Use of computed tomography to categorize the type of tracheal agenesis.

We report a case of type I tracheal agenesis with vertebral defects, anal atresia, tracheoesophageal fistula and/or esophageal atresia, radial dysplasia, renal defects, and cardiovascular defects association in which computed tomography was helpful in categorizing the diagnosis. This rare and fatal condition should always be considered when confronted with a neonate impossible to resuscitate at birth.

Airway obstruction following internal jugular vein cannulation.

A patient developed neck swelling following removal of an internal jugular vein cannula. Although tracheal extubation was delayed, severe airway obstruction occurred resulting in a cardiac arrest. He was successfully resuscitated. Doppler ultrasonography revealed a huge cervical haematoma surrounding a carotid artery pseudoaneurysm. The carotid artery pseudoaneurysm was obliterated using radiological intervention.

Pneumatic antishock trousers.

While pneumatic antishock trousers are a useful adjunct for the initial management of hypotension, they do not replace, but can augment, fluid replacement and other resuscitative procedures. They can be applied quickly and are relatively safe when used in the manner recommended. They must not be used in a patient with congestive heart failure. Finally, it is imperative that they are not removed suddenly from the hypovolemic, hypotensive patient.

Involving mothers in monitoring their unborn babies during labour: experience from Liberia.

AIM: To assess the impact on perinatal mortality and morbidity of involving mothers in partnership with maternity staff in monitoring their unborn babies. METHODS: Mothers attending 4 hospitals in Liberia consented and were trained to use doppler ultrasound probes to monitor for abnormalities or changes in fetal heart rates during labour. A range of 120 to 160 beats per minute was chosen as normal. Intermittent auscultation was undertaken sequentially, immediately following the end of every uterine contraction. RESULTS: One hundred eleven intrauterine fetal deaths occurred before mothers arr

A Prospective Observational Study on Serum Lactate Dehydrogenase and Uric Acid Levels in Hypertensive Disorders of Pregnancy and Prediction of Fetomaternal Outcome.

Introduction Hypertensive disorders of pregnancy are a major cause of maternal and perinatal morbidity and mortality, particularly in low- and middle-income countries. Early identification of women at risk using simple biochemical markers may aid in timely intervention and improved outcomes. Serum lactate dehydrogenase (LDH) and uric acid have been proposed as indicators of disease severity in hypertensive pregnancies. The study aimed to evaluate the association of serum LDH and uric acid levels with the severity of hypertensive disorders of pregnancy and their relationship with maternal and f

Investigating the impact of community first responders on out-of-hospital cardiac arrest outcomes in the North East of England: a retrospective cohort study.

INTRODUCTION: Community first responders (CFRs) are integrated within UK ambulance services to deliver early life-saving interventions prior to the arrival of emergency medical services (EMS). Although CFR attendance is associated with earlier cardiopulmonary resuscitation (CPR) and defibrillation, evidence regarding their independent impact on survival following out-of-hospital cardiac arrest (OHCA) remains mixed, and regional data from North East England are limited. The aim of this study was to evaluate the association between CFR attendance and survival outcomes following OHCA attended by

Blood product use in paediatric trauma: lessons from the TARN data.

BACKGROUND: Trauma contributes significantly to childhood morbidity and mortality. Uncontrolled haemorrhage leading to cardiovascular collapse remains the leading cause of preventable death in paediatric trauma. Evidence to support the early and accurate identification of paediatric trauma patients requiring blood products is growing. Consensus guidelines advocating for more liberal blood product use based on the concept of damage control resuscitation and the introduction of risk predictor tools to identify children requiring resuscitation with blood products may have altered use in recent ye

Anesthesiology competencies in undergraduate medical education: a comparative study of curriculum frameworks in Brazil, Spain, and the United Kingdom.

OBJECTIVES: Medical education is crucial for meeting local needs and achieving global standards, especially in anesthesiology. Key competencies, such as airway management, mechanical ventilation, and sedation, are essential for managing critically ill patients, maintaining surgical and intensive care services, and preventing deaths from surgically treatable conditions. This study evaluated anesthesiology competencies in the curricula of medical schools in Brazil, Spain, and the United Kingdom (UK), highlighting the importance of integrating these skills to prepare for critical situations and s

Thromboelastography versus Standard Coagulation Assays in Patients with Postpartum Hemorrhage.

