epilepsy, and peripheral neuropathy. We describe a novel Pol III-related disorder caused by monoallelic variants in POLR3A and presenting primarily with peripheral neuropathy. METHODS: We performed clinical and genetic investigation of the affected families. Biophysical characterization of mutant proteins

Unraveling GDAP1: Bridging Mitochondrial Biology and Peripheral Neuropathy.

The mitochondrial outer membrane (OMM) plays a crucial role in maintaining cellular homeostasis by regulating mitochondrial dynamics, organelle interactions, and stress responses. In peripheral neurons-cells with high metabolic demands and long axons-the OMM acts as a vital platform for coordinating bioenergetics, calcium signaling, and redox balance. Ganglioside-induced differentiation-associated protein 1 (GDAP1), an OMM-anchored protein, has emerged as a key regulator of mitochondrial fission and transport, redox homeostasis, and mitochondrial membrane contact sites (MCSs). Genetic variants

Touch and manual action in chemotherapy-induced peripheral neuropathy: a mixed-methods study.

Chemotherapy induced peripheral neuropathy (CIPN) is a frequent side effect of a number of chemotherapeutic agents which are widely used in the treatment of common cancers. Sensory symptoms primarily affect the fingers and toes and include numbness, tingling and pain. However, there is limited understanding of how these touch impairments may impact manipulation skills. Questionnaire and focus group methods were used to describe the experience of CIPN side effects impacting manual activities in 25 self-selected participants recruited from a cancer charity web site advertisement. Long COVID neuropathy: The role of mast cells.

Postacute sequelae of SARS-CoV-2 infection (PASC), or Long COVID, is estimated to affect over 60 million individuals globally, with almost half of COVID-19 survivors experiencing persistent symptoms such as neuropathic pain, fatigue, and autonomic dysfunction. Despite its prevalence, the pathophysiology of PASC remains poorly understood. This narrative review highlights activation of mast cells (MCs), the unique tissue immune cells as a central contributor to neuropathic manifestations in PASC. Mast cell locations near nerves and vessels allows them to regulate neuroimmune and neurovascular pr

Impact of Genetic Polymorphisms on Bortezomib-Induced Peripheral Neuropathy in Multiple Myeloma: A Systematic Review and Bioinformatics Analysis.

BACKGROUND: Bortezomib, a 26S proteasome inhibitor, has become a cornerstone in the treatment of multiple myeloma. However, its use is limited by a common and potentially serious adverse effect, bortezomib-induced peripheral neuropathy (BIPN), which manifests in 30-60% of multiple myeloma patients primarily as a sensory, distal, axonal neuropathy, often with pain, numbness, tingling, and in some cases, motor involvement, which can lead to dose reductions, therapy discontinuation, or long-term morbidity. BIPN is associated with genetic predisposition, and several studies suggest that single-nuc

Risk Factors of Chemotherapy-Induced Painful Peripheral Neuropathy: A Retrospective Study from A Single Cancer Center.

BACKGROUND: Chemotherapy-induced peripheral neuropathy (CIPN) affects approximately 50% of patients who receive chemotherapy. CIPN often results in dose reductions, therapy discontinuation, and long-term neurological impairment. Despite existing studies, identifying high-risk populations remains challenging, particularly in patients with diabetes, diabetic neuropathy, and those undergoing corticosteroid therapy. OBJECTIVES: We sought to evaluate the key risk factors associated with CIPN by analyzing patient demographics, comorbidities, and chemotherapy regimens, with a specific focus on diabe

Omega-3-Enriched Fish oil reduces the chemotherapy-induced peripheral neuropathy in mice.

Cancer is a leading cause of mortality and morbidity worldwide and conventional chemotherapy frequently induce irreversible adverse effects in patients. Chemotherapy-induced peripheral neuropathy (CIPN) is a common adverse effect of both paclitaxel (PTX) and oxaliplatin (OXA) chemotherapies, affecting approximately 30-50% of patients. As cancer survival rates have improved, the efforts of scientific community to develop new strategies for preventing CIPN are also growing. This study presents the effects of omega-3 (ω-3)-enriched fish oil supplementation on the hypersensitivity induced by PTX o

Repeated Valproic Acid Administration Fundamentally Ameliorated Cisplatin-Induced Mechanical Allodynia in Rats.

Cisplatin (cis-diamminedichloro-platinum; CDDP) is a chemotherapeutic agent that frequently induces peripheral neuropathy characterized by mechanical allodynia. Herein, we aimed to determine the effects of valproic acid (VPA) on cisplatin-induced mechanical allodynia in rats and elucidate the underlying mechanisms. A single administration of VPA (150 mg/kg) transiently suppressed CDDP-induced mechanical allodynia, correlating with serum VPA concentrations. Repeated VPA administration before or after the onset of CDDP-induced mechanical allodynia significantly attenuated allodynia even after VP

Regenerative failure of sympathetic axons contributes to deficits in functional recovery after nerve injury.

Renewed scientific interest in sympathetic modulation of muscle and neuromuscular junctions has spurred a flurry of new discoveries with major implications for motor diseases. However, the role sympathetic axons play in the persistent dysfunction that occurs after nerve injuries remains to be explored. Peripheral nerve injuries are common and lead to motor, sensory, and autonomic deficits that result in lifelong disabilities. Given the importance of sympathetic signaling in muscle metabolic health and maintaining bodily homeostasis, it is imperative to understand the regenerative capacity of s

A novel isoquinoline mitophagy inducer ameliorates paclitaxel-induced peripheral neuropathy in Drosophila and mouse models.

Chemotherapy-induced peripheral neuropathy (CIPN) resulting from neurodegeneration due to chemotherapy is a challenging complication of widely administered anticancer drugs including paclitaxel. Although CIPN is common and limits the use of chemotherapies, no curative treatment for CIPN has been developed. Recently, stimulation of mitophagy has emerged as a promising strategy for treating neurodegenerative diseases, but studies on its therapeutic effects on CIPN are limited. In this study, we examined the therapeutic effect of the recently developed mitophagy inducer ALT001 on paclitaxel-induc

Preventing neuropathy and improving anticancer chemotherapy with a carbazole-based compound.

While advances in cancer therapy have improved remission rates, chemotherapy-induced peripheral neuropathy (CIPN) remains a lasting and untreatable side effect. This study introduces Carba1, a bifunctional carbazole compound that protects against CIPN through two mechanisms. First, Carba1 interacts with tubulin, allowing for lower doses of taxanes, common chemotherapeutics known for causing CIPN, without reducing their anticancer effectiveness. Second, Carba1 activates nicotinamide phosphoribosyltransferase (NAMPT), enhancing NAD biosynthesis and boosting the metabolic resilience of neurons an

Vitamin B12 in Cancer Patients: Clinical Insights into Deficiency, Excess, Diagnosis, and Management.

Vitamin B12 (cobalamin) is a critical micronutrient involved in hematopoiesis and neurological function. Its deficiency, commonly presenting with anemia and neurological symptoms, is particularly relevant in oncology. While anemia affects up to 60% of cancer patients, the contribution of vitamin B12 deficiency to cancer-related anemia remains underexplored. Additionally, cobalamin-related neuropathy manifests or exacerbates existing chemotherapy-induced peripheral neuropathy (CIPN), a serious side effect of chemotherapy. Prevalence estimates in cancer populations range widely (6-48%), with hig
