How and why should we stabilize the upper airway?

This review focuses on the evidence for upper airway instability during sleep and on the methods used to correct this instability. Upper airway patency during sleep is determined by the balance between the forces tending to constrict the pharynx (i.e. negative suction force generated by the diaphragm) and those tending to dilate in the pharynx (i.e. force acting on the tongue, soft palate and pharyngeal dilator muscles). The evidence for reduction in genioglossus activity and tensor palatini activity, failure of compensatory mechanisms to maintain these activities, and increase in upper airway

Brain regions and genes affecting postural control.

Postural control is integrated in all facets of motor commands. The role of cortico-subcortical pathways underlying postural control, including cerebellum and its afferents (climbing, mossy, and noradrenergic fibers), basal ganglia, motor thalamus, and parieto-frontal neocortex has been identified in animal models, notably through the brain lesion technique in rats and in mice with spontaneous and induced mutations. These studies are complemented by analyses of the factors underlying postural deficiencies in patients with cerebellar atrophy. With the gene deletion technique in mice, specific g

Clinical Changes in Patients with Idiopathic Scoliosis After One Year of Schroth Treatment.

INTRODUCTION: Surface topography (ST) enables objective clinical evaluation of posture and monitoring of patients with spinal column deformities. The advantage of surface topography is reflected in the absence of radiation, which enables unlimited monitoring of changes on the body without risking the patient's health. In addition to the numerous advantages of ST, a limitation is the inability to precisely identify the Cobb angle. AIM: The aim of this research is to determine the effect of one-year Schroth treatment on the participants diagnosed with idiopathic scoliosis using surface topograp

The relationship between gait asymmetry and stability in people with sub-acute stroke.

BACKGROUND: People often walk with temporal asymmetry post-stroke; which is related to increased fall risk. The purpose of this study was to determine the relationship between temporal gait asymmetry and mechanical stability among people with sub-acute stroke. METHODS: Thirty-one people with sub-acute stroke (<6 months post-stroke) completed six walking trials. Three-dimensional motion capture was recorded. Swing symmetry was calculated as a ratio of swing time on the more affected limb divided by swing time on the less affected limb. Mechanical stability was the minimum margin of stability,

Effects of the 'ReacStep' training program on balance recovery and fall risk factors in older people: an assessor-blinded randomised controlled trial.

BACKGROUND: Reactive balance training (RBT) using repeated perturbations may reduce falls; however, training methods are not easily replicated or translatable to clinical settings. This study aimed to examine the effects of a novel RBT program on balance recovery from laboratory-induced trips and slips and fall risk factors in older people using simple and low-cost equipment. METHODS: We conducted a randomised controlled trial involving 88 older people. An intervention group (n = 43) received the ReacStep program, which involved tether-release reactive stepping and intentional slips once a we