OBJECTIVE: Thromboelastography (TEG), a point-of-care test that measures blood's dynamic viscoelastic properties, is routinely used to guide resuscitation in surgical specialties with high hemorrhage risk. Patients with ongoing postpartum hemorrhage (PPH) often develop coagulopathy and hypofibrinogenemia. Timely assessment of fibrinogen is crucial because cryoprecipitate for repletion requires thawing time prior to administration. TEG may provide rapid assessment of coagulopathy in ongoing hemorrhage but this has not been thoroughly studied. Our objective was to determine if TEG accurately ref

Traumatic diaphragmatic rupture: epidemiology, associated injuries, and outcome-an analysis based on the TraumaRegister DGU®.

INTRODUCTION: Traumatic diaphragmatic rupture is a rare injury in the severely injured patient and is most commonly caused by blunt mechanisms. However, penetrating mechanisms can also dominate depending on regional and local factors. Traumatic diaphragmatic rupture is difficult to diagnose and can be missed by primary diagnostic procedures in the resuscitation room. Initially not life-threatening, diaphragmatic ruptures can cause severe sequelae in the patient's long-term course if untreated. The objective of this study was to assess the epidemiology, associated injuries, and outcome of traum

Increased cerebral endothelium-dependent vasodilation in rats in the postcardiac arrest period.

Cardiovascular lability is common after cardiac arrest. We investigated whether altered endothelial function is present in cerebral and mesenteric arteries 2 and 4 h after resuscitation. Male Sprague-Dawley rats were anesthetized, intubated, ventilated, and intravascularly catheterized whereupon rats were randomized into four groups. Following 7 min of asphyxial cardiac arrest and subsequent resuscitation, cardiac arrest and sham rats were observed for either 2 or 4 h. Neuron-specific enolase levels were measured in blood samples. Middle cerebral artery segments and small mesenteric arteries w

Intraoperative development of pulmonary thromboembolism in a bedridden patient owing to a pelvic bone fracture with negative preoperative computed tomography pulmonary angiographic findings: A case report.

RATIONALE: Pulmonary thromboembolism (PTE) is a potentially life-threatening condition with high morbidity and mortality, and computed tomographic pulmonary angiography (CTPA) is an important diagnostic tool for patients in whom PTE is suspected; however, intraoperative PTE is very difficult to diagnose and often has a rapid clinical course. We experienced a case of intraoperative PTE with persistent tachycardia refractory to conventional treatments despite negative preoperative CTPA findings. PATIENT CONCERNS: A 53-year-old man with a pelvic bone fracture who had been on bed rest for 10 days

Comparison of Commercially Available Balanced Salt Solution and Ringer's Lactate on Extent of Correction of Metabolic Acidosis in Critically Ill Patients.

INTRODUCTION: Appropriate early fluid resuscitation is ubiquitous for critically ill patients with metabolic acidosis. Owing to harmful effects of normal saline, commercially prepared balanced salt solutions are being used. However, there is no study comparing use of Ringer's lactate (RL) and commercially available balanced salt solutions in critically ill patients. MATERIALS AND METHODS: A randomized controlled trial was conducted during July 2016 to December 2017. Fifty adult patients admitted to intensive care unit with metabolic acidosis were randomized into group RL or group acetate solu

Infrared pupillometry to help predict neurological outcome for patients achieving return of spontaneous circulation following cardiac arrest: a systematic review protocol.

BACKGROUND: Despite advances in resuscitation care, mortality rates following cardiac arrest (CA) remain high. Between one-quarter (in-hospital CA) and two-thirds (out of hospital CA) of patients admitted comatose to intensive care die of neurological injury. Neuroprognostication determines an informed and timely withdrawal of life sustaining treatment (WLST), sparing the patient unnecessary suffering, alleviating family distress and allowing a more utilitarian use of resources. The latest Resuscitation Council UK (2015) guidance on post-resuscitation care provides the current multi-modal neur

Citalopram Overdose: a Fatal Case.

BACKGROUND: Citalopram is a selective serotonin reuptake inhibitor (SSRI) with cardiac and neurologic toxicities as well as the potential for serotonin syndrome. In most instances, patients recover fully from toxic ingestions of SSRIs. We describe a fatal case of a citalopram overdose. CASE REPORT: A 35-year-old woman presented to the emergency department after having witnessed seizures at home. An empty citalopram prescription bottle was located, and an intentional overdose was suspected. At the scene, she was found to be in cardiac arrest with pulseless electrical activity and underwent car
